  • 저자들은 좌측 흉수액을 주소로 내원한 63세 여자에서 전산화 단층촬영상 복수와 난소의 종양이 발견되어 조직검사상 난소의 양성섬유종으로 확인되었고, 난소종양의 수술적 제거 후 흉수와 복수가 소실되어 Meigs 증후군으로 진단한 1예를 경험하였기에 이를 보고하는 바이다.

  • Central granular cell odontogenic tumor(CGCOT) is a very rare lesion that consists of densely packed granular cells with numerous scattered strands of odontogenic epithelium interspersed throughout the tissue. CGCOT was initially reported in 1962 by Cough et al as central granular cell ameloblastic fibroma. But, recently, this term is inappropriate because of histologic and chronologic differences. CGCOT is usually present as painless swellings. Radiographs show a well-demarcated radiolucent or mixed radiopaque-radiolucent lesion. The average age on presentation of CGCOT is 47.3 and women are 75% more likely to develop this lesion than men. The tumor only occur in tooth bearing areas of the jaw with 88% of cases occurring in the mandible and 12% involving the maxilla, usually in an equal distribution between the caninepremolar-molar areas. This tumor is benign, and care is effected by localized surgical excision. We report an additional case of CGCOT that occurred in the Rt. Maxillar premolar/molar region of a 32-year old man with literature review.

Evaluation of biopsies of oral and maxillofacial lesions: a retrospective study

  • Objectives: This study aimed to determine the prevalence of odontogenic cysts, tumors, and other lesions among reports in the archives of the Department of Oral and Maxillofacial Surgery at the Faculty of Dentistry affiliated with Kocaeli University collected over a four-year period. Materials and Methods: In this retrospective study, patient records from the archive of the Department of Oral and Maxillofacial Surgery from 2014 to 2018 were reviewed. Patient demographic information (age and sex) and lesion location were recorded and analyzed. Results: From a total of 475 files reviewed, odontogenic cyst was confirmed in 340 cases (71.6%), and odontogenic tumor was confirmed in 52 cases (10.9%). Regarding odontogenic cyst type, the most common was radicular cyst (216 cases), followed by dentigerous cyst (77 cases) and odontogenic keratocyst (23 cases). Among odontogenic tumors, the most frequent was odontoma (19 cases), followed by ossifying fibroma (18 cases) and ameloblastoma (9 cases). Giant cell granuloma was also reported in 35 cases. Conclusion: The distribution pattern of odontogenic cysts and tumors in our retrospective study is relatively similar to that reported in the literature. Complete clinical reports for final diagnosis of these lesions and routine follow-up examinations are very important for treatment.

Desmoplastic Fibroblastoma of the Scalp Accompanied by Severe Pain; Unusual Location and Symptom

  • Desmoplastic fibroblastoma is a rare fibrous tumor that usually presents as a painless, slow-growing mass in the subcutaneous tissues and skeletal muscles. It has a wide anatomic distribution, with the most common involvement being the arm and shoulder. Here, we report a case of a tiny painful desmoplastic fibroblastoma arising on the scalp. According to a microscopic examination, this tumor was composed of spindle-shaped fibroblasts in the dense collagenous stroma. On immunohistochemical staining, tumor cells were positive for vimentin and negative for smooth muscle actin, CD34, and S100. Our case is unique in that desmoplastic fibroblastoma developed on the scalp and there was presence of pain despite its small size.

Common conditions associated with displacement of the inferior alveolar nerve canal: A radiographic diagnostic aid

  • Purpose: This study reviewed the common conditions associated with displacement of inferior alveolar nerve canal. Materials and Methods: General search engines and specialized databases including Google Scholar, Pub Med, Pub Med Central, Science Direct, and Scopus were used to find relevant studies by using keywords such as "mandibular canal", "alveolar canal", "inferior alveolar nerve canal", "inferior dental canal", "inferior mandibular canal" and "displacement". Results: About 120 articles were found, of which approximately 70 were broadly relevant to the topic. We ultimately included 37 articles that were closely related to the topic of interest. When the data were compiled, the following 8 lesions were found to have a relationship with displacement of mandibular canal: radicular/residual cysts, dentigerous cyst, odontogenic keratocyst, aneurysmal bone cyst, ameloblastoma, central giant cell granuloma, fibrous dysplasis, and cementossifying fibroma. Conclusion: When clinicians encounter a lesion associated with displaced mandibular canal, they should first consider these entities in the differential diagnosis. This review would help dentists make more accurate diagnoses and develop better treatment plans according to patients' radiographs.

신경혈관 줄기를 침범한 수부종양의 미세현미경적 접근 (Microscopic Approach of Mass Involving Neurovascular Pedicle in the Hand)

  • Purpose: Mass can compress around tissue and cause deviation of normal anatomical structures. Often, mass grows toward neurovascular pedicle and encircles depending on the nature of mature mass. Neglecting neurovascular involvement of the mass is a serious problem not to be overlooked. Authors have performed microscopic approach regarding mass involving the neurovascular pedicle in the hand. Materials and Methods: From January 2007 through February 2012, retrospective analysis for nine cases of mass involving neurovascular pedicles was done. Patients were evaluated preoperatively by ultrasonography or MRI and checked intraoperative finding. Masses were evaluated by site, preoperative evaluation, involved neurovascular pedicle, histopathologic diagnosis, complication, and recurrence. Results: The site of mass involving neurovascular pedicles was 4 cases on the wrist, 2 cases on the palm, 2 cases on the finger, 1 case on the hand dorsum. Involved neurovascular pedicles were 3 radial arteries and nerves, 3 proper digital arteries and nerves, 1 radial artery, 1 superficial branch of radial nerve, 1 common digital artery and nerve. The histopathologic diagnosis of mass were 3 ganglions, 2 giant cell tumors, 2 epidermal cysts, 1 fibroma, and 1 benign spindle tumor. There were 2 cases of recurrence and secondary excisions were performed. Conclusion: Neurovascular pedicle injury can lead to serious complication like sensory and motor disorders, distal part ischemia, and so on. In case of mass suspected neurovascular invasion, accurate preoperative evaluation such as ultrasonography or MRI is necessary. To prevent any neurovascular related complication during mass excision, delicate surgical technique using a microscope becomes essential.

