골반 방사선 치료후의 골 변화와 손상 (Post Pelvic Radiotherapy Bony Changes)

  • 방사선치료후 발생할 수 있는 골 손상은 골반 부전골절, 방사선 골 괴사, 무혈성 괴사, 골 육종 등이다, 특히 최근 방사선치료후의 골탄성도의 변화로 발생하는 골반 부전골절에 관하여 많은 보고가 있었다. 과거 방사선치료 후의 골 손상 특히 골절의 빈도는 선형가속기를 이용한 메가볼트 방사선치료 바뀌면서 현저히 줄어 들어서 매우 낮은 것으로 보고 되었으나 최근에는 $8{\sim}20%$로 보고가 되고 있다. 따라서 골반 부전골절의 임상적 의미와 중요성과 예방 및 치료에 대하여 관심이 증가되고 있다. 저자는 자궁경부암 치료 후 관찰한 골반 부전골절, 골 괴사, 무혈성 괴사, 및 2차성 골육종등 골반골 변화와 손상에 관하여 저자의 경험과 문헌고찰을 통하여 보고 하고자 한다.

  • It is well known that the glucocorticoid induces osteoporosis by suppression of the osteoblast, but its effect on the osteoclast hRS some controversy whether it activates or suppresses the osteoclast. If the calcitonin, which is known to suppress the osteoclast, prevents the osteoporosis by glucocorticoid, then the suppression of the osteoclast by the glucocorticoid is not so significant. And if the calcitonin increases the osteoblastic activity, Tc-99m MDP uptake will be increased in spite of the glucocorticoid effect on the osteoblast. The immobilization operation was performed to the right leg of male Wistar rats weighing about 200gm. each. For 16 weeks after operation, rats were injected glucocorticoid alone or glucocorticoid anci calcitonin. The bone density was measured by means of photodensitometry under reference aluminum step wedge and Tc-99m MDP uptake was available to the index of the osteoblastic activity. 1. The bone density of femora! head was markedly reduced than that of femoral shaft following ration of cancellous and cortical components in both site. 2. Glucocorticoid caused decrease in bone density of spine and femur, md there is significantly increase of it when medication of glucocorticoid and calcitonin injection simultaneously than that of glucocorticoid. 3. Tc-99m MDP uptake was revealed significant reduction in medication of glucocorticoid but increase in gi;.:cocorticoid and calcitonin injection simultaneously in later experimental period. 4. There wail, a slight reduction in plasma osteocalcin in medication of glucocorticoid through experimental periods and an increase in its value in case of giving glucocorticoid and calcitonin simultaneously in later experimental period. From these results, we suggest that osteoporosis by immobilization is more pronounced by glucocorticoid hormone and osteoporosis induced by immobilization and glucocorticoid use is prevented by calcitonin administration with increasing osteoblastic activity.

  • 이 연구에서 저자들은 다음과 같은 결론을 얻었다. 1. 고관절 조영술은 수술전에 미리 유용한 정보를 줌으로써 수술전 계획을 세우고 수술을 체계적으로할 수 있을것으로 사료된다. 따라서 고관절 조영술은 도수정복 혹은 수술적 정복전에 필수적으로 시행하여야 할 것으로 사료된다. 2. 고관절 조영술은 비구 관절순, 원형인대의 평가에 매우 유용하였으며, 횡비구인대에 대한 정보는 제한적이었다. 3. 고관절 조영술은 단순 방사선 촬영에 비하여 중앙연각의 평가에 훨씬 유용하고 실제 병리적 소견에 합당한 정보를 주었다.

3상 골신티그램을 이용한 급성 일과성 활막염의 진단 (Three-Phase Bone Scintigraphic Diagnosis of Acute Transient Synovitis)

