Relationship between Low Back Pain and Health-Related Quality of Life among Some Elderly (노인의 요통과 건강관련 삶의 질과의 관련성)

  • Objectives: This study aimed to identify the relationship among health-related quality of life of the Cheon-nam region elderly with low back pain. Methods: Data were obtained from cross-sectional surveys conducted as a part of the Community Health Survey 2008. The final analysis included data from 7,003 of the 7,070 elderly participants (aged over 65 years), as 67 responses were excluded since they were inaccurate. Data were analyzed with SPSS for Windows (ver. 19.0), using a ${\chi}^2$-test, a t-test, an ANOVA, and multiple liner regression. The significance threshold was set as p<0.05. Results: Factors related to the health-related quality of life of the elderly were low back pain, age, education level, occupation, subjective health status, subjective stress, drinking status, number of chronic diseases, and sleep duration. Further, health-related quality of life was significantly lower in elderly adults with low back pain. Conclusions: In order to improve health-related quality of life of the elderly and the development of the program for the management of low back pain will be needed to determine, it is considered necessary to study more to follow through the various analysis of in the elderly and health-related quality of life.

Adaptation of Isolation Guidelines for Health Care Settings (수용개작방법을 활용한 의료기관의 격리주의지침 개발)

  • Purpose: This purpose of this study was to develop evidence-based practice guideline for isolation in health care settings to prevent transmission of infectious diseases utilizing guideline adaption process. Methods: The process of guideline adaptation was performed according to the Korean hospital nurses association's guideline adaptation manual which consisted of three main phases, 9 modules, and 24 steps. Results: The adapted isolation guideline consisted of introduction, overview of isolation guideline, summary of recommendations, recommendations, references, and appendices. The guideline includes 224 recommendations in 4 sections which are organizational administration, standard precautions, transmission-based precautions, and education/counselling. Conclusion: The adapted isolation guideline is recommended to be disseminated and utilized by nurses and clinicians nationwide to improve the isolation practices for infected or colonized patients with communicable diseases and to decrease the transmission of infections in the healthcare settings.

A Pilot Survey Examining Satisfaction for Integrated Medicine Based on Critical Pathways for Acute Facial Palsy

  • Background: The incidence of facial palsy has been increasing. Many patients consult both Korean and Western physicians; however, no critical pathway (CP) for facial palsy has been established based on integrated medicine including Korean medicine, Western medicine, and complementary and alternative medicine. Thus, we developed and implemented an integrated CP for patients with acute facial palsy and investigated the satisfaction with CP. Methods: Overall, 20 patients who received treatment following the CP and 20 medical staff members involved in their care responded to a questionnaire survey. The questionnaire was developed based on a review of previous studies and focused on the satisfaction with the CP. Results: Patients' satisfaction score with the integrated CP was ≥ 4.4 for all items using a 5-point Likert-type scale. Smooth and cooperative treatment procedures, time-saving practices, and a clear explanation of the integrated treatment plan were satisfactory factors. Additionally, they preferred incorporating specialized facial massage and receiving education on self-exercise or massage techniques as complementary therapies. The medical staff members expressed a high level of satisfaction with the CP; however, the work division and treatment guidelines must be improved. Conclusion: An integrated CP program for acute facial palsy was implemented, and the satisfaction levels of patients and medical staff members were assessed. The results revealed high levels of satisfaction, and several improvements identified will be incorporated into clinical practice going forward.

Attitudes on Medical Market Opening and Factors for Selecting a Foreign Hospital of Korean University Hospital Outpatients (환자들의 의료시장개방에 대한 인식도와 외국병원 선택요인 - S대학교병원 외래환자들을 대상으로 -)

