Effects of a Herbal Medicine, Yukgunja-Tang Extract Granule, on Functional Dyspepsia Patients by Sasang Constitution: Placebo-Controlled, Double-Blind, Randomized Trial (기능성 소화불량에 대한 육군자탕 엑스과립의 사상체질별 치료효과: 위약 대조, 이중 맹검, 무작위 시험)

  • Objectives This study was aimed to compare effects of Yukgunja-tang (YGJT) extract granule on functional dyspepsia (FD) by sasang constitution. Methods A placebo-controlled, double-blind, randomized, two-center trial was performed. We obtained 3D face images of FD patients participating in this study using face-only scanner and classified them into two types as narrow face and wide face by 3D facial shape diagnosis system (3-FSDS). 96 subjects (48 subjects per type) were enrolled and were randomly allocated into treatment or control groups in a 2:1 ratio. YGJT extract granule or placebo were administered to each group during the 8 week treatment period. One of sasang constitutional specialist diagnosed their constitutions by referring to questionnaires and 3D face images. The primary outcome was total dyspepsia symptom scale (TDS scale), the secondary outcomes were single dyspepsia symptom scale (SDS scale), visual analog scale (VAS) and so on. This trial was registered with clinical research information service identifier: KCT0001920, 15 May, 2016. Results In the Taeeumin treatment group, the TDS scale was significantly decreased after 8 weeks compared to the control group (t=2.331, p=0.025) and the SDS scale also significantly decreased. (t=2.042, p=0.048). The specific effects of the YGJT extract granule without the placebo effect on the Taeeumin with functional dyspepsia were 30.61% in TDS scale and 28.33% in SDS scale. Conclusions The effects of Yukgunja-tang extract granule on functional dyspepsia was different according to Sasang constitution. So the constitution should be considered in randomized controlled trials using herbal extract granules.

  • The purpose of this study is to observe the effectiveness of the Korean medical complex treatments for functional dyspepsia (FD). And we observed NDI-K (Nepean dyspepsia index-Korean version) and algometer on acupoints to confirm the treatment effectiveness. In this retrospective study, we investigated 77 patients with FD symptoms who admitted to 00 Korean Hospital for 2 weeks from April 14, 2020 to March 31, 2021. All patietns were treated with herbal medicines, acupuncture, pharmacopuncture, moxa treatment, aroma therapy, So-jeok and So-jeok II therapy. On the day of admission and discharge, NDI-K, algometer on acupoints (CV12, CV10, ST25, ST27, SP14) were examined twice. Among the NDI-K values, all 14 items except for poor breathing were decreased statistically significantly. The algometer values were increased statistically significantly in all acupoints (CV12, CV10, ST25, ST27, SP14). These results suggest that Korean medical treatment can be useful in the treatment of FD.

Pattern Identification of 97 Functional Dyspepsia Patients and the Characteristics of Each Pattern Type (기능성 소화불량 환자 97명의 변증유형별 특성)

  • Objective: This study was designed to identify and explore the pathological patterns of functional dyspepsia (FD) patients. We also evaluated the usefulness of the Pattern Identification Questionnaire by comparing it with other assessment tools for FD. Methods: We recruited 97 FD patients based on the Rome III criteria for FD diagnosis. The pathological patterns of the subjects were determined by the Pattern Identification Questionnaire. Their dyspepsia-related symptoms were assessed using the Gastrointestinal Symptom Questionnaire (GIS) and the Pyeongwi-san (Pingwei-san) Patternization Questionnaire. Depressive symptoms were evaluated with the Beck Depression Inventory (BDI) and quality of life with the Functional Dyspepsia-Related Quality of Life (FD-QoL) Questionnaire. Tongue coating was measured by the Digital Tongue Diagnosis System (DTDS). Results: The male to female ratio was 1:1.1, and the forties and fifties age groups were largest in number. The spleen deficiency and phlegm-dampness pattern was the most common pattern found among the FD patients. No significant differences in the GIS, BDI, FD-QoL, and DTDS scores were found among the five pattern types. All pattern types showed significant correlation with GIS, Pyeongwi-san Patternization Questionnaire, and FD-QoL scores. Conclusions: Pattern Identification Questionnaire can not only identify the pathological pattern types of FD patients but also evaluate the severity of their symptoms. Compared to conventional assessment tools for FD, it could enable a more dynamic evaluation of FD patients reflecting the severity of dyspeptic symptoms and the quality of life. Further studies on the Pattern Identification of FD patients are anticipated in order to improve the diagnosis and therapy for Korean FD patients.

