A Case of Chronic Relapsing Cystitis (만성 재발성 방광염 환자의 한방 치험 1례)

  • Purpose : The purpose of this report is to record the effects of acupuncture and moxibustion treatment on Chronic Relapsing Cystitis. Methods : This is a case report of female patient suffering from urinary disturbance based on the bladder stimulated. 28 years old woman diagnosed with chronic relapsing cystitis was enrolled in this study. She was treated with acupunctures at CV6(Qihai, 氣海), CV4(Guanyuan, 關元) and CV2(Qugu, 曲骨) for two weeks, seven times. During the treatments, we checked changes of symptoms. Results : After acupuncture treatments, clinical symptoms of chronic relapsing cystitis and general conditions were improved. Conclusion : This clinical case shows that acupuncture is effective in treatment of the chronic relapsing cystitis. More clinical data and studies are required for the treatment of chronic relapsing cystitis.

Effect of Placental Extract on Immobilization of Shoulder Joint in a Complex Regional Pain Syndrome Patient (복합국소통증증후군 환자의 견관절 운동제한에 미치는 자하거 가수분해물 약침요법의 효과)

  • Complex regional pain syndrome type 1(CRPS 1) is a neuropathic pain disorder that accompanies severe pain and motor deficit as well as changes in the skin in the extremities. The pathophysiology of CRPS 1 is still not exactly elucidated. However, the general consensus of involvement of inflammatory mediators in the development of CRPS 1 is amply made. On the basis that placental extract successfully inhibited the production of inflammatory cytokines and mediators in several experimental models, we have tried a long-term weekly injection of placental extract into acupuncture points to a CRPS 1 patient suffering pain and immobilization of shoulder joint. The results say that placental extract effectively resolved pain, restored skin color and improved immobilization of shoulder joint in the CRPS 1 patient.

Factors Related to Dropout in Clinical Trials of Acupuncture and Moxibustion (침구 임상시험에서의 중도탈락 관련요인)

  • Objective: This study aimed at providing preliminary data useful in reducing participant dropout and improving the quality of clinical trials, by analyzing the factors related to dropout. Methods: The data came from 15 acupuncture and/or moxibustion clinical trials (n=638; August 2005 to December 2009). Logistic regression analysis was used to reveal factors influencing participant dropout. Results: Gender, age, treatment method (intervention), treatment frequency, availability of follow-up, and presence of compensation treatment for the control group were factors influencing participant dropout. Conclusion: Subsequent studies of large-scale acupuncture and moxibustion clinical trials should address dropout factors that consider the character of each clinical trial, or general characters like participants' gender, age, occupation, and diverse diseases.

  • Object : Liver cirrhosis is a disease of the liver in which normal cells are replaced by scar tissue. The purpose of this case is to report the improvement of liver cirrhosis after herb-med, acupuncture & moxibustion complex therapy. Methods : We provided herb-med, acupuncture & moxibustion complex therapy to a patient who suffered from ascites and fatigue. We examined LFT & Child-Pugh class to evaluate the effectiveness of oriental treatment. Result and Conclusion : We observed that herb-mod. acupuncture & moxibustion complex therapy decreased symptoms of liver cirrhosis and improved general condition of a patient who suffered from ascites and fatigue. In the LFT result, Child-Pugh class was improved.

A Case Report of Chronic Insomnia Diagnosed Blood Stasis (어혈로 변증된 불면증 환자의 치험례)

  • Objectives: Insomnia is a common disease in the general population. The purpose of this paper was to suggest that insomnia subtype of blood stasis can be effectively treated with Danggwisu-san and acupuncture. Methods: This is a clinical case report of a patient with chronic insomnia that was diagnosed with blood stasis subtype. I treated him with Danggwisu-san (Dangguixu-san) two to three times daily and with acupuncture and moxibustion two to three times weekly. Results: After short-term treatment, the patient was gradually recovering from deteriorating chronic insomnia and multiple accompanying symptoms such as severe constipation, spontaneous bleeding and edema, darkened face, depressive moods, etc. Conclusions: Results revealed a possibility that chronic insomnia subtype of blood stasis may be successfully treated with Danggwisu-san and acupuncture and moxibustion.

