Topical Formulations of Water-Soluble Chitin as a Wound Healing Assistant -Evaluation on Open Wounds Using a Rabbit Ear Model-

  • Water-soluble chitin (WSC) was prepared by carefully deacetylating chitins to about $50\%$ of N-acetyl content. Topical formulations based on WSC were prepared and their effects on wound healing were evaluated on a rabbit ear model. Full-thickness, open skin wounds were made on the ears of rabbits and WSC ointments were embedded in the open wounds. The application of WSC ointments significantly accelerated wound healing and wound contraction. The areas of epithelial-ization and granulation tissues in WSC ointment group are remarkably larger than those in control group (no treatment) and in placebo group (treated with ointment-base materials). A large number of grown granulation tissues including dense fibroblast deposition were observed under the thickened epithelium of the wound treated with WSC ointments. The number of inflammatory cells in WSC ointment group was significantly decreased compared with those in control and placebo groups, indicating that WSC would give low stimuli to wounds and prevent excessive scar formation. Neovascularization was the most prominent in WSC ointment group. Wound contraction in WSC ointment group was much larger than those in control and placebo groups. Overall results demonstrate that the topical formulation based on WSC is considered to become an excellent dressing as a wound healing assistant.

Evolutionary Optimized Fuzzy Set-based Polynomial Neural Networks Based on Classified Information Granules

  • In this paper, we introduce a new structure of fuzzy-neural networks Fuzzy Set-based Polynomial Neural Networks (FSPNN). The two underlying design mechanisms of such networks involve genetic optimization and information granulation. The resulting constructs are Fuzzy Polynomial Neural Networks (FPNN) with fuzzy set-based polynomial neurons (FSPNs) regarded as their generic processing elements. First, we introduce a comprehensive design methodology (viz. a genetic optimization using Genetic Algorithms) to determine the optimal structure of the FSPNNs. This methodology hinges on the extended Group Method of Data Handling (GMDH) and fuzzy set-based rules. It concerns FSPNN-related parameters such as the number of input variables, the order of the polynomial, the number of membership functions, and a collection of a specific subset of input variables realized through the mechanism of genetic optimization. Second, the fuzzy rules used in the networks exploit the notion of information granules defined over systems variables and formed through the process of information granulation. This granulation is realized with the aid of the hard C- Means clustering (HCM). The performance of the network is quantified through experimentation in which we use a number of modeling benchmarks already experimented with in the realm of fuzzy or neurofuzzy modeling.

  • The paper concerns the hybrid optimization of fuzzy inference systems that is based on Hierarchical Fair Competition-based Parallel Genetic Algorithms (HFCGA) and information data granulation. The granulation is realized with the aid of the Hard C-means clustering and HFCGA is a kind of multi-populations of Parallel Genetic Algorithms (PGA), and it is used for structure optimization and parameter identification of fuzzy model. It concerns the fuzzy model-related parameters such as the number of input variables to be used, a collection of specific subset of input variables, the number of membership functions, the order of polynomial, and the apexes of the membership function. In the hybrid optimization process, two general optimization mechanisms are explored. Thestructural optimization is realized via HFCGA and HCM method whereas in case of the parametric optimization we proceed with a standard least square method as well as HFCGA method as well. A comparative analysis demonstrates that the proposed algorithm is superior to the conventional methods.

An extract of the root of Lithospermun erythrorhison accelerates wound healing

  • Metabolic disease such as diabetes, which is caused by stress or imbalanced diet, has been increasing. A diabetic tend to suffer from a delay or difficulty of wound healing. The extract of SHIKON (SK), that is the root of Lithospermun erythrorhison, has been reported to have an effect on healing for normal wound, but has never studies for intractable wound so far. Therefore we examined the effect of SK extract on wound healing with healing impaired mouse model. Full-thickness round wounds were created on the backs of db/db mice and applied SK, and we observed neovascularization and collagen synthesis, distribution of apoptotic cells, and vascular endothelial growth factor (VEGF)- positive cells in granulation tissue. After two weeks, a number of capillary vessel and collagen synthesis were increased in SK-treated wounds. Infiltration of VEGF-positive neutrophils was also seen in the wound, besides apoptotic fibroblasts and endothelial cells were appeared in the granulation tissue. After three weeks, the wound closed completely with SK-treated but not in control. These results suggest that SK enhanced neovascularization by VEGF and this kind of apoptosis process makes the scar smooth. In this study, it is obvious that SK also accelerates healing of intractable wound.

The structural optimization is realized via HFCGA and HCM method whereas in case of the parametric optimization we proceed with a standard least square method as well as HFCGA method as well. A comparative analysis demonstrates that the proposed algorithm is superior to the conventional methods. Particularly, in parameter identification, we use the UNDX operator which uses multiple parents and generate offsprings around the geographic center off mass of these parents.

