A STUDY ON DENTAL HEALTH STATUS AND THEIR DECENNIAL COMPARISONS OF KOREAN COLLEGE GIRL STUDENTS (10년 간격으로 관찰한 여대생의 구강보건 상태 변화에 대한 연구)

  • The author conducted a dental survey of college girl students and compared with that of college students 20 years and 10 years ago which based on the same criteria. The items of this survey included the prevalence of dental caries, DMFT and the kinds of restored materials. College girl students 2,294 were examined in May 1988, with the recommended criteria and method of W.H.O.. Among them, the data of 2,243 were analyzed and compared with the data of 1968 and 1978. The following results were obtained : 1. The average number of present teeth was $28.86{\pm}1.65$, which does not show much difference when compared to $29.00{\pm}1.94$ of 1968 and $28.97{\pm}1.64$ of 1978. 2. The prevalence of dental caries was 86.22%, which increased in comparison to 83.25% of 1968 and 84.92% of 1978. 3. DMF index was 19.98, which considerably increased in comparison to 14.56 of 1968 and 15.51 of 1978. 4. The number of DMFT was $5.75{\pm}5.04$ per student. Compared to $4.23{\pm}3.88$ of 1968 and $4.49{\pm}3.69$ of 1978, there is a considerable increase. I) The average number of decayed(D) teeth was $1.14{\pm}1.63$, which decreased from $1.16{\pm}1.57$ of 1968 and $2.09{\pm}2.16$ of 1978. II) The average number of missing(M) teeth was $0.23{\pm}0.73$, which also decreased from $0.69{\pm}1.11$ of 1968 and $0.32{\pm}0.79$ of 1978. III) The average number of filled(F) teeth was $4.39{\pm}4.91$, which increased from $2.38{\pm}3.33$ of 1968 and $2.09{\pm}3.17$ of 1978. 5. According to the analysis of filling material, filling baby including amalgam, gold inlay, resin, silicate cement and paladium inlay was 90.5%, which increased in comparison to 72.8% of 1968 and 83.6% of 1978. Among them, amalgam occupied 62.93%, which showed the most increase. On the other hand, crown and bridge was 6.5%, 3.0%, respectively, which showed continuous decrease from 12.1%, 15.1% of 1968 and 8.7%, 7.7% of 1978. Therefore, the prosthodontic treatment reduced due to the decrease of tooth extraction. In contrast, there is continuous increase of conservative treatment which maintaining natural tooth.

THE EFFECT OF PERIODONTAL REGENERATION AND ANTI-RESTORATION OF DEXAMETHASONE AND OP-1 FOLLOWING DELAYED REPLANTATION IN RAT MODEL (흰 쥐 모델에서 지연재식 시 dexamethasone과 OP-1의 표면처리가 치주조직 재생 및 항흡수 작용에 미치는 효과)

  • The purpose of this study was to observe the effect of dexamethasone and osteogenic protein-1(BMP-7) on bone, cementum and periodontal tissue regeneration. A total of 60 Sprague-Dawley white female mice were selected and beta-APN was used for five days to extract the maxillary first molar a traumatically. After the extraction of the teeth, the mesiobuccal root canal was filled with Caviton$^{\circledR}$. The teeth were etched with citric acid for 1 min and coated with one of four different experimental solutions : DEX(500nM/ml), DEX(1000nM/ml), OP-1(100$\mu\textrm{g}$/ml) and OP-1(500$\mu\textrm{g}$/ml) for three minutes depending on the group. All teeth were then replanted under microscope. All replantation procedures were done within 30 minutes. Teeth that were replanted after 30 minutes of bench dry only was used as positive control. All animals were sacrificed at 3 weeks following replantation and histologic observtion was done. The results were as follows ; 1. Active root resorption rate was decreased by the order of OP-1(500$\mu\textrm{g}$/ml), DEX(1000nM/ml), OP-1(100$\mu\textrm{g}$/ml), and DEX(500nM/ml). There was statistically less root resorption in OP-1 (500$\mu\textrm{g}$/ml) and DEX(1000nM/ml) group(P<0.05). 2. The group with higher concentration of dexamethasone(1000nM/ml) had statistically more bone union compared to positive control group(P<0.05),but there were no significant differences among four experimental groups. 3. OP-1(500$\mu\textrm{g}$/ml) and DEX(1000nM/ml) groups showed less degree of inflammation compared to the OP-1(100$\mu\textrm{g}$/ml). DEX(500nM/ml), and positive control group (P<0.05). In conclusion, the group with higher concentration of OP-1 had the best results on root resorption, bone ankylosis and anti-inflammatory effects compared to the other experimental groups, but a long-term study is also necessary to evaluate the exact pharmacological effects of the drugs in the future.

