병원 간호사의 환자중심 간호 경험 (Hospital Nurses' Experience of Patient-Centered Nursing)

  • Purpose: This study aimed to explore frontline nurses' experience of patient-centered care and understand the factors affecting its implementation in hospitals. Methods: Four focus group interviews were conducted with 30 nurses in two university hospitals. The following theoretical framework of patient-centered care was used: 1) Respect for patients' values, preferences, and expressed needs, 2) Care coordination and integration, 3) Information, communication, and education, 4) Physical comfort, 5) Emotional support and alleviation of fear and anxiety, 6) Involvement of family and friends, 7) Care transition and continuity, and 8) System issues. We performed a directed content analysis. Results: The most frequent patient-centered nursing practices of the hospital nurses were "promoting physical comfort" in inpatient settings and "providing information and communicating" in outpatient settings. The factors influencing patient-centered nursing included the health professionals' mindfulness, work overload and staff shortage, and unreasonable social demands and regulations. Conclusion: A more comprehensive patient-centered nursing practice should be implemented by improving "care transition and continuity," "family/caregiver involvement," and "system building." Health professionals' mindfulness is significant, and organizational supports addressing work overload and staff shortage are needed alongside change in social awareness.

재혼가족의 레질리언스 강화를 위한 실천모형 기초연구 (A Practice Model for Strengthening Resilience among Remarried Families)

  • Remarriage has been prevalent in Korea since the mid-1990s. Remarried families have reported that they face many challenges and difficulties. Life for those who remarry can worsen when the children from a previous marriage live together with new family members. It is interesting that some remarried families adjust well while others do not. This study focuses on developing a practice model for strengthening the resilience of remarried families with children based on a literature review. To understand and support remarried families, the resilience perspective was chosen, focusing on a strengths perspective with developmental, ecological, and preventative perspectives. In terms of resilience, the proposed practice model was developed on the basis of three concepts: belief systems, organizational patterns, and communication. The model can be used to help remarried families accomplish adequate integration by growing as a new family and can prevent another divorce. Moreover, it can assist practitioners and policy-makers develop programs and policies with a strengths perspective rather than a deficit or problem-oriented perspective. Based on the model, various programs need to be developed to strengthen the resilience of remarried families. The development of supportive programs is suggested. These can include remarriage preparation programs, remarriage life education programs, interactive web-based programs, and family camps. For a long time, remarried families have been labeled as abnormal and atypical owing to the normal family myth. This negative perception of remarried families must be altered so that they are accepted with a positive outlook in a society. For further studies, the development of diverse supportive adaptation programs for various types of remarried families and active research on them are recommended. The practice model can assist with the adaptation and growth of remarried families by strengthening their resilience.

장애인 탈시설 지원법 제정에 따른 장애인거주시설 이용인 가족대상 탈시설과 자립생활 인식에 대한 연구 (A Research of Recognition for Deinstitutionalization and Independent Living of the Disabled Living in Residential Facilities for the People of Family In accordance with the Enactment Deinstitutionalization of Disabled)

  • 본 연구는 탈시설 지원법 제정과 관련하여 장애인거주시설 이용자 가족을 대상으로한 탈시설과 자립생활에 대한 인식이 갖는 의미를 분석하는데 있다. 설문조사는 2018년 9월 10일부터 10월 12일까지 진행되었으며 최종 분석에 활용된 사례는 810명이다. 조사결과로 장애인 탈시설 지원법 제정을 인지한 이용자 가족 집단과 인지하지 못한 집단이 대략 절반으로 나뉘었으며 법제정 동의여부, 자치단체노력 여부, 자립가능성, 탈시설 동의여부에서는 동의하지 않거나 자립가능성이 낮다고 보는 가족의 인식은 제공된 정보가 없거나 낮은 수준임을 확인하였다. 이러한 결과는 이용인 가족을 대상으로 한 탈시설과 자립생활에 관련한 서비스나 정보제공이 부족했음을 의미하며 정확한 이해와 인식을 가질 수 있도록 정책과 서비스에 대한 올바른 정보전달교육이 필요함을 확인하였다. 또한 법제정 동의여부와 자치단체노력여부, 자립가능성에 대한 판단과 탈시설 동의여부는 퇴소 후 자립생활에 대한 긍정여부에 유의미한 영향을 미치는 것으로 나타났으며 퇴소 후 자립생활 준비에 유의미한 영향을 미치는 것으로 나타났다. 따라서 거주시설 이용자 가족의 올바른 인식과 나아가 이용자의 건강한 자립생활을 보장하기 위해서는 체계적인 정보교육지원과 지원체계가 필요함을 확인하였다.

