Bell's Palsy 환자의 건측-환측 안면부 피부온도차이에 관한 연구 (Differences Between Facial Skin Temperature of the Paralyzed Side and Those of the Normal Side in Bell's Palsy Patients)

  • Objectives : Bell's palsy in an acute peripheral facial nerve paralysis that usually affects only one side of the face. The seventh cranial nerve carries predominantly motor fibers, but also supplies some autonomic innervation, sensation to park of the ear, and taste to the anterior two thirds of the tongue. The aim of this study is to provide evidence of differences between facial skin temperature of the paralyzed side and normal side in Bell's palsy patients. Methods : the author studied 68 patients with Bell's palsy whose facial nerve function had been documented by the House-Brackmann grading system. We measured skin temperature of the forehead, zygoma, lower lip, temple and lower jaw area of the paralyzed side and those of the normal side. Results : there were significant facial skin temperature differences between the forehead area of paralyzed side and that of normal side of GrII(P<0.05) and III(P<0.05). The result also showed that the facial skin temperature difference according to the aflection period vanished when air temperature was calibrated (F=1.700, P=0.178). Conclusions : Thermography is a useful diagnostic tool in Bell's palsy if the air temperature is low enough to cool the facial skin temperature and the forehead area is evaluated as the sampling zone.

만성 재발성 벨마비 4예 (Four Cases of Chronic Recurrent Bell's Palsy)

  • Idiopathic facial nerve palsy, or Bell's palsy(BP) is an acute paralysis of the facial muscles innervated by the seventh cranial nerve. The cause and prognosis of recurrent BP are various. The frequency and heterogenicity of etiology suggest a predisposing factor or immune mechanisms. About 10% to 15% of patients with BP will suffer a recurrence, and less than 1.5% will have more than 4 episodes. We report four patients of chronic recurrent BPs.

흰쥐 안면신경핵 세포의 전기생리학적 및 형태학적 특성 (ELECTROPHYSIOLOGICAL AND MORPHOLOGICAL CHARACTERISTICS OF FACIAL NUCLEUS IN RAT)

  • 흰쥐의 안면신경핵을 구성하는 신경세포들의 시냅스 연결 양태 및 세포막 특성을 규명하기 위해 in vivo 필드전위 및 세포 내 전위 측정법을 이용하여 전기생리적 반응을 관찰하였다. 말초 안면신경 분지를 역행성으로 전기자극시 자극세기에 비례하여 전위의 크기가 증가되었고 필드 전위의 양태는 두 가지 반응으로 나타났는데 전기자극 직후 1ms 부근에서 정점을 나타내는 양태와 이와 더불어 $7\sim8ms$ 부근에서 후기 정점을 동반하는 양태가 있었다. 안면신경핵은 염색시 내측, 배외측, 중간측 및 외측등 4부분의 소핵으로 구분되었다. Neurobiotin으로 채워진 단일 신경세포를 형태학적으로 재구축하였는데 세포체는 추체형태를 나타내었고 주 수상돌기는 모든 방향으로 뻗어져 있었고 각 수상돌기의 영역은 해당 소핵 내에 한정되어 있었다. 일련의 과분극 전류 $(-1.2\sim+1.2nA)$를 세포내에 가하였을 때 동반되는 세포내 전위변화를 입력저항 값으로 계산하였을 때 그 기울기가 직선형으로 나타났다. 탈분극 전류를 세포내 주입시 지속적인 활동성 전위가 나타났으며 전류의 크기에 비례하여 각 전위의 개수가 증가하였고 spike-빈도 적응 현상이 나타났다. 그러나 시간 의존성 내향성 정류현상은 관찰되지 않았고 anodal break excitation이 나타났다. 이상의 실험결과로 보아 안면신경핵을 구성하고 있는 세포들 사이의 시냅스는 다양한 형태로 존재할 가능성이 있다고 사료되며 이들 시냅스간의 변화를 통하여 안면 신경마비, 반쪽 안면 경련, hypoglossal-facial anastomosis등에서 나타날 수 있는 임상적 신경성 증상 기전을 설명할 수 있을 것으로 여겨진다.

