Lumbar Intraspinal Extradural Ganglion Cysts

  • The lumbar intraspinal epidural ganglion cyst has been a rare cause of the low back pain or leg pain. Ganglion cysts and synovial cysts compose the juxtafacet cysts. Extensive studies have been performed about the synovial cysts, however, very little has been known about the ganglion cyst. Current report is about two ganglion cysts associated with implicative findings in young male patients. We discuss about the underlying pathology of the ganglion cyst based on intraoperative evidences, associated disc herniation at the same location or severe degeneration of the ligament flavum that the cyst originated from in young patients.

  • Spontaneous subcutaneous cysts were detected from three dogs, being a 4-year-old male shih tzu, an 11-month-old female britany spaniel and a 9-month-old male mongrel. All the cysts were occurred around the joints (one elbow's and two shoulderes', respectively). After surgical removal, the lesions did not recur for 6 months follow-up. Neither cyst communicated with the joint cavity. Grossly, the subcutaneous ovoid cysts had moderate to abundant mucinous fluid. Histologically, the cyst wall consisted of inner myxomatous and outer immature connective tissue. Some parts of the cyst wall had various stages of myxoid metaplasia of collagen tissue leading to new cyst formation. The true lining cells of the inner cyst wall were not observed in all the cysts. From these results, these cases were diagnosed as subcutaneous ganglion cysts. This is the first report of canine subcutaneous ganglion cysts in Korea.

  • Ganglion cysts of the cruciate ligaments of the knee are rare and their pathogenesis is uncertain. A few cases of the cyst of the posterior cruciate ligament were reported in the literature. We have arthroscopically treated sucessfully the ganglion cysts of the posterior cruciate ligament through the anteromedial portal. We reported three cases of ganglion cyst of the posterior cruciate ligament with brief of literatures.

Pudendal nerve entrapment syndrome caused by ganglion cysts along the pudendal nerve

  • Pudendal nerve entrapment (PNE) syndrome refers to the condition in which the pudendal nerve is entrapped or compressed. Reported cases of PNE associated with ganglion cysts are rare. Deep gluteal syndrome (DGS) is defined as compression of the sciatic or pudendal nerve due to a non-discogenic pelvic lesion. We report a case of PNE caused by compression from ganglion cysts and treated with steroid injection; we discuss this case in the context of DGS. A 77-year-old woman presented with a 3-month history of tingling and burning sensations in the left buttock and perineal area. Ultrasonography showed ganglion cystic lesions at the subgluteal space. Magnetic resonance imaging revealed cystic lesions along the pudendal nerve from below the piriformis to the Alcock's canal and a full-thickness tear of the proximal hamstring tendon. Aspiration of the cysts did not yield any material. We then injected steroid into the cysts, which resolved her symptoms. Steroid injection into a ganglion cyst should be considered as a treatment option for PNE caused by ganglion cysts.

  • 슬관절 후방십자인대의 결절종은 드물게 발생하며, 이에 대한 보고도 적다. 특별한 외상의 병력 없이 발생한 2례의 후방십자인대 결절종을 발견하고, 관절경을 이용하여 생검 및 제거를 하였기에 그 경험을 보고하는 바이다.

퍼지 소속 함수를 이용한 초음파 영상에서 결절종 추출 (Extracting Ganglion Cysts from Ultrasound Image with Fuzzy Membership Function)

  • 본 논문에서는 초음파 영상에서 결절종을 추출하는 방법을 제안한다. 제안된 방법은 초음파 영상에서 퍼지 스트레칭 기법을 적용하여 명암 대비를 증가시킨다. 퍼지 스트레칭 기법이 적용된 초음파 영상에서 퍼지 이진화 기법을 적용한다. 퍼지 이진화 기법이 적용된 영상에서 잡음 영역을 제거하기 위해 8방향 윤곽선 추적 기법을 적용한후, 결절종 후보 영역을 추출한다. 추출된 결절종 후보 영역에서 침식과 팽창 기법을 적용한 후, labelling 기법을 적용하여 결절종이 형태학적으로 타원 형태를 가지는 정보를 이용하여 결절종 영역을 추출한다. 제안된 방법을 결절종 초음파 영상을 대상으로 실험한 결과, 결절종 초음파 영상에서 결절종 영역이 비교적 정확히 추출되는 것을 확인할 수 있었다.

