A SURVEY ON THE USING STATUS AND PERCEPTION OF PIT AND FISSURE SEALANT (치면열구전색제 사용실태와 인식에 관한 조사)

  • The property of pit and fissure sealant has been improved and many studies on the bond strength, penetration, microleakage have been published. But there are few studies on the using status and perception of pit and fissure sealant within the country. Therefore, this study made a survey on it. Pedodontists and non-pedodontists were surveyed by interview. The Results were as follows; 1. On caries prevention effect, 96.7% of the pedodontists replied that sealants were effective on both permanent teeth and primary teeth. On the other hand, 13.5% of the non-pedodontists replied that sealants weren't effective on both. 2. All of the pedodontists and 27% of the non-pedodontists used rubber dams. 83.3% of the pedodontists and 40.5% of the non-pedodontists used bonding agents. 3. Non-pedodontists used enameloplasty more frequently than Pedodontists but the pattern was not significantly different. 4. The causes of sealant failures included salivary contamination, caries under sealant, low strength, low flowability, overfilling. 5. In the pedodontists, 90% replied that PRR application was desirable and PRR applications were more frequent than sealant application.

Measurements of the Palpebral Fissure (안검열의 측정)

  • The palpebral fissures of 1529 healthy Koreans ranging in age from one month to 92 years were examined. The length and width of palpebral fissure, distance of folded upper lid(FUL), inter outer canthal distance(IOCD) and appearance of double upper lid (DUL) were studied. The most commonly occuring length of the normal palpebral fissure was 30 mm(13.7%) followed by 28 mm(9.8%) and 27 mm(8.8%), and almost four fifths(79.1%) of the palpebral fissures measured less than 30mm in length. Of the ages 11~60 examined over 70 % averaged palpebral fissures measuring bet ween 27 mm and 32 mm. The commonest width of 1443 normal palpebral fissure was 8 mm(28.2%) followed by 9mm(16.3%) and 7mm(15.0%), and average widths according to 5 major age groups were 5~8mm, 6~10mm, 7~11mm, 7~11mm and 5~9mm, and there is some tendency to a decrease with age. The average of FUL without frontalis action was 8~11 mm. The distances between exocanthions, IOCD, according to age groups were estimated to be 77~82mm for one to 10, 94~101mm for 11 to 60 and 89~95mm for over-60 age group, respectively. The DUL was appeared in 44 % of sample, and DUL in female was more than in male.

Effect of Physical Properties and Bacterial Adherence Inhibition of Pit and Fissure Sealant Containing Bioactive Glass Nano Particles(BGn) (생체활성 유리 나노입자 첨가량에 따른 치면열구전색제의 물성평가와 세균부착 억제 효과)

  • In this study BGn-incorporated non-fluoride release of pit and fissure sealant $Concise^{TM}$ was developed to improve the mechanical properties and promote antibacterial effect of fit and fissure sealant with the original material. The mechanical properties and antibacterial activity of BGn incorporating vary-ing amounts bioactive glass nano particles(BGn) (0,0.5,1.0 and 2.0 wt% in sealant) were characterized composition of the resulting were investigated. The solubility to aid absorption was calculated by weighing specimens with a diameter of 10 mm and a thickness of 2 mm according to ISO 4049 (2009). The antimicrobial effect was evaluated using three strains of S. mutans, S. aureus and E. coli. The absorbance of the test results was as high as the addition of BGn increased, and the lower the solubility as the solubility was added(p<0.05). Adhesion experiment results S. mutans in contrast to the control group $Concise^{TM}$, BGn-added experimental group showed a somewhat lower adherent surface but no statistically significant difference was observed (p<0.05). However S. aureus and E. coli statistical analysis indicated a significant difference for antibacterial agents between control and BGn containing(p<0.05). It seems that this BGn proved that even a antibacterial effect was demonstrated. Therefore, it was suggest that the additional effects of BGn and research on a wide range of substances.

BUBBLE BEHAVIOR OF PIT AND FISSURE SEALANT (치면열구전색제의 도포 시 전색제에 따른 기포 발생 양태)

  • In clinical practice, air bubbles trapped in the pit and fissure may increase early loss of sealing materials for fracture, wear and microleakage. The purpose of this investigation was to examine the bubble behavior of pit and fissure sealant. The 140 replicas made of epoxy resin were used to this experiment. Following conditioning, light-polymerized sealants were applied and then exposed to the light source. After stereoscopic examination of standarized specimen by grinding, bubble behavior was analysed. The results obtained were as follows; 1. Ultraseal $XT^{(R)}$ plus grops irrespective of using time were higher than groups of $Helioseal^{(R)}$ with clinpro tip and metal tip in the frequency of bubble(p<0.05), 2. Ultraseal $XT^{(R)}$ plus old group was more than $Clinpro^{(R)}$, Teethmate $F-l^{(R)}$ and $Helioseal^{(R)}$ with brush tip in the number of bubble under 200 magnified cross section(p<0.05). 3. The widest mean area of bubble was shown in the Teethmate $F-1^{(R)}$. 4. No statistically significant difference of the frequency and the site of bubble between $Clinpro^{(R)}$ and $Helioseal^{(R)}$ groups(p>0.05).