베트남 미혼여성의 결혼이주 인식에 대한 탐색적 연구 (Exploratory Research on Marriage Migrant Recognition for Unmarried Vietnamese Women)

  • This study explored general marriage migration for 180 unmarried Vietnamese immigrant women and identified differences in recognition after the choice of marriage. The methods used were frequency analysis, kai verification, and independent t verification were conducted. The findings were as follows. First, unmarried Vietnamese women showed a receptive attitude towards marriage migration with the less-educated group more likely to opt for marriage migration. Economic benefit expectations topped other factors in regards to the image of marriage migrant women and motivation. Dual national identity benefits were also indicated. Second, the perception of external and illusionary images of the spouses of marriage migrant women was low; however, the perception of good occupations and gender equality was high. A vague expectancy of marriage was also found. The perception was high that children from multi-cultural families would be global bilingual talents and adjust to school; however, the perception of social discrimination or prejudice was low. The perception of social discrimination was low concerning the perception of social integration towards marriage migrant women; however, the perception of identities, cultural differences and employment was present. By contrast, the group opting for marriage migration showed a significantly low perception of social discrimination and difficulty in employment. The suggested measures are to regulate and maintain forms of marriage type, reinforce prior training systems for Vietnamese immigrant women (and spouses), enhance multicultural recognition, and supplement multicultural policies.

주부교육프로그램에 대한 요구도 분석 (A Study on the Needs for Housewives' Educational Program)

  • The purpose of this study was to analyze the housewives' needs for educational program. In this study the needs lot housewives educational program are consisted on 3 areas ; 1. program contents, 2. program management and 3. program activization plan. For the survey and statistical analysis the questionares from 445 full-time housewives who participate in educational program at present are used. The results of this study are following; 1, in the area of program contents the needs for housing/health, nutrition, family life planning for elderly stage and enhancement of parents-children relationship are very high. This contents are very related with research topics of human ecology. So the researcher of our science are expected to be involved more in the related program development and actualization in the field. 2. in the area of program management the needs for the concret management process such as human resource management and education period are much higher than the educational provision. 3. in the area of program activization the systematization of the program incording with program level is most needed. 4. the housewive who are keeping the social characteristics such as social orientation, the motivation of social integration, the participation in the social-oriented educational program have higher needs for the most entire program contents, management and activization plan.

환자중심성의 개념적 구성 요소: 환자와 가족구성원의 관점 (Conceptual Constructs of Patient Centeredness: Perspective of Patients and Family Members)

  • Purpose:The objective of this study was to identify the conceptual constructs of patient centeredness from the perspective of patients and family members in Korea, and to compare them with those included in the Picker Institute framework. Methods: Two focus group discussions were conducted. Each focus group consisted of six participants who had experienced being either a patient or a caregiver. We carried out a thematic analysis, and then compared the contents of our focus group discussions with the components of patient-centered care outlined by the Picker Institute. Results: Six conceptual constructs of patient centeredness emerged from the focus group discussions. Five of these overlapped with those outlined by the Picker Institute: 1)respect for patients' values, preferences, and needs, 2) coordination and integration of care, 3) information, communication, and education, 4) physical comfort, and 5) emotional support and alleviation for fear and anxiety. A new component that was not mentioned in the Picker Institute framework emerged from this study: "ease of making a complaint." Currently, "involvement of family and friends" and "continuity and transition" were not prominent components of patient centeredness according to our focus group discussions. Conclusions: This study presents the conceptual constructs of patient centeredness, five of which overlap with those outlined by the Picker Institute, and provides a qualitative basis of the patient experience survey currently being implemented by the Health Insurance Review & Assessment Service in Korea.

