  • The antiulcer effects of newly synthesized prostaglandin derivatives were investigated in various experimental ulcer models and on gastric secretion in rats. HK-3 and HK-4, PG $E_2$derivatives, prevented the formation of acute gastric ulcer induced by ethanol or aspirin in pylorus-ligated rats. The ulcer formation was moderately inhibited by HK-1 and HK-2, PG $F_{2{\alpha}}$ derivatives, and aggravated by SK-1, SK-2 and SK-3, PG $F_{2{\alpha}}$ derivatives. HK-3 and HK-4 reduced the volume, acid output and pepsin output of gastric juice in pylorus-ligated rats. The gastric perfusion with physiologic saline(pH 6.0) showed relatively constant acid secretion and indomethacin increased the acid secretion. The acid secretion was markedly decreased by PG $E_2$but PG $F_{2{\alpha}}$ caused little change. Prostaglandin derivatives, especially HK-3 arid HK-4, significantly inhibited the acid secretion induced by indomethacin. The results show that, PG $E_2$ derivatives, HK-3 and HK-4, inhibit acid secretion and also have protective effects on gastric ulceration induced by ethanol or aspirin.

Primary Gastric Malignant Melanoma Mimicking Adenocarcinoma

  • We report a case of primary gastric malignant melanoma that was diagnosed after curative resection but initially misdiagnosed as adenocarcinoma. A 68-year-old woman was referred to our department for surgery for gastric adenocarcinoma presenting as a polypoid lesion with central ulceration located in the upper body of the stomach. The preoperative diagnosis was confirmed by endoscopic biopsy. We performed laparoscopic total gastrectomy, and the final pathologic evaluation led to the diagnosis of primary gastric malignant melanoma without a primary lesion detected in the body. To the best of our knowledge, primary gastric malignant melanoma is extremely rare, and this is the first case reported in our country. According to the literature, it has aggressive biologic activity compared with adenocarcinoma, and curative resection is the only promising treatment strategy. In our case, the patient received an early diagnosis and underwent curative gastrectomy with radical lymphadenectomy, and no recurrence was noted for about two years.

?Effects of Insamyangwee-Tang on Functions of Stomach and Small Intestine in Experimental Animals (인삼양위탕(人蔘養胃湯)이 위장관(胃腸管)에 미치는 영향(影響))

  • Although the Insamyangwee-Tang has been widely used in clinical purposes in the oriental medicine and its clinical efficiency is documented for the cases of gastritis, gastric ulcer and enteritis but the experimental study on these has not been undertaken. So,To investigate the clinical efficiency, of Insamyangwee-Tang and validate its oriental medical theory, these experiments were undertaken, by being compared with animal experiment. ?The following results were obtained. ?1. By effect of Insamyangwee-Tang on isolated ileum from mice, rats, rabbits and Guinea pigs, the motility of ileums was inhibited remarkably. ?2. By effect of Insamyangwee-Tang on motility of the small intestine in rabbits, the motility was inhibited remarkably too. ?3. Insamyangwee-Tang inhibited charcoal transport functions in the small intestine of mice. ?4. By effect of Insamyangwee-Tang on isolated duodenum from rats, the motility of duodenum was inhibited remarkably.?5. Gastric juice and pepsin secretion was decreased, anti-ulceration effect was recognized.

The Effects of Strychni Ignatii Semen on Toxicity Levels and Gastrointestinal Tract (보두(寶豆)의 수치법(修治法)에 따른 독성(毒性) 및 소화기계(消化器系)에 미치는 영향(影響))

  • Objective : These experiments were designed to investigate the effects of Strychni Ignatii Semen on the gastrointestinal system and declining of toxicity. About experiments of acute toxicity, I investigated the quantity of Strychni $(C_{21}H_{22}N_2O_2)\;and\;LD_{50}$. In order to study the effects on gastrointestinal tract, I investigated the changes of gastric juice, discharging level of pepsin, inhibiting effects of ulceration, and transporting of intestine. Methods : Sample I : No making Strychni Ignatii Semen Sample II : Depositing for three days in water and then dry it. Sample III : Depositing for one hour in sesame oil and bum it. The results were as follows : 1. The average values of Strychnine decreased in Sample III. 2. The levels of $LD_{50}$ increased in Sample Ⅲ by about 70%. 3. In Sample III, inhibiting effects of ulceration and discharging level of pepsin were great. 4. In the level of gastric juice decreased in Sample I.II.III. 5. The transporting ability of large intestine elevated in Sample I.II.III. According to the results, making Strychni Ignatii Semen, especially Sample 3, toxicity decreased and has good effects on the gastrointestinal system.

