Objectives The purpose of this review is to analyze the trends of Korean medicine treatment for trigger finger. Methods Clinical papers using Korean medicine to treat trigger finger were searched in five online databases-Koreanstudies Information Service System, Oriental Medicine Advanced Searching Integrated System, ScienceON, Research Information Sharing Service, and PubMed. Nine studies were selected, and we analyzed their characteristics according to the author, year, number/gender/age of patients, duration of disease, duration of treatment, intervention method, measurement methods, and results. Results Seven case report studies, one prospective observational study, and one randomized controlled trials (RCTs) were selected in online databases, and there were no review studies. The most frequently used therapies were pharmacopuncture and acupuncture treatment. visual analogue scale, Quinnell's classification of triggering, was frequently used for measurement methods. All nine studies had therapeutic effects. Conclusions We reviewed studies of Korean medicine treatments for trigger finger, in this study. However, there are limitations that seven of the nine selected papers were case papers, and the number of papers was small. This paper suggests that a higher level of research and more studies on Korean medicine treatments of trigger finger need to be conducted.
Purpose: We aimed to analyze the incidence and prevalence of urolithiasis in Korea over the last decade using the National Health Insurance (NHI) sample cohort data. Materials and Methods: From January 2002 to December 2013, we enrolled sample cohort data from the NHI. Patients diagnosed with international classification of diseases code N20 or N13.2 were included. The incidence and prevalence rate was counted from the same period and patients previously diagnosed with urolithiasis were excluded. We compared the incidence and prevalence of urolithiasis by region, age, and sex, and identified the changes. Results: Total 1,111,828 subjects were included. Of these subjects, 36,857 had urolithiasis. The male-to-female ratio was 1.57:1, and total incidence rate was 3.27 per 1,000 person-years (1,000p-yrs). The annual incidence was lowest in 2013 (3,138 patients) and highest in 2005 (3,751 patients). Incidence rate by diagnostic code was highest in ureter stone only (2.49 per 1,000p-yrs) and was lowest in kidney and ureter stone both (0.17 per 1,000p-yrs). Prevalence gradually increased from 3,172 in 2002 and 5,758 in 2013. Jeollanam-do had the highest incidence rate of 3.70 persons per 1,000p-yrs, and Jeju had the lowest rate of 2.84 persons per 1,000p-yrs. In gender analysis, Daegu had the highest incidence (4.56) in males, Jeollanam-do had the highest incidence (3.20) in females. Conclusions: Annual incidence remained stable, whereas prevalence gradually increased. The incidence in male was 1.57 times higher than female, and the peak incidence age was 45-49 years, with the highest incidence occurring in Jeollanam-do and the lowest in Jeju.
The purpose of this study was to evaluate the prevalence of thyroid nodules and malignant findings of thyroid nodules in 1,954 patients (654 males and 1,300 females) aged 20 years or older who underwent thyroid ultrasound from January 2018 to December 2018. Examination of the thyroid gland was performed, and fine needle aspiration cytology was performed on the thyroid nodule. As a result, 108 (16.5%) out of 654 males and 368 (28.3%) out of 1,300 females showed higher prevalence than males. The prevalence of single nodules and multiple nodules in gender and age groups was significantly higher for women and for ages (male p=.001, female p=.001). There was a significant difference in males in the nodule size (p=.001) and no significant difference in females (p=.069). Fine - needle aspiration cytology of 476 patients with nodules was diagnosed as malignant in 46 patients (9.6%). Based on pathologic results, 383 benign and 93 malignant groups were analyzed. Ultrasonographic findings were as follows single nodule (p=.000), solid(p=.004), hypoechoic (p=.000), ill-defined peripheral boundary (p=.000), and calcification (p=.000), respectively. In the diagnosis of thyroid nodule, primary ultrasonographic findings through morphological classification of the nodules may reduce indiscriminate fine needle aspiration cytology in benign and malignant nodules.