A SURVEY ON THE CARIES PREVALENCE OF PRESCHOOL CHILDREN IN A SMALL CITY (소도시 거주 미취학 아동의 치아우식 실태조사)

  • The purpose of this study was to investigate the caries prevalence and caries pattern of preschool children in a small city. Six hundred twenty four kindergarten children in Milyang city were examined for their caries experience of individual teeth and surfaces by dmf index. The obtained results were as follows. 1. The rate of children with caries experience(dmft rate) in primary teeth was 74.0% in 3-year-olds, 83.8% in 4-year-olds, and 90.6% in 5-year-olds. The mean number of decayed, missed, and filled primary teeth(dmft index) was 3.47 in 3-year-olds, 5.41 in 4-year-olds, and 6.01 in 5-year-olds. 2. The caries prevalence of children in this study was higher than those of other researches in past and in other cities. 3. The caries-experienced teeth in order in 5-year-olds were as follows : mandibular 2nd primary molar, mandibular 1st primary molar, maxillary 2nd primary molar, maxillary primary central incisor and maxillary 1st primary molar. 4. The pattern of dental caries development was different between the incisors and molars. The prevalent surface of caries was proximal surface in primary incisors, but occlusal surface in primary molars.

THE EFFECT OF FLOWABLE RESIN LINING ON THE MARGINAL MICROLEAKAGE OF CONDENSABLE RESIN RESTORATION (응축형 복합레진 수복시 유동성 레진 이장이 변연부 미세누출에 미치는 영향)

  • The purpose of this study was to evaluate the marginal microleakage of condensable composite resin restorations according to flowable resin lining of internal cavity wall. The eighty extracted human molar teeth without caries and/or restorations are used The experimental teeth were randomly assigned into four groups of ten teeth each. Eighty caries-free extracted human molars were used in this study. The conventional class II cavities (box-shaped on mesial and distal surface, faciolingual width : 3mm, gingival wall depth : 1.5mm) were prepared 1mm below cementoenamel junction with a # 701 carbide bur. The teeth were divided into four groups, and then each group were subdivided into A & B group according to flowable resin & compomer lining ; Group 1-A : Tetric Ceram filling, Group 1-B : Tetric Flow lining and Tetric Ceram filling, Group 2-A Ariston pHc filling, Group 2-B : Tetric Flow lining and Ariston pHc filing, Group 3-A SureFil filling, Group 3-B : Dyract Flow lining and SureFil filling, Group 4-A : Pyramid filling, Group 4-B : Aeliteflo lining and Pyramid filling. To simulate as closely as possible the clinical situation during retoration placement, a "restoration template" was fabricated, and the condensable resin was filled using a three-sited light-curing incremental technique. All the materials used were applied according to the manufacturers' instructions. The specimens were stored in the 100% humidity for 7 days prior to thermocycling (100 thermal cycles of 5~55$^{\circ}C$ water with a 30-second dwell time) The specimens were immersed in 2% metyleneblue dye for 24 hours, and then embedded in transparent acrylic resin and sectioned mesiodistally with diamond wheel saw. The degree of marginal leakage was scored under stereomicroscope ($\times$20) and the data were analyzed by Kruskal-Wallis test and Wilcoxon signed ranks test. The results were as follows : 1. In the gingival margins of all the group, microleakage of subgroup B was less than subgroup A. 2. In the group 1, 2, 4, there was significant differences between subgroup A and B (p<0.05), but in the group 3, there was not significant different between group 3-A (SureFil) and group 3-B (Dyract flow/SureFil) (p>0.05). 3. In the subgroup A and B, there was significant different between all group except group 4 of subgroup A. From the results above, it was suggested that the cavity lining of flowable resin and flowable compomer in condensable resin restoration decrease microleakage at gingival margin, and does improve their ability to seal the gingival margin of class II preparation.

