도서형 신경 혈관 피판을 이용한 수지의 피부 및 연부조직 결손의 재건술 (Neurovascular Island Graft for Finger Tip Loss)

  • Purpose : Loss of sensibility over the finger tip resents a grave deficit and is an indication for sensible soft tissue reconstruction. This paper was performed to assess the long term results obtained by nerovascular island flap. Material and Methods : We performed neurovascular island graft for defective sensibility of finger tip loss in 94 cases since 1979 to 2000. The recipient sites were the thumb pulp defect in 79 cases, the amputated thumb in 9 cases, the amputated index in 4 cases, and the velar aspect of interphalangeal joint of thumb in 2 cases. The donor flaps were obtained from the radial side of ring finger in 63 cases, the ulnar side of the ring finger in 21 cases, and the ulnar side of the middle finger in 10 cases. A mean follow-up period was 5.7 years. Results : The flap quality was well vascularized and survived in 89 cases. The two-point discrimination was average 8.7mm. Because of scar contracture, the range of motion of the donor finger was decreased 3.5% of the normal finger in the distal interphalangeal joint, 8,2% in the proximal interphalangeal joint. A phenomenon of double sensibility occurred in 66 cases. Conclusion : This technique was excellent both aesthetically and functionally as a reconstruction of the Loss of fingertip.

손의 형태학적 특성과 손감성지수와의 상관관계 (Relationship between morphological characteristics and hand emotional quotient of hands)

  • 20~23세의 한국 여자대학생을 대상으로 아름다운 손에 대한 손의 형태적 특성과 감성과의 상관관계를 평가하였다. 감성평가 방법으로 손의 아름다움을 각각 10 등급으로 분류하고 이를 손의 형태학적 특성과 감성평가 등급파의 상관관계를 정성적 및 정량적으로 분석하는 방법을 적용하였다. 감성평가에 가장 많은 영향을 미치는 인자는 손 길이와 손 폭의 비이었고, 손 길이와 손가락 길이 및 손가락 굵기의 비율도 많은 영향을 주었다. 반면에 손 갈이와 손톱 길이의 비와 손가락 길이와 손톱 길이의 비는 감성평가에 마치는 영향이 상대적으로 미미하였다. 높은 감성평가를 받은 손(아름다운 손)의 특정은 손, 손가락 및 손톱의 길이가 모두 길고, 손과 손가락의 폭이 좁으며, 특히 낮은 감성평가를 받은 손에 비해 상대적으로 손톱의 길이가 모두 길고, 손과 손가락의 폭이 좁으며, 특히 낮은 감성평가를 받은 손에 비해 상대적으로 손톱의 길이가 더 길었다. 결과적으로 손의 모든 형태학적 특성들이 종합적으로 감성에 영향을 미친다는 것을 알 수 있었다. 이러한 연구 결과는 손의 형태학적 특성을 감성적으로 평가한 최초의 정량 및 정성적 보고로 손의 미적 요소에 관한 기초자료를 구축하였고, 이들 정보들은 손과 관련된 다양한 뷰티산업 제품 및 서비스 분야에 활용이 가능하다.

젖소에서 초음파기기를 이용한 난자 채취에 있어서 손가락 촉지를 이용한 난포란의 채란 (Study on Ovum Pick-up(OPU) with Finger-Sensibility using Oocyte Recovery in Holstein Heifers)

