A CASE OF ORAL LEIOMYOMA (구강내에 발생한 평활근종)

  • Leiomyoma is a benign smooth muscle tumor that usually arise in the uterus, skin and gastrointestinal system. Only 2 percent are located in the head and neck. Leiomyomas of oral cavity are uncommon as only two cases have been reported in the Korean literature. The common location of oral cavity has been tongue, but other sites include buccal mucosa, lips, palate. mouth floor, and gingiva. Our patient was 30 years old female who complained of swelling and discomfort in the mouth floor. Microscopically this tumor showed bundles of intertwining spindle cells within fibrous connective tissue stroma. The nuclei were generally pale staining and blunt ended. Masson's trichrome stain was positive for muscle, and immunohistochemical study for ${\alpha}$-smooth muscle actin revealed strong positivity. It was treated by surgical excision. We experienced a case of leiomyoma of oral cavity, so we report with literature reviews

THE PROTEIN COMPOSITION OF GINGIVAL CREVICULAR FLUID SAMPLED FROM NORMAL SUBJECTS AND PATIENTS WITH PERIODONTAL DISEASE (정상치은열구액과 치주질환시 치은열구액의 단백질 조성비교에 관한 연구)

  • Gingival crevicular fluid (GCF) is a promising source for markers of destructive periodontal disease activity. This study was undertaken to evaluate the protein composition of GCF in varying stages of the gingival inflammatory response. GCF sampled from 26 people with clinically healthy gingiva and 18 people with periodontitis were examined via sodium dodecyl sulphate polyacrylamide gel electrophoresis(SDS/PAGE). The result were as follows. 1. Total amount of GCF protein of diseased group significantly different from that of normal group. But difference in protein concentration was not that significant. 2. In analyzing GCF with SDS/PAGE, it was suggested that albumin is used as indicator plasma protein leakage because of heavily staining bond of albumin in patients with periodontal disease. 3. In diseased group, overall bonds of protein and bands of high molecular weight protein were heavily stained. It was proved useful information on high molecular plasma protein leakage with increasing vascular permeability due to inflammation.

  • Idiopathic gingival hyperplasia is a rare condition of undetermined etiology. The enlargement is usually associated with the emergence of the teeth into the oral cavity and may regress after extraction. The enlarged gingiva may be primarily attributed to hyperplasia of the subepithelial layer that is relatively avascular and consists of densely arranged collagen bundles and numerous fibroblasts. The recommended time for treatment is after completion of eruption of permanent teeth. But the most important thing is the patient's psychological and esthetic needs. Lately, Schluger has proposed modified gingivectomy procedure with horizontal, internal beveled incision for thinning of the flap resulting in less pain and bleeding after treatment, minimal opportunity of infection. The purpose of this report is to document a case of 8-year-old girl who had registered in Dept. of Pediatric Dentistry of Seoul National University dental hospital for treatment of her gingival hyperplasia and delayed tooth eruption

  • In the past, restoration of implant crown, ready-made abutment produced by implant manufacturer could only be used. Using straight, angled abutment, there was a limit in adaptation multiple implants. Recently, with the development of implant and CAD/CAM technology, CAD/CAM customized abutment use has become possible which is different from the past when restoration was possible with only prefabricated abutment. Not only it makes emergence profile possible which is similar to natural teeth, but also it makes insertion path possible on CAD in multiple implant restorations. However, on anterior teeth which dental esthetics is very important, another restorations which are formed with natural colored gingiva area could be required. Titanium-based zirconia prostheses which have titanium connection and zirconia structure from 1mm above fixture platform are alternative. Therefore, the purpose of this review is to analyze the characteristics, advantages and disadvantages of the abutment which is used in multiple implant restorations, and to choose right abutment when clinical trials.

  • Ameloblastic fibrosarcoma(AFS) is a rare malignant odontogenic tumor presented as painful swelling and intraosseous mass with occasional ulceration. The most frequent site is the mandible body. AFS of the jaw generally occurs in all ages($3{\sim}83$ years old), with the average age of 27.3. AFS was associated with high local recurrence rate of 37% in the areas of gingiva, floor of mouth and neck. Although metastasis is not a special feature of this lesion, 20% have died within 3 months to 19 years, due to locally aggressive tumor growth. This report describes an ameloblastic fibrosarcoma occurring in the mandible of a twenty-five year old male. The tumor was treated by partial mandibulectomy and reconstructed with a fibular flap. The patient has shown no signs of recurrence or complications during 18 months postoperatively. In this study, we report our case with a review of literatures.

  • Arsenic trioxide is one of the 'tooth pulp devitalizing agents' used through the dental history when anaesthesia was not available. But owing to its capacity to kill cells in surrounding tissues, the use of arsenic trioxide in vital pulpotomy has been reduced. Arsenic trioxide can cause necrosis of gingiva, bone which can cause osteomyelitis of the jaws. But some dentists still continue to use arsenic trioxide in their endodontic practices. The purpose of this article is to present arsenic trioxide induced osteomyelitis on maxilla and treatment process.