Evaluation for Therapeutic Effectiveness of Banwhasashim-tang in Functional Dyspepsia (기능성 소화불량증 환자에 대한 반하사심탕 엑기스제의 임상효과)

  • Objective : We evaluated the therapeutic effect of Banwhasasimtang dry extract powder on gastrointestinal symptoms and its side effects in patients with functional dyspepsia. Methods : Patients with functional dyspepsia, defined as discomfort in the epigastrium. A total 45 patients participated in this study. Dyspeptic symptoms ratio was obtained by patients, total point of symptom score/total 21 points x 100(%). On each of symptom after 4 weeks administration of Banwhasashim-tang was made by variation of symptom score: If symptoms score of posttreatment is lower than that of pretreatment, it is improved ; Otherwise aggravated, and if there is no change between them, unchangeable. When a full health condition of normal control is assumed 100%, relative health condition of dyspeptic patient could be calculated from 'full health condition-dyspeptic symptoms ratio'. Therefore a whole satisfaction rate on symptom improvement was presented with variation of a relative health condition to dyspepsia between pretreatment and post. Results : Banwhasashimtang significantly improved symptoms in patients. The significance was shown in both all seven items(p<0.01, respectively) and 19.2% (p<0.01) of increase of a whole satisfaction rate on symptom improvement. There is no remarkable side-effect in all laboratory test, excepting 1 case of abdominal pain and another one case of recurred pain of chronic prostatitis. Conclusion : Banwhasashimtang extract provides symptomatic relief in functional dyspepsia and is no remarkable side-effect drug.

Development of a Phlegm Pattern Questionnaire for Functional Dyspepsia and Analysis of Its Reliability and Validity (기능성소화불량에 대한 담음 변증설문지 개발과 신뢰도 및 타당도 검정)

  • Objectives: The aim of this study was to develop a Phlegm Pattern Questionnaire for Functional Dyspepsia (PPQ-FD) by modifying a previously developed Phlegm Pattern Questionnaire (PPQ) and to verify its reliability and validity. Further objectives were to obtain the optimal cut-off value for the PPQ-FD for standardization and for clinical use. Methods: The PPQ-FD was developed by extracting the major symptoms of the phlegm pattern in functional dyspepsia and by using the Delphi method to administer a requested importance survey to experts. The reliability, validity, and optimal cut-off value of the PPQ-FD were analyzed by enrolling a total of 60 subjects in this study. Thirty patients were diagnosed with both functional dyspepsia and phlegm pattern and thirty patients were diagnosed with only functional dyspepsia. All participants were requested to fill out the PPQ-FD. Results: No statistically significant differences were detected in the two groups for sex distribution, age, or body mass index. Five of the survey questions negatively affected its reliability; therefore, we decided to exclude those five questions on further inspection. The Cronbach's α coefficient of the revised PPQ-FD was 0.853, and clinical validity was verified. Construct validity was analyzed by factor analysis and identified four factors. Statistically significant positive correlations were found between the revised PPQ-FD and other dyspepsia scales, such as the SQDQ, SSQD, VAS, NDI-K, and FD-QoL scales. The VAS had particularly strong positive correlations with the PPQ-FD. Conclusions: The PPQ-FD developed in this study has fundamental reliability and validity for use as a pattern-diagnosis questionnaire. The PPQ-FD can help to diagnose the phlegm pattern in patients with functional dyspepsia.