  • This study was conducted to develop an improved method for oocyte pick-up(OPU) with finger-sensibility using ultrasound-guidance from ovarian follicles in Holstein heifers. Oocytes were aspirated from ovarian follicles of clear-outline (>2mm), obscure-outline and invisible($\leq$ 2mm) on ultrasound images with 3 different vacuum pressure(40, 80, 120mmHg). Total number of oocytes recovered/follicles were 309/237(130.4%). 113/80(141.3%) and 107/74(144.6%) with 40, 80 and 120 mmHg of vacuum pressure, respectively. Mean number of oocytes recovered was higher in 2 OPU/week (18.3$\pm$5.3) than 1 OPU/week(14.5$\pm$4.1), but this difference was not statistical1y significant. The recovery rates were not affected by the number of OPU as 135.6%(282 oocytes/208 follicles) in 1~20 OPU, 137.7% (168/122) in 21~40 OPU and 148.4%(92/62) in 41~60 OPU, respectively. The proportions of good oocytes (Grades I) recovered were not significantly different by the number of OPU until 40 OPU(12.4% in 1~20 OPU vs 16.7% in 21~40 OPU). However, a significantly(P<0.05) lower recovery rate resulted from more than 40 OPU compared to less than 40 OPU(7.6%). These results imply that more fertilizable oocytes can be produced from invisible-immature follicles by transvaginal aspiration with finger-sensibility from Holstein heifers.

  • Human uses sight, tactile sense to evaluate Total Hand Value(FUUAI) of textile fabrics. Tactile sense is important factor which decided the Total Hand Value of a textile fabric. When human feels the FUUAI, physical and physiological phenomena are occurred in finger. We first found out physical variable that is happened in fingertip when human is feeling the FUUAI. Such physical variable means characteristic of action tactual motion of finger such as moving range, tactile time, moved distance, speed of finger and applied force by finger. We study the relationship between action tactual motion and the ability in which the human distinguishes the textile fabric. As a result, we could know the characteristics of the tactual motion of fingertip to get high distinguishable ability. The characteristics were different in men and women respectively. In the case of man, touched time and moving range influenced to distinguish, and moving range, and the moving speed of finger influenced, in woman's case.

  • 본 연구는 집중력, 기억력 및 학습 능력의 뇌기능 증진을 위한 의식적인 손 운동에 관련된 연구이다. 우선 효과적인 재활을 위한 손가락 운동 패턴을 연구하였다. 단순한 손가락 운동(Simple Finger Movement ; SFM) 패턴과 의식적인 손가락 운동(Intentional Finger Movement ; IFM)패턴을 비교하였다. 다음으로 각각 두 패턴 운동을 시켜 피험자의 집중력과 학습 능력의 증진을 검증하고자 한다. SFM 패턴과 IFM 패턴의 비교와 집중력과 학습 능력의 증진의 검증은 뇌파(mid $\alpha$파)를 이용하였다. 실험은 먼저 SFM 패턴의 운동을 시키고 다음에 IFM 패턴의 운동을 시키는 실험을 하였다. 결과적으로 IFM 패턴에서 mid $\alpha$ 파의 증가가 이루어졌음을 측정함으로써, IFM 패턴이 뇌의 집중력과 학습 능력을 증진시킨다는 결과를 얻었다.

  • The friction behavior of human skin is determined by the complex interplay of the material and surface properties of the skin, as well as the contacting material, and strongly depends on the contact parameters (e.g., pressure and sliding velocity) and the presence of substances such as water, sweat, or skin surface lipids at the interface. Including a study on the effect of a surface's physical roughness for skin sliding over the surface, various studies have been conducted to understand human tactile sensibility. However, to investigate products in relation to human tactile sensibility, more objective research is needed. This study performed sliding experiments between the skin and the surfaces of phone cases to understand how the texture, friction, and stick-slip characteristics are related. Eight phone case surfaces with different topologies and chemical (or mechanical) compatibilities with skin were prepared and tested multiple times.