EVALUATION OF ENTEROCOCCUS FAECALIS REMOVAL EFFICACY OF THE ENDOVAC® AND ENDOACTIVATOR® INTRACANAL IRRIGATION METHODS (EndoVac®과 EndoActivator®를 이용한 근관세척법의 Enterococcus faecalis 제거 효율 평가)

  • The aim of this study was to evaluate endodontic irrigation methods with $EndoVac^{(R)}$ and $EndoActivator^{(R)}$ in the elimination of Enterococcus faecalis from the root canals. Extracted 70 human single-rooted teeth were used. The canals were instrumented by a crown-down technique with .04 taper ProFile to ISO size 40. After the teeth were autoclaved, the canals were inoculated with E. faecalis and incubated for 48 h. The teeth were randomly divided into three experimental groups of 20 teeth each according to canal irrigation methods and two control groups as follows: group 1 - $EndoVac^{(R)}$; group 2 - $EndoActivator^{(R)}$; group 3-Conventional needle irrigation method. After canal irrigation using 2.5% NaOCl. first samples (S1) were taken using sterile paper point. And the canals were filled with sterile brain heart infusion (BHI) broth and incubated for 24 h, then second samples (S2) were taken. The samples were cultured on BHI agar plate to determine the numbers of colony forming units (CFU). In first sampling (S1), only one canal of conventional method among the all experimental groups was positive cultured. In second sampling (S2), $EndoVac^{(R)}$ group showed the least positive culture numbers of E. faecalis. There was statistically significant difference between the $EndoVac^{(R)}$ and conventional needle irrigation methods in the mean value of Log CFU. According to the results of this study, $EndoVac^{(R)}$ showed better efficacy than conventional needle irrigation method in the elimination of E. faecalis from the root canal.

Comparative Histologic Study of 3-Root Canal Filling Materials for Dog's Teeth (성견 치아의 치수절제술 후 3가지 근관충전재에 따른 조직학적 반응의 비교)

  • Endodontic treatment of primary molars is a great challenge to the dentists because of their complex morphology. However, there have been limited controlled in vivo studies about filling materials for primary teeth. Hence, the aim of this study is to evaluate and compare histologic responses of different calcium hydroxide canal filling materials that are used in daily clinical practice. Pulpectomies were performed in 132 teeth of the dogs. The root canals were randomly filled with either $Vitapex^{(R)}$ (n = 44), $Metapex^{(R)}$ (n = 44), or $Metapaste^{(R)}$ (n = 44). After 4 or 13 weeks, respectively after operation, the dogs were sacrificed, and teeth were processed for histologic examinations. Inflammatory reaction was evaluated and scored in a blind manner. The Fisher's exact test and Kruskal-Wallis test were used to compare the tested groups. In 4-week-group, all inflammatory responses were in normal range except for fibrosis. In 13-week-group, increased response to various inflammation reaction was shown compared to the 4-week-group. However, there were no statically significant differences between the tested groups in all inflammatory reaction. Overall, there were no significant differences among three materials in responses of apical tissues and all of them showed favorable tissue responses.

5 Year Cumulative Survival Rate of Composite Resin Restorations in Permanent First Molars (제1대구치 복합레진 수복의 5년 누적 생존율)

  • The purpose of this retrospective study was to evaluate the survival rate of composite resin restorations in permanent first molars in pediatric patients focusing on the influence of risk factors related to patients and teeth. 172 patients (6 - 12 years old) who had their permanent first molars restored with composite resin from July 2010 to July 2012 were investigated. From the dental records, the influence of the risk factors on the survival of restorations was assessed. Location of teeth, classification of cavities, patients' age and caries risk were included as risk factors in this study. The caries risks of patients were evaluated by the value of the decayed-missing-filled teeth index with the records of patients taken at 5 years old. Among 354 restorations, 272 restorations retained and 82 restorations were replaced. The overall cumulative survival rate at 5 years was 73.9%. The main reason for replacement of restorations was secondary caries (81.7%). Patients with older age group and with lower caries risk group showed higher survival rate of restorations. No statistically significant influence was detected between the survival rates and the possible risk factors : location of teeth, patients' age and caries risk. The survival rate of restorations was significantly affected by the classification of the cavities (p = 0.002).

