Development of Algorithm for Nursing Interventions after Percutaneous Coronary Intervention (경피적 관상동맥중재술 후 간호중재 알고리즘 개발)

  • Purpose: The purpose of this study was to develop an algorithm for nursing care after percutaneous coronary intervention in order to improve patients' safety and prevent complications, because percutaneous coronary intervention is becoming a common treatment for coronary artery diseases. Methods: By reviewing related literatures and interviewing nurses, items and paths that were to be used in the algorithm for nursing care after percutaneous coronary intervention were drawn up and a draft algorithm was developed. The final algorithm was determined based on the results of the evaluation performed after clinical application. Results: According to the outcome after allowing nurses to apply the revised algorithm with 11 patients, suitability on items composing the algorithm were highly rated whereas promptness was lowly rated. Although the patients (n=11) to whom the algorithm was applied complained of less back pain (p=.001) and discomfort (p=.026) compared to the patients (n=17) to whom the algorithm was not applied, no significant difference in bleeding complication was found. Conclusion: The findings in the study support the clinical utilization of the algorithm for nursing care after percutaneous coronary intervention as the use of this algorithm reduced back pain and discomfort without increasing bleeding complications at the femoral puncture site.

  • Congenital coronary arteriovenous fistulas (CCAFs) are rare coronary artery abnormalities in which blood is shunted into a cardiac chamber or great vessel. If the fistula itself is large and tortuous, it is generally recommended to occlude the fistula to prevent several complications. In approaches of transcatheter occlusion, the transvenous approach is preferred over the transarterial approach. The transvenous approach would enable the cannulation of a relatively larger catheter or sheath without potential damage to the femoral vessels or normal coronary arteries, which can occur in the transarterial approach. The transvenous approach may also minimize the blind pouch after releasing the devices. Herein, we report the success of transvenous proximal closure of a CCAF using an Amplatzer vascular plug (AVP) in a 3-year-old patient with cardiomegaly. Complete occlusion was achieved by a single AVP and thrombus formation of the distal aneurysmal portion of the fistula. We suggest that this strategy of closing the proximal end with a dilated fistula using a single AVP by the transvenous approach may be a good option in treating CCAFs in a young child.

Cardiovascular Effects of Propofol Infused for Maintenance of Anesthesia in Dogs (개에서 Propofol 점적투여율 변화가 심혈관계에 미치는 영향)

  • Cardiovascular effects of propofol, were assessed after premedication with xylazine(1.0 mUkg, IM) under oxygen supply(200 ml/kg/min) via a endotracheal tube. Twelve adult mixed-breed dogs were divided into four groups; 0.2(Group 1), 0.4(Group 2), 0.6(Group 3) and 0.8 mg/kg/min(Group 4) of propofol respectively. Arterial blood pressure and electrocardiogram were monitored with a physiograph after an arterial catheter was inserted into the femoral artery. pH, arterial carbon dioxide tension($PaCO_2$and arterial oxygen tension($PaO_2$) were evaluated with arterial blood collected through the inserted catheter. Diastolic arterial pressure, systolic arterial pressure and mean arterial pressure were decreased slightly in Group I IIand III, but decreased significantly in Group IV. They were increased rapidly after stopping propofol infusion in Group IV pH was maintained in normal range in Group I, II and m, but was decreased in proportion to time passing in Group IV. $PaCO_2$ was increased significantly only in Group IV but $PaO_2$ was maintained in normal range in all groups Although heart rate was recorded in normal range for 90 minutes, arythmia was noted after stopping propofol infusion in all groups. It was concluded that propofol depressed the cardiovascular system in proportion to infusion dosage, and 0.8 mg/kg/min of propofol infusion rate was not appropriated in canine anesthesia with xylazine premedication.

  • Cervus elaphus, being known to reinforce Kidney, have tested to study the effects concerning damages of brain tissue induced by lipid peroxidation. In vitro, the level of lipid peroxide in brain tissue was decreased proportinally according to dose by Cervus elaphus extract solution for aqua-acupuncture (CESAA). It was much more decreased, when lipid peroxidation was induced with Fe(II). And, it was seen proportinally decrease according to the dose of CESAA on xanthine oxidase activities and type conversion ratio. However, I can not find special changes about aldehyde oxidase activities. And, I had observed the effects of CESAA on damages of rat's brain following ischemia and reflow. Before ischemia was caused, CESAA was applied 0.2 ml per 250 g through femoral vein in ischemia and reflow group and normal sailine was applied in normal group. Ischemia was caused by cervical artery's clamp for 30 min and reflowed by clamp remove after 15 min. It was increased on the content of lipid peroxidation, activies and type conversion ratio of xanthine oxidase following ischemia and reflow. However, they were decreased when CESAA was pre-appllied. However, it could not seen special changes on aldehyde oxidase activities, either. In conclusion. CESAA recovers the damage of brain due to ischemia and reflow by decreasing the lipid peroxidation through decreasing of xanthine oxidase activies and type conversion ratio.