Objectives The purpose of this study is to investigate present status of cooperation through Korean medicine and Western medicine collaboration process. Methods We have established Korean medicine and Western medicine collaboration process to examine the status of cooperation. The medical records were investigated retrospectively, the general characteristics of gender, age of cooperative patients, classification of disease, frequency of medical treatment and type of insurance were analyzed. Results The Korean medicine and Western medicine collaboration process conducted in 4 stages, the convenience of patient movements is considered. A total of 245 people received cooperative medical treatment, 156 were out-patient department (OPD) patients and 89 were hospitalized patients, both group have more women than men. When classified as disease, OPD patients were most diagnosed with Sprain and strain of lumbar spine, while hospitalized patients were most diagnosed with lumbar and other intervertebral disc disorders with radiculopathy. A total of 72.7% patients were received cooperative medical treatment at once. In a survey of cooperative satisfaction, 68.5% of all medical staff responded positively to the treatment effect, and 68.6% said the need for cooperation was necessary. Conclusions As a result of this study, we were able to see the present status of cooperation, and through this, we found an improvement in the continuation of the cooperation. Based on the present study, It is hoped that a cooperative process will emerge that can improve the problems shown in this study.

Background and Purpose: Previous studies have reported conflicting results about the prevalence of seizures in Alzheimer's disease (AD). There are few epidemiological studies on this topic in Asia. Thus, the objective of this study was to examine demographic and clinical characteristics as well as incidence for seizures in AD patients compared to non-AD patients in a prospective, longitudinal, community-based cohort with a long follow-up. Methods: Data were collected from National Health Insurance Service-National Elderly Cohort (NHIS-elderly) Database to define patients with AD from 2004-2006 using Korean Classification Diseases codes G30 and F00. We performed a 1:5 case-control propensity score matching based on age, sex, and household income. We conducted Cox proportional hazards regression analysis to estimate the risk of epilepsy in AD patients. Results: In the cohort study, patients with AD had higher risk for epilepsy than those without AD, with hazard ratio of 2.773 (95% confidence interval [CI], 2.515-3.057). This study also showed that male gender and comorbidities such as hypertension, hyperlipidemia, diabetes, and chronic kidney disease increased the risk of developing epilepsy. Patients with AD had 1.527 (95% CI, 1.375-1.695) times higher mortality rate than those in the control group. Conclusions: AD patients have significantly higher risk of developing epilepsy than non-AD patients.
Objectives : The purpose of this study was to investigate the differences in forensic demographic characteristics, interpersonal problems and personality disorder between early and late onset criminal patients with schizophrenia. Methods : The participants included 187 inpatients with schizophrenia who had committed crimes. They filled out the Korean Inventory of Interpersonal Problem Personality Disorder Scales and Psychopathic Personality Inventory-Revised. They were divided into early onset group and late onset group according to onset age of schizophrenia at 26 years old(under 26 versus 26 and older) and forensic demographic characteristics, interpersonal problem and personality disorders including psychopathy were compared between two groups. Results : There were no differences in gender, education years and family history between the two groups. Early onset group was associated with lower age, earlier onset of age and earlier age at the time of the crime. Duration from onset to diagnosis was not different between the two groups. Duration from onset to crime and after diagnosis to crime was shorter in late onset group. There were no differences between the two groups in the interpersonal problems, personality disorder and psychopathic personality evaluation. Conclusion : These results suggested that there may be forensic demographic differences related to crime between early and later onset schizophrenia. Psychiatrists should consider the age at onset of schizophrenia when assessing the risk of violence in patients with schizophrenia. In the future. it will be needed other study of age classification such as admixture analysis.
Objectives : Allergic rhinitis(AR) is a common chronic disease that accounts for 10-40% of the world's population. This study aims to analyze the status of claims, prevalence, and medical utilization of allergic rhinitis patients using representative patients sample data. Methods : This study used the National Patients Sample(HIRA-NPS) of the Health Insurance Review and Assessment Service for 7 years(2010-2016). And we defined AR patients as all statements including J30 or a subcategory of J30 as the main disease, using the Korean Standard Classification of Diseases(KCD-7). The trend of AR patients by year was divided into Western medicine(WM) and Korean medicine(KM), and analyzed by subgroup analysis such as inpatient/outpatient, gender, age, insurance type, and care institution. Results : Patients with AR were mainly claimed for first sub diagnosis in WM and major diagnosis in KM, and the number of claims increased about 1.3 times and 1.4 times compared to 2010 in WM and KM, respectively. In addition, the total annual medical expenses in 2016 increased 1.3 times and 1.7 times compared to 2010, respectively. Conclusions : Both WM and KM are showing a steadily increasing trend in medical use due to allergic rhinitis. Further research is needed by considering genetic and environmental factors and individual characteristics, and linking with additional data.