  • Acute transient synovitis of the hip presents clinically pain and limping. But in the majority of the cases, definite positive findings are not manifest in roentgenogram in its early phase. However radionuclide bone imaging combines with the assessment of vascularization and bone tracer uptake is of great value in solving this diagnostic problem. The materials for this study consisted of 29 children with acute transient synovitis of the hip, characterized by symptoms and physical signs of an arthritis, negative X-ray findings and disappearance of all symptoms and signs within a short period of time. They were twenty males and 9 females and age ranged from 1 to 12 years. We took pelvic reontgenogram in AP and frog-leg views. After intravenous bolus injection of 10 to 15 mCi of $^{99m}Tc-methylene$ diphosphonate, 24 sequential image of the pelvis was taken at 2-second interval for blood flow study. The scintigrams were made using a gamma camera with high resolution parallel hole collimator. Blood pool imaging was obtained at 2 minutes after tracer administration. After 3 hours, static images were taken and then closeup image of the hip using pin-hole collimator was followed. The results were as follows: 1) Bone scintigram was much more sensitive than conventional roentgenogram in diagnosis of acute transient synovitis of the hip. 2) Three-phase imagings showed increased vascular activities in blood pool scintigrams in 96%. 3) Pin-hole imaging showed increased tracer uptake in the regional bones of the hip, par ticularly in the medial aspect of femoral head and acetabulum. 4) We confirmed that three-phase imaging reinforced with pin-hole technique were very useful in diagnose of acute transient synovitis of the hip.

Scalp Free Flap Reconstruction Using Anterolateral Thigh Flap Pedicle for Interposition Artery and Vein Grafts

  • We experienced satisfactory outcomes by synchronously transplanting an artery and vein using an anterolateral thigh flap pedicle between the vascular pedicle and recipient vessel of a flap for scalp reconstruction. A 45-year-old man developed a subdural hemorrhage due to a fall injury. In this patient, the right temporal cranium was missing and the patient had $4{\times}3cm$ and $6{\times}5cm$ scalp defects. We planned a scalp reconstruction using a latissimus dorsi free flap. Intraoperatively, there was a severe injury to the right superficial temporal vessel because of previous neurosurgical operations. A 15 cm long pedicle defect was needed to reach the recipient facial vessels. For the vascular graft, the descending branch of the lateral circumflex femoral artery and two venae comitantes were harvested. The flap survived well and the skin graft was successful with no notable complications. When an interposition graft is needed in the reconstruction of the head and neck region for which mobility is mandatory to a greater extent, a sufficient length of graft from an anterolateral flap pedicle could easily be harvested. Thus, this could contribute to not only resolving the disadvantages of a venous graft but also to successfully performing a vascular anastomosis.

Rare Imaging of Fat Embolism Seen on Computed Tomography in the Common Iliac Vein after Polytrauma

  • Fat embolism refers to the presence of fat droplets within the peripheral and lung microcirculation with or without clinical sequelae. However, early diagnosis of fat embolism is very difficult because the embolism usually does not show at the computed tomography as a large fat complex within vessels. Forty-eight-year-old male with pedestrian traffic accident ransferred from a local hospital by helicopter to the regional trauma center by two flight surgeons on board. At the rendezvous point, he had suffered with dyspnea without any airway obstruction sign with 90% of oxygen saturation from pulse oximetry with giving 15 L of oxygen by a reserve bag mask. The patient was intubated at the rendezvous point. The secondary survey of the patient revealed multiple pelvic bone fracture with sacrum fracture, right femur shaft fracture and right tibia head fracture. Abdominal computed tomography was performed in 191 minutes after the injury and fat embolism with Hounsfield unit of -86 in his right common iliac vein was identified. Here is a very rare case that mass of fat embolism was shown within common iliac vein detected in computed tomography. Early detection of the fat embolus and early stabilization of the fractures are essential to the prevention of sequelae such as cerebral fat embolism.

비구 주위 육종의 저온 열처리한 골연골 이식술 - 증례 보고 - (Pasteurized Osteoarticular Graft for Periacetabular Sarcoma - A Case Report -)

  • 영상기술과 수술 술식의 발전으로 골반골의 악성종양에서 내 골반골 절제술이 보편화되었으나 절제 후 골 결손의 기능적인 재건 방법에 대하여는 아직 논란의 여지가 많다. 골반환 재건술식 중 열처리한 자가골과 대퇴골 전치환물을 조합하는 방법은 생역학적이며 해부학적인 적합성이 장점으로 적절한 증례를 선택하면 좋은 결과를 얻을 수 있다. 그러나 이 술식 후 감염이나 기계적인 파괴가 발생 시 내고정물과 자가골 전부를 제거하는 것이 불가피하다. 이런 경우 가관절을 형성하더라도 심한 하지 단축이 문제가 된다. 저자들은 이런 문제점을 해결하기 위해 대퇴골두를 제거하지 않고 골반골만 열처리 후 재삽입 한 1례를 보고하고자 한다.

