Treatment of Gingival Irritation Fibroma Using $CO_2$ Laser (이산화탄소 레이저($CO_2$ laser)를 이용한 치은에 발생한 자극성 섬유종의 치료)

  • Carbondioxide($CO_2$) laser is one of the most widely employed lasers in oral soft tissue surgery because of its excellent affinity for water based soft tissues. It has some inherent advantages such as hemostasis, less postoperative swelling, reduction of bacterial population at surgical site, less need for suturing, less scarring, and less postoperative pain compared to conventional surgical therapies including the use of scalpel, diathermy, cryotherapy and electrosurgery. A 30-years-old male was presented with gingival swelling. Clinical examination revealed a well?defined pedunculated fibrotic mass on the buccal gingiva near right maxillary 1st molar. In radiographic examination, no remarkable abnormality was seen. Excisional biopsy was performed with $CO_2$ laser (continuous wave mode, 4.0W). Histological diagnosis was "Irritation fibroma". $CO_2$ laser has advantages those are suitable for surgical treatment of intraoral lesion. If appropriate training and experience are provided, the dentist would be able to manage intraoral lesions more efficiently and successfully with the use of the $CO_2$ laser.

  • Various mucosal lesions are originated in oral cavity and trauma is most common cause of these lesions. Definitive treatment of these lesions is stop of traumatic events and removal. There are two representative removal methods in medical fields; conventional mess or LASER. Compare to conventional mess technique, LASER has several advantages such as bleeding control, pain reduction. Of the various LASER systems, $CO_2$ LASER is regarded as best choice for general practitioners due to its convenience, universal use and high cost/benefit effects. In these article, we report the treatment cases of mucocele, fibroma, and leukoplakias using $CO_2$ LASER. Eventhough there are malignancy potential of oral mucosal lesion especially leukoplakias, careful history taking, clinical examination, and regular check-up will help the general practitioners to manage these lesions. Simple and relatively safe oral mucosal lesions have to be treated more in general dental practitioners.

  • Central odontogenic fibroma (COF) is defined as a fibroblastic odontogenic tumor characterized by varying density of the tooth epithelium. It is an extremely rare benign neoplasm that occurs in the maxilla and the mandible; only a few reports of COF are available in the literature. Diagnosis of the lesion based only on the radiological features of COF is difficult due to variation in the findings regarding this condition. This report describes 2 clinical cases of middle-aged women with COF. Clinical examination revealed palatal mucosal depression; additionally, oral examination, as well as panoramic radiographs, intraoral radiographs, and computed tomography scans, revealed severe root resorption. This report highlights the clinical and radiological imaging features of COF, with the goal of enabling straightforward differential diagnosis of the lesion by the clinician and thereby appropriate treatment of the patient.

Desmoplastic Fibroma of the Skull;A Case Report, Review of the Literature, and Therapeutic Implications (두개골에 발생한 결합조직형성 섬유종 ; 증례보고 및 문헌 검토)

  • Desmoplastic fibroma(DF) is a rare neoplasm of the bone, and is histologically benign but locally aggressive disease. A total of nine cases of DF involving skull have been reported in the literature, and among these eight are females. In this report, the clinical findings and histopathology of a case with DF of the skull in a male patient is presented with a review of the literature with an emphasis on treatment modalities. A 21-year-old man presented with headache. CT scan revealed a solitary and lytic skull lesion without brain invasion. DF was confirmed by histological evaluation. On immunohistochemical staining of the tumor was negative for estrogen or progesterone receptors. After total resection of tumor with wide surgical margin, there was no recurrence during the 35 months of follow-up period. Although longer follow up period maybe needed, treatment of this type of tumor with complete resection of tumor tissue along with a wide margin may provide long disease-free state compare to the high recurrence rates in DF of other sites.

