감염성 두개강내 동맥류의 수술 및 내과적 치험 2례 - 증 례 보 고 - (Two Cases of Surgical and Medical Treatment of Infectious Intracranial Aneurysms - Case Report -)

  • Object : To determine whether to use surgical or medical therapy in treatment of infectious intracranial aneurysms, we reviewed two recent cases of infectious intracranial aneurysms and others known previous reports of aforementioned cases. Hence, we attempted to compare the validity and effectiveness of surgical and medical treatment. Method : Recently, we treated two cases of ruptured infectious intracranial aneurysms. In former case, the aneurysm was located distal to the middle cerebral artery in a patient with mild mitral regurgitation of the heart. In latter case, the aneurysm was multiple with varying hemorrhage. The hemorrhage was located bilaterally and a moderate mitral regurgitation and infective endocarditis were accompanied in this patient. Result : Due to the large size of the intracranial hematoma, stable medical condition, and easy resectability, we treated the former patient surgically. And, because of successive hemorrhage by multiple aneurysmal rupture, and the risk of heart failure, we treated the latter patient medically with serial follow-up angiography. Both patients are at present in good health. Conclusion : Because of the variability in associated factors, such as the patient's health, the number of lesions, location, anatomy of the aneurysms and the causative organism, each patient's care must be individualized and tailored to the patient's particular clinical situation.

한방치료를 통한 허로환자의 증상개선과 DHEAS 수치개선의 연관성에 관한 증례보고 2례 (Two Cases of Correlation between Improvement of General Weakness Symptoms and DHEAS Level through Korean Medical Treatment)

  • Objectives: This study reports on the efficacy of treating patients with general weakness symptoms with Korean medicine. Methods: Two patients with general weakness symptoms were treated with Korean medical treatment modalities, including acupuncture, moxibustion, and herbal medicines. Blood tests, the visual analogue scale (VAS), and patients' complaints were used to assess the treatment effects. Results and Conclusions: dehydroepiandrosterone sulphate (DHEAS), DHEAS/cortisol index, VAS, and the patients' complaints improved after treatment. Korean medical treatment is effective in improving DHEAS and the DHEAS/cortisol index according to the improvements in the patients' conditions. However, more studies are required to validate its use with other patients.

  • Objectives: This paper aims to report the effects of Korean medical treatment on three patients who underwent thyroid lobectomy for thyroid cancer. Methods: Three patients were hospitalized at hospital to treat pain and general weakness after thyroid lobectomy. The patients took Korean medical treatment including herbal medicine and acupuncture. Numerical Rating Scale (NRS) of symptoms was measured every morning. Results: After the treatment, the pain and general weakness following surgery decreased, and general conditions improved. Conclusions: This cases show that Korean medical treatment is effective for a patients who underwent thyroid lobectomy for thyroid cancer.

  • Haddad 증후군은 Congenital central hypoventilation syndrome과 Hirschsprung's disease가 함께 나타나는 질환으로 수면 시 호흡 저하를 특징으로 하나, 증상이 심할 경우 깨어있을 때에도 호흡 저하가 나타날 수 있다. 따라서 전신 마취 시, 중추성 억제 약물 사용에 주의를 기울여야 하고, 적절한 환기가 이루어지도록 하는 것이 중요하다. 또한 술 후에도 호흡 저하가 일어나지 않도록 주의를 기울여야 한다.

서울대학교치과병원 장애인진료실에서의 치과치료 분석 (A STUDY ON THE DENTAL TREATMENT UNDER OUTPATIENT GENERAL ANESTHESIA)

  • 전신마취하에서의 치과치료는 자발적인 협조가 불가능하거나 전신적인 문제가 있는 경우, 다수의 치료를 한 번에 하고자 하는 경우 등에 행해진다. 이 연구는 서울대학교치과병원 장애인진료실에서 외래 전신마취하에 행해진 환자와 치과치료를 분석함으로써 보다 나은 장애인 진료 시스템을 위한 방안 제시에 도움이 되고자 하였다. 2002년 6월 11일부터 2006년 12월 31일까지 장애인진료실에서 치료 받은 432명의 환자들을 대상으로 조사한 결과 15세 이하의 환자가 50.46%로, 소아 및 청소년 환자가 높은 비율을 차지하고 있었다. 진료과는 보존과, 소아치과, 구강외과, 치주과, 보철과 순으로 나타났으며 치료내용을 보면 보존치료와 발치가 주를 이루고 있었다. 또 2회 이상 전신마취를 시행한 환자가 14.1%나 되었다. 본 연구는 장애인 환자를 위한 보다 포괄적인 치과 치료 및 진료과들 간의 긴밀한 협진 체계를 구축하고 필요한 인력과 설비를 보충함으로써 장애인 환자들에게 보다 보존적 인 치과치료와 편안한 치료 환경을 제공해야 할 필요성을 제시하고 있다.

