Allergic reactions to local anesthetic mepivacaine in dental procedures: a case report

  • Local anesthetics are an essential part of pain control during dental treatment. Despite its effectiveness and safety, patients should constantly be aware of potential adverse effects, including allergic reactions. Allergic reactions to amide-type local anesthetics (LAs), such as lidocaine and mepivacaine, are rare compared to those to ester-type LAs. Herein, we report the case of a patient with a history of allergy to lidocaine and mepivacaine, with symptoms of itching, diffuse erythema of the wrists and hands, dizziness, and pectoralgia. This case report emphasizes the importance of collecting medical and dental histories of patients is necessary, and how an allergy test in the allergy and clinical immunology department helps select safe LAs for patients.

Airway anesthesia with lidocaine for general anesthesia without using neuromuscular blocking agents in a patient with a history of anaphylaxis to rocuronium: a case report

  • We experienced a case of induction of general anesthesia without using neuromuscular blocking agents (NMBAs) in a 40-year-old woman with a history of anaphylaxis immediately after the administration of anesthetics lidocaine, propofol, and rocuronium to perform endoscopic sinus surgery 2 years before. The skin test showed a positive reaction to rocuronium and cis-atracurium. We induced general anesthesia without using NMBAs after inducing airway anesthesia with lidocaine (transtracheal injection and superior laryngeal nerve block). Deep general anesthesia was maintained with end-tidal 4 vol% sevoflurane. Hypotension was treated with phenylephrine infusion. The operation condition was excellent, and patient recovered without complications after surgery. Airway anesthesia with local anesthetics may be helpful when we cannot use NMBAs for any reason, including hypersensitivity to NMBA and surgery that needs neuromuscular monitoring.

  • 전신마취제의 잉여 몰부피는 Halobacteriun Halobium에서 추출분리된 Purple membrane과 Red membrane에서 vesicle의 현탁액에서 $25^{\circ}C$ 잉여부피 dilatometer로 측정되었다. 우리의 연구에 의해 전신마취제인 Propofol은 잉여몰부피와 상관 관계가 있는 것으로 밝혔다. Halobacteriun Halobium에서 비중 차이에 의해 추출 분리된 Purple membrane과 Red membrane의 잉여몰 부피는 vesicle의 아미노산과 지질의 영향이다. 이들은 잉여 몰부피 dilatometer로 잉여 몰 부피를 측정되었으며, 흡광세기는 280 nm 330 nm 에서 연구되었다. Purple membrane과 Red membrane이 Propofol에 의한 입자크기 분석, 상대적 혼탁도가 기계적 특성을 위해 연구되었다. 확산 교환에 의한 점성 중합체 용액의 입자 크기 분석은 측정에 의한 중요한 단계이다. Propofol이 vesicle에서 재구성된 시료들 중에서, 특히 PM+RM+Propofol의 잉여 몰 부피가 PM, RM의 잉여 몰 부피보다 가장 크게 측정되었다.

Enflurane으로 인하여 발생한 마취 중 급성과민증 (Intraoperative Anaphylatic Reaction to Enflurane -A Case Report -)

  • Anaphylactic reactions to anesthetic drugs could potentially produce life-threatening immune-mediated crisis. Most published reports are associated with neuromuscular blockers and anaphylactic reactions to inhalation anesthetics are rare. A 25-year-old male patient with no significant medical history and no previous abnormal drug reaction was scheduled for orthognathic surgery under general anesthesia. After uneventful anesthetic induction and nasotracheal intubation, generalized urticaria and erythema were detected during the maintenance period with $O_2-N_2O$-enflurane. No severe changes of vital signs and no ventilation problem were accompanied. The operation was cancelled and the cutaneous lesions were faded away during the recovery with 100% $O_2$. The skin-prick and intradermal tests showed that he was hypersensitive to all halogenated inhalation anesthetics including enflurane and not to intravenous anesthetics and neuromuscular blockers. The re-operation was safely carried out under intravenous anesthesia with propofol-fentanyl-vecuronium. We report this case of intraoperative anaphylactic reaction to enflurane with literature review.

Does the general public have concerns with dental anesthetics?

  • Background: Consumers and patients in the last two decades have increasingly turned to various internet search engines including Google for information. Google Trends records searches done using the Google search engine. Google Trends is free and provides data on search terms and related queries. One recent study found a large public interest in "dental anesthesia". In this paper, we further explore this interest in "dental anesthesia" and assess if any patterns emerge. Methods: In this study, Google Trends and the search term "dental pain" was used to record the consumer's interest over a five-year period. Additionally, using the search term "Dental anesthesia," a top ten related query list was generated. Queries are grouped into two sections, a "top" category and a "rising" category. We then added additional search term such as: wisdom tooth anesthesia, wisdom tooth general anesthesia, dental anesthetics, local anesthetic, dental numbing, anesthesia dentist, and dental pain. From the related queries generated from each search term, repeated themes were grouped together and ranked according to the total sum of their relative search frequency (RSF) values. Results: Over the five-year time period, Google Trends data show that there was a 1.5% increase in the search term "dental pain". Results of the related queries for dental anesthesia show that there seems to be a large public interest in how long local anesthetics last (Total RSF = 231) - even more so than potential side effects or toxicities (Total RSF = 83). Conclusion: Based on these results it is recommended that clinicians clearly advice their patients on how long local anesthetics last to better manage patient expectations.