요추부 후종인대에서 발생한 결절종 (Spinal Ganglion Cyst of Lumbar Posterior Longitudinal Ligament)

  • 결절종은 말초 관절이나 인대에서 비교적 흔히 발견되나 척추 내 결절종은 드물게 보고되고 있다. 저자들은 5예의 요추후종인대에서 발생한 결절종을 경험하고 이들의 임상적, 방사선학적 특징, 수술소견 등에 대하여 고찰하였다. 저자들의 예에서 요추 내 결절종은 요통 및 하지통증을 유발하여 임상적으로 요추간반탈출증과 유사한 소견을 보였다. 요추 MRI상 진단이 가능하였고 5예 모두에서 특징적으로 요추디스크의 퇴행성 변화와 연관되어 있었으며 결절종이 요추디스크와 바로 인접된 부위에 위치하였다. 본 소견으로 저자들은 요추후종인대에서 발생한 결절종은 디스크의 퇴행성 변화와 밀접한 관계가 있다고 추론하였다. 수술은 5예 모두에서 완전절제가 가능하였고 전 예에서 수술 후 증상호전을 보였다.

Scolopendra Pharmacopuncture Combined with Electroacupuncture for the Treatment of Ganglion Cysts: A Retrospective Study

  • Objectives: Ganglion cysts require a sustainable treatment that suppresses their frequent recurrence. This study aimed to explore the clinical effects of Scolopendra pharmacopuncture (SP) and electroacupuncture on ganglion cysts. Methods: We retrospectively reviewed the patient records and follow-up reports for 20 patients with wrist ganglion cysts who received SP and electroacupuncture from April 2016 to March 2017. The cyst diameter, recurrence, visual analog scale (VAS) scores for pain, the Korean version of the disabilities of arm, shoulder, and hand (K-DASH) score, and the Korean version of the patient-rated wrist evaluation (K-PRWE) score before and after treatment were noted. Results: After treatment, the cyst diameter decreased significantly from 13.61 ± 6.41 mm to 5.15 ± 6.18 mm (p < 0.001), and VAS score for pain decreased from 1.31 ± 1.77 to 0.41 ± 0.33 (p = 0.021). Further, the K-DASH score decreased significantly from 8.97 ± 12.66 to 2.21 ± 7.39 (p = 0.016), and score for the function subscale of K-PRWE decreased from 11.37 ± 4.48 to 9.1 ± 3.67 (p = 0.046). No recurrences were reported from the followed-up patients. Any complication related to SP or electroacupuncture was not observed, except mild rash, itching, and swelling at the injection site in four patients. Conclusion: Combination of SP and electroacupuncture may be effective in treating ganglion cysts; further prospective studies with large population are needed to clarify the effect of SP and electroacupuncture.

Ganglion Cyst of the Posterior Longitudinal Ligament Causing Lumbar Radiculopathy

  • Degenerated conditions such as herniated disc or spinal stenosis are common etiologies of lumbar radiculopathy. Less common etiologies include spinal extradural cyst such as synovial cysts and ganglion cysts. Ganglion cyst of the posterior longitudinal ligament (PLL) of the spine is a rare entity that can result in classical sciatica. Posterior longitudinal ligament cyst has no continuity with the facet joint and has no epithelial lining. Two young male patients presented with unilateral sciatica and were found to have intraspinal cystic lesions causing lumbar radiculopathy. Magnetic resonance imaging demonstrated rounded, cystic lesions (i.e., hypointense on T1-but hyperintense on T2-weighted images) adjacent to minimally dehydrated, nonherniated disc spaces in both cases. These patients underwent posterior decompression and cysts were excised, and their sciatic symptoms were completely resolved. Histological examination showed typical features of ganglion cysts in these cases.

Bilateral Thoracic Ganglion Cyst : A Rare Case Report

  • Ganglion cysts usually arise from the tissues around the facet joints. It is usually associated with degenerative cahanges in facet joints. Bilateral thoracic ganglion cysts are very rare and there is no previous case that located in bilateral intervertebral foramen compressing the L1 nerve root associated with severe radiculopathy. We report a 53 years old woman who presented with bilateral groin pain and severe numbness. Magnetic resonance imaging revealed bilateral cystic mass in the intervertebral foramen between 12th thoracal and 1st lumbar vertebrae. The cystic lesions were removed after bilateral exposure of Th12-L1 foramens. The result of hystopathology confirmed the diagnosis as ganglion cyst. The ganglion cyst may compromise lumbar dorsal ganglion when it located in the intervertebral foramen. The surgeon should keep this rare entity in their mind for differential diagnosis.