서울대학교치과병원 소아치과에서 시행된 전신마취 하 치료에 대한 연구 (A Study on Treatment under General Anesthesia in Pediatric Dentistry at Seoul National University Dental Hospital)

  • 의학적 관리가 필요하거나 공포로 인해 치과치료를 기피하는 경우 등 다양한 경우에서 소아환자의 치과치료는 일반적인 행동조절만으로는 불가능한 경우가 많다. 이에 최근 전신마취를 이용한 치과치료에 대한 관심이 증가하는 추세이다. 본 연구는 서울대학교치과병원 소아치과에서 시행된 전신마취 하 치과치료에 대한 분석을 위하여 시행되었다. 2011년 10월부터 2015년 12월까지 서울대학교치과병원 소아치과에서 만 18세 미만의 환자를 대상으로 시행한 전신마취 1378례(1322명)에 대한 자료를 조사하였다. 성별은 남자가 805명(60.9%)로 여성보다 더 많았으며, 연령은 5 - 9세(56.0%)가 가장 많았다. 치료 전 전신상태는 ASA 1등급 67.8%, 2등급 이상이 32.2%이었다. 주된 치료는 치아우식증 치료(51.1%)가 가장 많았으며, 다음으로 외과적 치료(42.2%)가 많았다. 전신마취는 행동조절법 중 하나로 다양한 장점이 있으며, 협조가 부족한 환자에서 유용한 방법이 될 수 있다.

정신지체 환아의 치과치료를 위한 외래마취관리 -증례보고- (Anesthetic Management of a Mentally Retarded Child during Dental Treatment -A case report -)

  • General anesthesia is often required for mentally retarded children undergoing extensive dental treatment. We experienced a case of dental treatment under general anesthesia in a 14-year-old boy with mental retardation. He was treated on an outpatient basis. He was diagnosed of Noonan syndrome and received heart surgery when he was six years old. Induction using thiopental and vecuronium was uneventful and nasotracheal intubation were carried out. General anesthesia was maintained with sevoflurane for 2.5 hours. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful anesthetic management of a mentally retarded child during dental treatment in as an out-patient.

  • Manifestations in tuberous sclerosis such as seizure, mental retardation, end-stage renal disease (ESRD), and heart problems present a number of challenges to the dentist and anesthesiologist. Lack of cooperation in addition to the severe medical condition makes dental treatment more difficult. General anesthesia is often required for mentally and physically handicapped patients undergoing extensive dental treatment. We experienced a case of dental treatment under general anesthesia in a 36-year-old male patient with tuberous sclerosis associated with ESRD and mental retardation. He was treated on an outpatient basis followed by hemodialysis without any complications.

  • The patient who has congenital hypothyroidism and pseudohypoparathyroidism could have mental retardation even though adequate hormone treatment and cannot endure conventional dental treatment. In this case, general anesthesia is selected to administer effective dental treatment. But, there could be symptoms such as anemia, neuropathy, associated pituitary or adrenal hypofunction, cardiac failure even in euthyroid state. And, bradycardia, mental dullness, hypothermia, slow reflexes can appear in case of inadequate thyroid hormone replacement. Especially, macroglosssia, slow drug metabolism, exaggerated responses to anesthetic agents and decreased ventilatory responses could be problem during general anesthesia. The presentation of hypoparathyroidism also varies depending on the chronicity of the result of hypocalcemia. Muscle spasms/tetany, paresthesias, and seizures may occur in an acute onset. Chronic hypocalcaemia causes fatigue, muscle cramps, lethargy, personality changes, and cerebration defects.

Multiple implant therapy with multiple inductions of general anesthesia in non-compliant patients with schizophrenia: A case report

  • The effectiveness of dental implants in patients with disability, who are non-compliant during treatment, is controversial because of their poor oral health. Thus, oral health-care and management in such patients is concerning. Moreover, limited information is available on prognosis after implant placement. Herein, we describe a patient with schizophrenia who underwent dental implantation under multiple inductions of general anesthesia (5 times) and required conservative treatment and tooth extraction for multiple dental caries and retained roots because of inadequate oral health-care. Postoperatively, fracture of the prosthodontics and progression of dental caries were observed, and with 3 additional inductions of general anesthesia, conservative treatment, implant surgery, and prosthesis implantation were conducted. Postoperative 12-month follow-up since the last prosthesis implantation showed successful results. For patients with schizophrenia, multiple implantation can reduce horizontal bone loss and achieve aesthetic results compared to treatment with removable prosthodontics and could serve as an alternative treatment modality.