Analysis of symptom pattern through comprehensive diagnosis of Qui Xui Shui in patients with functional dyspepsia (기혈수변증(氣血水辨證)에 의한 기능성 소화불량 환자의 변증유형 분석)

  • Background & Object : It is well known that functional dyspepsia is one of the most common diseases. While many dyspepsia patients have been helped with oriental medical therapy, there has not been a study based on the concepts of oriental medicine. The aim of this study was to perform a fundamental epidemiological survey and to analyse the symptom pattern of functional dyspepsia. Methods : 86 patients(27 males, 59 females) diagnosed with functional dyspepsia in Kyunghee Oriental Medical Center from May to December 2002 were involved in this investigation. The disease characteristics of functional dyspepsia(based on Rome criteria II) and the Qui Xue Shui diagnostic procedure were investigated by questionnaire and physical examination. The total score and composition ratio of each comprehensive diagnosis were calculated from the symptom score described in the questionnaire. Results : The total score from the Qui Xue Shui diagnostic procedure was found to be influenced by the number of functional dyspepsia symptoms(p=0.026) and the patient's own cognition of their current dyspeptic situation(p=0.006), in addition to digestive ability and general congnition(p=0.006), and was not associated with sex, duration of dyspeptic symptoms in one year, or the total illness period, In the composition ratio of the Qui Xue Shui diagnostic procedure, only Qui-yu(p=0.048) diagnosis was accurate regarding the number of symptoms in one year, total illness period, or the patients' own congnition of their current dyspeptic situation, But the value of the composition ratio among the total factors involved was most similar to that of sex. Conclusions : Thus, it is shown here that the total score of using the Qui Xue Shui diagnostic procedure is mainly dependent on dyspeptic symptoms and the patinets' own cognition of their current dyspeptic situation, and that regular symptom patterns in the Qui Xue Shui diagnostic procedure exist in dyspeptic patients.

Relationship between Gastric Motility and Health Condition Graded by Total Symptom Scores in Comprehensive Diagnosis of Qui Xue Shui in Functional Dyspeptic Patients (기능성 소화불량증 환자의 변증증후(辨證證候)에 의한 건강상태와 위 운동성의 상관성에 대한 연구 (기혈수(氣血水) 변증(辨證)과 위전도 지표를 중심으로))

  • Background & Object : The aim of this study was to investigate the usefulness of electrogastrography in the diagnosis field of traditional medicine through a study of the relationship between gastric motility and health condition based on oriental medicine diagnostic theory in functional dyspepsia. Method : 86 patients (male 27, female 59) with functional dyspepsia and 10 healthy control subjects (male 5, female 5) were involved in the investigation. The disease information of functional dyspepsia (based on Rome criteria II) was used for dyspeptic index and scores were obtained from the comprehensive diagnosis of Qui, Xue, Shui was applied as index for health condition, those were all investigated by questionnaire. Gastric motility were recorded and analyzed using electrogastrography in fasting and postprandial period. Results : The total score of comprehensive diagnosis of Qui Xue Shui was influenced by the number of functional dyspepsia symptoms (p=0.026). In terms of electrogastrographical parameters, both postpranial normal slow waves regularity(p=0.003) and power ratio (p=0.001) in the patients had the statistical significance and they showed an incremental inverse correlation with the number of symptoms. Dominant frequency and fasting normal slow waves regularity ratio had no statistical significance. Conclusion : Results suggest that electrogastrography is useful in evaluating the health condition of patient by comprehensive diagnosis of Qui Xue Sui.

