Sensory Bearing Scapular Free Flap (감각 유리견갑피판술)

  • Among many kinds of introduced free flaps, scapular freeflap is one of the most popularly using modalities in fasciocutaneous defect coverage with minimal donor defect and easier procedure and constant vascular patterns of the donor. Many surgeons who had experience of this flap pointed out deficit of the reliable sensation of the transplanted flap is the main shortcoming of the scapular free flap. If we can subjugate that point, scapular free flap is the most excellent procedure in such a cases as heel pad reconstruction and hand reconstruction which are relatively important to have skin with protective sensation. Author performed anatomical literature review, 10 cadaveric dissections and 12 clinical dissections. In surgical anatomical aspect, the upper six dorsal rami of the thoracic nerves have medial branches which pierce Longissimus thoracis and Multifidus muscle with small cutaneous twigs which pierce Latissimus dorsi and Trapezius muscle. Among that cutaneous twigs, several twigs distribute to the skin of the back from midline to lateral aspect which territory is identical to scapular free flap. We analysed clinical experiences of that sensory bearing scapular free flap surgical anatomy and one year follow-up studies with several results. 1) Two to three cutaneous twigs which pierced from the Trapezius muscle over the scapular free flap region. 2) Each twigs has two to four nerve fascicles with small artery. 3) The nerve distributed to the ordinary scapular free flap and large enough size and pedicle length to neurorrhapy with various recipient site nerves. 4) The inconvenience of this procedure is the vascular pedicle and nerve pedicle have opposite directions, vascular pedicle of that comes from lateral direction from subscapular vessels, but nerve pedicle comes from medial direction from trapezius muscle. Author can found constant cutaneous nerve branches which come from piercing the Trapezius. This nerves are helpful for protective sensation in transplanted scapular free flap. We can't had enough follow-up and evaluation of the nerve function of this procedure, we need continuous research works to application of this procedure. The in conveniences come from directional differences of pedicle can solve with longer harvest neural pedicle and change direction of the neural pedicle.

Effect of Duck Free-Ranging Density on Duck Behavior Patterns, and Rice Growth and Yield under a Rice-Duck Farming System in Paddy Field (논오리 방사밀도가 오리의 행동양상 및 벼 생육 ${\cdot}$ 수량에 미치는 영향)

  • The current study was conducted to elucidate the optimum density of free-ranging ducks in a rice-duck farming system in terms of effects on duck behavior, and growth and yield of rice plants. Four paddy fields were used for this experiment, with 6, 9, 12 and 15 birds per plot, respectively. Ducklings at eight days of age were free ranged in experimental paddy plots (4.0 a each) on the 9th day after the transplantation of rice plants. Ducks were kept in the plot for seventy days, at which point rice plants reached the earing stage. Foraging, moving, working, resting, diving and pecking behaviors of the ducks were observed two times during the experimental period. Also, rice plant growth and yield according to the density of ducks per plot were examined. The foraging and moving behavior of free-ranging ducks in paddy fields for 12 hours during the daytime tended to be longer in the 12-bird plot, and working behavior was significantly (P<0.01) longer in the 12-bird plot than in the other three plots. The resting behavior was significantly (P<0.01) higher in the 9- and 15-bird plots than in the 12-bird plot. The frequency of moving behavior for 6 hours during the daytime in the 15-bird plot tended to be lower than that in the other three plots, but this difference was not significant. The amount of diving and pecking behavior in the 9-bird plot was significantly (P<0.05) higher than that in the other three plots, and the number of hills pecked tended to be higher with increasing of duck density. From thirty days after ducks were introduced to the paddy fields, the length of rice plants tended to be significantly (P<0.05) shorter in high free-ranging density plots as compared to low free-ranging density plots. The number of tillers per hill was not affected by the free-ranging density. The culm length of rice plants was significantly (P<0.05) shorter in the 12- and 15-bird plots than in the other two plots, however, the duck free-ranging density did not affect panicle length. The dry weight of the root of rice plants was increased with high free-ranging density, but there was no such increase in the top parts of the rice plants. The percent of rice plants badly damaged by free-ranging density tended to be lower in the order of 12-, 9-, 6- and 15-bird plots. The number of ears, ripening grains and crop yield per hill of rice plants in the 12-bird plot were significantly (P<0.05) higher than those of the other three plots. Therefore, the yield of each rice plant per 10 a was significantly increased in the 12-bird plot.