Dynamic smile reanimation in facial nerve palsy

  • Objectives: Long-term facial paralysis results in degeneration of the distal nerve segment and atrophy of the supplied muscles. Options for these patients include free muscle transfer, temporalis myoplasty, and botulinum toxin injections for smile reanimation. In this study we aimed to evaluate the subjective and objective outcomes of these procedures. Materials and Methods: In our study, we retrospectively analyzed smile symmetry in patients with facial palsy (n=8) who underwent facial reanimation procedures. Results: Subjective analysis showed high satisfaction in seven out of eight patients. Objective analysis showed statistically significant improvement postoperatively in both vertical and horizontal smile symmetry at rest and during maximum smile (P<0.001). Conclusion: Choosing the ideal procedure for the patients is the most critical aspect for facial reanimation. Though free muscle transfer is considered gold standard procedure, temporalis myoplasty also gives satisfactory results. Residual synkinesis which can lead to disturbing aesthetic deformity can be effectively treated with botulinum toxin.

Bell's palsy after concomitant chemoradiotherapy: a case report and literature review

  • Concomitant chemoradiotherapy (CCRT) treated patients experience various complications. We present a rare case of post-CCRT Bell's palsy and describe its various possible causes, so as to increase awareness among clinicians about Bell's palsy being a CCRT-associated adverse effect. The patient was a 48-year-old man diagnosed with squamous cell carcinoma who presented with post-CCRT Bell's palsy. After radiotherapy for 6 weeks (overall 67.5 Gy) and four rounds of cisplatin chemotherapy, he complained of paralysis of the entire left face. A test was performed 33 days after the last CCRT session to differentiate Bell's palsy from other causative factors. Based on magnetic resonance imaging findings, facial nerve invasion due to tumor size increase was determined to not cause Bell's palsy. Inflammation of the left Eustachian tube was observed. Hence, steroids and famciclovir were administered, which markedly improved the facial paralysis symptoms within 56 days after facial paralysis development. In conclusion, patients can develop Bell's palsy owing to complex effects of various CCRT mechanisms. Although the exact cause of Bell's palsy has not been identified and the effectiveness of drug treatment was questionable in this case, unlikely causative factors should be excluded through various tests and appropriate and timely measures must be adopted.

외상성 후두골과 측두골 골절에 대한 고찰 (Clinical Study of Patient with Traumatic Temporal and Occipital Bone Fracture)

  • This study shows that oriental medical treatment affected a patient with headache, dizziness, dim eyes, facial palsy and hard-of-hearing caused by traumatic temporal and occipital bone fracture. Traumatic facial palsy mainly occures by temporal bone fracture. Facial palsy caused by traumatic temporal bone fracture corresponds to gu-an-wa-sha (口眼?斜)of oriental medicine. Functional disorder of an auditory organ corresponds to yi-rong(耳聾) and hyun-hoon(眩暈) of oriental medicine. In general, everyone consider surgical operation first of all, in the case of having traumatic facial nerve paralysis. But, this case shows that oriental medical treatments(acupuncture and herbal medicine) have a good effect on a patient with traumatic temporal and occipital bone fracture.

특발성 구완와사로 입원한 환자에 대한 사상체질별 유형분석 (A Clinical Study about Factors and Improvement Score on Sasang Constitution of Bell's Palsy Patients.)

  • Objectives : This study was conducted in order to analyze the admission patients of facial nerve paralysis by Sasang constitution. Methods: A clinical study was done on 61 admission patients who were diagnosed and treated as facial nerve paralysis (Bell's palsy) from September, 2005 to May 2006 at the Dept. of Acupuncture and Moxibution, Chung-ju Hospital, college of Daejeon Oriental Medicine. We classified 61 patients according to Sasang constitution by QSCC II, attack side, sex, cause factor, concomitant symptom and Yanagihara' s score at the admission and discharge, and analyzed the improvement of Yanagihara's score between Sasang constitution groups. Results: 1. By sex, the ratio of Taeumin was higher than that of Soyangin in female. 2. By attack side, there is no significant difference between constitution groups In concomitant symptoms and cause factors. 3. By improvement of Yanagihara's scores between admission and discharge, the improvement score in Soyangin was a little bit higher than that in Soeumin, but there is no significant difference. Conclusion : The results suggest that there is no significant difference in all aspects except sex in Sasang constitution. But it is considered that we need to keep on studying afterwards because we had limits in getting enough patients.

