Incidence of Osteoporosis and Falls and Predictors of Fracture Risk in Postmenopausal Women (폐경 후 여성 골다공증과 낙상의 실태 및 골절 위험 예측요인)

  • Purpose: The purpose of this study was to investigate the incidence of osteoporosis and falls and their consequences, and to identify predictors of fracture risk in the postmenopausal women. Methods: A total of 687 postmenopausal women were recruited through a stratified convenience sampling. A structured questionnaire was used to obtain osteoporosis and fall history and details of their most recent fall. To predict fracture risk factors, we collected demographic and physical health variables related osteoporosis and fall. Fracture risk was measured by FRAX$^{(R)}$ to calculate 10-year probability of major osteoporotic and hip fracture. Results: The prevalence of osteoporosis was 22.1%, and 66.4% of them had treatments for osteoporosis. The incidence of falls during the past year was 19.2% and 38.6% of those who fell suffered consequent fractures. Women with history of osteoporosis and falls were significant predictors of 10-year probability of major osteoporotic and hip fracture. Other significant predictors were history of fracture, chronic disease, surgical menopause, lower BMI, poorer perceived health and no job. Conclusion: It appears that history of osteoporosis and falls are main predictors of fracture risk. Nursing assessment should be performed by detail history taking for osteoporosis, fall, chronic disease, and fracture to screen fracture risk group among postmenopausal women.

Risk Factors for Hip Fracture among the Elderly (노인들의 고관절골절 위험요인에 관한 환자-대조군 연구)

  • Objectives: This study was conducted to find out risk factors related to elderly hip fracture, so that the result could provide basic data to establish prevention programs for hip fracture among the elderly. Methods: The data were collected from 199 cases hospitalized in 4 university hospitals in Pusan from January, 1996 to December, 1998, 193 healthy controls who visited elderly facilities in Pusan. The data were obtained from medical records and by using questionnaires through telephone contact or direct personal interview with the subjects or their family, and were analyzed for 2-test and multiple logistic regression. The risk factors were shown with odds ratios and their 95% confidence intervals. Results: In univariate analysis, the odds ratio of hip fracture risk was estimated to be 1.9 for the elderly aged 75 years as compare with those aging less than 65 years; 42 for those with job as compared with those without job; 3.3 for those with more than 6 children as compared with those with one or two children. For the variables related to physical characteristics, small height (p=0.015), light weight (p=0.000), and low BMI (p=0.014) were risk factors for elderly hip fracture. Sane variables related to health, such as previous history of illness (OR=3.3.), abnormal blood pressure (OR=1.6), previous fracture history (OR=22), lower limbs weakness (OR=12.1) and gait disturbance (OR=42.6), were significantly associated with the risk of hip fracture. In multiple logistic regression, risk factors for hip fracture were age, having job, lower limb weakness and previous history of illness. The adjusted odds ratios of hip fracture risk among the elderly were age (OR=1.1), having jobs (OR=11.7), weak lower limb (OR=10.8) and previous history of illnesses (OR=3.3), respectively. Conclusion: This study suggests that the plan for improving the daily living environment for the elderly systematically should be implemented to avoid the chances of fall, and that programs encouraging to practice regular exercise for physical activity and to promote health of the elderly should be developed.

  • The machine structure or parts has been arrive at a fracture, depend on crack by repeated load. Machine designer is estimate of fatigue strength in early stages the design for prevent fatigue fracture, then necessary its load history. This research study is using diesel locomotive in measurement of load history for fatigue design of rolling stock. The relation between velocity and amplitude of load history was found. We confirmed that the increase of velocity make to increase the magnitude of amplitude.

Contribution of Lateral Ankle Instability Evaluation with MRI to Proximal Fifth Metatarsal Fracture (제5 중족골 근위부 골절 환자의 자기공명영상 검사를 통한 족관절 외측 불안정성 평가의 기여도)

  • Purpose: One of the main contributors to proximal fifth metatarsal fracture is ankle inversion and the incidence of recurrence may increase in patients with ankle instability. So, the authors confirmed the patients of proximal fifth metatarsal fracture with ankle instability by checking the history and magnetic resonance imaging (MRI) and assessed the value of MRI as therapeutic prognosis and clinical indicators for prevention of recurrence. Materials and Methods: Patients with proximal fifth metatarsal fractures visited our hospital during recent five years were reviewed. 35 patients with suspected damage by ankle inversion had been identified a history of ankle instability and checked the hindfoot malalignment through hindfoot alignment view and MRI was performed prospectively. The patients was devided to three groups on the location of fracture site and the groups were compared each other. Results: The mean time from injury to checking MRI was 10.7 days. There was no structural abnormality and was no significant difference according to the location of fracture. The patients with history of ankle inversion were 31(88.6%) and the patients with history of chronic or recurrent injury were 22 patients (62.9%). The lesion of MRI related to lateral ankle instability were identified in all patients. Conclusion: This study noted a high incidence of lateral ankle instability that was identified by MRI in the patients of proximal fifth metatarsal fracture. Aggressive treatment for lateral ankle instability should be needed for complications as proximal fifth metatarsal fracture to reduce the recurrence and occurrence.