Effect of cylinder aspect ratio on wake structure behind a finite circular cylinder located in an atmospheric boundary layer (대기경계층 내에 놓인 자유단 원주의 형상비가 후류유동에 미치는 영향에 관한 연구)

  • The flow around free end of a finite circular cylinder(FC) embedded in an atmospheric boundary layer has been investigated experimentally. The experiments were carried out in a closed-return type subsonic wind tunnel with varying aspect ratio of the finite cylinder mounted vertically on a flat plate. The wake structures behind a 2-D cylinder and a finite cylinder located in a uniform flow were also measured for comparison. Reynolds number based on the cylinder diameter was about Re=20,000. A hot-wire anemometer was employed to measure the wake velocity and the mean pressure distributions on the cylinder surface were also measured. The flow past the FC free end shows a complicated three-dimensional wake structure and flow phenomenon is quite different from that of 2-D cylinder. The three-dimensional flow structure was attributed to the downwashing counter rotating vortices separated from the FC free end. As the FC aspect ratio decreases, the vortex shedding frequency is decreased and the vortex formation length is increased compared to that of 2-D cylinder. Due to the descending counter-rotating twin-vortex, in the region near the FC free end, regular vortex shedding from the cylinder is suppressed and the vortex formation region is hardly established. In the wake center region, the mean velocity for the FC located in atmospheric boundary layer has large velocity deficit, compared to that of uniform flow.

Treatment of Urethral Stricture using Sensated Ulnar Forearm Free Flap (척골측 전완부 감각유리 피판술을 이용한 요도협착의 치료)

  • In the treatment of urethral stricture, many problems still remain with the current methods making it a field of further exploration for reconstructive surgeon. Furthermore, when total or multiple strictures of the penile urethra exist, the methods of surgery become difficult due to a necessity for a long neourethra. Introduction of vascularized free flap has broadened the choice and improved the results of reconstruction for the urethra. The authors used a sensate ulnar forearm free flap in a patient with multiple penile urethral strictures for reconstruction. Uroflowmetry, 30 months after surgery, revealed that maximal flow rate was 15.5 ml/sec, average flow rate was 9.5 ml/sec, and voided volume was 157 ml. A urethrogram was performed 30 months postoperatively and a good result was achieved. The ulnar forearm free flap used by the authors are thin and pliable and is good for providing sufficient length to reconstruct the neourethra for a long urethral defect.

Metabolic Changes in Growing Rats Fed Diets with Different Levels of Lead and Lipid (납(Pb) 과 지방수준을 달리한 식이로 사육한 성장기 흰쥐의 체내대사변화)

  • This study was performed to investigate the effects of lead poisoning and different levels of dietary lipid on lead and lipid metabolism in the rats. Forty eight male weanling rats of Sprague-Dawley strain weighing 73.1$\pm$11.1g were blocked into 6 groups according to body weight. Dietary lipid were given at the lev딘 of 5, 20 and 40% of total caloric intake and lead were either administered or not. The results obtained are summerized as follows ; 1) Food intake, weight gain, F.E.R. liver and epididymal. fat pad weights, weight and length of bone, hematocrit and hemoglobin content in Pb-adminstered groups were lower than these in Pb-free groups. 2) Plasma lipid and cholesterol levels were tended to be higher in Pb-administered groups than in Pb-free groups, while liver lipid and cholesterol levels were tended to be lower in Pb-adminstered groups. 3) Fecal dry matter excretion was tended to be higher in Pb-adminstered groups than in Pb-free groups, and were increased with increasing dietary lipid level. Daily fecal excretions of lipid and cholesterol were higher in high lipid groups than in low lipid groups and these levels were even higher in the animals exposed to Pb than in Pb-free groups. 4) Pb contents in blood, liver, kidney and bone were significantly higher in Pb administered groups than in Pb-free groups. Pb levels of blood, liver and bone did not show any significant difference among groups with different levels of dietary lipid, but Pb concentration in kidney of Pb-adminstered groups increased with increasing dietary lipid level.

FREE SURFACE FLOW ANALYSIS BY SOROBAN GRID BASED CIP MEHTOD (Soroban grid 기반 CIP법을 이용한 자유표면 유동해석)

  • In this study, we provide a comprehensive review of the CIP(Constrained Interpolation Pro file/Cubic Interpolated Propagation) method with a pressure-based algorithm that is known as a general numerical solver for soled liquid, gas and plasmas. And also we introduce a body-fitted grid system(Soroban grid) for computation of strongly nonlinear marine hydrodynamic problems such as slamming water on deck, wave impact by green water. This grid system can keep the third-order accuracy in time and space with the help of the CIP method. The grid system consists of the straight lines and grid points. In the 2-dimensional grid case, each grid points moving in these lines like abacus - Soroban in Japanese. The length of each line can be different and the number of grid points in each line can be different. Mesh generation and searching of upstream departure point are very simple and possible to mesh-free treatment. To optimize computation of free-surface and multi-fluid flows, We adopt the C-CUP method. In most of the earlier computations, the C-CUP method was used with a staggered-grid approach. Here, because of the mesh free nature of the Soroban grid, we use the C-CUP method with a collocated-grid approach.

