Patient Safety Personnel's Task analysis using Importance and Performance Analysis (환자안전 전담인력 업무 분석: 중요도-수행도 분석기법 활용)

  • Purpose: This study aimed to identify tasks performed by patient safety personnel using importance-performance analysis. Methods: An online survey was conducted during a mandatory educational course. The questionnaire consisted of 43 items categorized into four subscales: management of patient safety accidents, education of healthcare personnel, education of patients and guardians, and patient safety activities. Importance-performance analysis was employed to ascertain the relationship between the importance and performance of tasks conducted by patient safety personnel. Results: A total of 145 patient safety personnel participated in the survey. The perceived importance of tasks by participants averaged 3.67 out of four, while the average performance was 3.40 out of four, indicating a significant difference (t=8.04, p<.001). Activities such as collecting patient safety reports, conducting root-cause analyses, and educating new employees were identified as low-performance tasks compared to their perceived importance. Conclusion: Tasks recognized as having low importance but high performance among patient safety personnel should be addressed through increased awareness and education. Analyzing the causes of tasks with low importance and performance is crucial for recognizing their importance and implementing improvement measures.

Salt Removal and Agricultural Application of Food Waste-Biochar (음식폐기물바이오차의 염분 제거 및 농업적 활용)

  • Food waste (FW) emissions in South Korea amounted to 4.77 million tons in 2021, and continue to increase. Various technologies have been developed to treat FW, with recent research focusing on biochar production through pyrolysis to reduce FW. However, the agricultural application of food waste-biochar (FWBC) is limited by the salt accumulated during pyrolysis. This study investigated salt removal from and the kinetic characteristics of FWBC, and subsequently evaluated its agricultural applications. FW was pyrolyzed at 350℃ for 4 h, and subsequently washed for 0.1, 0.25, 0.5, 0.75, 1, 5, 15, and 30 min to remove salt. FWBC had a salt concentration of 5.75%, which was effectively removed through washing. The salt concentration decreased rapidly at the beginning (1 min) and then slowly decreased, unlike in FW, in which the salt decreased continuously and slowly. The salt removal speed constant (K) was 1.5586 (Stage 1, FWBC) > 0.0445 (Stage 2, FWBC) > 0.0026 (FW). In a lettuce cultivation experiment, higher biomass was achieved using washed FWBC than when using unwashed FWBC and FW, and soil properties were improved. Overall, these findings suggest that although FW reduction using pyrolysis causes a salt accumulation problem, the salt can be effectively removed through washing. The use of washed FWBC can enhance plant growth and soil properties.

The Analysis on Surgical Result and Prognostic Factors of Thoracic Spinal Stenosis (흉추강 협착증의 수술적 치료 결과와 예후인자에 관한 분석)

  • Objective : To describe the underlying causes, surgical results, and prognostic factors in thoracic stenosis causing myelopathy, retrospective analysis for 28 cases of thoracic stenosis with surgery was performed Materials & Method : Twenty-eight patients(male, 15 ; female, 13) who underwent decompressive surgery for thoracic stenosis between 1987 and 1997 were analyzed. The mean age was 49 and the mean follow-up was 30.6 months. Statistical analysis with $SPSS^{(R)}$ was performed. Chi-square test was used for the analysis of relationship between subjects and multivariate analysis with general linear model was used to find prognostic factors. Result : Degenerative spondylosis was the most common cause, and three cases were associated with systemic diseases. Decompressive laminectomy was done in 23 cases, anterior decompression in four cases, and combined decompression in one case. Ossification of ligamentum flavum was found in 18 cases, facet hypertrophy in 13, ossification of posterior longitudinal ligament in six, and ventral spur in four. Postoperatively, 16 patients improved functionally and 4 patients worsened. The group of which initial symptom duration was less than two years showed better result(p=0.003). The group with sufficient decompression and no additional proximal stenosis had better outcome(p=0.002, p=0.001). Conclusion : Chronic myelopathy caused by thoracic stenosis can be reversible with appropriate decompression.

