Alternative Prevention on Human Error of Fatal Injuries by the Mobile Cranes (이동식 크레인 사망재해의 인지공학적 대안)

  • According to the statistics, occupational fatal injuries by mobile cranes were about 12 per year in whole industrial. Mobile cranes are widely used in various parts of industries to improve the efficiency of the work. However considerable number of fatal injuries happen each year during the operation of the machines. In this study, the current regulations to be adequate in industrial site have to be renew in order to prevent the fatal injuries by mobile cranes. Fatal injury analyses were conducted with several accident cases by the mobile cranes. For each accident, the causes of the injuries were examined and proper safety measures were proposed. In this study, the mobile crane showed a high fatality rate in industrial accidents and no detailed cause analysis of fatal accidents was conducted in terms of unsafe acts or conditions. This study proposed a revision of the standard guideline as an accident prevention measures through in-depth analysis of fatal accidents. First, among the mainly five machines caused the accidents, mobile crane was higher for the second showed 0.6% for number of fatalities compared to number of mobil cranes and for the third showed 11% for number of fatalities compared to number of injuries. Second, main cause of cognitive engineering agenda was visibility, responsibility, affordance. As the measures to prevent accidents before starting operation, alternative revision for the fool proof including visibility, responsibility, affordance etc. for the fool proof measures was proposed. Third, alternative revision as cognitive accident prevention for the fail safe measures was proposed.

The Clinical Feature and Prognostic Factor of Glyphosate Intoxication Patients (글리포세이트 중독 환자의 임상 양상 및 사망 관련 인자 분석)

  • Purpose: Glyphosate is widely used and its toxic exposures are not rare. Occasionally, glyphosate intoxication can lead to death. The aim of this study is to analyze clinical findings and fatality in glyphosate intoxication. Methods: Clinical data on acute glyphosate intoxication were prospectively collected at 28 hospitals nationwide between August 2005 and July 2006. The patients' clinical symptoms and characteristics of fatalities were investigated and statistical analysis was performed. Results: Among 105 patients who were finally included, gastrointestinal symptoms(59%) were the most common. A significant difference in the amount ingested was observed between patients with higher systolic blood pressure and those with systolic blood pressure less than or equal to 80 mmHg (p<0.001). The more the patients ingested, the more aggravated their mental status became (p=0.004). Seven patients(6.7%) died, and all of them had ingested greater than or equal to 200 ml. Patients who died had ingested greater amounts than the survivors (p<0.001), and their mental status was worse (p<0.001), and systolic blood pressure was lower (p<0.001). According to the result of logistic regression analysis, relative risk was 24.1-fold higher in the 'poor' mental status group compared with 'good'. Conclusion: Patients who ingested large amounts of glyphosate showed poor mental status and lower blood pressure. Statistical difference in amount ingested, mental status, and systolic blood pressure was observed between survivors and patients who died. Ingested amounts and mental status were the most important factor of the prognosis of glyphosate intoxication.

Different Clinical Courses for Poisoning with WHO Hazard Class Ia Organophosphates EPN, Phosphamidon, and Terbufos in Humans (WHO 분류 1 등급 EPN, Phosphamidone, Terbufos 유기인계 중독환자의 임상 양상)

  • Purpose: Extremely hazardous pesticides are classified as World Health Organization (WHO) hazard class Ia. However, data describing the clinical course of WHO class Ia OP (organophosphate) poisonings in humans are very scarce. Here, we compare the clinical features of patients who ingested hazard class Ia OPs. Methods: This retrospective observational case study included 75 patients with a history of ingesting ethyl p-nitrophenol thio-benzene phosphonate (EPN), phosphamidon, or terbufos. The patients were divided according to the chemical formulation of the ingested OP. Data regarding mortality and the development of complications were collected and compared among groups. Results: There were no differences in the baseline characteristics and severity scores at presentation between the three groups. No fatalities were observed in the terbufos group. The fatality rates in the EPN and phosphamidon groups were 11.8% and 28.6%, respectively. Patients poisoned with EPN developed respiratory failure later than those poisoned with phosphamidon and also tended to require longer mechanical ventilatory support than phosphamidon patients. The main cause of death was pneumonia in the EPN group and hypotensive shock in the phosphamidon group. Death occurred later in the EPN group than in the phosphamidon group. Conclusion: Even though all three drugs are classified as WHO class Ia OPs (extremely hazardous pesticides), their clinical courses and the related causes of death in humans varied. Their treatment protocols and predicted outcomes should therefore also be different based on the chemical formulation of the OP.

Effect of Seizure on Prognosis in Acute Endosulfan Intoxication (급성 endosulfan 중독환자에서 경련이 예후에 미치는 영향)

  • Purpose: In highly doses, endosulfan lowers the seizure threshold and elicits central nervous system stimulation, which can result in seizures, respiratory failure, and death. Management of seizure control is essential for survival and prognosis of intoxicated patients. This study assessed whether seizure time was an independent predictor mortality in patients with endosulfan poisoning. Methods: This retrospective study enrolled patients with endosulfan poisoning presenting to Masan Samsung Hospital and Gyeongsang National University Hospital from January 2003 to December 2008. The data were collected from clinical records and laboratory files. Using a multivariate logistic analysis, data on the total population was retrospectively analyzed for association with mortality. Results: Of the 24 patients with endosulfan poisoning, nineteen (79.1%) experienced seizure. The patients in the seizure group showed significantly lower Glasgow coma scale score, base excess, bicarbonate, and significant existence of mechanical ventilation, as compared to the non seizure group (n=5). Seizure, Glasgow coma scale score, systolic blood pressure, bicarbonate level, need for respiratory support, pulse rate, respiratory rate, pH, base excess, and seizure time were associated with mortality. The fatality rate of endosulfan poisoning was 54.1% with higher mortality among patients experiencing. Longer seizure time was associated with higher mortality. Conclusion: Seizure time can be a significant independent predictor of mortality in patients with acute endosulfan poisoning. Physicians should aggressively treat for seizure control in patients with acute endosulfan poisoning.