외래소수술시 적용하는 치과국소마취 술식들 (Dental Local Anesthetic Techniques for Minor Operations at Outpatient Clinic)

  • The most widely used method of pain control in dental practice is to block the pathway of painful impulses by local anesthesia. For a maximum effect of pain control at outpatient minor operation, regional anesthesia including nerve block must be performed. This issue includes various dental local anesthetic techniques on mandible, maxilla and considerations for selection of dental local anesthetics according to patient's age, duration of operation. Additionally, current conscious sedation technique combined with local anesthesia will be introduced for control of patient's anxiousness. Based on patient's general condition, severity of operation, local anesthesia sometimes must be changed into general anesthesia to avoid failure of dental practice. The exact choice of anesthetic techniques for minor operations at dental outpatient clinic must not be underestimated for complete success of pain control.

Hyaluronidase를 사용한 경추간공 경막외 차단의 효과 (The Effect of Transforaminal Epidural Block with Hyaluronidase and Triamcinolone)

  • Background: Epidural steroid injections benefit certain patients with radicular pain, and often have only a limited duration. We compared the efficacy of hyaluronidase and triamcinolone and triamcinolone alone in patients with lumbar herniated disc disease treated with transforaminal epidural block. Methods: Forty patients who had undergone a transforaminal epidural injection were retrospectively reviewed. The T group received triamcinolone and local anesthetics; whereas, the HT group received hyaluronidase, triamcinolone and local anesthetics. We evaluated the improvement as being good, moderate, mild or no improvement, and in those where the improvement was good or moderate, also evaluated the duration of pain relief. Data were collected from the medical records of patients or via phone calls, which were analyzed using Student t- and chi-squared tests. A value of P < 0.05 was considered significant. Results: There were no significant differences in the degree of pain improvement or duration of pain relief between the two groups. Conclusions: A hyaluronidase and triamcinolone injection during transforaminal epidural block has on benefit with respect to the degree of pain improvement or its duration compared to a triamcinolone only injection.

경막외마취에 따른 생체신호 변화의 분석 (Analysis of Biosignal Variations caused by Epidural Anesthesia)

  • This study was performed to extract and analyze the biosignals to find the relationship between the level of anesthesia and the variations of physiological parameters during epidural anesthesia. Seven male and twenty female patients(ages from 45 to 70 years old) were participated for the experiment, and ECGs, PPGs, SKTs, SCRs were obtained during anesthesia. As results, the HF/LF ratios of HRV were decreased after the injection anesthetics. For skin temperatures, values measured from the palm was reduced and the temperatures from four channels, measured from armpit through the right side of the body, were increased. SCRs were decreased for all channels after the injection of anesthetics. However the heart rate and PPGs showed no significant changes. It was concluded that the injection of anesthetics result the changes in biosignals, and it could be explained by the degree of the sympathetic and/or parasympathetic nerve activities. Results of this study could provide the valuable information for the estimation of level for the spinal and general anesthesia, and could be extended to the development of a system which could quantify the level of anesthesia.

Dental anesthesia for patients with allergic reactions to lidocaine: two case reports

  • Lidocaine, a local anesthetic commonly used in dental treatments, is capable of causing allergies or adverse effects similar to allergic reactions. However, the frequency of such occurrences in actual clinical settings is very rare, and even clinical tests on patients with known allergies to local anesthetics may often show negative results. When adverse effects, such as allergy to lidocaine, are involved, patients can be treated by testing other local anesthetics and choosing a local anesthetic without any adverse effects, or by performing dental treatment under general anesthesia in cases in which no local anesthetic without adverse effects is available. Along with a literature review, the authors of the present study report on two cases of patients who tested positive on allergy skin tests for lidocaine and bupivacaine and subsequently underwent successful dental treatments with either general anesthesia or a different local anesthetic.

광순환 에너지를 갖는 Halobacterium Halobium에서 마취제의 혼합에 따른 Membrane의 부피 변화 (Volume Change of Membrane Mixed with Various Anesthetics in Halobacterium Halobium with Photo-Cyclic Energy)

  • The excess volumes of mixing of benzyl alcohol, ethyl alcohol, halothane, and procaine in vesicle and suspensions of several lipids have been determined at $25^{\circ}C$ using a excess volume dilatometer. The potency of general anesthetics has long been known to be correlated with lipid solubility. Denaturations of the bacteriorhodopsin, which is a sole membrane protein in the purple membrane of Halobacteriun Halobium, were studied by UV/Vis absorption changes. The excess volumes of mixing of benzyl alcohol and procaine in egg lecithin were all found to be negative and this result was confirmed as Miller's supposition.
