A Study on Legislation Background and Application of the General Duty Clause of the Occupational Safety and Health Act in U.S. (미국 산업안전보건법에서 일반의무조항의 제정배경과 운용에 관한 연구)

  • The primary purpose of the general duty clause is to offer an extra measure of protection to employees in the workplace. Most standards implemented under OSHA are targeted at a specific hazard. The general duty clause, however allows inspectors to cite employers for exposing its employees to a recognized hazard that has not been specifically addressed in the regulations. Congress intended the general duty clause to be a limited means of advancing the purposes of the OSHAct. But OSHA has not always regarded the general duty clause as the limited means for protecting the safety and health of employees that Congress intented. OSHA attempted to expand the scope of the general duty clause, at times improperly, to make it a more flexible enforcement tool. OSHA's interpretation of each of the restrictions on the scope of the clause has changed over the years. In recent years the general duty clause has been utilized as a sometimes controversial mechanism for enforcement of safety guidelines that have not yet been specifically addressed by statute or regulation. The most notable example of this was application of the general duty clause to ergonomic hazards.

Protective dental splint for oroendotracheal intubation: experience of 202 cases

  • Background: Dental injury as a result of oroendotracheal intubation during general anesthesia is very common. We report our experiences of using mouthguard to prevent dental injury during intubation based on our protocol. Methods: This retrospective study enrolled patients referred for preanesthetic evaluation, those patients with a history of any of the dental treatments to their anterior teeth listed on our fabrication protocol from January 1, 2009 to June 30, 2010. Results: No cases of dental trauma during oroendotracheal intubation were reported among the 202 patients who used a protective device. 66% of the patients had risk factors for hard tissue damage aged 10-40 years. At the ages of 40-70 years, the incidence of risk group for periodontal damage was higher. Conclusions: Preanesthetic consultation was effective for preventing dental injury, so preanesthetic questionnaire and proper dental consultation would be helpful.

The Analysis of General and Apparel Color Preference and Apparel Possession Color (일반색. 의복색 선호도와 의복색 소유도의 특성 및 차이)

  • This study intended to provide more refined information on color preferences by systematizing the concept of the color preference(general color preference, apparel color preference) and revealing different trends in the color preference and the actual possession of apparels. We collected data from 303 females in the ages between 20 and 40 in Seoul and analyzed by paired t-test. The results were as follows: 1. The analysis on general color preference, apparel color preference, and apparel possession color revealed that the cold colors such as blue and violet marked higher, and the warm colors such as yellow and orange marked lower in general color preference. For apparel color preference, neutral and brown were among the highest and the rest of the colors were in the same trend as general color preference. Apparel possession color showed defined accordance with apparel color preference. In terms of tones, pale tone marked the highest in general color preference and was followed by vivid, dull, and dark tones in decreasing order. For apparel color preference, pastel, dul1, and dark tones were among the highest. 2. Comparison between general color preference and apparel color preference in terms of hue showed that, except for orange, every hue showed disagreement in general color preference and apparel color preference. Neutral and brown hues marked higher in apparel color preference than in general color preference where as a]1 the other hues marked higher in general color preference. Comparison between apparel color preference and apparel possession color in terms of hue revealed that in most hues, apparel possession color was lower than apparel color preference. In terms of tones, vivid and pale tones marked higher in general color preference than in apparel color preference while dull and dark tones marked higher in apparel color preference than in general color preference.