  • In this paper, the estimation of dynamic interlaminar fracture toughness on fracture mode II in CFRP(carbon fiber reinforced plastics) laminates in made. Dynamic ENF(End Notched Flexure) apparatus used in this paper is manufactured by suing Split Hopkinson Pressure Bar. The static and impact load history in the CFRP specimen is measured by using manufactured dynamic ENF tester and 3-point bending test is carried out to find the load history. Also dynamic interlaminar fracture toughness can be found by using the J integral obrained from dynamic analysis in consideration of intertia-force effect.

  • Background: Prevention of osteoporosis and bone fracture is one of the important issues for liver transplant recipients because a long history of liver disease and lifelong use of immunosuppressants, including corticosteroids, may cause these diseases. In this study, we aimed to analyze liver recipient bone status, 10-year fracture risk, and medication history. Methods: The electronic medical records of adult patients aged >40 years who received liver transplantation at Seoul National University Bundang Hospital between January 2009 and June 2017 were reviewed retrospectively. On the basis of their bone mineral density and fracture history, their fracture risks were analyzed using the Korean fracture risk assessment tool. Results: A total of 57 liver transplant recipients were treated with corticosteroids during a mean of 8.8 months after transplantation. 30 patients (52.6%) showed bone metabolism dysfunction such as osteopenia or osteoporosis. The 10-year femoral fracture risk was 2.1%, and dual-energy X-ray absorptiometry monitoring was performed, including right before liver transplantation every 27.5±19.2 months. The mean femoral bone mineral density decreased by -7.2%±7.3%. Four patients (7.0%) had a fracture after liver transplantation. Osteoporotic fracture occurred in 3 patients with osteoporosis (25.0%). Among the osteopenia patients with moderate fracture risk who were not treated with bisphosphonate, 1 patient (12.5%) had a history of bone fracture after liver transplantation. Conclusions: Considering the deterioration of bone density and moderate fracture risk, medication for osteoporosis should be prescribed to liver transplant recipients with regular monitoring of bone density after transplantation.

  • The creep behavior and creep fracture of alumina at high temperature were investigated under four point flexural test. The steady-state creep behavior was observed at low bending stress and the primary creep until fracture was observed at high bending stress. The loading history of bending stress did not affect on the steady-stated creep rate. Intergranular fracture was dominant for fracture of alumina at room and high temperature. However, transgranular fracture was dominant on creep fracture of alumina under high temperature by nuclueation and growth of microcracks due to residual flaws or cavities in the material.

  • Numerical modeling was conducted to estimate the amount of dislocation that may occur across a frictionless fracture during an earthquake using commercial code FLAC3D (Fast Lagrangian Analysis of Continua in 3 Dimensions). The applied motion was calculated to represent a Richter 6.0 magnitude earthquake at distances of 2 km from the fracture. The velocity-time history was generated from Svensk $K{\ddot{a}}arnbr{\ddot{a}}anslehantering$ AB report. In the report, The velocity field resulting from an earthquake on a fault located in the near-field (2 km distance) was modelled using a finite difference program, WAVE. The stress-time history was substituted for velocity-time history to perform dynamic analysis using FLAC3D. During the earthquake, the maximum dislocation and change of shear stress were about 1 cm and 2MPa, respectively. Because the fracture is frictionless in this study, all dislocations relax to zero after the earthquake motions have ceased.

  • This study aims to investigate the dynamic fracture characteristics of rocks and rock-like materials subjected to the Nonex Rock Cracker (NRC), a vapor pressure crushing agent that produces vapor pressure by instantaneously vaporizing a liquid mixture crystallized through the thermite reaction. Furthermore, the study seeks to develop an analytical technique for predicting the fracture pattern. A dynamic fracture test was performed on a PMMA block, an artificial brittle material, using the NRC. High-speed cameras and dynamic pressure gauges were employed to capture the moment of vapor pressure generation and measure the vapor pressure-time history, respectively. The 2-dimensional Dynamic Fracture Process Analysis (2D DFPA) was used to simulate the fracture process caused by the vapor pressure, with the applied pressure determined based on the vapor pressure-time history. The proposed analytical method was used to examine various fracture patterns with respect to granite material and high-performance explosives.