Postprandial Epigastric Fullness and Pyloric Valve Disturbance in Patients with Functional Dyspepsia;Analysis of Bowel Sounds and Electrogastrography (기능성(機能性) 소화불량증(消化不良症) 환자(患者)의 식후(食後) 심하비만과 pyloric valve의 기능장애(機能障碍);장음과 위전도를 중심으로)

  • Background & Object : Postprandial epigastric fullness is one of common symptoms in functional dyspepsia, but its pathophysiological mechanism has still been unknown. In this study, its association between postprandial epigastric fullness and pyloric valve disturbance was studied through analysis of bowel sounds and electrogastrography. Methods : Bowel sounds and electrogastrography were recorded together for fasting 15 min and postprandial 40 min. Parameters of bowel soundincluding motility index, sound to sound interval, standard deviation of sound to sound interval, sounds per minute, % of bowel sound, sound length, intensity, dominant frequency of sounds, and postprandial / fasting ratio of dominant frequency of sounds were analyzed with a specialized program. By electrogastrography, regularity of slow waves and power ratio were obtained. Results : Significances of bowel sounds appeared in motility index (p=0.046), dominant frequency of fasting (p=0.048), postprandial (p=0.003), and the ratio of postprandial/fasting (p=0.000); those of EGG parameters were shown in postprandial regularity of slow waves (p=0.006) and power ratio (p=0.011). Conclusion : Pyloric valve disturbance was a cause of postprandial epigastric fullness in patients with functional dyspepsia. Analysis of bowel sound might be useful in diagnosing its existence.

Effect of Naesowhajung-tang on Electrogastrography in children with functional dyspepsia (기능성 소화불량증 환아(患兒)의 위전도에 대한 내소화중탕(內消和中湯)의 효과(效果))

  • We have surveyed the epidemiology of functional dyspepsia in 7 cases of Children and investigated the therapeutic effect on Functional Dyspepsia following 4 weeks medication of Naesowhajung-tang using Electrogastrogram. The Results were as followings: 1. Sex ratio are 6 males, 1 female; their mean ages, $9.0{\pm}1.41(7-11years)$; length, $137.2{\pm}13.86cm$; body weight, $35.3{\pm}l1.34kg$. 2. Most of subjects have a normal growth state. But one case have severe growth disturbance. Familial tendency for this disease is noted in 85.8%. 3. Abdominal pain was seen in 100% of subjects. The indicated symptom was lowed in order of abdominal pain(100%), nausea vomiting(71.43%), general fatigue(65.34%), vertigo headache(57.14%), belching(55.38%), etc. 4. The EGG finding after treatment was improved in 53%, aggrevated in 43. In cases, postprandial improvement was more remarkable than fasting improvement. Common cold and antibiotics seems to have functioned as an aggrevating factor on gastric motility. From the above results, Naesowhajung-tang seems to have been used in the treatment of functional dyspepsia for children.

A Case Report of the Traditional Korean Treatment of Female Patients with Both Functional Dyspepsia and Chronic Atrophic Gastritis (기능성소화불량과 만성 위축성 위염이 병발된 여성 환자에 대해 한방치료 경험 1례)

  • Objective: The aim of this study was to introduce gastric dysmotility as a common cause in patients with concurrent functional dyspepsia and chronic atrophic gastritis. Method: Dyspeptic symptoms, the Rydoraku score, gastric motility (electrogastrography, bowel sound analysis), gastric mucosa (gastroendoscopy), and blood and blood chemistry were all evaluated. For the treatment method, Pyengwi-san (solution) and Banwhasashim-tang (extract) were used as herbal drugs. Both ST36 electrical stimulation and simple immersion stimulation of CV11, 12, and 13 in the abdomen were applied. Results: Dyspeptic symptoms including indigestion, headache, and insomnia were all relieved. Gastric myoelectrical activity and gastric pyloric function were additionally improved. The condition of the gastric mucosa was changed from atrophic to erosive. Other side-effects of the treatment were not noted. Conclusion: The traditional Korean treatment showed effectiveness in the relief of dyspeptic symptoms and mucosal improvement of chronic atrophic gastritis. Gastric dysmotility is a common cause of the condition being concurrent with both functional dyspepsia and chronic atrophic gastritis without Helicobacter pylori infection.
