겨울철 도시부 노면결빙사고 발생에 미치는 요소에 관한 연구 (A Study on the Effects of Factors of Traffic Accidents Caused by Frozen Urban Road Surfaces in the Winter)

  • PURPOSES: According to accident statistics for road built in 2004, the ratio of accidents on frozen roads to normal roads is 0.9%, whereas the fatality ratio is 2.7%. The risk of accidents on frozen roads is very high. Measures taken every year to prevent traffic accidents of frozen roads in the winter season are still insufficient. Additionally, measures have been established mainly on rural roads. Therefore, for urban roads, analyses and measures to prevent accidents are lacking. In this study, data on accidents on frozen roads was used to search for the causes behind these accidents and measures to reduce accidents have been recommended. METHODS: In this study, collected data from the TAMS (Traffic Accident Management System), which were collected by the Seoul National Police Agency was used. The data were divided into vehicle, people, and condition of road. The analytical model used here was the Logistic Regression Model, which is frequently used for traffic safety and accident analysis. This study uses the odds ratio analysis to search for variables related to frozen road traffic accidents in each category. A total of 18 out of 47 variables were found to be the causes of accidents. RESULTS: From the results of the comparative analysis of 18 variables, the category of the condition of the road was found to be the most critical. Contrary to expectations, more accidents occurred in clear weather than in other conditions. Accidents on bridges occurred frequently, and its odds ratio was the highest compared with other road types. When BPT is operated, the probability of accidents on frozen roads is lower than in general conditions, and accidents occurred frequently on roads with less than four lanes. CONCLUSIONS : Based on the results of this study, suggestions for reducing the risk of future domestic road accidents in freezing conditions are indicated as follows. First, it is necessary to perform a technical review of the urban road traffic accidents caused by frozen roads. Second, it is necessary to establish criteria for the study of the road environment based on the major causes of road accidents on frozen roads. Third, improvements in urban road environmental factors should be made.

회전익항공기용 연료탱크 충돌충격시험 수치모사 연구 (Numerical Simulation of Crash Impact Test for Fuel Tank of Rotorcraft)

  • 항공기 기체와의 체결부위가 많은 연료탱크는 체계연관성이 큰 대표적인 핵심 구성품 중의 하나로 다루어져 왔다. 회전익항공기에 광범위하게 적용되고 있는 내충격성 연료탱크는 항공기 추락 시 탑승자의 생존성 향상에 크게 기여하고 있다. 미육군에서는 항공기 추락 후 화재에 의한 인명손실을 원천적으로 방지하기 위해 군용 회전익기 역사의 초기 단계부터 연료탱크 고유의 내충격성에 관련된 군사규격을 제정하여 적용해 왔다. V-22 등의 잘 알려진 사례에 따르면 미군사규격(MIL-DTL-27422D)에서 요구하고 있는 연료탱크 충돌충격시험을 원활하게 통과하지 못하는 경우, 해당 연료탱크가 장착되는 항공기 자체의 개발일정에 심각한 지장을 초래한다. 연료탱크 충돌충격시험은 시편자체의 제작비용 및 준비기간이 상당히 소요되므로, 설계 초기단계부터 충돌충격시험에 대한 일련의 수치적 모사(numerical simulation)를 통해 실물에 의한 시행착오의 가능성을 최소화해야 한다. 본 연구에서는 충돌모사 프로그램인 Autodyn을 이용하여 연료탱크 충돌충격시험에 대한 수치적 모사를 수행하였으며, 그 결과로 구해진 등가응력 및 내부압력 평가를 통해 회전익항공기용 연료탱크의 내충격 성능을 고려한 설계방향을 제시하였다.

중환자실로 입원한 폐결핵 환자의 임상 양상과 예후 인자 (Clinical Characteristics and Prognostic Factors in Patients with Pulmonary Tuberculosis Admitted to Intensive Care Units)