The Effect of Level of Wheat Inclusion in Diets for Growing and Finishing Pigs on Performance, Nutrient Digestibility and Gastric Ulceration

  • Four experimental diets were formulated to contain 700 g/kg cereal with decreasing levels of wheat:barley inclusion. Diet 1 contained 700 g/kg wheat, diet 2 contained 600 g/kg wheat and 100 g/kg barley, diet 3 contained 500 g/kg wheat and 200 g/kg barley and diet 4 contained 400 g/kg wheat and 300 g/kg barley. The diets were offered to pigs on three trials to investigate effects on the performance of individually (n = 72) and group housed (n = 480) pigs and on nutrient digestibility in pigs housed in metabolizm crates (n = 24). Performance was assessed from 10 wks of age until slaughter and carcass characteristics were measured. For the group performance study, one pig from each pen (in total 24) at 10, 15 wks and at finish were slaughtered to ascertain scores for stomach ulceration, stomach weights and intestinal length. Level of wheat inclusion did not significantly (p>0.05) affect liveweight gain (LWG) or feed conversion ratio (FCR). Feed intake was lowest (p<0.05) for individually housed pigs offered diets containing 700 g/kg wheat during the 10-15 wk period, which indicated that individually housed pigs attempted to eat to a constant energy intake. There was little evidence of stomach ulceration across treatments and increasing wheat inclusion had no detrimental effect. Higher levels of wheat inclusion tended to increase backfat depth at the $P_2$ position which could lead to increasing grading penalties in a commercial situation although more research is required in this area. Increasing level of wheat inclusion increased digestible energy (DE) content but the lack of effect on FCR and killing out percentage indicated that utilization of energy from barley and wheat was similar. Digestibility coefficients increased linearly with increasing wheat content, which can be attributed to the lower level of fibre and higher level of starch in wheat compared with barley.

Quercetin, A Bioflavonoid, Protects Against Oxidative Stress-related Gastric Mucosal Damage in Rats

  • Quercetin and its sugar conjugates are the most abundantly distributed bioflavonoids and represent the largest proportion of flavonols in the plant kingdom. The present study was undertaken to demonstrate the effect of quercetin on the role of reactive oxygen species (ROS) in the development of gastric ulcers in rats. Administration of quercetin in doses of 50, 100 and $200\;mg\;kg^{-1}$ twice daily for 5 days, showed dose dependent significant protection against ethanol (EtOH), aspirin (ASP), cold-restraint stress (CRS) and pylorus ligation (PL) -induced gastric ulcer models and the results were comparable with those elicited by sucralfate. The thiobarbituric acid reactive substances in the stomach mucosa, an index of lipid peroxidation and regulation of plasma corticosterone were significantly increased in CRS-induced gastric ulceration. The queroetin $(100\;mg\;kg^{-1})$ and reduced glutathione effectively inhibited gastric lesions induced by CRS with a significant decrease in the lipid peroxidation and plasma corticosterone. These results indicate that quercetin a bioflavonoid exerts its antiulcer effect in light of free radical scavenging and plasma corticosterone in cold restraint stress ulcers.

The Effects of Dietary Proteins on Curative Effect, Protein Metabolism and Nitrogen Balance of Rats with Gastric Ulcer Induce by Restraint and Water Immersion Stress (침수 속박 스트레스에 의한 위궤양 흰쥐에서 식이 단백질 종류가 궤양 치유, 단백질 대사 및 질소평형에 미치는 영향)