  • Korea is to open its medical markets to foreign hospitals starting in the year 2006 regardless of our will(DDA, Doha Development Agenda). To accurately understand the characteristics of Korean medical users, their detailed and various needs, their attitudes toward the opening of Korean medical markets, and factors affecting these users in choosing foreign medical service providers would be first step needs to be taken by the Korean medical facilities that need to survive and develope through the fiercely competitive era coming with the opening of Korean medical markets to foreign medical service providers and would be very important in hospital management. The subjects of this study were 500 patients randomly selected from the outpatients who visited one of university hospitals in Seoul on the 14th-16th days of April 2003, and conducted a self-completion questionnaire. The answers of 463 respondents among the selected patients(93% of a responding rate)were analyzed through the Excel and statistics programs. The attitudes on the opening of the medical markets were shown in agreement 56.5%(247 persons), disagreement 6.9%(30 persons), and no idea 36.6%(160 persons). In consideration of only the answers as agreement and disagreement exclusive of the answer as no idea, 89.2% of the respondents agreed to the opening of the medical markets while 10.8% objected to the opening. The approval rate was higher with the higher education and income levels. Moreover, The approval rate for the opening of the medical markets was relatively high regardless of the satisfaction in the medical service, and the most important reason of the agreement was the guarantee of the patients(national)option. The main reason of the disagreement was high medical fee(50.5%), and the other reasons showing low rates were outflow of the domestic fund to the foreign countries(13.6%), damage of medical influences on the public(11.4%), lack of competition of the domestic medical industry(9.1%)and so on. As for the factors of selecting the foreign hospitals in the opening of the medical markets, the patients considered the authority(competency)of doctors firstly, and the other principal factors were worldwide fame and reliance, specific explanation of doctors, modernized medical instruments, convenient consultation procedure, etc. The patients agreed to the opening of the medical markets at a high rate regardless of the satisfaction in the medical service, and the most principal reason of the agreement was the guarantee of the patients(national)option for the medical care. Connected with the factors to select the hospitals, the approval reasons for the opening of the medical markets were the authority(competency)of the doctors as the first one, and then fame and tradition, reliance, overall diagnosis and modernized medical instruments, doctors specific explanation, and so on. However, these factors are actually associated with the Quality of the medical care, and consequently the approval reasons for the opening of the medical markets are connected with the security of the medical care. Accordingly, the guarantee of the patients(national)option answered as the main reason of the agreement can be also understood as the awareness of the right to have a variety of options for the security of the medical quality.

A Study on Cognition about 119 Rescue·First Aid Team - Gwangju Area College Student as the Central Figure - (119구조·구급대에 대한 인식도 조사 연구 - 광주지역 보건계열과 비보건계열 대학생을 중심으로 -)

  • The purpose of this study is to provide the basic materials for searching the way of improvement to heighten the emergency medical welfare level by one step further. To achieve this purpose, the subjects of this study were selected 452 college students in Gwangju, using a random sampling method. The statistical analysis methods utilized for analyzing the collected data are frequency analysis, $x^2$ test. The conclusions obtained from these analyses are as following ; 1. In question about necessary optimum number of persons for rescue first aid activity, health and non-health major college students responded by 39.2%, 45.3% respectively that rescue team 15 people, first aid team 3 people is most suitable. But there was no significant difference in major department(p<0.05). 2. In question about security of the public health doctor and the emergency medical technician, all health and non-health major college students are recognizing necessity urgently, but there was no significant difference in major department(p<0.05). 3. In question about 119 rescue first aid team member applying for an examination qualification grant to the department of EMT's graduate, all health and non-health major college students were highest by 52.9%, 52.4% respectively in "necessity" item. But there was no significant difference in major department(p<0.05). 4. Because rescue first aid equipment level appears higher than 41.7% in non-health major college student's case by 54.2% in health major college student's case, health major college students are recognizing that equipment level should be supplemented more but there was no significant difference in major department(p<0.05). 5. In question about equipment supplement, all health and non-health major college students appeared highest by 64.8%, 69.3% in accident type different special equipment. But there was no significant difference in major department(p<0.05). 6. In question about rescue ambulance car size, we could know being thinking that health and non-health major college student each 61.2%, 56.5% is small and narrow that large size of the rescue ambulance amount need. But there was no significant difference in major department(p<0.05). 7. In question about patient's state is worsened, because rescue first aid equipment is inferior, health major college student responded sometimes 55.1%, many 29.5%. very many by 11.5%, while non-health major college student responded 65.8%, 23.1%, 4.0% respectively. There was significant difference in major department(p<0.05). 8. In question about emergency patient must utilize for 119 rescue ambulance car, all health and non-health major college students appeared highest by 38.8%, 41.3% in "not so" item. In question about rescue first aid team's first-aid treatment ability improves more, all health and non-health major college students appeared highest by 58.1% and 58.7% respectively in "improve" item. In question about "119 rescue ambulance car must go more rapidly than now", all health and non-health major college students are recognizing that should be quicker by 58.1%, 60.9% respectively. When called to 119 all health and non-health major college students responded highest by 55.5%, 53.3% respectively that we must receive first-aid treatment direction from a doctor. In question about "119 rescue ambulance car must be made the pay system", all health and non-health major college students responded 74%, 80% respectively in "not so" item. There was significant difference in major department(p<0.05). In conclusions, In oder to provide superior rescue first aid service to people, a public health doctor should be placed in the situation room inside the fire station so that the doctor could instruct the proper emergency treatment suitable for each situation to the rescue first aid team. Also, national education about a first-aid treatment that do to all people is necessarily necessary in emergency delivery system and this should be spread extensively through school education and broadcasting medium and education should be gone side by side, and see that will can save emergency patients' life which is more when these education consists continuously fixed period for public institution of policeman, fire officer etc. specially. And for reinforcement of patient transfer system, public organization must procure special ambulance car so that emergency patient receive first aid treatment while transfer.