  • Mucous membrane pemphigoid is uncommom disease in oral cavity and synonymous with cicatricial pemphigoid. This disease is caused by autoimmune reaction that autoantibody reacts antigen located in basement membrane and epithelium is separated from underlying connective tissue. It affects female over sixth decade, commonly. Oral mucosa, especially gingiva is common site but conjunctival, nasal, pharyngeal, laryngeal, esophageal, varginal mucosa and skin are involved. Intraoral findings show Nikolsky sign, irregular erythema, erosion, vesicle, and ulceration at mucous membrane. To differentiate from diseases of positive Nikolsky sign, should perform histologic, immunologic test. Histologic features show subbasilar cleft and direct immunologic features show IgG, C3 deposits at basement membrane in linear pattern. Mucous membrane pemphigoid is incurable disease because symptoms are repetitively improved or worsed for several years. Patiens are commonly managed with topical and systemic steroid. To avoid side effects of prolonged steroid therapy and to maintain immunosupressive effects, combination therapy of azathioprine with steroid is effective. This case reports that mucous membrane pemphigoid is diagnosed based on clinical and histologic features, is treated with topical, systemic steroid and azathioprine therapy.

EFFECTS OF ARTIFICIAL CROWN MARGIN ON GINGIVA AND GINGIVAL SULCUS (금관변연이 치은 및 치은열구에 미치는 영향에 관한 연구)

  • A total of 200 patients, ranging in age from 20 to 60 years, were selected for the study. Each had at least one tooth which was restored with complete cast gold crown and a nonrestored contralateral tooth with no clinical evidence of caries and periodontal disease. The gingival tissues adjacent to the crowned and nonrestored teeth were examined to determine the evaluation of the severity of inflammation and probed to determine individual pocket depth. The findings are listed here. 1. The average sulcus bleeding index of the gingival tissues adjacent to crowned teeth was 1.99. The average sulcus bleeding index of the gingival tissues adjacent to nonrestored teeth was 0.67. 2. The average gingival sulcus depth adjacent to crowned teeth was 2.19mm. The average gingival sulcus depth adjacent to non restored teeth was 1.68mm. 3. No difference could be found between the average gingival sulcus bleeding index and average gingival sulcus depth of male and those of female. 4. The difference between sulcus bleeding index of the gingival tissues adjacent to crowned teeth and sulcus bleeding index of the gingival tissues adjacent to nonrestored teeth increased with increased age of the cast crown.

  • A radical maxillectomy causes a defect of the alveolar bone, gingiva, palate, and orbital floor and causes cosmetical problems and masticatory and phonatory functions. Defect after a radical maxillectomy was reconstructed with skin or dermis graft was introduced, but recently wide resection of the tumor and functional reconstruction with free flap was introduced by several methods. The defect due to radical maxillectomy was reconstructed with scapula, iliac crest, radius. But reconstruction with a fibular osteocutaneous free flap was rarely introduced to defect of radical maxillectomy. The fibular osteocutaneous free flap was firstly introduced by Taylor. The fibular osteocutaneous free flap has several advantages. We experienced the first case of radical maxillectomy and reconstruction with the fibular osteocutaneous free flap, so we reported that case with literatures. The patient has a right maxillary sinus squamous carcinoma (T2N0M0), and performed a radical maxillectomy with right supraomohyoid neck dissection, and reconstruction with fibular osteocutaneous free flap. Donor site morbidity was little, and phonatory and masticatory function were nearly normalized. And cosmetical result was very acceptable.

A STUDY OF IMPRESSION TECHNIQUE USING PUTTY MATERIAL OF PVS IMPRESSION MATERIAL (Putty 인상재를 채용한 PVS 인상채득 기법에 관한 연구)

  • In order to compare the accuracy of impression technique using the addition silicone putty and improved heavy body material, impression were taken for the natural 6 maxillary anterior teeth, and for the metal mast model that has full arch shape with 4 cylindrical abutment teeth. Marginal gingiva was retracted by routine method using retraction cord, and two techniques were tried to compare the length of impression materials in the gingival sulcus. This was aimed to see the effect of viscosity and hydraulic pressure of impression materials for the subgingival impression. Impressions for the full arch-shaped metal master model were taken to compare the linear stability of the different impression materials and their related techniques. The conclusions were as belows : 1. The one-step impression technique showed longer extension of impression material in gingival sulcus than two-step impression technique. 2. High viscosity and hydraulic pressure of impression material were useful to take subgingival impression. 3. There was no statistically significant difference for the two-dimensional accuracy of impression technique, but two-step technique showed better trend than one-step technique.