말초성 안면신경마비 경과에 대한 임상적 예후인자 및 신경생리검사의 유용성 분석 (An Analysis of Clinical Prognosis Factors of Peripheral Facial Palsy and the Effects of Electrodiagnostic Test)

  • Objectives : Facial Nerve Paralysis is one kind of common diseases and it can be treated by natural therapy and the efficiency of treatment is relatively high. In clinical trial, it is not difficult to find patients who were not completely recovered from Facial Nerve Paralysis, so the symptoms are fixed permanently. This leads many doctors and patients to have interests in the progress and prognosis of the disease, so this study was to analyze clinical prognosis factors and verify the effects of Electrodiagnostic Test. Methods : The 378 subjects were chosen from 987 patients who were suffering from Peripheral Facial Palsy, diagnosed with Bell's palsy and Ramsay Hunt Syndrome and had admission treatment. They got Oriental-Western Medicine Treatment within two weeks after outbreaks of the disease and treated at least over 3 weeks using Oriental-Western Medicine Treatment. Results : 1. There was a significant difference in the results of treatment according to gender, age, types of Facial Palsy, existence of Post Auricular Pain, existence of Labyrinth Symptom, HBGS, and existence of onsets of recovery as clinical prognosis factors of Peripheral Facial Palsy, However, a statistically significant difference was not shown in the results of treatment according to the position of Facial Palsy(left or right), existence of a relapse, and diabetes, hypertension. 2. As a result of overall treatment, 77.2% of patients were recovered almost entirely and 22.8% were not, and the quelae of incomplete recovery were Synkinesis, facial contracture, facial spasm, crocodile tears and scheroma in order of frequency. 3. The results of electrodiagnostic test represented useful correlation to predict the final effects of treatment. Conclusion : Based on the above results, the prognosis factors, the degree of recovery, and the sequelae of incomplete recovery were analysed and the effects of electrodiagnostic test was verified.

한 대학병원에서 분석한 재발 벨마비 환자의 임상양상 (Clinical Analysis of Recurrent Bell's Palsy in One University Hospital)

  • Background: Bell's palsy (BP) is the most common cause of unilateral lower motor facial palsy. Recurrent paralysis of the facial nerve is unusual and reported in only 7-8%. Methods: A total of 394 consecutive patients with acute BP patients were enrolled at Daegu Catholic University Hospital from July 2005 to September 2012. We classified the patients into two groups-single BP and recurrent BP-and compared them by patient characteristics, clinical features, MRI findings, electrophysiologic findings and prognosis. The degree of BP was graded according to the House and Brackmann facial nerve grading system. Results: Recurrent BP was observed in 31 (7.9%) patients. The number of recurrence was varied from 2 to 5. The recurrent BP (9.7%) had more incidence of family history and MRI enhancement than those of single BP (2.2%, p=0.047). The single BP (63.4%) had better recovery than recurrent BP (45.2%, p=0.045). Conclusions: The recurrent BP had more incidence of family history, MRI enhancement and poor prognosis than the single BP.

Recurrent Pleomorphic Adenoma of the Parotid Gland

  • Pleomorphic adenoma is the most common type of salivary gland tumor and the most common tumor of the parotid gland. Because of its propensity for invasion, pleomorphic adenoma of the parotid requires superficial parotidectomy or total parotidectomy to minimize the risk of tumor recurrence. We report a case of pleomorphic adenoma of the parotid gland with repeated recurrences. A 23-year-old male patient presented with a protruding neck mass. Six years prior to this presentation, the patient had undergone superficial parotidectomy for the removal of pleomorphic adenoma of the right parotid gland at our institution. The patient experienced recurrence at 17 months after the initial resection, which required a total parotidectomy with partial resection of the facial nerve. Pathologic examination revealed histologic findings consistent with pleomorphic adenoma across the surgical specimen from all three of the operations. The patient suffered from facial nerve paralysis, with facial expressions partially recovered over a year.
