New proposal for skin grafts on tendon-exposed wounds

  • Performing a skin graft is not feasible for tendon-exposed defect reconstruction because tendons are fibrous connective tissues with relatively poor blood supply. This study proposes a method to effectively perform skin graft surgery in tendon-exposed wounds. A 48-year-old male patient with diabetes mellitus had a very large left dorsal foot defect (8×8 cm). The wound bed had healthy granular tissue, with tendon exposure. The tendons were turned over so that the posterior side would behave as the anterior side. The edge of the paratenon was then fixed together to the surrounding granulation tissue or dense remnant fascia using absorbable sutures, and the close granulation tissue was approximated and buried. A split-thickness skin graft was performed after 1 week. The graft site was stably taken on postoperative day 3. A small disruption was then observed at the graft site within 1 week postoperatively, but conservative treatment was continued for 1 month, after which the defect site was completely restored. This technique can increase the success rate of skin grafts for defects with tendon exposure.

A Study of Dispersion Characteristics of Sunsik Using Fluidized-Bed Granulator (유동층 과립기를 이용한 선식의 분산 특성)

  • To improve dispersibility of cereal powder without additives, granulation of cereal powder was conducted using fluidized-bed granulator. Operation condition was sample 300 g, internal temperature $40^{\circ}C$, ventilation speed $30-90m^3/h$, inlet temperature $90^{\circ}C$ and spray pressure 2.5 bar. The amount of distilled water (20-45%) as binder, granulation time (10-15 min) and drying time (3-10 min) were controlled. Mean diameter over volume (Brouckere mean, $D_{4,3}$) was increased from $123{\mu}m$ to $263{\mu}m$ and dispersibility was improved from 73% to 92.25% at experiment conditions. Wettability (wetting time) was drastically decreased from 5,000 second to 7 second. Granulation of cereal powder did not affect sinkability and mean diameter over volume as wet analysis was about the same between raw and granulated cereals. Such phenomenon means that granulation with only water as binder enables cereal powder to disperse in water or milk without rapid sedimentation.

A Clinical Study of Hearing Disturbance and Middle Ear Pathology in Chronic Otitis Media (만성중이염의 교실내 병변과 청력에 관한 임상적 연구)

  • The degree of hearing impairment of chronic otitis media will indicate the nature and severity of middle ear pathology especially condition of ossicular chain, size of ear drum perforation and location of granulation tissue in the middle ear cavity. The subjects were 189 ears of tympanoplasty for chronic otitis media and divided into four groups as follows: Normal ossicular chain with only ear drum perforation (group I), normal ossicular chain with granulation tissue only around the ossicles regardless of any other region (group II), ossicular ankylosis or fixation of handle of malleus to promontory with or without granulation tissue around the ossicle (group III) and ossicular interruption by partial or complete destruction(groupf IV). The results were concluded as follows: 1) The average hearing threshold of chronic otitis media was 44.6 dB and hearing threshold was closely related to the condition of ossicular chain. Hearing threshold became greater in order of normal ossicular chain, ankylosis and interruption. 2) The average hearing threshold of ossicular interrupted group was 49.1 dB and it was greater in the cases of total destruction than that of partial destruction. 3) The hearing loss in the cases of normal ossicular chain with only tympanic membrane perforation was within 45 dB and this level was presumed to represent normal ossicular function. The degree of hearing loss was in proportion to the size of ear drum perforation and when over 45 dB, other middle ear pathology was suggested. 4) In the cases of small ear drum perforation with normal ossicular function, the hearing threshold was within 30 dB. 5) In the type of audiogram, flat type was 30.2% and ascending type 35.4%. Descending type was more frequent in the cases of normal ossicular mobility with granulation tissue around the ossicle and flat type was observed frequently in the cases of ossicular ankylosis. 6) Carhart's notch was seen in 14 cases (7.4%) and observed mainly in ossicular ankylosis. 7) There was no relation between hearing threshold and histopathological type of granulation tissue in chronic otitis media. However the degree of hearing impairment was related to the location of granulation tissue in the middle ear cavity. 8) Authors recognized the granulation tissue compensated the function of interrupted ossicular chain.

Alveolar ridge preservation using granulation tissue for esthetic implant restoration on maxillary anterior tooth (상악 전치부의 심미적 임플란트 수복을 위한 육아 조직(Granulation tissue)을 이용한 치조제 보존술)

  • Esthetic factors are very important in the success of maxillary anterior implant restoration. However, achieving esthetic results is difficult, especially in cases where periodontitis has resulted in severe alveolar bone loss. In the case of maxillary anterior teeth, the alveolar ridge resorption that begins immediately after tooth extraction interferes with the esthetic implant restoration. Therefore immediate implant placement can be performed to minimize the alveolar ridge resorption. However, in severe bone loss cases, immediate implant placement could result in esthetic failure, and this result might cause irreparable problems. We can also perform alveolar ridge preservation and then place implants later. On JCP published in 2019, there is the consensus of European academy of periodontology on the extraction socket management and the timing of implant placement. This consensus states that alveolar ridge preservation should be considered when there is severe labial bone loss in an esthetically important area such as maxillary anterior region. On performing the alveolar ridge preservation, we cannot obtain the primary wound closure, so secondary wound healing is induced with open membrane technique or soft tissue grafting should be performed for primary wound closure. However, the secondary wound healing can have a negative impact on bone regeneration, and soft tissue grafting such as FGG or CT graft can be burdensome for both patients and dentists. On the other hand, by using the granulation tissue in the extraction socket, primary closure can be achieved without soft tissue grafting. Also some studies have shown that granulation tissue in periodontal defects contains stem cells that may help in tissue regeneration. Based on this, implant restorations were performed on maxillary anterior teeth with severe alveolar bone loss by alveolar ridge preservation using granulation tissue. In spite of the severe bone defect of the extraction socket, relatively esthetic results could be obtained in implant restorations.

