환축에서 분리된 Escherichia coli의 plasmid-mediated quinolone resistance genes 분포도 조사 (Prevalence of Plasmid-Mediated Quinolone Resistance Genes in Escherichia coli Isolated from Diseased Animals in Korea)

  • 본 연구는 동물유래 E. coli에서 plasmid-mediated quinolone resistance (PMQR) 유전자의 분포도를 조사하였다. 55주의 E. coli를 대상으로 PCR을 수행한 결과, PMQR 양성균은 11주이었으며 그 중 2주는 두 개의 PMQR 유전자를 동시에 가지고 있었다. PMQR 유전자의 염기서열 분석 결과, qnrS, aac(6')-Ib-cr, qepA로 확인되었고 qnrS (1.8%)와 aac(6')-Ib-cr (7.3%) 보다는 qepA (14.5%)가 높은 분포도를 나타내었으며, qnrA와 qnrB는 검출되지 않았다. 11주의 PMQR 양성균의 MIC를 측정한 결과, 대부분의 PMQR 양성균이 검사한 항생제에 내성을 보였지만 일부에서 감수성 균주도 확인되었다. 또한, aac(6')-Ib-cr 유전자는 단독으로 존재할 때 보다 qnrS 또는 qepA와 함께 존재할 때 내성이 더욱 증가함을 알 수 있었다. 내성유전자 전달능 실험에서 11주 모두 PMQR 유전자를 전달하였으며, PMQR 유전자를 전달받은 수용체에서 공여체와 동일한 내성 phenotype 및 PMQR 유전자를 확인 할 수 있었다.

Prevalence and antimicrobial resistance of Klebsiella species isolated from clinically ill companion animals

  • Background: Klebsiella spp. is an important conditional pathogen in humans and animals. However, due to the indiscriminate use of antibiotics, the incidence of antimicrobial resistance has increased. Objectives: The purpose of this study was to investigate antimicrobial resistance in strains of Klebsiella strains and the phylogenetic relatedness of extended-spectrum cephalosporin (ESC)-resistance among Klebsiella strains isolated from clinically ill companion animals. Methods: A total of 336 clinical specimens were collected from animal hospitals. Identification of Klebsiella species, determination of minimum inhibitory concentrations, detection of ESC resistance genes, polymerase chain reaction-based replicon typing of plasmids by conjugation, and multilocus sequence typing were performed. Results: Forty-three Klebsiella strains were isolated and, subsequently, 28 were identified as K. pneumoniae, 11 as K. oxytoca, and 4 as K. aerogenes. Eleven strains were isolated from feces, followed by 10 from ear, 7 from the nasal cavity, 6 from urine, 5 from genitals, and 4 from skin. Klebsiella isolates showed more than 40% resistance to penicillin, cephalosporin, fluoroquinolone, and aminoglycoside. ESCresistance genes, CTX-M groups (CTX-M-3, CTX-M-15, and CTX-M-65), and AmpC (CMY-2 and DHA-1) were most common in the K. pneumoniae strains. Some K. pneumoniae carrying CTX-M or AmpC were transferred via IncFII plasmids. Two sequence types, ST709 and ST307, from K. pneumoniae were most common. Conclusions: In conclusion, this is the first report on the prevalence, ESCresistance genotypes, and sequence types of Klebsiella strains isolated from clinically ill companion animals. The combination of infectious diseases and antimicrobial resistance by Klebsiella in companion animals suggest that, in clinical veterinary, antibiotic selection should be made carefully and in conjunction with the disease diagnosis.

Characterization of antimicrobial resistance and application of RFLP for epidemiological monitoring of thermophilic Campylobacter spp. isolated from dogs and humans in Korea

  • An antimicrobial susceptibility test was conducted to compare the resistance rates among Campylobacter spp. isolates from dogs (n = 50) raised under diverse conditions and humans (n = 50). More than 60% of Campylobacter (C.) jejuni from dogs and humans showed resistance to nalidixic acid, enrofloxacin and ciprofloxacin. C. jejuni isolates from humans showed higher resistance to tetracycline (83.3%) and ampicillin (91.3%) than those from dogs. None of the C. jejuni or Campylobacter coli isolates from humans or dogs were resistant to erythromycin. Overall, 85% of Campylobacter spp. isolates showed a multidrug resistant phenotype. Nucleotide sequencing analysis of the gryA gene showed that 100% of $NA^R/CIP^R$ C. jejuni isolates from dogs and humans had the Thr-$86^{th}$-Ile mutation, which is associated with fluoroquinolone resistance. flaA PCR restriction fragment length polymorphism (RFLP) typing to differentiate the isolates below the species level revealed 12 different clusters out of 73 strains. The human isolates belonged to eight different RFLP clusters, while five clusters contained dog and human isolates.