  • The four principles of treatment of odontogenic infection are as follows : (1) removal of the cause, (2) establishment of drainage, (3) institution of antibiotic therapy, and (4) provision of supportive care, including proper rest and nutrition. A separate incision is required to establish drainage, especially in the case of extensive fascial space infections. There are four principle causes for active bleeding in the immediate incision & drainage phase; (1) vascular wall alteration (infection, scurvy, chemicals), (2) disorder of platelet function, (3) thrombocytopenic purpuras, (4) disorders of coagulation (liver disease, anticoagulation drug). If the hemorrhage from incision & drainage site is aggressive, the site must be packed with proper wet gauze and wound closure & drainage dressing are applied. The specific causes of bleeding may be associated with hypoxia, changes in the pH of blood & chemical changes affecting vascular contractility and blood clotting. This is a case report of bleeding control by the circumferential suture & drainage on active bleeding incision & drainage site of temporal space abscess due to advanced odontogenic infection in a multiple medically compromised disabled patient.

Transport and Deposition Characteristics of Coarse Grained Soil According to the Flow Velocity and Grain Size (유속 및 입경에 따른 조립토의 이동 및 퇴적 특성)

  • Dredging and land reclamation works has actively carried out for the efficient use of land and secure of agricultural and industrial site. During the reclamation, a portion of landfills are lost from the desired location due to a variety of causes. However, these causes has been rarely studied, and water flow velocity has a great influence on the movement of landfills. For the economical and efficient reclamation, it is important to predict the movement of landfills in water. In this paper, an experimental study was carried out to investigate the deposition distance according to the flow velocity and soil grain size. We have made a large open-channel apparatus that can reproduce a laminar flow, and the deposition test was carried out on the four grain size (0.638, 1.425, 3.375, 7.125 mm) and four flow velocity (0.1, 0.3, 0.5, 0.67 m/s) with high definition video recording. As a results, average deposition distance increased with the flow velocity, and its relationship is shown linearly. For the grain size, the average deposition distance were drastically increased as the grain size becomes smaller.

Management of Femoropopliteal Vascular Injuries after Trauma: Surgical Outcomes (외상 후 대퇴-오금 혈관손상의 치료: 수술 성적)

  • Purpose: Vascular injuries caused by traffic, industrial accidents and by outside activities have increased in Korea. Especially, vascular injuries to the extremities can lead to limb loss and even mortality if they are not appropriately treated. The aim of the study was to evaluate the surgical outcomes of femoropopliteal vascular management after trauma. Methods: The medical records of 12 patients with femoropopliteal vascular injuries who were treated at Dankook University Hospital from 2011 to 2013 were reviewed. Iatrogenic vascular injuries were excluded. The clinical data including the causes of injury, associated injuries and surgical outcomes were analyzed retrospectively. Results: All patients were male, with a mean age of $46.8{\pm}16.3years$ (range: 26~69 years). The causes of vascular injuries were four traffic accidents, three industrial accidents, two iron plates, one outside activity, one glass injury and one knife injury. The average transit time between the place of the accident place and the emergency department was $3.0{\pm}2.1$ (0.5~12.5) hours, and the average preparation time for surgery was $8.0{\pm}6.7$ (1.7~23.3) hours. The anatomic injuries included the popliteal vessel in seven cases and the femoral vessel in five cases. The average Injury Severity Score (ISS) was $12.0{\pm}5.0$ (5~17), and the average Mangled Extremity Severity Score (MESS) was $5.7{\pm}2.1$ (2~9). The operation methods were four interposition grafts, three end-to-end anastomoses, two direct repairs and three patch angioplasties. One case required amputation of the injured extremity. Conclusion: Early recognition and revascularization of the injured vessel are mandatory to reduce limb loss and to obtain satisfactory outcomes. Therefore, careful/rapid evaluation of the vascular injuries and timely/successful surgical treatment are the keys to salvaging an injured limb.