EVALUATION OF THREE FLOWABLE COMPOSITES AS PIT AND FISSURE SEALANTS WITHOUT USING BONDING AGENTS (접착제 미 사용시 치면열구전색제로서의 유동성 복합레진 평가)

  • The aims of this study were to determine if flowable composites can be used as pit and fissure sealants without bonding agents. Three flowable composites(Filtek Flow, Tetric Flow, Charmfil Flow) and a filled sealant (Ultraseal XT plus) were used. The pattern of resin tag formation in the four sealant materials were compared using SEM. For the microleakage assessment, 54 extracted human premolar teeth were randomly divided into 3 groups containing 18 premolars each. In each group, a conventional filled sealant and one of the three flowable composites were applied to occlusal fissures. The teeth were thermocycled(1200 cycles between $5^{\circ}{\pm}2^{\circ}C\;and\;55^{\circ}{\pm}2^{\circ}C$ with a dwell time of 30 seconds) and immersed in a 1% methylene blue solution for 48 hours. Each tooth was sectioned and examined to determine the extent of dye penetration. Three flowable composites and a filed sealant showed a similar resin tag formation pattern. The three flowable composites showed significantly more microleakage in each group than the filled sealant. The level of microleakage was similar in the three flowable composites. Flowable composites are not recommended as pit and fissure sealants because more microleakage can occur even when occlural fissures are mechanically widened.

INFLUENCE OF THE ENAMEL TREATMENT WITH ER:YAG LASER ON THE MICROLEAKAGE OF PIT AND FISSURE SEALANT (Er:YAG 레이저를 이용한 법랑질 표면처리가 치면열구전색제의 미세누출에 미치는 영향)

  • The aim of this study was to assess the microleakage underneath a pit and fissure sealant bonded to occlusal surfaces treated by Er:YAG laser To determine the most effective energy density of laser, fourteen specimens were irradiated from 50mJ to 300mJ at 3Hz. After irradiation, the lased specimens were observed under the scanning electron microscope. Thirty six non-carious extracted premolars were randomly assigned to four groups of nine teeth: group 1, no treatment on the occlusal surface; group 2, acid etching for 15 seconds; group 3, Er:YAG laser irradiation; group 4, acid etching followed by Er:YAG laser irradiation. The pits and fissures were sealed with unfilled sealant(Helioseal F) and the specimen teeth were thermo-cycled, immersed in 2% Rhodamine B solution, longitudinally sectioned and analyzed for microleakage with fluorescent microscope. The results were as follows: 1. Er:YAG lased surfaces with 50mJ, 3Hz showed a similar pattern of irregularity with acid etched enamel surfaces 2. The mean microleakage score increased in the order of group 2, 4, 3 and 1. There was no significant difference among group 1, 3 and 4(p>0.05), however group 2 showed significantly less microleakage compared with group 1 and 3. Conclusively, the laser irradiation seemed not enough to replace the acid etching for proper retention of pit and fissure sealants.

A Photogrammetic Study of the Eyes in Korean Youths (한국 청년층 눈의 사진 계측학적 연구)

  • Purpose: Photogrammetry has been introduced as an alternative to direct measurement to obtain facial distances for a variety of anthropometric applications. The aim of this study is to establish morphological and functional value of an eye in Korean youths using specially designed computer software with digital photographic images. Methods: Authors measured anthropometry of an eye in Korean youths 1,449 individuals(622 males, 827 females) and analyzed stastically them. Results: The mean measurements are as follows. The diameter of cornea was $11.6{\pm}0.9mm$ in males and $11.3{\pm}0.9mm$ in females. The vertical dimension of palpebral fissure was $7.4{\pm}1.7mm$ in males and $8.2{\pm}1.6mm$ in females. The intercanthal width was $35.8{\pm}3.2mm$ in males and $34.4{\pm}2.9mm$ in females. The biocular width was $90.4{\pm}5.7mm$ in males and $87.4{\pm}5.3mm$ in females. The endocantion-superior margin of palpebral fissure distance was $12.7{\pm}1.4mm$ in males and $11.6{\pm}1.4mm$ in females. The horizontal dimension of palpebral fissure was $28.5{\pm}2.2mm$ in males and $27.4{\pm}1.9mm$ in females. The slant of palpebral fissure was $10.7{\pm}2.6^{\circ}$ in males and $11.3{\pm}3.0^{\circ}$ in females. The height of upper eyelid was $10.6{\pm}2.3mm$ in males and $10.7{\pm}2.3mm$ in females. The width of double fold was $2.0{\pm}0.7mm$ in males and $1.9{\pm}0.6mm$ in females. A double fold was seen in 21.9% of males and 44.9% of females. The most frequent shape of the double fold was type IV. The most common type of epicanthal fold was type III in both males and females. Conclusion: Photogrammetic measurement of large population using specially designed software could offer reliable data and these data could be useful reference for preoperative consultation, surgical planning and anthropometic study of eye.

