Colchicine Induced Myopathy Presenting Electrophysiologic Myotonia (근전도상 근긴장증이 관찰되는 콜치신에 의한 근병증 1예)

  • Colchicine is widely used in the treatment of gouty arthritis, and chronic use of colchicine can cause vacuolar myopathy. We report colchicine-induced myopathy with electrophysiological myotonia in a patient with gouty arthritis. A 86-year-old man with gouty arthritis presented with progressive proximal weakness and gait disturbance. Electrophysiological finding showed myotonic myopathy and muscle biopsy revealed a vacuolar myopathy. His symptoms soon resolved with the discontinuation of colchicine.

Hologram Based QSAR Analysis of Xanthine Oxidase Inhibitors

  • Xanthine Oxidase is an enzyme, which oxidizes hypoxanthine to xanthine, and xanthine to uric acid. It is widely distributed throughout various organs including the liver, gut, lungs, kidney, heart, brain and plasma. It is involved in gout pathogenesis. Hence, in the present study, Hologram based Quantitative Structure Activity Relationship Study was performed on a series of Xanthine Oxidase antagonist named 2-(indol-5-yl) thiazole derivatives. The best HQSAR model was obtained using Atoms, Bonds, Connection, Hydrogen, Chirality and Donor Acceptor as fragment distinction parameter using hologram length 71 and 4 components with fragment size of minimum 2 and maximum 5. Significant cross-validated correlation coefficient ($q^2$= 0.563) and non cross-validated correlation coefficients ($r^2$= 0.967) were obtained. The model was then used to evaluate the six external test compounds and its $r^2{_{pred}}$ was found to be 0.798. Contribution map show that presence of propyl ring in indole thiazole makes big contributions for improving the biological activities of the compounds. We hope that our HQSAR model and analysis will be helpful for future design of xanthine oxidase antagonists.

Topomer CoMFA Analysis of Xanthine Oxidase inhibitors

  • Xanthine Oxidase is an enzyme, which oxidizes hypoxanthine to xanthine, and xanthine to uric acid. It is widely distributed throughout various organs including the liver, gut, lungs, kidney, heart, brain and plasma. It is involved in gout pathogenesis. Hence, in the present study, topomer based Comparative Molecular Field Analysis (topomer CoMFA) was performed on a series of Xanthine oxidase antagonist named 2-(indol-5-yl) thiazole derivatives. The best topomer CoMFA model was obtained with significant cross-validated correlation coefficient ($q^2$ = 0.572) and non cross-validated correlation coefficients ($r^2$ = 0.937). The model was evaluated with six external test compounds and its $r^2{_{pred}}$ was found to be 0.553. The steric and electrostatic contribution map show that presence of bulky and electropositive group in indole thiazole ring is necessary for improving the biological activities of the compounds. The generated topomer CoMFA model could be helpful for future design of novel and structurally related xanthine oxidase antagonists.

  • A microwave incoherent imaging method for a conducting cyliner by using multi-frequency tiem-harmonic field is presented in this study. In this paper, an incoherent intensity pattern of th econducting cylinder is obtained by averagin gout the multi-frequency intensities of the coherent field such as the time-harmonic field scattered from this cylinder. This phenomenon is hsown numerically in scattering by a conducting circular cylinder illuminated by the time-harmonic plane wave, and is interpreted analytically by the mutual coherence functon defined as a frequency-averaged intensity of the time-harmonic fields in th frequency domain.

Effect of Ginseng Saponins on Xanthine Oxidase Activity after Ethanol Treatment in Mouse Liver (알코올 투여후 마우스 간 크산틴 산화효소 활성에 미치는 인삼의 영향)

  • A dose, 1g/kg of ethanol produced experimental hyperuricemia in mouse. Ginseng saponins were tested for their ability to alter the hepatic xanthine oxidase activity and the blood level of uric acid in the ethanol-treated mouse. Intraperitoneal injection of ginseng saponin 4mg/kg markedly decreased the xanthine oxidase activity in the ethanol-treated mouse liver. It was also observed that ginseng saponin reduced the blood concentration of uric acid in experimentally induced hyperuricemia by alcohol treatment. In vitro, it was found that a low concentration of ginseng saponin in the reaction mixture incresed the hepatic xanthine oxidase activity, while a high concentration inhibited both enzyme preparations of normal and ethanol treated mice. In contrast with the xanthine oxidase, uricase activity was not influenced by ginseng saponin as well as in vivo. These results suggest there is a possibility that ginseng saponin may have some therapeutic effect on gout and other hyperuricemia syndrome.

  • Arthritis may be broadly classified as degenerative - related to defects in cartilage and other joint constituents, often age-associated - or inflammatory disease. Inflammatory arthritis called as rheumatoid arthritis (RA) is a chronic inflammatory arthropathy and characterized by a destructive arthritis. RA encompasses infectious arthritis, arthritis caused by intra-articular deposits of crystalline material (gout), syndromes associated with genetic defects (familial Mediterranean fever), and the immune-mediated inflammatory arthropathy. Degenerative arthritis called as osteoarthritis (OA), which is most frequently occurring, causes degenerative figures of knee, waist and knuckle, and accompanies severe pain around the cartilage. Also, it may cause morning stiffness, gelling effect, tenderness, bone swelling, crepitus, and motion disorders.

  • Urine is a gift of God for the health of human being in proverbs of the Old Testament in Hindu. Urine therapy (UT) is not a mysterious folk remedy, it is doctor's examined prescription based on modern science and UT is recommended strongly with absolute confidence. It is effective and amazing to a number of incurables such as chronic fatigue, lingering, a cold, nose allergy, diabetes, high blood pressure, and gout. Also, UT is applied not only for human beings but also for animals, the latter including amazing effects for pigs and cows. The purpose of this manuscript is to help change the misunderstandings of urine and UT and to help reader realize it is one of the helpful alternative remedies.

  • Spontaneous rupture of the extensor tendon has been reported in association with predisposing inflammatory conditions including rheumatoid arthritis, diabetes, trauma, tophaceous gout, and steroid injection. The authors experienced a case of spontaneous rupture of the extensor digitorum longus tendons caused by an osteophyte of the tarsometatarsal joint in a patient with rheumatoid arthritis. The authors stress that aggressive treatment including surgery could be considered for prevention of spontaneous tendon rupture in a patient with predisposing conditions despite an asymptomatic spur.

  • The King Hyo-Jong is the 17th King in the Chosun dynasty. King Hyo-Jong carried forward the subjugation of the northern areas, but the subjugation of the northern areas was failed for the reason that he was died in 41 years old. After that time, his death is wrapt in a shround of suspicion, such as a rumor of killing by poison. But the cause of his death in direct was the septicemia arised from the boil. Besides, We suppose that he was afflicted with gout and renal failure.
