Right Atrium Rupture as a Result of Blunt Trauma from a Traffic Accident - One case report - (교통사고에 의한 둔상으로 발생한 우심방 파열 - 1예 보고 -)

  • Traumatic cardiac injury is an extremely serious medical condition. It is possible to overlook a cardiac injury where there is no chest wall trauma. We here report the 47-year-old woman who got a crach car accident and had a tear of the right atrium. The distortion force from a decelerating injury may cause cardiac rupture at a fixed point. The most common symptom that alerts the clinician to a potentially fatal cardiac injury is the change in vital signs. Therefore cardiac injury should be considered in any patient with unexplained hypotension who has experienced decelerating trauma, even without external injury to the chest wall.

  • Parainfluenza virus infection is one of the causes of fatal rhabdomyolysis. Rhabdomyolysis can be aggravated by mitochondrial fatty acid ${\beta}$-oxidation disorders during prolonged periods of fasting. Moreover, in patients with late-onset isovaleric acidemia, hyperammonemia may occur following catabolic stress. In the present report, we describe a case of a 4-year-old boy with parainfluenza virus infection and late-onset isovaleric acidemia that rapidly progressed to coma, seizures, and cardiorespiratory collapse. His serum ammonia and creatinine kinase (CK) levels were $385{\mu}Mol/L$ and 23,707 IU/L, respectively. Continuous renal replacement therapy (CRRT) was initiated using continuous venovenous hemodiafiltration, after which the ammonia and CK levels returned to normal. Thus, we recommend the immediate initiation of CRRT in the management of patients with life-threatening rhabdomyolysis and hyperammonemia.

  • In the present study, a Cook Book method and a Static System Analysis method were compared with each other on the basis of a seismic design criteria of fire-fighting facilities and analyzed. The Cook Book method is analyzed by dividing a pipeline in each same section. In this method, a stress analysis is not possible except for the section analyzed in such a way that a brace is designed according to the weight of pipe, water and pipe fitting. To the contrary, in case of the Static System Analysis method, the stress analysis for the whole pipeline can be performed because the whole pipeline is regarded as a single structure. For the fatal stress values locally generated, it is necessary to actively perform a pipeline analysis by installing a device capable of locally relieving the stress of the pipeline. In Korea, only the Cook Book method is provided as the seismic design criteria of fire-fighting facilities, which causes many problems with diversification of seismic design. Thus, it is necessary to apply the seismic design method of the pipeline by using various kinds of engineered Static System Analysis methods.

  • Subsequent to an allogenic stem cell transplantation(ASCT) on patients with hematologic malignancy(AML, ALL, CML, multiple myeloma, lymphoma etc.), chronic GVHD(graft versus host disease), which is an immunological reaction, occurs. With treatment results from patients who were diagnosed with ALL(acute lymphocytic leukemia), undergone BMT(bone marrow transplantation) and showed oral and skin lesions due to GVHD, treatment of oral manifestations of leukemia and its general management were studied. 90% of patients with chronic GVHD show change in the oral mucosa causing oral manifestations such as leukoplakia, lichenoid change of the oral mucosa, mucosal atrophy, erythema, ulceration and xerostomia. In treating GVHD, extensive systemic immunosuppression cause bacterial, viral, fungal infection that are fatal, and even if the treatment is successful, the patient is already in a severe immunosuppressed state. Therefore, localized target therapy is preferred. In another words, topical application(rinse, cream, ointment etc.) of cyclosporin and steroid in treating oral chronic GVHD is highly recommended, and the use of PUVA(Psoralen Ultraviolet A) and thalidomide is reported to be effective. In treating such diseases, dental treatment to control pain and prevent secondary infection of oral manifestations is very important. To those patients with systemic diseases who show limited effect by general dental treatment, non-invasive treatment such as the dental laser, in addition to the use of drugs, may be necessary to actively treat pain and help the healing process. For greater results, new effective methods are to be developed for treatment.

Cancer of Unknown Primary Site Mimicking Retroperitoneal Fibrosis (후복막강섬유화증으로 오인된 원발 부위 불명암 1예)

  • Retroperitoneal fibrosis (RPF) is a rare disease characterized by marked fibro-inflammatory tissue in the retroperitoneum. Approximately 70% of cases of RPF are idiopathic, while the rest can be secondary to several other causes. The diagnosis is mainly obtained by imaging modalities such as computed tomography (CT). However, histological examination should be considered when the clinical manifestations and imaging studies suggest malignancy. In particular, in malignant diseases with retroperitoneal metastases, abnormal collagen plaques are formed from an exuberant desmoplastic response, which may not be distinguishable from RPF on CT scans. Therefore, even if CT suggests RPF, biopsy is essential to identify malignant disease because it typically results in a fatal prognosis. Here, we report a case of metastatic adenocarcinoma of unknown primary site that developed only in the retroperitoneum and was initially diagnosed as RPF based on CT findings.