A Study of Nursing Services Quality, General Satisfaction of Medical Services, and Revisiting Intent of Hospital (간호서비스 질, 의료서비스에 대한 전반적인 만족과 병원 재이용 의도에 관한 연구)

  • Purpose: This study was performed to identify the perception gaps of nursing services quality between consumers and providers, and identify the perceived influences of nursing services quality on general satisfaction of medical services and revisiting intent of hospital by consumers. Method: The Joo's questionnaire founded on SERVQUAL was modified and distributed to 300 patients and nurses at 6 general hospitals in 5 provincial cities, Korea. Result: It is identified that the perception gaps between consumer and providers exist, and consumers perceive nursing services quality has a influences on general satisfaction of medical services and revisiting intent of hospital. And it is also identified that consumers also perceive general satisfaction of medical services has a powerful influences on revisiting intent of hospital. Conclusion: It is concluded that the marketing strategies being based on the perception gaps between consumers and providers should be specifically planned and applied to improve the perceived nursing services quality by consumers. And it is needed to accurately identify the impact attributes and factors of nursing services quality on general satisfaction of medical services and revisiting intent of hospital that consumers perceive.

  • Primary diffuse large B-cell lymphoma of the seminal vesicle is an extremely rare disorder, with only two cases reported in the English literature. Here, we present imaging findings of a case of primary diffuse large B-cell lymphoma of the seminal vesicle. On transrectal ultrasonography, the mass presented as a 3.0-cm-sized heterogeneous, hypoechoic lesion in the right seminal vesicle. Computed tomography (CT) revealed a mass with rim-like enhancement in the right seminal vesicle. On magnetic resonance imaging (MRI), the tumor showed iso-signal intensity on T1-weighted images and heterogeneously intermediate-high signal intensity on T2-weighted images. The tumor showed rim-like and progressive enhancement with non-enhancing portion on dynamic scanning. Diffusion restriction was observed in the mass. On fluorodeoxyglucose positron emission tomography-computed tomography (FDG PET/CT) imaging, a high standardized uptake value (maxSUV, 23.5) by the tumor was noted exclusively in the right seminal vesicle.

Although saikosaponins isolated from the Bupleuri Radix had metabolic and anti-inflammatory action, it's toxicity was not screened completely. A Study on the Recognition on Home Care Services by Nurses, Doctors and General Employees (일 종합병원의 가정간호사업에 대한 의사, 간호사 및 일반직원의 인식 조사)

  • Purpose: The purpose of this study was to provide the data for improving home care services through investigating the recognition of home care services by nurses, doctors and general employees. Method: The data were collected from 231 nurses, 103 doctors and 157 general employees who were working at a general hospital in Seoul from Sept 11 to Sept 24, 2006 and analyzed using descriptive statistics and ANOVA. Results: The recognition on necessity of home care services was lower in doctors(M=2.73, SD=.39) and general employees (M=2.83, SD=.31) than nurses(M=3.13, SD=.39). In terms of the recognition on content of home care services, the affirmative percentages of most items were lower in doctors and general employee than nurses. The recognition on effects of home care services was also lower in doctors(M=2.90, SD=.32) and general employees(M=2.99, SD=.31) than nurses(M=3.26, SD=.35). Conclusion: This study indicates that the continuous educations and advertisements on home care services are necessary to facilitate home care services in general hospitals.

Analysis of Medical Resources according to number of beds in Korean General Hospitals (국내 종합병원의 병상규모에 따른 의료자원 분석)

  • Purpose: This study analyzed the status of general hospitals as an expanded concept of medical resources including medical staff and equipment. The purpose of this study is to provide a basic for the feasibility study of the scale and establishment of facility guidelines at the planning stage of general hospitals. Methods: The subjects of this study were limited to general hospitals. The status of medical resources was based on the data of the Health Insurance Review and Assessment Service. The number of beds, doctors, nursing grades and major medical equipment were surveyed in 335 general hospitals. Results: 1) The characteristic of general hospitals varies depending on the number of inpatient beds. To be concrete, there were differences in the number of medical staffs and equipments in general hospitals based on 300 500 800 1,000 beds. 2) As the number of hospital beds increases, the number of medical staff increases more than medical equipment and facilities. Medical equipment and facilities remain constant, even when the number of beds increases. On the other hand, the number of medical staff increased about 1.5 times in each level. Implications: Architectural plans for medical staff should be considered differently depending on the number of beds. In particular, architectural planning and facility guidelines should be applied differently based on 300 and 500 beds.