Condensation Heat Transfer and Pressure Drop of R-134a in the Oblong Shell and Plate Heat Exchanger

  • Condensation heat transfer experiments were conducted with a oblong shell and plate heat exchanger without oil in a refrigerant loop using R-134a. An experimental refrigerant loop has been developed to measure the condensation heat transfer coefficient $h_r$ and frictional pressure drop ${\Delta}p_f$ of R-134a in a vertical oblong shell and plate heat exchanger. Four vertical counter flow channels were formed in the oblong shell and plate heat exchanger by four plates having a corrugated sinusoid shape of a $45^{\circ}$ chevron angle. The effects of the refrigerant mass flux, average heat flux, refrigerant saturation temperature and vapor quality were explored in detail. Similar to the case of a plate heat exchanger, even at a very low Reynolds number, the flow in the oblong shell and plate heat exchanger remains turbulent. The results indicate that the condensation heat transfer coefficients and pressure drops increase with the vapor quality. A rise in the refrigerant mass flux causes an increase in the $h_r\;and\;{\Delta}p_f$. Also, a rise in the average heat flux causes an increase in the $h_r$. But the effect of the average heat flux does not show significant effect on the ${\Delta}p_f$. On the other hand, at a higher saturation temperature, both the $h_r\;and\;{\Delta}p_f$. found to be lower. Based on the present data, the empirical correlations are provided in terms of the Nusselt number and friction factor.

  • It would be helpful to evaluate the potential flood damage and compare quantitatively with each other when establishing the regional flood countermeasure and determining the execution of the restoration works and emergency action plans. The Flood Damage Index (FDI) in Korea, possible to estimate localized potential risks caused by flood damages, therefore, was proposed in this study. It was considered with the scale of regional flood damages including the regional characteristics and quantitative grounds. First, the four significant causes were categorized as natural, social, politic, and facilitative ones. And the eleven selected factors representing four causes were determined. Finally, the FDI was obtained by the weighting linear summation of the corrected 11 factors multiplied by the weighting values based on the professional questionnaires. Employing the FDI, the potential risk analysis about flood damages for 229 cities and counties in Korea was conducted. These results would be utilized as the essential basis for more rational and practical countermeasures and plans against flood damage.

Macroscopic and microscopic observation of white spot in carapace of imported white leg shrimp Litopenaeus vannamei (수입산 냉동 흰다리새우(Litopenaeus vannamei) 두흉갑의 흰 반점 육안 및 현미경 관찰)

  • In this study, white spots appearing in carapace of imported white leg shrimp Litopenaeus vannamei were observed by macroscopic and microscopic examination. White spots due to white spot disease showed two types, white spots with a regular circular shape ranging from 0.4 mm to 1.7 mm and white spots with an irregular shape ranging from 0.05 mm to 2 mm. White spots due to other causes showed four types that appeared as crystalized materials in range of 100 ㎛ around the tegumental gland, as polygonal inorganic deposits ranging from 58 ㎛ to 188 ㎛, as Linear inorganic deposits ranging from 0.8 mm to 9 mm, as brownish spots ranging from 0.1 mm to 0.4 mm, and brownish spots containing melanin ranging from 0.1 mm to 2 mm. As a result, white spots occurred in imported white leg shrimp Litopenaeus vannamei were divide two type, due to white spot disease or other causes, for quarantine works.

Clinical Ewperiences of Rib Fixation Using Judet's Strut (Judet's strut를 사용한 늑골 고정술의 임상적 고찰)

  • For the purpose of evaluation of clinical characteristics in multiple rib fracture due to accident , 24 cases treated by surgical rib fixation using Judet`s strut for multiple rib fracture and flail chestduring the period from June 1993 to October 1994 were reviewed. There were 17 males and 7 females.They ranged in age from 19 years old to 56 years old. The causes of rib fracture were traffic accident in 18 cases, fall down in 3 cases, compression in 2 cases stab wound in 1 case. The number of rib fracture were five in 7 cases, six in 5 cases, four in 5 cases, three in 3 cases. Associated intrathoracic injuries were hemopneumothorax in 12 cases, hemothorax in 10 cases, lung laceration or hemorrhagic contusion in 7 cases. Associated extrathoracic injuries were abdominal injuries in 21 cases, orthopedic problem in 7 cases, head trauma in 4 cases. The most common fractured site was posterolateral portion of the ribs. The causes for operation were flail chest, severe rib displacement and pain, hemothorax or hemopneumothorax with continuous air leakage and stab wound. There were 6 postoperative complications ; one with hydrothorax, two with fibrothorax, two with wound infection and one case of death due to multiful organ failure. Postoperatively, all patients became comfortable and complained less painful. Twenty patients restored spontaneous breathing without ventilator support, three patients were ventilated during a day and one patient expired after 2 days. There were no morbidity and mortality related to operation.