An Analysis of Change in Traffic Characteristics with Fog (안개 발생에 따른 교통 특성 변화 분석)

  • The adverse weather is known as a factor that interrupts traffic flow and causes traffic accidents and traffic congestion by lowering visibility of drivers. Especially, in case of fog unlike any other weather conditions, traffic accidents lead to serious accidents and the fatality of the accidents is known to be high. This paper aims to analyze uninterrupted traffic flow characteristics under foggy conditions among adverse weathers. The traffic volumes and speeds under foggy and normal conditions were analyzed. Results indicated that fog with low visibility causes the most insignificant reduction in traffic volumes. On the other hand, the reduction in the speeds due to low visibility was evident. In addition, the relationship between flow, speed, and density in fog were analyzed. Analysis results showed that the fog with less than 200m visibility had clear impact on traffic flow.

  • Recent researches show that the traffic fatality rate in fog is higher than that in rain and snow. In that sense, methods to measure visibility accurately and to notify the measured visibility information to drivers are required. We thus developed a system for measuring the road visibility in realtime by employing CCTV cameras. In this paper, we also propose a wide-area visibility information system through measuring the visibility of every highway by using the developed system. The proposed wide-area visibility information system uses the video clips from the CCTV camera instead of the output from the expensive fog sensor. Hence, we expect that it can reduce the cost of installation and operation dramatically and the measured visibility information has a number of applications.

  • Most nations including Korean government make a great endeavor to realize low-carbon and green-growth world. We also work hard to expand bicycle facilities and bicycle road in order to increase bicycle transportation rate. Nowadays number of cyclists is increasing but fortunately, bicycle accidents also increase rapidly. Most data of bicycle accidents published by National Police Agency annually are represented as frequencies in two dimensional contingency tables. In this work, risk rates and characteristics of bicycle accidents are analyzed by using concepts of the probability and conditional probability. Especially with numbers of estimated cyclists and registered cars, risk rates of various kinds of bicycle accidents are obtained. Under the assumption of the conditional independence, probability of bicycle accident occurred at realistic situations could be estimated. Furthermore we discuss to reduce bicycle accidents with these results obtained in this work.

Study on Survival Effectiveness of Intelligent System for Warrior Platform by using AWAM (지상무기효과분석모델(AWAM)을 활용한 워리어 플랫폼 지능형 조절 시스템 생존 효과도에 관한 연구)

  • Survivability in a battle field is the most important aspect to the warriors. To analyze the survival effectiveness of warrior platform, the simulation via war-game model is an essential step in advance to the development of platform. In this study, Army Weapon effectiveness Analysis Model(AWAM) was utilized for analysis. Several weapon parameters were adjusted to apply the characteristics of warrior platform in some cases of the defense and survival system. Especially, adjusted triage possibility, probability of kill, fatality and accuracy were employed as parameters in the simulation program to evaluate the survival effectiveness of intelligent system based on the previous researches. In the future battle field or virtual space in the AWAM, the warrior platform intelligent system could react emergency treatment on time by expoiting the bio-information of man at arms. Considering the order of supply priority, special force was selected as operating troops and battle scenario without engagement was selected to measure accurate survival effectiveness. In conclusion, the survivability of defence and survival system of the warrior platform was about 1.47 times higher than that of current system.

Measures of Reducing Traffic Accidents by Aging Bus Drivers (시내버스 운전자의 고령화에 따른 교통사고 저감대책 마련에 관한 실증적 연구)

  • The Implementation Semi-Public Management System of Intra-City Bus brings improvements of the position stability, work environment and welfare of the bus drivers. As these benefits prevent drivers' job changes, average age of the drivers increases. Some side effects such as fatality increase after the implementation of the new system are becoming social problems. To minimize such problems, it is necessary to prepare the measurements reflected drivers' age increase and driving characteristics of old drivers to decrease traffic accidents of intra-city buses. The existing measurements are mostly simple policy. To get over such limitations, this study grasped driving characteristics by surveys and driving aptitude test targeting old drivers and non-old drivers who brings about traffic accidents actually. As a result, the characteristics of the old drivers were understood precisely. The measures of decrease of the traffic accidents are suggested by the analysis of the behavior characteristics of the old drivers through the structural equation model.

An Accident Analysis for Reducing Railway Staff Fatalities (종사자의 직무사고 현황분석에 관한 연구)

  • As a result of investment in railway safety focusing on passenger safety, the train accident rate has been reduced by half domestically and we have reached a high level of safety compared to other countries and other transportation modes. However, accidents related to staff are still at a high ratio compared to other countries. There have been few studies on staff safety and no relevant systematic safety measures have been implemented. More than 90% of staff fatalities occurred from rolling stock in motion and electrification, which are representative railway accidents. In this study, causes of accidents, and current safety measures for staff are analyzed focusing on trains in motion. The results can be utilized for developing new safety measures.