A Case Study on Sintering Characteristics of Yttria Stabilized Zirconia Powder Prepared by Two-Fluid Spray Drying

  • As a case study on yttria stabilized zirconia ceramics, the sintering characteristics of submicron powders and the granulation prepared by two-fluid spray drying of submicron particles were investigated. As-received powders of yttria stabilized zirconia particles were reduced to a uniform size of less than about 200 nm by repeated milling. Granulation size obtained by the two-fluid spray drying was affected by the organic matter and the primary particle size. Sintering behavior such as porosity, water absorption ratio, density, and transparency was influenced by processing conditions of the powder, and the discontinuous interfaces in a green body were reduced.

  • Matrix tablets of nifedipine (NP) were prepared with Eudragit, diluent (lactose or Ca. phosphate) and Mg. stearate employing two different preparation methods (wet granulation and direct compression) to develop its sustained-release dosage forms. The effects of various formulation factors on the dissolution rate of the drug were investigated. Dissolution test was studied in pH 6.8 phosphate buffer containing 1% sodium lauryl sulfate using the paddle method. Formulation factors were the type and content of Eudragit, the type of diluent and the tablet preparation method. The optimum formula of NP matrix tablet, which resulted in a similar dissolution profile to that from Adalat Oros used as a reference, was 30 mg NP, 10% Eudragit RS, 2% Mg. stearate and an adequate quantity of lactose to yield 500 mg weight using the wet granulation method.

  • We introduce a hybrid identification of information granulation(IG)-based fuzzy model to carry out the model identification of complex and nonlinear systems. To optimally design the IG-based fuzzy model we exploit a hybrid identification through genetic alrogithms(GAs) and Hard C-Means (HCM) clustering. An initial structure of fuzzy model is identified by determining the number of input, the seleced input variables, the number of membership function, and the conclusion inference type by means of GAs. Granulation of information data with the aid of HCM clustering help determine the initial paramters of fuzzy model such as the initial apexes of the membership functions and the initial values of polyminial functions being used in the premise and consequence part of the fuzzy rules. And the inital parameters are tuned effectively with the aid of the GAs and the least square method. Numerical example is included to evaluate the performance of the proposed model.

Management of Mediastinitis after Open Heart Surgery (개심술후 발생한 종격동염의 치료)

  • Between January 1984 and January 1994, 13 patients developed mediastinitis after cardiac operations. There were 7 women and 6 men with a mean age of 24.2 years[range 0.7 - 61 . Initial operation included 7 valve replacements and 6 congenital cardiac repairs. The duration between initial operation and diagnosis was 13.5 days[range 4-57 . Organisms cultured from debridement material included S.aurus[n=9 , S.epidermidis[n=3 and Enterobacter[n=1 . Fever was the most frequent complaint and purulent drainage was noticed in 9 patients[69% . Seven patients were treated with radical debridement followed by closed irrigation. In other 6 patients, the wounds were managed by debridement, open granulation and delayed wound closure. Two hospital deaths [15.4% following open granulation method, resulted from sepsis. The 11 survivors were followed-up for 1-120 months, averaging 42 months and had healed wounds. One late death occurred due to massive hemorrhage.

Anaerobic Treatment of High Strength Organic Wastewater by Granulated Microorganism (입상미생물을 이용한 고농도 유기성폐수의 혐기성처리)

  • This research aims to find granulation and organic removal of the piggery wastewater with the upflow blanket filter(UBF) reactor. UASB process had the effect of high pH on the granulation phase. But teh effect was decreased after the granule formation. The filter zone of the UBF reactor had the function of GSS and contributed to removing the organic because of its biofilm formation. The removal rates of total COD and soluble COD were 70% ~ 80% and 60 ~ 80% at an influent organic loading range of $2{\sim}17.4kgCOD/m^3{\cdot}d$, respectively. The methane production rate with the organic removal was $0.21{\sim}0.34{\ell}CH_4/gCOD_{rem}$ and the maximum methan production rate was $0.34CH_4{\ell}/gCOD_{rem}$ at the volumetric loading $5kgCOD/m^3{\cdot}d$.

Efficient Extraction of Hierarchically Structured Rules Using Rough Sets

  • This paper deals with rule extraction from data using rough set theory. We construct the rule base in a hierarchical granulation structure by applying core as a classification criteria at each level. When more than one core exist, the coverage is used for the selection of an appropriate one among them to increase the classification rate and accuracy. In Addition, a probabilistic approach is suggested so that the partially useful information included in inconsistent data can be contributed to knowledge reduction in order to decrease the effect of the uncertainty or vagueness of data. As a result, the proposed method yields more proper and efficient rule base in compatability and size. The simulation result shows that it gives a good performance in spite of very simple rules and short conditionals.