A Study on the Contents of Child Rearing in Household Textbooks during the 1920s-30s ($1920{\sim}30$년대 가사 교과서 육아단원의 외형 및 내용 분석)

  • This study analyzed the unit of child rearing in four Household textbooks approved by the Department of Education during the 1920s-30s. These Household textbooks is contained a unit related to child rearing among Household textbooks approved by the Department of Education at that time. The results of the analysis are summarized as follows. First, the structure of the child rearing unit in the Household textbooks were largely composed of 'part, chapter and section' and this structure and contents were similar among all the textbooks. Second. in general, the four textbooks spared a large space for 'childcare,' nursing' and 'new-born babies and infants,' suggesting the importance of these contents. Third, cautions for pregnant women were taught to call attention not only of pregnant women but also of their families to importance of pregnancy. Fourth, the child rearing unit contained many illustrations and pictures and some pictures were shared among the textbooks. Firth, in acquiring medical knowledge related to diseases, the textbooks demanded child-rearing mothers to 'observe' and 'scrutinize' any symptoms appearing in the child. These contents were connected with commercial products but far distant from the realities. The detailed method of 'nursing' also required a lot of time and accuracy impractical in real life.

Preliminary Study of the Educational Effects of the Geriatric Dental Hygiene Curriculum (노인치위생학 교육과정에 따른 교육효과 분석을 위한 예비연구)

  • Background: This study involved a geriatric oral care expert who developed the competencies of students in the Department of Dental Hygiene and conducted preliminary research to develop an effective curriculum. Methods: A questionnaire was conducted in the last week of class targeting students who took courses in geriatric dentistry, geriatric dental hygiene, and practice. In order to confirm the educational effect according to the differences in the geriatric dental hygiene curriculum, the differences in students' achievement of major competencies, awareness of the geriatric dental hygiene process, class satisfaction, and ageism were analyzed. Results: Regarding major competency attainment, 'communication competency' was significantly higher in PBL education that combined theory and practice than that theory-oriented PBL education (p=0.038). For ageism, the tendency to avoid older adults was low in PBL education, which combined theory and practice, and was statistically significant (p=0.040). For class satisfaction, the rate of responding 'very high' for the 'class atmosphere' was significantly high (p=0.025) for PBL education that combined theory and practice. Conclusion: The PBL teaching method can be useful as a geriatric dental hygiene curriculum. However, it would be more effective to create a curriculum so that education in geriatric dental hygiene care practice can be combined with theory rather than a theoretical education alone.