Molecular subtyping and antimicrobial susceptibility of Streptococcus dysgalactiae subspecies equisimilis isolates from clinically diseased pigs

  • Background: Streptococcus dysgalactiae subspecies equisimilis (SDSE) acts as an etiological agent for lameness, neurological signs, and high mortality in pigs. Despite its importance in pig industries and zoonotic potential, little is known about the effects of this pathogen. Objectives: This study aimed to determine the molecular characteristics and antimicrobial resistance of SDSE strains isolated from diseased pigs. Methods: A total 11 SDSE isolates were obtained from diseased pigs. Bacterial identification, PCR for virulence genes, emm typing, and antimicrobial resistance genes, multilocus sequence typing, and antimicrobial susceptibility test were performed. Results: Nine isolates were from piglets, and 8 showed lameness, sudden death, or neurological signs. The isolates were PCR-positive for sla (100%), sagA (100%), and scpA (45.5%), and only 1 isolate amplified the emm gene (stL2764). Eight different sequence types were detected, categorized into 2 clonal complexes and 4 singletons. All the isolates in this study were included in a small cluster, which also contained other strains derived from humans and horses. The minimum inhibitory concentrations for the tested beta-lactams were low, while those for macrolides, tetracyclines, and fluoroquinolones were relatively high. PCR analysis of the macrolide and tetracycline resistance genes demonstrated that the isolates carried erm(B) (18.2%, n = 2), mef(A/E) (9.1%, n = 1), tet(M) (18.2%, n = 2), and tet(O) (90.2%, n = 10). Two isolates presented a mutation in parC, which is associated with fluoroquinolone resistance. Conclusion: This study provided insight into swine-derived SDSE, as it is related to veterinary medicine, and elucidated its zoonotic potential, in the context of molecular epidemiology and antimicrobial resistance in public health.

  • Twenty isolates resistant to seven quinolones were isolated from major rivers in Korea. All isolates had three mutations, Ser83$\rightarrow$Leu and Asp87$\rightarrow$Asn in GyrA and Ser80$\rightarrow$Ile or Ser80$\rightarrow$Arg in ParC and three isolates had an additional mutation Glu84$\rightarrow$Gly or Glu84$\rightarrow$Val in ParC. In addition, a clonal spread was not found in these isolates.

Anti-inflammatory and Anti-bacterial Effects of Aloe vera MAP against Multidrug-resistant Bacteria

  • Multidrug-resistant Acinetobacter baumannii and Pseudomonas aeruginosa are highly dangerous nosocomial pathogens, cause the symptoms of skin infections, pressure sores, sepsis, blood stream and wound infections. Unfortunately, these pathogens are immune to the most common antibiotics, such as, carbapenem, aminoglycoside and fluoroquinolone. Therefore, it is imperative that new and effective antibiotics be developed. In the present study, the antimicrobial effects of Aloe vera MAP (modified Aloe polysaccharide) on Staphylococcus aureus and Bacillus subtilis, Escherichia coli and Enterobacter aerogenes, and clinical Pseudomonas aeruginosa and clinical Acinetobacter baumannii were comprehensibly investigated. Prior to the growth inhibition effect measurement and antibiotic disc diffusion assay on gram-positive and gram-negative bacteria and selected multidrug-resistant Pseudomonas aeruginosa and Acinetobacter baumannii, antimicrobial resistance screening was performed for the multidrug-resistant bacteria obtained from clinical isolates. The results for showed the Aloe vera MAP had a concentration-dependent effect on all of examined bacteria, particularly on Pseudomonas aeruginosa. Anti-inflammatory and anti-oxidant experiments were also performed dose dependently effects to confirm the beneficial physiological effects of Aloe vera MAP.

닭에서 동정된 플르오르퀴놀론 내성 대장균 균주의 분자생물학적 성상에 관한 연구 (Molecular Characterization of Fluoroquinolone Resistant Escherichia coli Isolates from Chickens in Korea)

  • 본 연구에서는 한국의 닭에서 분리된 E. coli 균주들로부터 퀴놀론계 항생제 내성을 나타내는 균주를 분리 동정하고 그 내성 기전과 유병률에 관하여 조사하였다. 또한 multilocus sequence typing (MLST)을 이용하여 E. coli 균주들의 분자생물학적 성상을 분석하였다. 항생제 감수성 테스트에서 63.5% (54/85) 의 E. coli 균주들에서 퀴놀론계 항생제 내성률을 보였다. 또한 퀴놀론계 항생제 내성을 보이는 54개 모두에서 gyrA 유전자의 sense mutations과 parC 유전자의 $57^{th}$, $80^{th}$, or $84^{th}$residues에서 점돌연변이를 관찰할 수 있었다. MLST를 통한 분석에서 E. coli ST는 parE 유전자의 염기치환과 깊은 상관관계를 보이는 것으로 관찰되었다. 이 결과들을 바탕으로 우리가 먹는 가축 및 가금류에 대한 무분별한 항생제 사용은 항생제 내성균의 증가와 유전변이를 초래함을 알 수 있었다. 따라서 식용 동물에 대한 지속적인 감시와 모니터링을 통하여 항생제 내성균의 확산방지를 통제하는 것이 필요할 것으로 사료된다.

