The Meaning of Good Dying of Chinese Terminally Ill Cancer Patients in Taiwan

  • The purpose of this hermeneutic study was to investigate the meaning of "good dying" of Chinese terminally ill cancer patients in Taiwan; the factors related to this morning; and the strategies cancer patients used to ensure "good dying". Indepth unstructured interviews, prolonged participant observations, and review of clinical records were selected as the methods for data collection. In the four and one-half month period of data collection, the researcher was in the role of a full time clinical nurse specialist who directly took care of the subject patients in 4 hospitals and in patients' homes. The 20 subject were selected purposively according to selection criteria and various demographic backgrounds. Interview transcripts and field notes comprised the data for analysis. The results were composed by 3 constitutive patterns and 12 themes. Achieving inner peace appeared to herald the good dying state. The "good dying" for Chinese terminally ill cancer patients in Taiwan meant peace of body, peace of mind, and peace of thought. The constitutive pattern of peace of body included 4 themes: (1)minimizing the agony of physical symptoms; (2)short period of dying process without lingering death; (3) cleanliness, neatness, and integrity of the body; and (4) mobility. The constitutive pattern of peace of mind included 5 themes: (1) yielding; (2) non-attachment; (3) not to be lonely; (4) settle down all affairs; and (5) being in a preferred environment and enjoying nature. The third constitutive pattern of peace of thought included 3 themes: (1) getting through day by day without thinking; (2) meaningful life; and (3) expectation that the suffering would be ending. Through understanding of the terminally ill cancer patient' needs in their meanings of "good dying", recommendations can be made for humanistic care. The findings of this study have recommendations for care givers daily contact with dying patients and for medical and nursing education.

Profiling of T Cell Receptor β-Chain Complimentary Determining Regions 3 Repertoire in Subarachnoid Hemorrhage Patients Using High-Throughput Sequencing

  • Objective : The adaptive immune response following subarachnoid hemorrhage (SAH) is not well understood. We evaluated and compared the T cell receptor (TCR) immune repertoire of good-grade and poor-grade SAH patients to elucidate the T cell immunology after ictus. Methods : Peripheral blood from six SAH patients was collected at two different times, admission and at the 7-day follow-up. Composition and variation of the TCR β-chain (TCRB) complimentary determining regions (CDR) 3 repertoire was examined using high-throughput sequencing; the analysis was based on sampling time and disease severity (good vs. poor-grade SAH). Results : Clonality at admission and follow-up were 0.059 (0.037-0.038) and 0.027 (0.014-0.082) (median, 25th-75th percentile). Poor-grade SAH (0.025 [0.011-0.038]) was associated with significantly lower clonality than good-grade SAH (0.095 [0.079-0.101]). Poor-grade SAH patients had higher diversity scores than good-grade SAH patients. CDR length was shorter in good-grade SAH vs. poor-grade SAH. Differences in clonotype distribution were more prominent in TCRBV gene segments than TCRBJ segments. TCRBV19-01/TCRBJ02-04 and TCRBV28-01/TCRBJ02-04 were the most increased and the most decreased V-J pairs in the 7-day follow-up compared to admission in good-grade SAH. The most increased and decreased V-J pairs in poor-grade SAH patients were TCRBV28-01/TCRBJ02-06 and TCRBV30-01/TCRBJ02-04, respectively. Conclusion : The TCRB repertoire is dynamic in nature following SAH. TCRB repertoire may facilitate our understanding of adaptive immune response according to SAH severity.

  • Korea has established a code for the economic and cultural industry that covers a wide range of classes unlike the old information era which was limited to the simple knowledge, technology, tools, morals, and customers that had been established in our society. On the other hand, 'culture marketing' is a series of marketing activities in which value-added items are created and shared. The combination of business management and culture seemed pretty awkward considering how rapidly the world was changing, however, the nature of today's culture has changed and the economic nature of culture products has resulted from those changes. In this paper, two authors who expressed the cultural identity in the culture products are studied through a comparison analysis; the author Min Yeongsoon expressed the identity confusion that she experienced in a foreign country and how she overcame homesickness in her work; and the author Kim Ata expressed his confidence in Asia and used his clear and loud voice to express the culture in his work. In addition, today's culture products are affected by the surrounding cultures such as the member's traditional society, environmental culture, and individual's culture. It is a good source for good works of art. In this study, the efforts of seeking the cultural identity, structuring the concept, seeking a new market and possibilities are used to make a consistent process in order to make a series of the systems required for planning and developing culture products.

A Comparative Study on Effective Leadership in Combat and Noncombat Situation (전투 및 비전투 상황에서의 효율적 리더십에 대한 비교 연구)

  • The current problems with the changing nature of the battlefield of the future point up the serious need for more and better research on the nature of effective military leadership. The purpose of this study was to examine effective leadership traits and behaviors of junior officers in combat situation. During times of peace, leader study battles and imagine themselves in all sorts of combat situations while at the same time, they must cope with numerous challenges and fill a variety of roles that they perceive are not battle related. This illustrates one of the fundamental paradoxes of the peacetime environment. Early studies, showing that combat leadership and noncombat leadership needed different talents, produced some clusters of traits which good combat leader were said to possess. Good combat leaders, for example, were described as possessing courage(e.g., bravery, fearlessness, daring, prowess, gallantry, guts, intrepidity, undaunted courage, fighting spirit, aggressive action), personal integrity(e.g., sincerity, flair, calmness, modesty), adaptability(e.g., flexibility, rapidity in action, speedy decision-making, clarity of thought) and so on. Behaviors found to be important in both combat and noncombat situations bore some relation to role requirements common to both situations. Behaviors important in one situation but not the other could be explained in terms of situational differences in role requirements for effective leadership. In order to achieve this purpose, a number of literature reviews were analysed. These results, though obtained in a somewhat rough and ready fashion, were useful not because they pointed to different leaders in war and in peace, but because they showed leaders the different things that were expected of them in different situations. It was also worth knowing how develop combat leadership. While these findings clearly suggest combat and noncombat differences, they do not necessarily confirm the complete study on effective leadership in combat situation. In conclusion, this study would be useful basis for further improvement on effective combat leadership and some further researches were recommended.