Trend of Socioeconomic Inequality in Participation in Cervical Cancer Screening among Korean Women (자궁경부암 검진 수검률의 불평등 추이)

  • Objectives : While cervical cancer is one of the leading cancers among women worldwide, there are a number of effective early detection tests available. However, the participation rates in cervical cancer screening among Korean women remain low. After the nationwide efforts in 1988 and thereafter to encourage participation in cervical cancer screening, few studies have investigated the effects of socioeconomic inequality on participation in cervical cancer screening. The purpose of this study was to investigate 1) the level of socioeconomic disparities in receiving cervical cancer screening by age group and 2) if there was an improvement in reducing these disparities between 1995 and 2001. Methods : Using data from the Korean National Health Status, Health Behavior and Belief Survey in 1995, the Korean National Health and Nutrition Examination Surveys from 1998 and 2001 (sample sizes of 2,297, 3,738, and 3,283), age-standardized participation rates were calculated according to education level, equivalized household income, and job status. Odds ratios and the relative inequality index (RII) were also calculated after controlling for age. Results : Women with lower education levels were less likely to attend the screening test, and the disparities by education level were most pronounced among women aged 60 years and older. The RIIs among women 60 years and older were 3.64, 4.46, and 8.64 in 1995, 1998, and 2001, respectively. Higher rates of participation were reported among those in the highest income category, which was more notable among the middle aged women (40s and 50s). An inconsistent trend in the rate of participation in cervical cancer screening by occupational level was found. Conclusions : Indicators of socioeconomic position seem to have varying impacts on the inequalities in the rates of participation in cervical cancer screening according to age group. These results demonstrate the need for more aggressive and age-based interventions and policy programs to eliminate the remaining inequalities.

Impact of Registration Program after Hypertensive or Diabetic Patient Detection through Community Partnership on Compliance and Blood Pressure or Blood Sugar Control (고혈압·당뇨병 신규 환자 발견 이후 지역사회 협력을 통한 등록관리가 치료순응도 및 혈압 및 혈당 조절에 미치는 영향)

  • = ABSTRACT = Objectives : Incheon metropolitan city has set a model program of community partnership for hypertensive or diabetic patients detection and follow-up since 2005. This study examines the effects of the model on the compliance and the control of blood pressure or blood sugar level Methods : Telephone-surveys were done for 140 persons selected from 408 patients who were detected newly as hypertensive or diabetic patients at Gangwha-Gun and Seo-Gu in Incheon between January 1st and August 31th in 2006, and finally completed in 110 patients(78.6%). Survey questionnaires included socio-demographic(age, gender, and educational level), health behavioral (smoking, drinking, physical activity, and diet), and therapy-related (registration state, compliance, blood pressure or blood sugar control) variables. Odds ratio and 95% CI were derived from logistic regression model. Results : Registered group exhibited high compliance and well managed blood pressure or blood sugar level. The odds ratio of registration were 5.55(95% CI:1.83~16.89) for compliance and 3.78(95% CI:1.43~9.99) for blood pressure or blood sugar control after adjusting for age, gender, disease, and area Conclusions : It is independently related to compliance and blood pressure or blood sugar control whether the patient is registered or not. To control hypertension or diabetes mellitus in terms of compliance and control state, a community-based registration program through community partnership could be a powerful tool.

A study on Quality Control of Mammography Equipment in the Gwang-Ju (일부지역 유방 촬영 장비의 정도관리 실태조사 연구)

  • As breast cancer shows the highest incidence among women's cancers, the use of mammography is also increasing as a screening test. Mammography should produce high-quality images for accurate diagnosis. For this, it is necessary to manage the performance and image quality of mammography. Thus, in order to investigate quality control, the present study conducted a questionnaire survey of 37 hospitals registered as breast cancer medical examination centers in Gwang-Ju, concerning their quality control of mammography. In the results of surveying the characteristics of apparatus use for mammography, many respondents did not know about the equipment that they were using. Of the hospitals, 19 (49%) were using film, 19 (49%) CR, and 1 DR. In the results of asking how to do quality control, only 38% answered that they inspected according to the manual. In addition, all the surveyed hospitals had specialized agencies do quality control for them. As to the reason for using agencies, 65% mentioned limitations in personnel, time, distance, etc, and 44% mentioned the absence of machines and materials. These results suggest that quality control is being performed perfunctorily, as well as passively and indifferently as it relies on specialized agencies. Accordingly, it is necessary to provide regular education for enhancing people's perception of quality control and to perform quality control adequately in the presence of Radiologist.