거주지역에 따른 결혼이민자 여성의 자아분화 및 문화적응이 결혼만족도에 미치는 영향 (Influence of Self-Differentiation and Acculturation on Marriage Satisfaction Among Immigrant Women by Residential Area)

  • This is a study that explores the influence of self-differentiation and acculturation among married immigrant women on their feelings of marriage satisfaction by residential area. The aim was to verify the level of self-differentiation and acculturation that married immigrant women secure from multicultural marriage. To achieve this objective, the study widely distributed a questionnaire that targeted women who were participating in education and other services at health and family support centers, multi-cultural family support centers, general social welfare centers, immigrant women shelters, and Korean language classrooms which are located in Seoul, Gyeonggi, Chungcheong, Jeolla, and Gyeongsang. Data analysis involved frequency analysis, descriptive statistics, one-way-Anova, and multiple regression analysis. Based on the results of descriptive statistics, two factors, namely, (1)interpersonal-relation differentiation, a sub-scale of self-differentiation, and (2)marginalization, a sub-scale of acculturation, had the lowest average. In verifying its various hypotheses, the study achieved the following results. Firstly, among demographic characteristics, there was difference of the mean in the marriage period, average monthly income, the frequency of meetings with the married woman´s parents-in-law and her own parents, and the average cost of supporting the woman's parents-in-law and her own parents. Secondly, among demographic characteristics, the variable of influencing marriage satisfaction showed negative influence in the case of women dwelling in farming and fishing villages. This shows that women residing in cities whether small, medium or large have higher marriage satisfaction. Also, as a result of verifying whether self-differentiation has influence on marriage satisfaction, the element of interpersonal-relations differentiation had a negative influence on marriage satisfaction. Thirdly, as for influence of acculturation upon marriage satisfaction, the study showed that just integration, which is a sub-scale of acculturation had positive effect on marriage satisfaction. In other words, the study showed that the interpersonal-relation differentiation among the sub-scales of self-differentiation among married immigrant women had a negative influence, and that integration among the sub-scales in acculturation had a positive influence on marriage satisfaction. Based on these results, in order to increase interpersonal-relation differentiation, as well as marriage satisfaction among immigrant women, the study suggests the integration of the women's families with the nuclear and extended families in the communities where the women reside.

다문화가정자녀에 대한 일반아동의 사회적 거리감에 영향을 미치는 요인 연구 (A Study of factors influencing on Children's Social Distance towards Children from Multicultural families)

  • 사회적 거리감(social distance)은 사회집단에 대한 태도를 측정할 수 있는 개념으로서 취약층의 사회행동 연구에 유용하다. 일반 아동들이 보이는 사회적 거리감이 두드러질수록 다문화가정자녀들의 자신감과 사회적 유능성이 저해될 수 있다. 이에 본 연구는 다문화가정자녀에 대한 일반아동들의 사회적 거리감에 영향을 미치는 요인을 다문화접촉경험, 긍정적 정서와 인지로 본 고정관념, 단일민족의 식을 중심으로 고찰하여 일반아동의 사회적 거리감에 관한 경험적 근거를 제시하고자 하였다. 주요결과는 첫째, 다문화접촉경험 중 높은 친밀도, 외국인 이웃의 인종(특히 백인과 흑인), 미디어와 다문화 교육을 받은 경우가 높은 수준의 사회적 거리감과 관련되었다. 둘째, 다문화가정자녀에 대해 긍정적으로 느끼고 인식할수록 사회적 거리감은 낮아졌다. 셋째, 위계적 회귀분석 결과, 다문화가정자녀에 대한 긍정적 정서, 긍정적 인지가 사회적 거리감을 유의하게 낮추는 효과가 나타났고. 친밀도와 다문화 교육은 사회적 거리감을 오히려 높이는 결과를 보였다. 이러한 결과에 근거하여 다문화가정자녀를 대하는 일반아동의 행동에 관한 사회복지실천을 위한 함의를 제시하였다.