Utilization of aerobic granulation to mitigate membrane fouling in MBRs

  • Membrane bioreactor (MBR) is a compact and efficient wastewater treatment and reclamation technology; but, it is limited by membrane fouling. The control of membrane fouling significantly increases operational and maintenance costs. Bacteria and their byproducts - extracellular polymeric substances (EPS) - are major contributors to membrane fouling in MBRs. A recent attempt at fouling mitigation is the development of aerobic granular sludge membrane bioreactor (AGMBR) through the integration of a novel biotechnology - aerobic granulation - and MBR. This paper provides an overview on the development of AGMBR to mitigate membrane fouling caused by bacteria and EPS. In AGMBR, EPS are used up in granule formation; and, the rigid structure of granules provides a surface for bacteria to attach to rather than the membrane surface. Preliminary research on AGMBR using synthetic wastewater show remarkable membrane fouling reduction compared to conventional MBR, thus improved membrane filtration. Enhanced performance in AGMBR using actual municipal wastewater at pilot-scale has also been reported. Therefore, further research is needed to determine AGMBR optimal operational conditions to enhance granule stability in long-term operations and in full-scale applications.

후두기관협착증에 있어서 mitomycin 국소 도포 : 예비결과 (Preliminary Results of Topical Mitomycin Application in Laryngotracheal Stenosis)

  • Restenosis frequently develops with granulation and overgrowth of scar following surgical treatment for laryngotracheal stenosis. Various methods such as stenting or CO2 laser application have been used to prevent restenosis, but they were seldom unsatisfactory. Mitomycin is an antineoplastic antibiotics derived from Streptomyces caespitosus; it inhibits fibroblast proliferation and acts as an alkylating agent to inhibit DNA synthesis. This study was desinged to evaluate effectiveness and determine indications of usage of topical mitomycin for laryngotracheal stenosis as a nonsurgical means of reducing postoperative granulation and scar tissue formation. Patients and Method : A retrospective study was performed on eight cases of laryngotracheal stenosis with topical mitomycin application. The author analyzed clinical outcomes of operative treatment with topical mitomycin. Patients underwent laryngotracheal reconstruction, endoscopic granulation removal, or bronchoscopic bougienage followed by topical application of mitomycin (0.4 mg/$m{\ell}$, 4minuntes) on the lesion intraoperatively. Result : Overall success rate of decannulation was 38% ($\frac{3}{8}$). Successful decannulation was possible in 75% ($\frac{3}{4}$) of laryngeal stenosis patients, 75% ($\frac{3}{4}$) of children, 60% ($\frac{3}{5}$) of the patients without previous surgery, and 75% ($\frac{3}{4}$) of bronchoscopic bougienage. Conclusion : The topical application of mitomycin in laryngotracheal stenosis was effective in untreated pediatric laryngeal stenosis which underwent bronchoscopic bougienage. Our results show that the topical mitomycin application for laryngotracheal stenosis could be a effective adjuvant treatment.

Superiorly based flap을 이용한 기관절개술 (Superiorly Based Flap Tracheostomy)

  • The superiorly based flap tracheostomy(SBFT) has been advocated as an new technique of tracheostomy to manage a wide variety of causes of upper airway obstruction. This technique has particular applicability in patients who require long term tracheostomy such as in bilateral vocal cord paralysis and severe obstructive sleep apnea. SBFT has numerous advantages such as shortening of the gap between the skin and trachea : construction of a self-sustaining tract ; circumferential mucocutaneous junction to reduce infection, granulation tissue, bleeding, and stenosis of the tract : avoidance of the laryngotracheal damage : easy placement of a tracheostomal stent to promote speech, coughing and swallowing. Most of all, this technique can reduces the suprastomal buckling by the support of the superiorly based tracheal flap, and thus prevents the stenosis of suprastomal airway. The disadvantage of SBFT is more time-consuming procedure than the conventional tracheostomy, A retrospective analysis of 8 patients undergoing SBFT between June, 1994 and March, 1995 in Dankook University Hospital was performed to present the surgical technique and com-plication rates. The average duration of follow up was 11 months. The complications were consisted of a wound infection and a sternal granulation. The other complications including wound dehiscence, tracheitis, pneumonia, tracheal granulation, sternal narrowing and subglottic stenosis were not experienced.

Study on the Granulation Behavior of TiO2-PVA Composite Powders Prepared Via Spray Drying Technique

  • In this study, TiO2-polyvinyl alcohol (PVA) composite granules were prepared via spray drying technique. To investigate the effects of solid content and binder/powder ratio in the slurry on the granulation behavior of TiO2 powders, the feed compositions were designed to vary over a wide range. The morphology, actual densities, and average granule size and size distribution of the TiO2-PVA composite granules were characterized by using scanning electron microscopy technique, a gas pycnometer, and an image analyzing program (Image-J), respectively. The results indicate that solid content and the amount of PVA in the feedstock slurry are the dominant factors determining the granule morphology, size, and size distribution of TiO2-PVA composite. Moreover, it was observed that increasing the solid content and the amount of PVA in the slurry improved the granulation process and reduced the granule defects. For the preparation of spherical TiO2-PVA composite granules with the minimum amount of non-granulated powders, the optimized composition of the feedstock slurry was found to be 35 wt.% solid and 3 wt.% PVA.