결핵균에서 gyrA 유전자 돌연변이에 따른 Fluoroquinolone계 약제들의 교차내성 (Cross Resistance of Fluoroquinolone Drugs on gyrA Gene Mutation in Mycobacterium tuberculosis)

  • 배 경 : FQ계 약제는 다제내성 결핵환자들의 치료를 위한 주용 2차 항결핵약제의 하나이다. 그러나 FQ계 여러 약제들 간 교차내성정도와 내성관련 유전자인 gyrA의 돌연변이간 관련성을 조사하게 되었다. 방 법 : 결핵균 중에서 OFX 내성인 63균주와 감수성인 10균주를 대상으로 Lowenstein-Jensen 배지에 CIP, LVX, MXF, GAT, SPX 등에 대한 교차내성을 조사하였다. 퀴놀론계 각 약제를 $0.5-3{\mu}g/ml$ 농도로 첨가한 배지에서, $2{\mu}g/ml$ 이상에서 균이 발육한 경우를 내성으로 판정하였다. OFX 내성인 균주의 gyrA 및 gyrB 유전자의 돌연변이가 잘 발생하는 곳을 염기서열 분석하여 약제별로 그 교차내성 양상을 비교하였다. 결 과 : OFX 내성인 63균주 모두가 CIP약제에서는 교차내성을 나타내었고, LVX에는 90.5%, MXF에는 44.4%, GAT에는 36.5%, SPX에는 46.0%가 교차내성을 보였다. 이들 내성균주의 81.0%가 gyrA 유전자에 돌연변이를 가지고 있었으며, 그 분포는 아미노산 88번, 90번, 91번, 94번에서 나타났다. 그 중에서도 Gly88Ala, Ala90Val, Asp94Ala 돌연변이는 제3세대 FQ계 약제인 MXF, GAT, SPX에 감수성을 나타내는 경향이 있었다. 한편 gyrB 유전자에서는 63개 OFX 내성균주 중 4균주만이 돌연변이를 나타내었으나, 이들과 gyrB 유전자내 돌연변이가 없는 균주들 사이에서 FQ의 내성에 관련해서 차이가 없었다. 결 론 : OFX 내성균 중에서 약 60% 정도가 제3세대 FQ계 약제에서 감수성을 보였다. 따라서 이 들 약제에 대해서는 별도로 감수성검사를 실시하여 처방에 활용한다면, OFX 내성환자의 치료를 크게 향상시킬 수 있을 것으로 판단되었다.

CTX-M-15형 Extended Spectrum β-lactamase와 ArmA 동시 생성 Enterobacter cloacae의 출현 (Emergence of CTX-M-15 Extended Spectrum β-lactamase and ArmA-Producing Enterobacter cloacae)

  • 본 연구에서는 세균의 항균제 내성기전을 연구하기 위해 일개의 대학병원에서 분리된 Enterobacter cloacae를 대상으로 extended spectrum ${\beta}$-lactamase (ESBL) 및 16S rRNA methyltransferase 유전자를 검출하고 항균제 감수성 양상을 조사하였다. 대상균주 중 총 8 균주가 CTX-M-15형 ESBL을 생성하는 것으로 확인되었으며 이 균주들 중 3 균주는 16S rRNA methyltransferase의 한 종류인 armA 유전자도 동시에 가지고 있는 것으로 나타났다. CTX-M-15형 ESBL 유전자와 armA 유전자를 동시에 가지고 있는 E. cloacae는 3세대 cephalosporin 계열 및 aminoglycoside 계열의 항균제 뿐 만 아니라 fluoroquinolone 계열의 항균제에도 내성을 보였다. 더구나 이러한 항균제 내성 유전자들은 플라스미드를 통해 다른 세균으로 전달 될 수 있어 다제내성 세균의 출현 및 확산을 촉진 할 수 있다. 따라서 E. cloacae를 대상으로 지속적인 항균제 내성 유전자를 모니터링 하는 것은 항균제 내성 확산방지를 위해 중요할 것으로 사료된다.

Factors Affecting in Vitro Activity of LB20304, a New Flu-oroquinolone

  • LB20304 is a novel fluoroquinolone that exhibits a potent broad spectrum antibacterial activity against both gram-positve and gram-negative bacteria. The MICs (Minimal Inhibitory Concentration) of LB20304 were determined against both gram-positve and gram-negative bacteria under various conditions including several media, pHs, and inoculum concentrations. The in vitro activity of LB20304 was not significantly affected by the changes in testing conditions such as components of media and inoculum concentrations, but it was slightly reduced by acid condition. The MICs and MBCs (Minimal Bactericidal Concentration) of LB20304 against Staphylococcus aureus, Escherichia coli and Pseudomonas aeruginosa were hardly affected by the presence of 50 % human serum, mouse serum, guinea pig serum or horse serum, and the MBCs were equal to or at most four-times higher than the MiCs. The activities of LB20304 were decreased by the presence of high concentraion of $Mg^{++}$ or human urine (pH, 5.5) in the test media. The frequencies of mutants resistant to LB20304 were similar to or lower than those found in ciprofloxacin and sparfloxacin.