딥러닝 기반의 대퇴골 영역 분할을 위한 훈련 데이터 증강 연구 (Data Augmentation Method for Deep Learning based Medical Image Segmentation Model)

  • 본 연구에서는 CT 영상의 대퇴골 부위를 해부학적으로 의미 있게 변형하여 CT 영상의 대퇴골 영역을 분할하기 위한 컨벌루션 신경망(CNN)의 훈련 데이터를 증강하는 방법을 제안한다. 먼저 CT 영상으로부터 삼차원 삼각형 대퇴골 메쉬를 얻는다. 그 후 메쉬의 국소부위에 대한 기하학적 특성을 계산하고, 군집화하여 메쉬를 의미 있는 부분들로 분할한다. 마지막으로, 분할한 부분들을 적절한 알고리즘으로 변형한 뒤, 이를 바탕으로 CT 영상을 와핑하여 새로운 CT영상을 생성하였다. 본 연구의 데이터 증강 방법을 이용하여 학습시킨 딥러닝 모델은 기하학적 변환이나 색상 변환 같이 일반적으로 사용되는 데이터 증강법과 비교하여 더 나은 영상분할 성능을 보인다.

Dosimetric Evaluation of Plans Converted with the DVH-Based Plan Converter

  • Plans converted using dose-volume-histogram-based plan conversion (DPC) were evaluated by comparing them to the original plans. Changes in the dose volumetric (DV) parameters of five volumetric modulated arc therapy (VMAT) plans for head and neck (HN) cancer and five VMAT plans for prostate cancer were analyzed. For the HN plans, the homogeneity indices (HIs) of the three planning target volumes (PTV) increased by 0.03, 0.02, and 0.03, respectively, after DPC. The maximum doses to the PTVs increased by 1.20, 1.87, and 0.92 Gy, respectively, after DPC. The maximum doses to the optic chiasm, optic nerves, spinal cord, brain stem, lenses, and parotid glands increased after DPC by approximately 4.39, 3.62, 7.55, 7.96, 1.77, and 6.40 Gy, respectively. For the prostate plans after DPC, the HIs for the primary and boost PTVs increased by 0.05 and 0.03, respectively, and the maximum doses to each PTV increased by 1.84 and 0.19 Gy, respectively. After DPC, the mean doses to the rectum and femoral heads increased by approximately 6.19 and 2.79 Gy, respectively, and those to the bladder decreased by 0.20 Gy when summing the primary and boost plans. Because clinically unacceptable changes were sometimes observed after DPC, plans converted by DPC should be carefully reviewed before actual patient treatment.

다중 트레이닝 기법을 이용한 MASK R-CNN의 초음파 DDH 각도 측정 진단 시스템 연구 (A Study on a Mask R-CNN-Based Diagnostic System Measuring DDH Angles on Ultrasound Scans)

  • 최근 영유아 성장기에 발생하는 고관절 이형성증(Developmental Dysplasia of Hip, DDH)의 숫자가 늘어나고 있다. DDH는 영유아 성장을 방해하고 다른 부작용도 많이 발생시키기 때문에 최대한 조기에 발견하여 치료해야 한다. 최근 들어 Convolutional Neural Networks (CNN) 및 개선된 Resnet50을 활용한 머신러닝 기법이 초음파 영상 분석에 많이 활용되고 있다. 연구 결과를 보면 컴퓨터 보조 이미지 분석이 의료현장에서 객관성과 생산성을 크게 향상시키고 있다. 본 연구의 결과는 정형외과에서의 난제인 초음파 영상을 통한 DDH 컴퓨터 보조 진단 알고리즘에도 충분히 활용될 수 있다는 것을 보여주고 있다. 본 논문에서는 CNN을 활용하여 DDH를 자동으로 측정하고 진단할 수 있는 컴퓨터 보조 진단 알고리즘을 제안하였다. DDH 측정을 위해 유아 고관절의 정상/비정상 판독을 위해 Acetabulum-Femoral head의 angle을 자동으로 계산하였으며 기존 영상을 딥 러닝하여 진단을 자동으로 하는 알고리즘을 설계하였다. 실험 결과 의사와 비교하여 진단의 속도와 정확도가 향상된다는 것을 확인하였다.