  • Background: Pulmonary tuberculosis (TB), requiring the intensive care unit (ICU) care, has been a high-mortality condition until now. In the present study, we aimed to investigate clinical features and parameters associated with TB mortality. Methods: From August 2003 to December 2008, patients with microbiologically or histologically confirmed pulmonary TB then admitted to the ICU, were retrospectively enrolled into the study. Upon enrollment, their medical records were reviewed. Results: Forty three patients (30 males, 13 females) were included and their mean age was 63.8 years (range: 17~87 years). Twelve patients died, an overall in-hospital mortality of 27.8%. The main reason for the ICU care was dyspnea or hypoxemia requiring mechanical ventilation (n=17). Other diagnoses for ICU care were hemoptysis, monitoring after procedures, neurologic dysfunction, shock, and gastrointestinal bleeding. On univariate analysis, the factors affecting the mortality were malnutrition-related parameters including low body mass index, hypoalbuminemia, lymphocytopenia, and hypocholersterolemia, as well as severity-related variables such as high acute physiology and chronic health evaluation (APACHE) score, number of involved lobes, and high C-reactive protein. In addition, respiratory failure requiring mechanical ventilation and acute respiratory distress syndrome contributed to patient fatality. It was shown on multivariate analysis that respiratory failure and hypoalbuminemia were significantly independent variables associated with the mortality. Conclusion: Acute respiratory failure is the most common reason for the ICU care and also the most important factor in predicting poor outcome. In addition, our data suggest that the parameters associated with malnutrition could be possible factors contributing to mortality.

우리나라 유행성뇌염(流行性腦炎)의 역학적(疫學的) 및 혈청학적(血淸學的) 조사연구(調査硏究) (Epidemiological and Serological Investigation on Epidemic Encephalitis in Korea)

  • The author has investigated epidemiological features of human cases of epidemic encephalitis (E. E.) in the Republic of Korea and the status of antibody requisition in pre-and post-epidemic time. And virological and serological studies with regarding the relationship of E. E. infection between human and piglet, and field survey against its vector by means of virus isolation from mosquitoes were carried out. Finally, vaccine field trial against human population has also been evaluated in order to confirm its effectiveness. The results of the studies are summarized as follows : 1. The annual incidence of reported cases during the past 25 years (1949-1973) in the Republic of Korea has shown two patterns, one was typical cyclic incidence and the other one was irregular. Annual average morbidity and mortality rate per 100,000 population were 5.7 and 2.1 and fatality rate was 34.6% in typical cyclic years. 2. With regard to the geographical distribution of E. E., the province of Jeolla-Bug-Do illustrated the highest incidence regardless of the epidemic size. 3. The main epidemic period was between mid-August and mid-September (above 90% of the total number of cases). The first case was reported in middle of July and the epidemic ceased in late of October. 4. An analysis of the age distribution of cases of E. E., has shown that above 90% of the total cases occurred in the age groups under 14 years and it was noted that about its 54% were occurred in the age groups between 5-9 years group. 5. Through the Haemagglutination Inhibition (H-I) test for the laboratory diagnosis of E. E., it was found that higher H-I antibody titer was usually detected in the convalescent phase, 15 days after onset. 6. The H-I antibody survey against 563 healthy population by age groups during the pre-epidemic season showed that 422(75%) were less than H-I titer, 1:20 and 122(21.7%) were positive H-I titer, 1:20. Among the 94 American in Seoul who had not been in E. E. endemic area previously only one person had appeared sero-conversion as a H-I titer of 1:80 after post-epidemic season. 7. The E. E. virus could be isolated from the mosquitos pools-C, tritaeniorhyncus which were caught between late July and middle August. 8. E.E. Virus was also isolated from piglet blood on early August and H-I antibody conversion was occurred mostly on middle of August. 9. H-I antibody sero-conversion rate reached to high level when vaccine purified by mouse brain tissue inoculated, showing 98.9%. Higher antibody titer was acquired when booster inoculation was performed, Four fold rise of H-I add N-T antibodies was confirmed with 93.2% and 82.1% respectively.

  • Purpose: There are an insignificant number of studies done on the demographics of intoxication patients and on the characteristics of toxic exposure on a long term basis in Korea. The objective of our survey is to investigate the epidemiologic characteristics of intoxication in a metropolitan emergency department in order to more efficiently manage intoxication patients. Methods: We retrospectively reviewed the medical records of intoxication patients who visited the emergency department of a high end medical facility between January, 1998 and June, 2009. We investigated the trend of the substances people became intoxicated with during the study period and we analyzed the age, gender, year and distribution of patients and the outcome of the patients. Results: There were 1544 cases of intoxication during the study period, and the cases made up 0.37% of the total visitors to our emergency department, which is a high end medical facility located in the city. Most of the patients were female (70%) in their twenties and thirties. The most commonly ingested intoxication substances were sedatives, analgesics and pesticides. Unlike in the province, antidepressant abuse is on the rise while pesticide abuse is falling. The overall admission rate was 24.8% and the mortality rate was 1.6%. Pesticides intoxication was the most common cause of death (76%). Pesticides intoxication, a male gender and old age were the most significant fatality-related factors. Conclusion: We think that there is a need to investigate the actual conditions of drug intoxication in the city and prepare measures to prevent drug intoxication.

