Deleyed Cardiac Tamponade After Open Heart Surgery (Two Cases Report) (개심술후에 발생한 지연성 심장압진증)

  • Delayed cardiac tamponade in an uncommon and frequently fatal complication after open-heart surgery. We had been experienced two cases of delayed cardiac tamponade as a complication of open-heart surgery and treated successfully by reinsertion of pericardial drain through subxiphoid route. First case was 60 years old female patient and underwent MVR under impression of MSi + Ti Second case was 19 years old male patient and underwent total correction of T.O.F.with Blalock shunt [Lt]. Both cases had Initial symptoms, which were epigastric pain, chest tightness, dropped blood pressure, and increased pulse rate and respiratory rate, mimic as low cardiac output syndrome after open-heart surgery. Roentgenogram of the chest showed a rapid increased cardiothoracic ratio. It is important to realize the presence of late cardiac tamponade for proper diagnosis of complication after open-heart surgery.

Congenital Aortic Stenosis: Report of 2 Cases (선천성 대동맥 판막 협착증 치험 2예)

  • Congenital aortic stenosis, a relatively uncommon congenital heart disease, may cause heart failure and may be fatal. In recent years, increased clinical awareness and improved diagnostic and operative technique has made accurate diagnosis and successful treatment possible. Recently we experienced 2 cases of congenital aortic stenosis, and which was corrected surgically. The first case was 9 years old boy, and second case was 16 years male. Preoperative diagnosis was entertained by angiography and cardiac catheterization in both cases. In each case, aortic valve opening was widened by incision along the fused commissure between the combined left and right coronary cusp on one side, and the noncoronary cusp on the other side. Post-op. pressure gradient between the aorta and left ventricle markedly reduced, in the first case, 50mmHg, and in the 2nd case, 55mmHg.Both patients discharged with good results 2 weeks after open heart surgery.

  • Paraquat is a very potent herbicide which causes fatal toxicity when ingested, and there is no specific antidote against it. Human ingestion induces acute renal failure, hepatic dysfunction and progressive respiratory failure with high mortality rate. Clinical investigation and medical treatment were done on two cases of acute renal failure caused by paraquat poisoning admitted to the Department of Internal Medicine, Wonkwang University Oriental Chonju Medical Hospital. We report two cases of patients who survived after acute paraquat intoxication, by means of oriental medicine such as Gamdutang, a typical antidote of toxins, chinese ink as an absorbent and burned powder of Rhei Radix et Rhizoma for laxative and so on, western medicine such as gastric lavage, diuretics and fluid therapy. We suggest more experiments and studies related to such treatment for paraquat poisoning be conducted.

Surgical Treatment of VSD with Endocarditis: 2 Cases (심내막염을 합병한 심실중격결손증의 외과적 치료 : 2례 보고)

  • Bacterial endocarditis has been well recognized as an important complication of congenital heart disease. The most common. form of congenital heart disease is the VSD, of which natural history is spontaneous closure, pulmonary vascular disease, symptoms, and endocarditis. The incidence of endocarditis is relatively low. But endocarditis is almost universally fatal if untreated. Two cases of VSD with endocarditis, 4 \ulcorneryear male and 17 \ulcorneryear female, were treated at Department of Thoracic and Cardiovascular Surgery, Chonbuk National University. In the First case, the VSD was perimembranous type and vegetation located on the septal leaflet of the tricuspid valve. After 7 week medical treatment, simple closure of the VSD, removal of vegetation, and tricuspid annuloplasty were performed. In the second case. the VSD was subpulmonic type and the pulmonic valve was destructed due to vegetation. So the VSD was closed with interrupted 4 \ulcorner0 Prolene sutures and the pulmonic valve was excised. Postoperative course of all cases was uneventful.

  • Esophageal perforation due to the air pressure generated by forcefully evaporating gas is seldomly reported. If the diagnosis is confined to the injury of the oral cavity and the pharynx, missing the injury of the esophagus, the result may be fatal. Cases like this must be managed by early diagnosis and appropriate surgical intervention. The most important thing for early diagnosis is suspicion of esophageal injury from history and physical examination. We report two cases of esophageal pneumatic perforation caused by an explosive gas from the carbonated beverage bottle.

Malignant Fibrous Histiocytoma of the Heart A Case Report and Review of the Literature (심장내 악성 섬유성 조직구종)

  • A 45-year old woman with congestive heart failure due to left atrial tumor was operated on. Three tumor masses arising from posterior wall, atrial septum, anterior portion of mitral valve were resected. Pathological diagnosis was malignant fibrous histiocytoma [MFH]. Above one case and sixteen previous reports are reviewed. Eleven cases out of 17 were females. The tumors all originated primarily in the left atrium and 8 had distant metastases. The metastatic sites are lung [4 cases], brain [2 cases], liver, jejunum, cervix and pleura etc. Careful pathologic study is necessary to differentiate the uniformly fatal MFH of the heart from the more common benign atrial myxoma. The treatment modalities are surgical resection, chemotherapy, and radiation therapy & the prognosis of intracardiac MFH is poor. We underwent partial resection of left atrial MFH and obtained symptom relief and patient still alive 7 months post-operatively in state of NYHA class II.

  • Frequently, we have been noted foreign bodies in air way, which can produce different signs and symptoms needed for differential diagnosis with other diseases. Occasionally, it can be followed by some pulmonary complications and removal difficulty. Therefore it can be serious and fatal in few cases, especially in children. Three cases were evaluated with clinical and roentgenographic features of air way obstruction due to foreign bodies and discussed with previous many literatures.

Surgical Treatment of Boerhaave's Syndrome (Boerhaave증후군 환자의 외과적 치료)

  • Boerhaave's syndrome is a very fatal condition occuring esophageal rupture during emesis and has the worst prognosis of the esophageal perforation. From May 2007 to March 2012 11 patients underwent surgical treatment for Boerhaave's syndrome at Inje University Sanggye Paik Hospital. There were 11 males whose mean age was 49.9 years ranging from 42 to 59. 2 cases of primary closure was performed and 9 cases of anastomosis using EEA (Esophago-Enteric Anastomisis) stapler. Mean operation time was 154.4 minutes and one patient who undergone primary repair died because of mediastinitis and sepsis on $35^{th}$ post-operative day. There was no leakage at other patients. Post-operative complication was one operative wound infection and one post-operative bleeding which was treated completely. Surgical treatment for Boerhaave's syndrome using EEA stapler is simple and effective technique but further studies with large number of cases should be carried out for better outcome.

  • Foreign bodies in the airway are very important and not uncommon conditions in the field of E.N.T., and practically there are many reports of them. Occasionally, it can be followed by serious complications and technical difficulty to remove. Therefore, foreign body in the airway is serious and fatal in some cases. Recently, the authors experienced 8 interesting cases of foreign bodies in the airway and a brief review of literature was made.

  • Babesiosis, caused by Babesia microti and B. divergens, is transmitted by Ixodid ticks. Symptoms of babesiosis vary from a mild flu-like illness to acute, severe, and sometimes fatal and fulminant disease. In Korea, 7 imported babesiosis cases and 1 endemic case have been reported. We report 2 cases of severe babesiosis initially mistaken as malaria. The first patient was complicated by shock and splenic infarction, the other co-infected with Lyme disease. As the population traveling abroad increases every year, physicians should be aware of babesiosis which mimics malaria, co-infection with other diseases, and its complications.