Expression of TGF-β1 and EGFR in Irritation Fibroma and Oral Leukoplakia (면역조직화학염색법을 이용한 자극성 섬유종과 구강 백반증에서의 TGF-β1과 EGFR 발현 비교 연구)

  • Irritation fibroma (IF) is the most common tumor-like oral lesion that is evolved by proliferation of collagen in response to chronic irritation. Oral leukoplakia (OL) is considered as precancerous lesion characterized by proliferation of epithelial cells due to chronic irritation, smoking and drinking. TGF-${\beta}1$ and EGFR are important factors that play an essential role in extracellular matrix remodeling during normal wound healing process. The epithelial reaction by chronic irritation may be connected with pathogenesis of IF and OL. In the present study, we examined the expression of TGF-${\beta}1$ and EGFR in the IF and OL using immunohistochemistry. We used 88 cases of IF, 44 cases of OL and 9 cases of normal oral mucosa as normal control. TGF-${\beta}1$ was decreased in the epithelium of IF and OL. As for EGFR, the epithelial cells revealed the increased positive expression in IF and OL. In case of OL, the Spearman correlation coefficient of TGF-${\beta}1$ and EGFR was -0.10 (p< 0.05), which showed weak correlation. In the fibrous tissue, TGF-${\beta}1$ was increased only in IF. The expression difference of TGF-${\beta}1$ and EGFR may be involved in the pathogenesis of IF and OL.

Successful Conservative Surgical Treatment of Ameloblastic Fibroma in the Posterior Maxilla : A Case Report (상악 구치부에 발생한 법랑모세포섬유종의 성공적인 보존적 수술 : 증례 보고)

  • Ameloblastic fibroma (AF) is a rare odontogenic ectomesenchymal tumor that is frequently seen in the first two decades of life, and occurs in the mandible. The most proper management of AF has been a recent topic of debate because of its recurrence and malignant transformation. This report describes AF in a 4-year-old male, which was a unilocular radiolucency on the maxillary right primary molar area with a scalloped border and corticated margin. The tumor was treated conservatively with enucleation and curettage, and the decision was made to preserve the right primary second molar. A biopsy confirmed it as AF. During the 43 months of follow-up, the patient had no evidence of recurrence or malignant transformation. Moreover, the radiographic examination revealed the generation of tooth germ to be a permanent second premolar. This report shows a case of AF in the posterior maxilla of a 4-year-old boy and discusses the conservative therapeutic approach to this tumor. Therefore, the age of the patients should be an important consideration when choosing conservative or radical surgery in a young AF patient.

종양성 병변으로 인한 구강점막질환

  • 구강점막에 발생하는 종양성 병변으로는 대표적으로 구강점막암(oral mucous membrane cancer), 유두종(papilloma),혈관종(hemangioma), 임파관종(lymphangioma),섬유종(fibroma)등이 있는데 이들 병변들은 구강내 협점막, 치은, 경구개, 연구개, 구강저, 혀 등에 궤양이나 증식성 병소로 나타나서 동통, 출혈, 치아동요, 종창 등의 증상을 유발시킨다. 이러한 병변들에 대한 명확한 치료와 예후를 결정하기 위해서는 무엇보다도 이들 병변들에 대한 정확한 소견에 근거한 진단이 우선되어야 할 것이다. 따라서 본 장에서는 정확한 진단과 치료를 위해 이들 병변들에 대해 소개하고자 한다.

  • The author observed a 35-year-old male patient who came to the Dental Infirmary of Kyungpook National University Hospital who had complained of gradual swelling on both side of the mandible for 15 years. As a result of careful analysis of clinical, radiological and histopathological findings, the authors diagnosed it as cementifying fibroma and obtained the results as follows: 1. In clinical examination, main clinical symptom was facial deformity due to gradual swelling on both side of the mandible. 2. In radiographic examination, radiolucent lesions with central radiopaque foci were seen on both mandibular body areas bilaterally, and cortical thinning and expansion of the mandibular body were seen buccolingually. And loss of lamina dura and root resortption of adjacent teeth were also seen. In histopathological examination, this lesion was composed of delicate interlacing collagen fibers interspersed by the fibroblasts and cementoblasts. And round or oval shaped basophilic masses of cementum-like tissue were observed in the connective tissue.