ENDODONTIC TREATMENT WITH CALCIUM HYDROXIDE OF REPLANTED TOOTH : A CASE REPORT (재식한 치아에서 수산화 칼슘을 사용한 근관치료)

  • Tooth avusion implies total displacement of teeth out of its socket. Its frequency range from 0.5 to 16% of traumatic injuries. Replantation procedure is used for the treatment of avulsed tooth. However, its major complications are pulp necrosis, inflammatory root resorption and replacement root resorption. This paper describes 10 years and 2 months old male patient whose both maxillary central incisors were avulsed due to fall-down with slightly underdeveloped root apices. Teeth were replanted 2 hours after accident. Right central incisor's pulp tissues were extirpated and filled with $Vitapex^{(R)}$(calcium hydroxide) at 3-4 weeks after replantation, but left central incisor was filled at 8-9 weeks. Right central incisor showed little inflammatory resorption in apical portion, whereas left central incisor showed severe root resorption. During 18 months' follow-up period, left central incisor showed slightly ankylosis while right central incisor did not. Based upon the above-mentioned results, the following conclusions can be drawn: 1. Inflammatory resorption could be suppressed by endodontic treatment with calcium hydroxide. 2. When apex formation is doubted in replanted tooth due to avulsion, early endodontic treatment with calcium hydroxide seems to act positively for better prognosis.

Effects of National Dental Screening on Dental Caries Experience, Treatment, and Cost in Children (국가 구강검진이 어린이의 치아우식 경험, 치료 및 비용에 미치는 영향)

  • This study aimed to evaluate the effects of national dental screening for children in Korea in the aspects of prevention of dental caries, the number of treatments related to dental caries, and cost of dental care. The study used the national cohort data on children health screening provided by National Health Insurance Service, and analyzed 32,937 1st dental screening, 22,608 2nd dental screening, 13,708 3rd dental screening, and 241,043 cases from the dental care of 27,944 children born in year 2008 and 2009. The decayed-filled teeth index and decayed-filled person rate, calculated from the results of the 2nd and 3rd dental screening, decreased as the number of preceding dental screening increased. Glass ionomer and amalgam restoration, pulp treatment of primary teeth and extraction of primary canine and molar were shown to decrease as the number of examined dental screening increased, and the total cost of dental care covered by national health insurance also decreased as the number of dental screening increased. In conclusion, national dental screening for children is an efficient and economical oral health care method that prevents dental caries and lowers the possibility of dental caries related treatment, thereby reducing cost of dental care.

Influence of plugger penetration depth on the area of the canal space occupied by gutta-percha (Plugger 삽입깊이가 근관내 gutta-percha 점유면적에 미치는 영향)

  • To evaluate the ratio of gutta-percha area in the canal after canal obturation with Continuous Wave of Condensation Technique (CWCT) with varying depths of plugger penetration, forty root canals of extracted human teeth were prepared up to size 40 of 0.06 taper with $ProFile^{(R)}$. Canals of three groups were filled with CWCT with System $B^{TM}$ (Analytic Tech. , USA) and different plugger penetration depths of 3, 5, or 7 mm from the apex. Canals of one group were filled with lateral condensation technique as a control The felled teeth were cross-sectioned at 1, 2 and 3 mm levels from the apical foramen. The ratio of gutta-percha area in the canal was analyzed using $Auto^{(R)}$ Cad 2000. Data were analyzed with one-way ANOVA and Duncan's multiple range test. At all levels, higher gutta-percha area ratio was found with deeper plugger penetration depth in CWCT, and cold lateral condensation group showed higher ratio than group of plugger penetration to apical 7 mm in CWCT. At apical 1 mm and 2 mm levels, group of plugger penetration to apical 3 mm showed significantly high or gutta-percha area ratio than those of apical 7 mm and lateral condensation (p<0.05). It is concluded therefore that, under the conditions of the present study, deeper plugger penetration depth results in more favorable and efficient obturation in CWCT.