Schedule-Dependent Effects of Kappa-Selenocarrageenan in Combination with Epirubicin on Hepatocellular Carcinoma

  • Hepatocellular carcinoma (HCC) has a relatively higher incidence in many countries of Asia. Globally, HCC has a high fatality rate and short survival. Epirubicin, a doxorubicin analogue, may be administered alone or in combination with other agents to treat primary liver cancer and metastatic diseases. However, the toxic effects of epirubicin to normal tissues and cells have been one of the major obstacles to successful cancer chemotherapy. Here, we investigated the effects of epirubicin in combination with kappa-selenocarrageenan on mice with H22 implanted tumors and HepG-2 cell proliferation, immune organ index, morphology, cell cycle and related protein expressions in vivo and in vitro with sequential drug exposure. The inhibitory rate of tumor growth in vivo was calculated. Drug sensitivity was measured by MTT assay, and the King's principle was used to evaluate the interaction of drug combination. Morphological changes were observed by fluorescent microscopy. Cell cycle changes were analyzed by flow cytometry. Expression of cyclin A, Cdc25A and Cdk2 were detected by Western blotting. In vivo results demonstrated that the inhibitory rate of EPI combined with KSC was higher than that of KSC or EPI alone, and the Q value indicated an additive effect. In addition, KSC could significantly raise the thymus and spleen indices of mice with H22 implanted tumors. In the drug sensitivity assay in vitro, exposure to KSC and EPI simultaneously was more effective than exposure sequentially in HepG-2 cells, while exposure to KSC prior to EPI was more effective than exposure to EPI prior to KSC. Q values showed an additive effect in the simultaneous group and antagonistic effects in the sequential groups. Morphological analysis showed similar results to the drug sensitivity assay. Cell cycle analysis revealed that exposure to KSC or EPI alone arrested the cells in S phase in HepG-2 cells, exposure to KSC and EPI simultaneously caused accumulation in the S phase, an effect caused by either KSC or EPI. Expression of cyclin A, Cdc25A and Cdk2 protein was down-regulated following exposure to KSC and EPI alone or in combination, exposure to KSC and EPI simultaneously resulting in the lowest values. Taken together, our findings suggest that KSC in combination with EPI might have potential as a new therapeutic regimen against HCC.

고속도로 선형 동질구간 기반의 안전성능함수 개발 (Development of Safety Performance Function Based on Expressway Alignment Homogeneous Section)

  • 과거 고속도로는 이동성 중심의 기능을 주로 담당하였으나, 삶의 질 향상과 의식 수준의 향상에 따라 '빠른 고속도로'에서 '안전한 고속도로'로 고속도로 기능의 패러다임이 변화하고 있다. 고속도로 교통사고는 2012년 기준 3,550건이 발생하였으며, 371명이 교통사고로 사망한 것으로 집계되었고, 치사율은 일반국도의 약 2배에 달하는 것으로 보고되고 있다. 본 연구에서는 고속도로 주요 6개 노선을 대상으로 교통사고건수와 교통량 기반의 사고예측모형(안전성능함수)을 개발하였다. 안전성능함수는 각 노선별로 도로의 선형 및 규모 등이 달라 정확하게 각 노선을 예측하는데 어려움이 있을 것으로 판단되어, 6개 노선의 통합 안전성능함수를 구축하고, 각 노선별로 교통사고를 보정할 수 있는 계수를 산출하였다. 본 연구의 결과는 향후 교통사고 예방을 위한 노선별 교통안전 전략수립의 기초자료로 활용함으로써 보다 안전한 고속도로 관리에 기여할 것으로 판단된다. 향후에는 각 노선별 특성에 따른 그룹별 통합모형을 통한 노선별 보정계수를 산출하여 신뢰성 있는 사고 예측값을 제시하는 연구가 수행되어야 할 것이다.