Reconstruction of the Finger Defect with Free Vascularized Reversed Radial Forearm Flap (유리 반전 전완피판술을 이용한 수지부 결손의 치료)

  • Radial forearm flap is one of the most useful skin flap in hand reconstructuion with distally based reverse pedicled or free vascularized fashion. Athors modified that flap into reverse pedicled and free vascularized flap which has advantages of both methods. The modification composed with harvesting flap on recipient side distal forearm just as free flap, than apply it as reverse distal pedicled flap fashion with microvascular anastomosis with distal vascular stump of donor radial vessels. We underwent this method in 5 cases in finger reconstruction from 1996, all of the cases had sucessful results. The advantages of this method are: 1. Thin flap which is compatible to finger skin can harvest from distal forearm with very long vascular pedicle that can be passed under the subcutaneous tunnel which avoid additional skin incisions on the hand. 2. The vessels of donor site and recipient site are same vessel in effected side of forearm, which can preserve contralateral side forearm and hand keep intact. 3. The flap can cover the defects on distal portion of the fingers which is difficult in conventional reversed radial forearm pedicled flap because of limited mobilization of flap due to limitation of pedicle length reach to tip of the fingers.

Flow structure of wake behind a finite circular cylinder (자유단이 있는 원주의 후류 유동특성에 관한 실험적 연구)

  • Flow characteristics of the wake behind a finite circular cylinder(FC) mounted on a flat plate was experimentally investigated. Three finite cylinder models having aspect ratio (length to diameter ratio, L/D) of 6,10 and 13 were tested in this study. Wake velocity was measured by a hot-wire anemometry at Reynolds number of 20,000, and the results were compared with those of two-dimensional circular cylinder. As a result, the free-end effect on the wake structure becomes more dominant with decreasing the aspect ratio(L/D) of the finite cylinder. Invisid flow entrained into the wake region decreases the turbulence intensity and periodicity of the vortex shedding due to existence of the free end. From spectral analysis and cross correlation of the velocity signals, vortices having 24Hz frequency characteristics are found in the down wash flow just behind the free end. There exists very complicated flow near the free end due to interaction between the entrained flow and streamwise vortices. Vortex formation region is destroyed significantly in the near wake and shows quite different wake structures from those of 2-D cylinder.

Free tissue transfer for reconstruction of axillary defects: two case reports

  • Axillary defects need pliable, vascular tissue to cover the critical structures traversing the axilla and to allow near-normal range of motion in the shoulder. Although local flaps are the first choice, free tissue transfer is a good option when local tissues are injured or scarred. Herein, we report two cases of axillary defects that were reconstructed using anterolateral thigh free flaps. One was a post-electric burn axillary defect for which a thoracoacromial pedicle was used as the recipient, and the other was a posttraumatic axillary defect with the transverse cervical vessels as the recipient. In both patients, the flap survived well with no complications and resulted in adequate functional recovery. In large defects of the axilla with a scarcity of local tissues, free flaps can yield optimal results. The proper selection of recipient vessels and a donor flap with adequate pedicle length impact the outcomes of such reconstruction.

Free Rectus Muscle or Myocutaneous Flap for Reconstruction on the Various Sites (다양한 부위의 재건에 있어 유리복직근 피판술의 이용)

  • A free rectus abdominis flap can include a variable amount of muscle length depending on recipient site requirements. There is also great flexibility in flap design in terms of size, orientation of its axis, and the level of its location over the muscle. It is safe to design the skin island across the midline. Though skin islands designed over the most inferior portion of the abdomen have not always proved reliable when based on the superior epigastric artery, free flaps based on the inferior pedicle can be successfully designed in this area. As free flap based on the inferior epigastric vessels, this flap has been useful for large head and neck defects following ablative procedures, for facial contour restoration as a buried flap, for upper extremity defects, for lower extremity defects such as coverage of grade III tibial fractures and for breast reconstruction. A free rectus abdominis muscle or myocutaneus flap was used in 8 patients. The operations were performed between Sep. of 1994 and April of 1996. The patients were tongue cancer 1 case, chronic facial palsy 1 case, unilateral breast reconstruction 1 case, upper and lower extremity injury 5 cases. The free rectus abdominis muscle flaps were 4 cases and the free myocutaneous flaps were 4 cases. There was no failure of the flap, except one partial necrosis. One case of the skin grafts on the muscle flap was regrafted. One case of reoperation due to venous thrombosis was performed. In tongue cancer patient, a orocutaneous fistula was occurred, but conservative treatment and secondandry skin graft were done. In conclusion, a free rectus abdominis flap has many advantages such as a long and constant pedicle, easy dissection, enough soft tissue available, scar on the donor site to be hiddened, no need for changing position. So we think that this flap is the most useful one for small or moderate sized defects on the various sites.