Innervated Cross-Finger Pulp Flap for Reconstruction of the Fingertip

  • Background Fingertip injuries involving subtotal or total loss of the digital pulp are common types of hand injuries and require reconstruction that is able to provide stable padding and sensory recovery. There are various techniques used for reconstruction of fingertip injuries, but the most effective method is functionally and aesthetically controversial. Despite some disadvantages, cross-finger pulp flap is a relatively simple procedure without significant complications or requiring special techniques. Methods This study included 90 patients with fingertip defects who underwent cross-finger pulp flap between September 1998 and March 2010. In 69 cases, neurorrhaphy was performed between the pulp branch from the proper digital nerve and the recipient's sensory nerve for good sensibility of the injured fingertip. In order to evaluate the outcome of our surgical method, we observed two-point discrimination in the early (3 months) and late (12 to 40 months) postoperative periods. Results Most of the cases had cosmetically and functionally acceptable outcomes. The average defect size was $1.7{\times}1.5$ cm. Sensory return began 3 months after flap application. The two-point discrimination was measured at 4.6 mm (range, 3 to 6 mm) in our method and 7.2 mm (range, 4 to 9 mm) in non-innervated cross-finger pulp flaps. Conclusions The innervated cross-finger pulp flap is a safe and reliable procedure for lateral oblique, volar oblique, and transverse fingertip amputations. Our procedure is simple to perform under local anesthesia, and is able to provide both mechanical stability and sensory recovery. We recommend this method for reconstruction of fingertip injuries.

수지 첨부 절단의 재접합술 (Microsurgical Replantation of Very Distally Amputated Finger)

  • Introduction: Microsurgical replantation of amputated digit have become common procedure in recent years. However replantation of fingertip amputation, Zone I by Yamano classification, is still difficult because digital arteries branch into small arteries and also digital veins are hard to separate from the immobile soft tissue. So, fingertip amputation was covered by volar V-Y flap, composite graft, cross finger flap and groin flap. But patients who have been treated by these methods experience shortening of digit, nail deformity, excessive tenderness and persistent pain. Replantation could solve most of these problems. Material & Methods: In our department, from March 2004 to August 2007, 36 digits in 32 patients with complete amputation at distal to nail base were replanted using a microsurgical technique. Results: The overall survival rate of the replanted finger was 75%. Venous anastomosis was possible in 8 cases and impossible in 28 cases. In latter cases external bleeding technique was applied with medical leech. Conclusion: After replantation, a few patient complained decreased sensibility, nail deformity and cold intolerance. But most of patients were satisfied with the functional and cosmetic appearance of the viable replanted digits. We believe the replantation should be the first choice in fingertip amputation.

P300 숨긴정보검사와 대응수단 (P300-based concealed information test and countermeasures)

  • P300 숨긴정보검사는 전통적인 대응수단에 큰 영향을 받지 않는 것으로 알려져 있다. 본 연구에서는 전형적인 세 자극 프로토콜을 사용한 P300 숨긴정보검사가 새로운 대응수단에 영향을 받는지 검증하였다. 세 가지 유형의 대응수단을 사용하였다. 첫 번째는 순차적 대응수단으로, 무관련자극이 제시될 때마다 왼손 검지손가락에 힘주기, 오른쪽 발가락에 힘주기, 마음속으로 어머니 이름 부르기, 마음속으로 아버지 이름 부르기를 순서대로 하나씩 수행하는 것이었다. 나머지 두 개의 대응수단은 부분 매칭 대응수단으로 네 개의 무관련자극 중 특정한 2개의 무관련자극에만 반응을 하는 것이었다. 두 번째는 행동적 부분 매칭 대응수단으로, 특정한 한 무관련자극이 제시되면 왼손 검지손가락에 힘을 주고, 특정한 다른 무관련자극이 제시되면 오른쪽 발가락에 힘을 주는 것이었다. 세 번째는 정신적 부분 매칭 대응수단으로, 특정한 한 무관련자극이 제시되면 마음속으로 어머니 이름을 부르고, 특정한 다른 무관련자극이 제시되면 마음속으로 아버지 이름을 부르는 것이었다. 실험결과, 순차적 대응수단을 사용하는 것은 예상과는 달리 P300 숨긴정보검사의 정확판단율을 77%에서 92%로 증가시켰다. 부분 매칭 대응수단은 P300 숨긴정보검사의 정확판단율에 부정적인 영향을 미쳤다. 물리적 대응수단은 정확판단율을 77%에서 46%로 감소시켰으며, 정신적 대응수단은 정확판단율을 100%에서 69%로 감소시켰다. 대응수단의 사용을 억제하거나 탐지할 수 있는 기법의 개발 필요성을 논의하였다.