Traumatic Posterior Fossa Epidural Hematomas in Children : Experience with 48 Cases and a Review of the Literature

  • Objective : Epidural haematoma (EDH) most commonly occurs in the supratentorial area, particularly in the temporal region, of the brain. Posterior fossa epidural haematoma (PFEDH) is less frequently observed, accounting for only 1.2% to 12.9% of all EDH cases. Because of the non-specific symptoms and the potential for rapid and fatal deterioration in children, an early computed tomography (CT) scanning is necessary for all suspicious cases. The aim of the present study was to share the experience of 48 cases and review the literature concerning PFEDH. Methods : A retrospective analysis was conducted for 48 paediatric cases diagnosed with PFEDH and admitted to Yuying Children's Hospital of Wenzhou Medical University from January 2010 to August 2015. The clinical features and outcomes were analyzed and compared with previous literature. Results : Seventeen patients were surgically treated in this series and 31 patients received non-operative treatment. The outcomes were good in 46 patients, evaluated using the Glasgow outcome score (GOS), while mild disability was observed in one patient, and only one case showed severe disability. There were no cases of mortality in this series. Conclusion : Posterior fossa epidural haematoma is relatively rare compared with supratentorial epidural haematoma. Early and serial CT scans should be performed for all suspicious cases. The criteria for the surgical treatment of paediatric patients with PFEDH were concluded. The overall prognosis was excellent in paediatric patients.

A Study on the Characteristics of Low Temperature sintering Ceramic Siding Using Natural Minerals (천연광물을 활용한 저온소결 세라믹 사이딩의 특성에 관한 연구)

  • Recently, skyscraper building and apartment fires, which were rapidly spread out from a low floor to a rooftop, have become a frequent occurrence in mass media. This fire problems have a fatal disadvantage that the exterior wall finish of the building emits toxic gas in case of fire by using dry bit method or organic insulating material. Therefore, in order to remedy these problems, many exterior wall finishing construction methods have been proposed, but the current trend is to use existing construction methods due to problems such as economy, weight, and durability. On the other hand, in countries such as Germany and Japan, ceramic sidings are used as exterior finishing material for buildings, which is environmentally friendly, excellent natural beauty, long life, easy maintenance and high-quality exterior materials. However, those ceramic sidings have still the problems such as manufacturing cost and weight problem because of boosting the sintering temperature up to 1,350℃ or more. Also, conventional CRC, MgO, FRP sidings which are composed of pulp, glass fiber and organic materials, have been reports of deformation due to ultraviolet rays, discoloration, corrosion and scattering, surface rupture, lifting and peeling. Therefore, in this study as an alternative to solve this problem, halosite nano kaolin produced in Sancheong in Korea and frit flux were used to satisfy the required properties as ceramic siding using low temperature sintering (below 1,000℃) and lightweight materials such as pearlite. This study aims to design the optimal formulation and process of materials and to study the characteristics of nano-coated ceramic siding material development and to present relevant basic data. The findings show that ceramic siding for nanocoated building materials is excellent as a natural ceramic siding building material. The fire resistance of natural minerals and nano particle refining technology satisfy the bending strength of 80kgf / cm2, the volume ratio of 2.0 and the absorption rate of less than 10.0%.

  • Leucine rich glioma inactivated (LGI1) encephalitis is an uncommon neurological disorder rarely encountered in clinical practice. However, it is a potentially fatal autoimmune syndrome that can decrease the level of consciousness, possibly progressing to coma. Additionally, unless diagnosed and promptly treated, it can cause permanent cognitive impairment. Since LGI1 encephalitis can initially present with psychiatric symptoms, there can be delays in reaching a proper diagnosis. This report describes a case of a 47-year-old woman with LGI1 antibodies-associated limbic encephalitis who initially presented with psychosis. Her blood tests were normal and no MRI and EEG abnormalities were found. Cerebrospinal fluid analysis was negative for other possible infectious causes. Three months after admission, she was found to be LGI1 antibody positive. LGI1 encephalitis should be suspected in patients with symptoms such as memory loss, confusion, seizures, and psychiatric symptoms. Prompt diagnosis and treatment of LGI1 encephalitis are warranted because prognosis becomes worse when such actions are delayed.

  • As meters become digital and smart, energy data such as electricity, gas, heat, and water can be accurately and efficiently measured with a smart meter, providing consumers with data on energy used, so that real-time demand response and energy management services can be utilized. Although it is developing from a simple metering system to a smart metering industry to create a high value-added industry fused with ICT, illegal counterfeiting of electronic meters is causing problems in intelligent crimes such as manipulation and hacking of SW. The meter not only allows forgery of the meter data through arbitrary manipulation of the SW, but also leaves a fatal error in the metering performance, so that the OIML requires the validation of the SW from the authorized institution. In order to solve this problem, a quantitative confirmation device was developed in order to eradicate the act of cheating the fuel oil quantity through encoder pulse operation and program modulation, etc. In order to prevent the act of deceiving the lubricator, a device capable of checking pulse forgery was developed, manufactured, and verified. In addition, the performance of the device was verified by conducting an experiment on the meter being used in the actual field. It is judged that the developed quantitative confirmation device can be applied to other flow meters other than lubricators, and in this case, accurate measurement can be induced.

  • Recently, safety issues in companies and public institutions are no longer a task that can be postponed, and when a major safety accident occurs, not only direct financial loss, but also indirect loss of social trust in the company and public institution is greatly increased. In particular, in the case of a fatal accident, the damage is even more serious. Accordingly, as companies and public institutions expand their investments in industrial safety education and prevention, open AI learning model creation technology that enables safety management services without being affected by user behavior in industrial sites where high-risk situations exist, edge terminals System development using inter-AI collaboration technology, cloud-edge terminal linkage technology, multi-modal risk situation determination technology, and AI model learning support technology is underway. In particular, with the development and spread of artificial intelligence technology, research to apply the technology to safety issues is becoming active. Therefore, in this paper, an open cloud platform design method that can support AI model learning for high-risk site safety management is presented.