Habermas의 세 행동체계를 융합한 초·중·고등학교 가정교과 식생활 교육과정 개발 (Development of Dietary Life Curriculum in Elementary and Secondary Schools Home Economics that Integrated Habermas's Three Systems of Action)

  • 이 연구의 목적은 세 가지 행동체계를 융합한 초·중·고등학교 가정교과 식생활 교육과정의 내용체계를 개발하여 가정교과에서의 융합교육 방안을 마련하는 데 목적이 있다. 이러한 목적을 이루기 위해서 문헌 고찰을 통해 세 행동체계를 융합한 초·중·고등학교 식생활 교육과정의 특징, 개발의 중점사항, 핵심역량, 목표를 도출하였고, 국내외 식생활교육 문헌, 국내외 가정과교육과정과 교과서를 고찰하여 항구적 문제와 하위 실천적 문제를 설정하고, 내용 요소를 선정하였다. 개발된 교육과정의 내용체계가 타당한지 전문가 타당도 검사를 하여 수정·보완하였고, 세 행동체계를 융합한 중학교 가정교과 식생활 교육과정의 성취기준을 개발하였다. 세 행동체계를 융합한 초·중·고 가정교과 식생활 교육과정의 내용 체계는 실천적 문제를 중심으로 되어있으며, 항구적 문제는 '건강한 식생활을 실천하기 위해 무엇을 해야 하는가?'이다. 이 교육과정의 영역은 5가지로 '건강과 식생활', '식단계획', '식품 구매와 관리', '조리', '식사'이며, 이 영역이 '개인과 가족', '사회'로 교육과정의 범위가 반복적으로 확장되어 있다. 이에 따라 5개 영역의 실천적 문제는 초·중·고등학교 급별, 두 차원(개인과 가족, 사회)으로 10개씩의 실천적 문제로 구성되어 있으며, 계열화의 원리가 적용되어 실천적 문제와 내용 요소가 나선형적 계열화가 되어있다. 이 교육과정의 내용 체계를 우리나라 2015 초등학교 실과와 중·고등학교 기술·가정과, 가정과학 교육과정의 식생활 영역 성취기준 및 학습 요소와 비교한 결과, 2015 개정 교육과정의 내용을 모두 포함하면서 더 추가된 내용이 있었다.

장애 유아 통합보육을 위한 교사 지원이 어린이집 교사의 교사 효능감에 미치는 영향 (The Effect of Teacher Support Program for the Integration of Handicapped Children on Teaching Efficacy of Daycare Center Teachers)

  • 본 연구는 장애 유아 통합보육을 위한 교사 지원이 어린이집 교사의 교사 효능감에 미치는 영향을 알아보고자 하였다. 연구에는 서울과 경기 지역 4개 어린이집 12명의 어린이집 교사가 실험 집단에 선정되었고, 5개 어린이집 12명의 교사가 통제 집단에 선정되었다. 교사 지원은 교사 교육과 현장 관찰 및 자문, 온라인 자문을 통해 실시되었다. 교사 교육은 집단 교육을 통해 발달 영역의 이해, 교육과정 수정, 활동중심삽입교수, 협동 학습, 직접 교수, 장애 이해 교육, 행동 지원, 가족 지원 등에 대한 교육을 실시하고, 개별 교육을 통해 각 교사들의 장애유아 통합보육의 요구를 반영한 장애유아 보육의 실제에 대한 교육을 실시하고 자문을 제공하였다. 실험집단 어린이집 당 10회씩 현장 관찰 및 자문을 실시하였고, 이메일과 핸드폰 자문을 통해 교사들의 수업을 지원 하였다. 교사의 교사 효능감 변화를 측정하기 위해 사전-사후 검사 통제 집단 설계를 적용하였으며, 장애 통합에 대한 일반 교사의 교사 효능감 척도(Teachers' Efficacy in Inclusive Practices: TEIP)로 사전-사후 검사를 실시하였다. 연구 결과의 분석을 위해 두 집단 간 교사 효능감의 사전-사후 검사 간 차에 대해 두 독립표본 t 검정을 실시하였고, 결과 두 집단 간 교사 효능감과 하위 영역인 통합교육 효능감, 협력 효능감, 행동관리 효능감의 사전 사후 변화에 유의미한 차이를 보였다. 본 연구에서 장애 유아 통합보육 현장에서 즉각적인 피드백을 제공하고, 어린이집 교사들의 통합보육 전문성을 향상시키고, 교사들의 실제적 요구에 반응적이고, 장애 유아 통합의 다양한 변인을 반영하고, 참여자들 간의 대등한 협력적 관계를 강조한 교사 지원을 제공한 점이 연구 결과에 긍정적 영향을 주었다. 본 연구 결과는 장애 유아 통합보육 지원에 대한 연구가 부족한 현장의 실질적인 자료로 활용될 수 있을 것이다.