The Effect of Melatonin on Morphological Changes of Rat Skeletal Muscle after Ischemia-Reperfusion Injury (멜라토닌이 허혈-재관류 손상에 의한 골격근의 형태학적 변화에 미치는 효과)

  • The effect of melatonin on morphological changes after ischemia-reperfusion injury was investigated in rat skeletal muscle. Dimethyl-sulfoxide(DMSO) was also tested for comparison. Muscle injury was evaluated in 4 groups as a single laparotomy group(control), ischemia-reperfusion group, DMSO group, melatonin group. Left hind limb ischemia was induced for 4 hours by vascular clamping of the common femoral artery and followed by 24 hours of reperfusion. The midportion of gastrocnemius muscle was taken for histological evaluation. In light microscopic study, ischemia-reperfusion group showed severe neutrophil infiltration, interstitial edema, and partial loss or degeneration of muscle fibers. The muscle tissue of melatonin group showed relatively normal architecture with mild inflammatory cell infiltration. In electron microscopic study, dilated cisternae of sarcoplasmic reticulum, dilated mitochondria with electron loose matrix and dilated cristae, disordered or loss of myofilament, indistinct A-band and I-band, intracytoplasmic vacuoles, and markedly decreased glycogen granules were observed in ischemia-reperfusion group. But relatively well maintained A-band, I-band, Z-line, M-line, and mildly dilated mitochondria with well preserved cristae were observed in melatonin group. The DMSO group showed intermediately attenuated ultrastructural changes. The results show that melatonin improves morphologically ischemia-reperfusion injury more effectively than DMSO. In conclusion, melatonin seems to be a promising agent that can salvage the skeletal muscle from severe ischemia-reperfusion injury.

  • The evaluation of the effectivess of ongoing cardiopulmonary resucitation efforts is dependent on the commonly used methods, such as the presence of femoral or carotid artery pulsations, arterial blood gas determinations, peripheral arterial pressure and intracardiac pressure monitoring. But recent studies suggest that end-tidal carbon dioxide tension serves as a non-invasive measurement of pulmonary blood flow and therefore cardiac output under constant ventilation. A prospective clinical study was done to determine whether end-tidal carbon dioxide monitoring in open heart surgery under cardiopulmonary bypass could be used as a prognostic indicator of bypass weaning. We monitored end-tidal PCO2 values continuously during cardiopulmonary bypass in 30 patients. "Ohmeda 5210 CO-2 monitor" under infrared absorption method were incorperated into the ventilator circuit by means of a side point adaptor between endotracheal tube and ventilator tubing. 18 patients[Group I ] were res-ucitated from partial bypass followed by aorta cross clamp off and 12 patients[Group II ] from aorta cross clamp off followed by partial bypass. But there was no difference between two groups[p>0.05]. The value of end-tidal carbon dioxide tension during ventricular fibrillation or nearly arrest state was 6.6$\pm$2.9 mmHg, and at the time of spontaneous beating was 19.3$\pm$5.6 mmHg[Mean$\pm$Standard deviation], In conclusion end-tidal carbon dioxide tension monitoring provides clinically useful, continous, noninvasive and supplementary prognostic indicator during cardiopulmonary bypass weaning procedures.rocedures.

Coarctation of Aorta: A Report of 4 Cases (대동맥교약증 수술치험 4예)

  • Recent advances in the surgical treatment of congenital disorders of the heart have necessitated an accurate preoperative diagnosis. Right heart catheterization has become widely accepted as a research tool and diagnostic test to detect the heart diseases, especially in the congenital heart anomalies. Right heart catheterizations were carried out in 50 cases of congenital heart diseases at Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital, during the period of June, 1975 through September 1978. In age distribution, 27cases were below 10 years of age, 18 cases between 11 and 20, and 5 cases above 20 male to female ratio was 2.8:1. The distribution of congenital heart diseases was VSD [42%], TOF [36%], PDA [10%], ASD [8%], and PS[4%]. Of these, 44 cases [88%], were compatible with the clinical impressions that were made preliminarily before cardiac catheterization, and all the cases except 1 case of VSD was correlated well with the postoperative diagnosis. The right heart catheterization is considered to be reliable and accurate toll in the preoperative diagnosis of congenital heart diseases. These procedures caused complications such as left side hemiplegia [lcase], occlusion of the femoral artery [lcase], and transient ventricular tachycardia [1case], and so the complication rate of right heart catheterization was 6% [3 cases]. None of patients who have undergone right heart catheterization was died.

  • The aortic coarctation is located in the distal thoracic aorta or abdominal aorta, or both and is often called ‘middle aortic syndrome’ or ‘mid-aortic dyspastic syndrome’. Etiology is controversial and most cases are seen in young female women. Severe complication such as cardiac or renal dysfunction as well as cerebral hemorrhage may occur, so aggressive surgical intervention may take effect. Lately we experienced a middle aortic syndrome which was not typical because of the patient's advanced age at the time of clinical presentation. The Axillo-femoral artery bypass graft with 6 mm PTFE vascular graft was done.

  • In veterinary medicine amitraz has been used as an insecticide to eliminate mites, lice, and ticks in dogs, cats, goats, swine and cattle. We performed the experiment on bradycardiac effect of arnitraz in rabbits. Under anesthetized with urethane (1g/kg, 25% w/v, SC), the femoral artery was cannulated by a polyvinyl tube which was connected with pressure transducer for continuous measurement of heart rate. Amitraz (0.5-2.5mg/kg, IV) reduced the heart rate in a dose-dependent manner. Pretreatment of yohimbine (2.0mg/kg) or atropine (2.0mg/kg and 4.0mg/kg) affected the response of amitraz but pretreatment of prazosin (0.5mg/kg) or propranolol (1.0mg/kg) did not. Moreover, the effect of amitraz was blocked by vagotomy. The result of our experiment suggest that amitraz may reduce the heart rate by increasing acetylcholine through activating vagal nerves and ${\alpha}_2$-adrenoceptors could be involved in activating vagal nerves.