The Effect of Children's Beverages on Degradation of Dental Resin-Based Pit and Fissure Sealant (어린이 음료가 레진계 치면열구전색제의 화학적 분해에 미치는 영향)

  • The consumption of beverages among children is rising. The purpose of this study was to examine the effect of kid's drink on dental resin-based pit and fissure sealant. Pororo, I-kicker, Sunkist kids were included in the experimental groups, and Samdasu was included in the control group. A conventional dental sealant material ($Clinpro^{TM}Sealant^{(R)}$) was selected for this study. Resin specimens (8 mm in diameter and 1 mm in thickness) were prepared according to manufacturers' instructions and the initial roughness (Ra) was then measured. The pH of all the four groups was measured using a pH meter. The specimens were individually immersed in 5 ml of the experimental solutions and stored at $37^{\circ}C$ for 72 hours. Following this, the surface roughness of the resin specimens was measured by Surftest. The concentration of residual monomer released was determined by high performance liquid chromatography (HPLC). The surface morphology of the resin specimen was evaluated before and after storage by scanning electron microscopy (SEM). Data were statistically analyzed using Kruskal-Wallis and Duncan's test. The results showed that all the children's beverages examined in this study contained citric acid. The pH of I-kicker was the lowest ($3.03{\pm}0.01$), followed by that of Sunkist kids ($3.26{\pm}0.02$) and Pororo ($3.47{\pm}0.02$). We observed an increase in the surface roughness of resin specimens after 72 h of immersion in all the beverages tested (p<0.05). There was matrix degradation after immersion, visualized on SEM image, in all the beverage groups. Bisphenol-A-glycidyl methacrylate was not detected after 72 hours, but triethylene glycol dimethacrylate levels were increased in all the beverages tested during the 72 hours by HPLC. These results suggest that intake of beverages containing acid can cause degradation of the resin-based pit and fissure sealants in children.

The caries preventive effect on permanent teeth adjusted for number of fissure-sealed teeth and surfaces from 6-year community water fluoridation program in Geoje City (거제시 수돗물불소농도조정사업 6년 경과 후의 영구치우식 예방효과)

  • Objectives: The water fluoridation program has been implemented since 2008, in a region of Geoje City. This study aimed to evaluate the effectiveness of the water fluoridation program on caries prevention in the permanent dentition. Methods: Evaluation surveys were conducted on 8-, 10- and 12-year-old children in the fluoridated and non-fluoridated regions of Geoje City, in 2015. The survey included 834 children from the fluoridated regions and 703 from the non-fluoridated regions. The data obtained from the survey were analyzed using the IBM SPSS statistical package version 23.0. The effectiveness of community water fluoridation in caries prevention was estimated by the differences in the decayed, missing, filled teeth (DMFT) and decayed, missing, filled surfaces (DMFS) scores, between the program and control groups. The mean numbers of fissure-sealed teeth and surfaces were adjusted to reduce their confounding effect. Results: The mean DMFT and DMFS scores (0.69 and 0.91, respectively) of 10-year-olds from the fluoridated regions, after adjusting for the numbers of fissure-sealed teeth and surfaces, were significantly lower than those of the 10 year olds (DMFT: 0.95, DMFS: 1.32) from the non-fluoridated regions. The effectiveness of water fluoridation on caries prevention in the permanent dentition, estimated based on the differences in the mean DMFT and DMFS scores, adjusted for the number of fissure-sealed teeth and surfaces, between the program and control groups of 10-year-old children, was 27.4% and 31.1%, respectively. Conclusions: These results suggest that the effectiveness of water fluoridation on caries prevention in the permanent dentition, in Geoje City was so high that this program should be implemented in other regions in Korea as well.