법랑모세포섬유치아종(Ameloblastic fibre-odontoma)의 치험례 (AMELOBLASTIC FIBRO-ODONTOMA : A CASE REPORT)

  • 법랑모세포 섬유치아종(Ameloblastic fibro-odontoma)은 하악 구치부에 호발하는 드문 혼합 치성종양이다. 방사선적 소견으로는 경계가 명확하고, 대부분에서 방사선 투과상과 불규칙적인 크기와 형태의 불투과상이 혼재된 양상으로 관찰된다. 조직학적으로 법랑모세포 섬유치아종은 법랑모세포 섬유종의 연조직 성분과 복합성 치아종의 경조직 성분을 모두 가지고 있다. 본 증례의 13세 2개월된 남자 환아는 원광대학교 치과대학 치과병원 소아치과에 하악 좌측 견치의 맹출 지연을 주소로 내원하였다. 맹출을 방해하는 병소를 적출하고 조직 생검한 결과, 법랑모세포섬유치아종으로 진단하였다. 지속적인 예후관찰 중 병소 제거 수개월 후 견치는 정상적으로 맹출하였다.

초음파에서 족부 결절종으로 오인한 유사 병변 (Ganglion-like Lesions of the Foot on the Ultrasonography)

  • Purpose: The authors have experienced various lesions that simulate ganglion of the foot on the ultrasonography. The purpose of this study is to evaluate ultrasonographic findings of soft tissue lesions, which were interpreted as ganglion but confirmed as different lesions in the foot. Materials and Methods: We reviewed a database of patients with ganglion on the ultrasonography from two different institutions. There were 109 patients who underwent both ultrasonography and surgical confirmation. Twenty one lesions were identified, of which initial interpretation on the ultrasonography included ganglion which pathology revealed to be different lesions. All images were evaluated by one musculoskeletal radiologist, regarding size, margins, internal echogenicity of lesions, and presence of posterior enhancement. Results: Of 21 lesions, there were 6 fibrous tumors including fibroma, giant cell tumor of tendon sheath, and fibromatosis, 3 hemangiomas, 2 epidermal inclusion cysts, 2 chondromas, 2 angioleiomyomas, 1 trichilemal cyst, 1 neurofibroma, 1 granular cell tumor, 1 neurilemmoma, 1 neuromyxoma, and 1 nodular hidradenoma. Mean size of the lesion was 1.1 cm. Margins were smooth in 10, mild lobulation in 8 and marked lobulation in 3 lesions. Lesions were hypoechoic in 16, anechoic in 4 and isoechoic in 1 case. Posterior acoustic enhancement was definitely present in 5 lesions. Conclusion: On the ultrasonography, various soft tissue lesions of the foot may be confused with ganglion. During surgical resection care should be given even to a simple ganglion as it might turn up to be a solid lesions such as fibrous tumors.

Squamous Cell Carcinoma as the Most Common Lesion of the Tongue in Iranians: a 22-Year Retrospective Study

  • The tongue has been globally considered as an indicator of general health for millennia. This study aimed to determine the prevalence and distribution of tongue lesions in an Iranian population. In this retrospective study, data from 6,435 oral biopsy reports over a 22-year period (1992-2014) were retrieved from archives of Oral and Maxillofacial Pathology Department, Shahid Beheshti Dental School, Tehran, Iran. These reports were analyzed according to age, sex, type of lesion and location. Prevalence of tongue lesions were reported as percentages. Out of total oral lesions, 238 (3.7%) were found in the tongue, with the incidence peak (42%) being between 41-60 years. Men constituted 53% and women 47%of patients. The youngest patient was a 3-year-old girl with pyogenic granuloma and the oldest one was a 93-year-old man with squamous cell carcinoma (SCC). SCC was the most common (25%) lesion generally found in the lateral border of the tongue with a male predilection. The second and third most prevalent lesions of the tongue were benign keratosis (frictional keratosis) (13.4%) and leukoplakia (13%).White-red lesions (38.6%) were the most frequent subgroup followed by neoplastic lesions (28%). Moreover, irritation fibroma, non-specific ulcers, squamous papilloma, and hemangioma were found as the most frequent lesions in their related subgroups.Given the high rate of SCC of the tongue in Iranian patients, this area should be examined more carefully by dental practitioners and physicians.

늑골에 발생한 연골점액유사 섬유종 -1예 보고- (Chondromyxoid Fibroma of the Rib Report of one case -)

  • 연골점액유사 섬유종은 뼈에 발생하는 드문 양성 종양으로 양성 골종양의 $2\%$ 미만을 차지한다. 연골점액유사 섬유종은 무릎부위의 경골 근위부와 대퇴골 원위부에 가장 호발하며 골반, 비골, 발꿈치 뼈, 늑골에서는 드물게 발생한다. 내원 1달 전부터 시작된 좌측 7번째 늑골부위의 부종과 간헐적으로 지속된 경한 동통으로 내원한 54세 남자가 방사선 검사상 양성 늑골 종양 진단 하에 늑골 절제술을 시행 받았으며 조직 검사상 늑골에 발생한 연골점액유사 섬유종으로 최종 진단되었고 현재까지 재발이나 별다른 증상 없이 외래 추적 중으로 이에 보고하는 바이다.