Diabetes Mellitus (DM) is one of common chronic diseases leading to severe health complications that may cause death. The disease influences individuals, community, and the government due to the continuous monitoring, lifelong commitment, and the cost of treatment. The World Health Organization (WHO) considers Saudi Arabia as one of the top 10 countries in diabetes prevalence across the world. Since most of the medical services are provided by the government, the cost of the treatment in terms of hospitals and clinical visits and lab tests represents a real burden due to the large scale of the disease. The ability to predict the diabetic status of a patient without the laboratory tests by performing screening based on some personal features can lessen the health and economic burden caused by diabetes alone. The goal of this paper is to investigate the prediction of diabetic and prediabetic patients by considering factors other than the laboratory tests, as required by physicians in general. With the data obtained from local hospitals, medical records were processed to obtain a dataset that classified patients into three classes: diabetic, prediabetic, and non-diabetic. After applying three machine learning algorithms, we established good performance for accuracy, precision, and recall of the models on the dataset. Further analysis was performed on the data to identify important non-laboratory variables related to the patients for diabetes classification. The importance of five variables (gender, physical activity level, hypertension, BMI, and age) from the person's basic health data were investigated to find their contribution to the state of a patient being diabetic, prediabetic or normal. Our analysis presented great agreement with the risk factors of diabetes and prediabetes stated by the American Diabetes Association (ADA) and other health institutions worldwide. We conclude that by performing class-specific analysis of the disease, important factors specific to Saudi population can be identified, whose management can result in controlling the disease. We also provide some recommendations learnt from this research.
Background: In Korea, the length of stay and medical expenses incurred by medical aid patients are increasing at a rate faster than the national health insurance. Therefore, there is a need to create a management strategy for each type of hospitalization to manage the length of stay of medical aid patients. Methods: The study used data from the 2019 National Health Insurance Claims. We analyzed the factors that affect the length of stay for 186,576 medical aid patients who were hospitalized for more than 31 days, with a focus on the type of hospitalization in long-term care hospitals. Results: The study found a significant correlation between gender, age, medical aid type, chronic disease ratio, long-term care hospital patient classification, and hospitalization type variables as factors that affect the length of hospital stay. The analysis of the differences in the length of stay for each type of hospitalization showed that the average length of stay is 291.4 days for type 1, 192.9 days for type 2, and 157.0 days for type 3, and that the difference is significant (p<0.0001). When type 3 was 0, type 1 significantly increased by 99.4 days, and type 2 by 36.6 days (p<0.0001). Conclusion: A model that can comprehensively view factors, such as provider factors and institutional factors, needs to be designed. In addition, to reduce long stays for medical aid patients, a mechanism to establish an early discharge plan should be prepared and concerns about underutilization should be simultaneously addressed.
Purpose: The purpose of this study was to analysis pattern of nonconforming farmers who is one of the factors of unconformity in residual pesticides. Methods: Pattern analysis of nonconforming farmers were analyzed through convergence of safety data and farmer's DB data. Exploratory data analysis and association rule analysis were used for extracting factors related to unconformity. Results: The results of this study are as follows; regarding the exploratory data analysis, it was found that factors of farmers influencing unconformity in residual pesticides by total 9 factors; sampling time, gender, age, cultivation region, farming career, agricultural start form, type of agriculture, cultivation area, classification of agricultural products. Regarding the association rule analysis, non-conformity association rules were found over the past three years. There was a difference in the pattern of nonconforming farmers depending on the cultivation period. Conclusion: Exploratory data analysis and association rule analysis will be useful tools to establish more efficient and economical safety management plan for agricultural products.