GIS를 이용한 교통사고의 분석 기법 개발 (A GIS Based Technique for Analyzing Traffic Accidents)

  • 본 논문은 도시의 가로망에서 발생하는 교통사고의 효과적인 관리와 분석을 수행하기 위한 방안을 제시한 글이다. 즉 지리정보시스템을 교통사고 관련 데이터의 구축과 분석의 기본적인 틀로서 이용하여 교통사고의 분석을 보다 과학적으로 수행할 수 있는 기법을 제시하고자하였다. 교통사고는 네트워크 상에서 발생하는 공간적 특징을 가지고 있다. 공간적 특성 및 관련 데이터와 사고데이터를 지리정보시스템을 이용하여 데이터베이스를 구축하고, 이러한 기반 위에서 점, 선, 면적인 분석을 수행할 수 있는 기법을 제시하였다. 이는 단순히 문자위주의 교통사고 관리시스템보다 진일보한 시스템으로 판단되고, 효과적인 원인 분석 등에 활용 필 수 있다는 측면에서 본 연구에서는 수원시의 특정부분에 있어서의 사고데이터를 지리정보데이터베이스로 구축하였다. 이를 기반으로 대상지역에 대한 점, 선, 면적인 제반 분석을 수행하였다. 한편, 구축된 데이터베이스를 바탕으로 특정공간 상에서 사고가 얼마나 많이 발생하는지, 또는 어떤 유형의 사고가 많이 발생하는지를 판단할 수 있는 계량적인 판단을 할 수 있는 지수들을 소개하여 교통사고에 대한 각 소구역별 세부분석을 실시하였다. 끝으로, 지형분석자료를 바탕으로 도로의 연장과, 도로의 교차점 수를 설명변수로 하는 교통사고 예측모형에 대한 교통사고빈도 회귀식을 도출하였다.

대면교행터널에서 배연시스템의 구축과 화재모형실험에 의한 적정성 평가 (Construction of the Smoke Exhaust System and Its Applicability by the Fire Model Test for a Bidirectional Tunnel)

  • 대면 교행 터널의 경우 차량의 교행으로 사고 위험성이 일방향 터널에 비해 1.5배 정도 높으므로 화재가 발생할 위험성도 커지게 되며, 터널내 화재시 입출구로 이용객이 대피하므로 인명 대피 완료전까지 제트팬을 가동하지 못하는 문제가 있다. 특히 장대터널의 경우 연기확산으로 인하여 대형 인명 피해가 발생하기 때문에 터널내에 별도의 인명 대피 통로와 배연 시스템을 구축하여 인명 피해를 최소화하는 것이 필요하다. 본 연구에서는 화재 시뮬레이션 및 축소모형실험을 수행하여 터널내에 구축된 배연시스템의 효과를 검증하였다. 실험결과 대피 통로만 설치시는 최종 인명 대피 시간이 335초가 소요되었으나, 배연 시스템을 구축할 때는 185초가 소요되었다. 또한 화재원 부근의 온도가 $60^{\circ}C$ 정도 감소되었으며, 연기 확산 속도는 배연 시스템 미가동시 $0.36\~0.82\;m/sec$이고, 가동시는 $0.27\~0.58\;m/sec$로 현격히 감소하였다. 위 결과로부터 대피자의 이동속도가 $0.7\~1.0\;m/sec$인 점을 고려할 때, 150 m 간격으로 설치된 피난문을 통해 안전한 대피가 가능하였다. 본 연구는 배연시스템에 대한 화재모형실험 분석을 통해 향후 대면 교행 터널의 방재에 대한 기초자료를 제공하는데 그 의미가 있다.

  • 2019년 12월에 중국 후베이성 우한시에서 원인 미상 폐렴이 무리 지어 발생하기 시작하였다. 환자의 하기도에서 이전에 알려지지 않은 새로운 코로나바이러스가 분리되었으며 severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2)로 명명되었다. 세계보건기구는 SARS-CoV-2에 의한 질병을 코로나바이러스감염증-19(coronavirus disease 2019, COVID-19)로 명명하였다. 2020년 3월 11일에 COVID-19는 전세계 최소 114국으로 퍼졌으며 약 4,000명이 사망하여 세계보건기구는 COVID-19가 세계적 대유행임을 선언하였다. 지난 20년 동안 새로운 코로나바이러스에 의한 두 번의 유행이 있었다. 2002-2003년 중증급성호흡기증후군(severe acute respiratory syndrome, SARS) 유행 시에는 8,098명의 확진 환자와 774명의 사망자가 발생하였으며, 2012년에 사우디 아라비아에서 시작되어 현재까지 주로 아라비아 반도에서 발생하고 있는 중동호흡기증후군(Middle East respiratory syndrome, MERS)는 2019년까지 총 2,499명의 환자와 858명의 사망자를 발생시켰다. 본 종설의 목적은 2020년 3월 12일까지 알려진 SARS-CoV-2의 특징과 전파 양상 및 COVID-19의 임상 증상을 알아보고 SARS와 MERS와의 유사점 및 차이점에 대하여 간략하게 소개하는 것이다.