  • This study was designed to determine the effects of protein sources on the curation of gastric ulceration, protein metabolism, and nitrogen balance in rats with gastric ulcer induce by restraint and water immersion stress. After the rats were fed 10% casein diet for 3 weeks, four groups of the rats were forced in 5$\times$5$\times$15cm plexiglass cage. The restraint and water immersion stress was carried at 20$\pm$2$^{\circ}C$ for 8-hour. The other one group(control group) was not exposed to stress. After stress 4 kinds of different diets containing 20% protein were given for 5 days. The protein sources were casein, whey protein, soy protein, gluten. The control group was fed to 10% casein diet. The results were as follows ; the weights of rats were not different among the diet groups During the experiment period follows ; the weights of rats were not different among the diet groups during the experiment period (for 5 days). The ulcer index of rats fed 10% gluten and soy protein diet was significantly higher than those of casein and whey protein diet groups(p<0.05). The level of serum albumin was not significantly different among diet groups. But hematocrit and the level of $\alpha$-amino-N, BU and UUN of plant protein diet groups were higher than animal diet groups, the urinary hydroxyproline of soy protein group was the highest and the whey protein was the lowest. The digestibility and BV of nitrogen of gluten diet group were significantly higher than those of casein and whey protein diet groups(p<0.05). The animal proteins had more curative effects of ulcer than plant animals. The results of this study provide useful information concerning diet therapy for the patients with gastrointestinal diseases and the field of enteral diet materials.

A Case of Gastric Glomus Tumor (위 사구종양 1예)

  • Gastric glomus tumor is a rare mesenchymal tumor that originates from modified smooth muscle cells of the glomus body. Glomus tumors are commonly observed in peripheral soft tissue, such as dennis or subungal region, but rarely in the gastrointestinal tract. A 39-year-old woman was admitted due to epigastric soreness. Upper gastrointestinal endoscopy revealed a subepithelial mass measuring 3.5 cm with central ulceration at the lesser curvature-posterior wall of the antrum. Characteristically, contrast enhanced abdominal computed tomography scan demonstrated high enhancement of the submucosal mass up to the same level of the abdominal aorta in the arterial phase; this enhancement persisted to delayed phase. Due to the risk of bleeding and malignancy, wedge resection of the submucosal tumor was performed. Histologic findings were compatible with a glomus tumor.

  • Hyangsayangwee-Tang, a combined preparation of crude drugs, which has been used for stomach and intestinal disorder, was examined for anti-spasmodic, anti-ulcerative and anti-emetive effects. Spontaneous motility of isolated ileum was strongly suppressed and inhibitory effects against contraction of isolated ileum induced by acetylcholine and barium chloride were shown in mice. And, contraction of isolated guinea-pig ileum by histamine was inhibited. In rats fundus preparations, Hyangsayangwee-Tang elicited strong relaxation and had antagonist effects against the spasm induced by acetylcholine and barium chloride. A significant inhibitory effect on the intestinal propulsion of barium sulfate in mice was shown. In pylorus-ligated rats, Hyangsangwee-Tang inhibited gastric secretion and showed a strong anti-peptic activity. Protective effects against gastric ulceration induced by pyloric ligation, water-immersion stress, histamine and aspirin were significantly recognized in mice and rats. Hyangsayangwee-Tang decreased cupric sulfate-induced vomitting in frogs.

Comparative Transcriptome Analysis of Caryophyllene-Treated Helicobacter pylori

  • Helicobacter pylori (H. pylori) establishes long-term infections associated with severe gastric diseases such as peptic ulceration and gastric cancer. Exposure to an antibacterial agent can help regulate the expression levels of its pathogenic genes. In this study, we analyzed the transcriptional changes in H. pylori genes induced by β-caryophyllene. We used next-generation sequencing (NGS) to analyze RNA expression changes, and reverse transcription-polymerase chain reaction (RT-PCR) was performed as required to verify the results. The NGS results showed that 30 out of 1,632 genes were expressed differentially by β-caryophyllene treatment. Eleven genes associated with DNA replication, virulence factors, and T4SS components were significantly downregulated. RT-PCR confirmed that treatment reduced the expression levels of 11 genes. RT-PCR showed the reduced expression of 11 genes (dnaE, dnaN, holB, gyrA, cagA, vacA, secA, flgE, virB2, virB4, and virB8) following β-caryophyllene treatment. These results suggest that β-caryophyllene can modulate various H. pylori pathogenic determinants and be a potential therapeutic agent for H. pylori infection.