Study on the Composition of Acupoints of Eight Constitution Acupuncture (8체질침의 혈위구성에 관한 연구)

  • As eight constitution acupuncture began to be applied in clinics, the acupuncture method and the method of differentiating constitution became widely known. However, no official principles of composing acupuncture prescriptions have been established so far. Therefore, this study examined unique and novel interpretations among a variety of theories on the composition of acupoints of eight constitution medicine. The basic principles of the general formula of unit prescription discussed in the study are that the compatibility and incompatibility of the five elements is acknowledged not in one way but in interactive way, and that when an energy shown in the compatibility and incompatibility chart of five elements is tonified, its two neighboring competible energies are tonified due to the effects of compatibility. On the other hand, two other energies which are incompatible with the energy and exist on the opposite side of it are sedated due to the effect of incompatibility. In contrast, when an energy is sedated, its two neighboring competible energies are sedated while two other energies in incompatible with it are tonified. In addition, eight constitution acupuncture could be explained as a variant of Saam acupuncture. It is unfortunate that there exist no established theory but hypotheses with regard to the composition of acupoints of eight constitution acupuncture. It is expected that young scholars will produce better results if they continue researching eight constitution acupuncture.

  • Objectives : The Hwabyung Research Center of The Korean Society Of Oriental Neuropsychiatry has attempted to develop the 'Clinical Guidelines for Hwabyung'. Methods : The Hwabyung Research Center constructed a committee of experts and advisory group. Relevant literature was collected and evaluated in order to find out effective Pharmacotherapy (Herbal medicine), acupuncture and moxibustion, and cupping of Hwabyung. Results : We found some evidences that proved the effectiveness of Pharmacotherapy (Herbal medicine), acupuncture and moxibustion, and cupping in the treatment of Hwabyung. 1) We recommend Pharmacotherapy (Herbal medicine), acupuncture and moxibustion, cupping, psychotherapy and management, other therapeutic approaches and education for Hwabyung. 2) Pharmacotherapy (Herbal medicine) should be determined according to the pattern identification of Korean medicine. Further, the prescription should be cost-effective, safe and have no side effects. Also, the interaction between Herbal and Western drugs should be considered carefully. 3) There are general acupuncture, scalp acupuncture, Pharmacopuncture, Sa-am acupuncture as well as acupuncture treatments for Hwabyung. 4) Moxibustion and cupping are useful, relieving various somatic symptoms of Hwabyung. Conclusions : Pharmacotherapy (Herbal medicine), Acupuncture and Moxibustion, Cupping for Hwabyung were studied. We hope that the 'Clinical Guidelines for the Treatment of Hwabyung' are helpful for oriental medical doctors and patients.

Acupuncture Therapy and Herbal Medicine Accelerating Temporal Space Abscess after Tooth Extraction: A Case Report

  • Numerous oral and maxillofacial surgeons have found facial space infections after tooth extraction. Most of these infections can be managed easily, but some of them could be life-threatening. Among the facial infections, temporal space infections are rare. Most temporal space infections could be observed as secondary to maxillary third molar infections, maxillary sinusitis, and maxillary sinus fractures. Note, however, that there are insufficient studies on temporal space abscess due to mandibular second molars, especially with acupuncture. A 74-year-old female came to our hospital with severe trismus and facial swelling on the right temporal, buccal, posterior auricular, and cervical regions. The patient had undergone extraction of tooth #47 secondary to dental caries by a general dentist about a month ago. After the dental procedure, the patient had been treated with acupuncture therapy around the right temporomandibular joint area at the oriental medicine clinic. We performed emergency incision and drainage under general anesthesia and started antibiotic treatment with IV ampicillin/sulbactam 3 g every 24 hours and vancomycin 1 g every 24 hours for 5 days. The patient's symptoms subsided and ultimately disappeared. Temporal space abscess after mandibular molar extraction is quite rare. In this case, the spreading mechanism against gravity is considered to be acupuncture therapy.