  • Today manny contemporaries struggle to live because they disregard essential matters and indulge in materialism. They should not pursue wealth and prosperity and ought to find true meaning of life by studying death where one can reveal the essence of life. Zhuang Zi transcended death. His view of death is magnificent. This dissertation has ruminated on how contemporaries live and established righteous view of life and death on the basis of that. In addition, this study has researched what implications his view of death suggests to us today. There is no study as good as death. There is no great teacher as good as death in life. Studying death is studying a life. We can find life meaning and value through death. The matter of death is prone to be thought that a solution can be got only though existing religions. However, when it comes to Zhuang Zi's death, the matter of death can be resolved, not relying on religions. Zhuang Zi has played a role of our taking death in a positive way and relieving the fear of death. He has taught us that it is well off to adapt to the change of nature, leading to meeting our death in comfort.

  • Confucius made endless thoughts on such themes as enlightening for the internal value of human, how to draw out such value and how to make personal relationship in order to overcome the chaotic Chun-qiu age. The spirit of 'Harmony but not Sameness' stated by Confucius passes us the theoretical meaning for making a good relationship with the others, which is realization of Benevolence. The spirit of 'Harmony but not Sameness' pursues moral, public and golden-mean value. This pursuit is made to look for harmonization and coexistence in personal relation, community based relation and, furthermore, relation with the nature, so that a great man is an entirely-good person who has realized the spirit of 'Harmony but not Sameness'. The spirit of 'Harmony but not Sameness' throws to us many issues to think of for the social unity, 'living together', which is the central theme of today. The objects for which the 'living together' shall be embodied are the minor groups of our society and those who are relatively expelled from the power and benefit including multi-culture based families, north Korean refugees and etc. Such open mind for harmonization and coexistence gives us, furthermore, another important issue to think of also in the matter of environment that includes the ecosystem and the entire nature.

A Survey on Qualitative Analysis of Directional VANET MAC Protocols

  • Since vehicles' trajectories are so complex and dense traffic changes in nature frequently, the VANET (Vehicles Ad-hoc Network), using Omni-directional Antenna, has many channel collisions (or overlapping) on Data Link phrases (MAC layers). It is not easy to keep a good seamless communication status for VANET because of its unpredictable network environment. Among VANET research, Directional Antenna have been proposed as one of the most common systematical solutions to reduce (or to mitigate) this miss-communication problems by narrowing communicational ranges and making use of its customized error-detection process. However, even though Directional Antennas help VANET keep good seamless communication, many VANET researchers have reported that Directional VANET still has miss-communicational problems - this has lead to problems like 'Directional Hidden Terminal Problem', 'Deafness', 'Un-accuracy Lobe Scopes' and 'High Deployment Cost' being reported in various papers. To establish well-organized design assessments for a good Directional VANET MAC protocol to overcome these problems, we rearranged and grouped current Directional VANET' qualitative criteria from several current survey papers using these categories- 'Directional Discovery', 'Directional Forwarding' and 'Directional Handover'. In addition, based on the results of the following analysis, we show the essential design concerns that need to be looked at in order to develop a well-designed Directional-VANET MAC protocol.

Development of a New Copper(II) Ion-selective Poly(vinyl chloride) Membrane Electrode Based on 2-Mercaptobenzoxazole

  • Copper(II) ion-selective PVC membrane electrode based on 2-mercaptobenzoxazole as a new ionophore and o-nitrophenyl octyl ether (o-NPOE) as plasticizer is proposed. This electrode revealed good selectivity for $Cu^{2+}$ over a wide variety of other metal ions. Effects of experimental parameters such as membrane composition, nature and amount of plasticizer, and concentration of internal solution on the potential response of $Cu^{2+}$ sensor were investigated. The electrode exhibits good response for $Cu^{2+}$ in a wide linear range of 5.0 ${\times}$ 10−.6-1.6 ${\times}$ $10^{-2}$ mol/L with a slope of 29.2 ${\pm}$ 2.0 mV/decade. The response time of the sensor is less than 10 s, and the detection limit is 2.0 ${\times}$ $10^{-6}$ mol/L. The electrode response was stable in pH range of 4-6. The lifetime of the electrode was about 2 months. The electrode revealed comparatively good selectivities with respect to many alkali, alkaline earth, and transition metal ions.

  • Ultrasound has been found useful as a therapeutic modality for the reduction of muscular and tendinous spasm. It has also been utilized for pain and other pathologic conditions through the ability of soundwaves to introduce molecules of chemical substances through the skin by a process. Choice of the transmission medium is very important for effective ultrasound treatment in clinical field. The purpose of this study was to analyze the effects of various ultrasound conduction media in regard to ultrasound conductivity and degree of absorption, evaporation and of skin irritation. The media used in this study were Antiphlamine, Sacch lotion, Stereogel, Trastgel, Antiphlamine S lotion, and Mentholatum lotion that have been used in clinical medicine. The study revealed that Antiphlamine was not compatible with a good ultrasound transmitter. Other media excluding Antiphlamine were compatible with a good ultrasound conductor, but they had some drawback with their nature of higher absorption, evaporation and skin irritation. The medium that was prepared by mixing of Antiphlamine with Gel in 1 to 10 ratio was a good ultrasound transmitter and extents of absorption and evaporation and of skin irritation of it were less than the other media.