  • Objective : To broaden understanding about acupuncture and moxibustion therapy on AIDS and to promote base studies and clinical trials Materials and Methods : Analysis was given to more than 30 literatures including acupuncture and moxibustion therapy on AIDS-related sites explored by internet search engine named NAVER from Nov., 2000 to Feb. 20th, 2001 Results : 1. Acupuncture and moxibustion played great role as a complementary therapy in enabling AIDS patients to keep their antiretroviral therapy by enhancing immune system, ameliorating AIDS-related symptoms and side effect of antiretroviral drug 2. Acupuncture and moxibustion therapy had a broad spectrum indication from systemic or local signs of AIDS patients to signs of antiretroviral drug-related side effect 3. Contraindication of acupuncture and moxibustion therapy against AIDS patients include abstraction and moxibustion on the skin lesion, because of their easy exposure to inflammation 4. AIDS patients were regarded as the state of KI-HE(氣虛), EUM-HE(陰虛), YEOL-DOK(熱毒) in general 5. BO-KI(補氣), BO-HYUL(補血), BO-EUM(補陰), CHEONG-YEOL-HAE-DOK(淸熱解毒) were shown as a principle of acupuncture and moxibustion therapy for AIDS patients 6. Principle of selecting acupoints for AIDS patients had characteristics of enhancing immune system, detoxicating detrimental agents and relieving each AIDS related symptom appropriately 7. Acupuncture on 合谷(HAPKOK, LI4), 內關(NAE-GWAN, P6), 足三里(CHOK-SAMNI, S36) were applied to the early stage of AIDS in order to enhance immune system. Acupuncture on 血海(HYOLHAE, SP10), 三陰交(SAMUMGYO, SP6), (KOHWANG, B43) were applied to the intermediate stage of AIDS so as to enhance immune system and eliminate YEOL-DOK(熱毒) in blood. Moxibustion on 湧泉(YONGCHON, K1), 足三里(CHOK-SAMNI, S36) were applied to the late stage owing to enhance immune system more. Conclusion : The efficacy of acupuncture and moxibustion therapy on AIDS has been acknowledged to the world, moreover, it is proved to be significant as a complementary therapy on AIDS patients. Thus, more control group studies of the efficacy of acupuncture and moxibustion therapy on AIDS and clinical trials are considered to be necessary.

An Analysis of Emotional and Cognitive Factors on Acupuncture (침에 대한 정서와 인지요소 분석)

  • Objectives : Placebo phenomena have been considered as a confounding factor of clinical trial. Expectancy and belief of acupuncture have not been evaluated quantitatively. The present study was performed to analyze the emotional and cognitive factor .of acupuncture and investigate whether the expectancy of acupuncture treatment is associated with the cognition of acupuncture. Methods : The expectancy and the perception of bodily sensation (PBS) of 22 participants were assessed using self-reported questionnaire. The subjects used the self assessment manikin (SAM) to rate each of the standard affective image of the international affective picture system (lAPS) and other acupuncture-related image. Based on the degree of expectancy, the high expectant (HE) and the low expectant (LE) group were classified. The thermal and pressure pain threshold was objectively evaluated using radiant-heat device and algometer. The degree of expected pain of acupuncture and the actual pain of painful stimulation was subjectively evaluated using facial pain scales (FPS). Results : Using SAlVI analysis, we identified the negative correlation between hedonic valence and arousal dimension on acupuncture-related visual cue. The degree of the PBS and general pain threshold did not show any significant difference between the HE and the LE group. The HE group rated the acupuncture images as more pleasant, more arousing, than the LE group. In addition, we also found that the higher expectancy marked the lower FPS of the expected pain of acupuncture, but not of the actual pain of painful stimulation. Conclusions : Our preliminary study identified the psychological dimensions of acupuncture-related visual cue. These findings indicate that the expectancy of acupuncture could affect the cognition of acupuncture.
