ERUPTION GUIDANCE OF IMPACTED MANDIBULAR FIRST MOLAR (매복된 하악 제1대구치의 맹출 유도)

  • Incidence of tooth impaction varies from 5.6 to 18.8% of the population. Failure of eruption of the first and second permanent molars is rare; the prevalence in the normal population is 0.01% in case of the first permanent molar, and 0.06% in case of the second permanent molar. Permanent molars are particularly important for providing sufficient occlusal support and co-ordinating facial growth. Failure of eruption of permanent molars may result in various complications such as decrease in vertical dimension, posterior open bite, extrusion of antagonistic teeth, resorption and inclination of adjacent teeth, formation of cyst and so on. Treatment options of impacted teeth are periodic observation, surgical exposure, surgical exposure with subluxation, orthodontic relocation, and surgical extraction before prosthetic treatment. Early diagnosis and treatment are important, because delayed treatment induces various problems such as decreased spontaneous eruptive force, decreased successful percentage, increased treatment period, increased various complications. Prevalence of the failure of mandibular first molars is rare but eruptive guidance before extraction of impacted teeth is necessary due to importance of permanent molars. We reported two cases of surgical exposure of impacted mandibular first molar. In these cases, we could observe different result of the impacted mandibular first molar after surgical exposure.

Systematic Approach to The Extraction of Effective Region for Tongue Diagnosis (설진 유효 영역 추출의 시스템적 접근 방법)

  • In Oriental medicine, the status of a tongue is the important indicator to diagnose the condition of one's health like the physiological and the clinicopathological changes of internal organs in a body. A tongue diagnosis is not only convenient but also non-invasive, and therefore widely used in Oriental medicine. However, the tongue diagnosis is affected by examination circumstances like a light source, patient's posture, and doctor's condition a lot. To develop an automatic tongue diagnosis system for an objective and standardized diagnosis, segmenting a tongue region from a facial image captured and classifying tongue coating are inevitable but difficult since the colors of a tongue, lips, and skin in a mouth are similar. The proposed method includes preprocessing, over-segmenting, detecting the edge with a local minimum over a shading area from the structure of a tongue, correcting local minima or detecting the edge with the greatest color difference, selecting one edge to correspond to a tongue shape, and smoothing edges, where preprocessing consists of down-sampling to reduce computation time, histogram equalization, and edge enhancement, which produces the region of a segmented tongue. Finally, the systematic procedure separated only a tongue region from a face image with a tongue, which was obtained from a digital tongue diagnosis system. Oriental medical doctors' evaluation for the results illustrated that the segmented region excluding a non-tongue region provides important information for the accurate diagnosis. The proposed method can be used for an objective and standardized diagnosis and for an u-Healthcare system.

Surgical and Orthodontic Treatment of Unicystic Ameloblastoma Related to an Impacted Molar Tooth in the Mandible: Case Report (낭종성 법랑모세포종으로 인하여 매복된 하악 구치의 교정-외과 치료: 증례보고)

  • Ameloblastoma is an aggressive benign odontogenic epithelial tumour that may arise from the enamel organ, remnants of dental lamina, or the lining of an odontogenic cyst. It is usually categorized into solid or multicystic, unicystic, and peripheral types. Treatment ofameloblastomas include conservative methods such as marsupialisation, enucleation, and curettage; and radical treatments such as marginal or segmental resection. Radical treatments have resulted in lower recurrence rates; however, may also encounter esthetic, functional, and reconstructive problems. Unicystic ameloblastoma has been considered less aggressive and a lower recurrence tendency. Thus, many authors have recommended conservative treatment in cases of unicystic ameloblastoma. An 11 year-old boy presented with displaced second and third molars by luminal unicystic ameloblastoma in the mandible. Cyst enucleation, curettage, and third molar extraction were done. No signs of recurrence or esthetic problems such as facial asymmetry were seen radiologically and clinically, up to 8 years 2 months postoperatively.

Deep learning based face mask recognition for access control (출입 통제에 활용 가능한 딥러닝 기반 마스크 착용 판별)

  • Coronavirus disease 2019 (COVID-19) was identified in December 2019 in China and has spread globally, resulting in an ongoing pandemic. Because COVID-19 is spread mainly from person to person, every person is required to wear a facemask in public. On the other hand, many people are still not wearing facemasks despite official advice. This paper proposes a method to predict whether a human subject is wearing a facemask or not. In the proposed method, two eye regions are detected, and the mask region (i.e., face regions below two eyes) is predicted and extracted based on the two eye locations. For more accurate extraction of the mask region, the facial region was aligned by rotating it such that the line connecting the two eye centers was horizontal. The mask region extracted from the aligned face was fed into a convolutional neural network (CNN), producing the classification result (with or without a mask). The experimental result on 186 test images showed that the proposed method achieves a very high accuracy of 98.4%.

  • A dentigerous cyst is an epithelium-lined sac that surrounds the crown of an unerupted tooth or odontoma. And the most common sites of this cyst are the mandibular and maxillary third molar and maxillary cuspid a areas. Clinically, expansion of bone with subsequent facial asymmetry, extreme displacement of teeth, severe root resorption of adjacent teeth and pain are all possible sequelae of this cyst. The standard treatment for a dentigerous cyst is enucleation and extraction of the involved tooth. But in large cysts, this can lead to functional, cosmetic and psychologic consequences to the patients. So recently, more conservative methods are used. We report 5 cases of dentigerous cysts in pediatric patient which were treated by a conservative approach, By this methods, we can preserve teeth and guide eruption of the teeth which are involved in cystic area.

Evaluation of the Convergence Efficacy of Cosmetic Products Containing Pleurotus eringii Extracts (Pleurotus eringii 추출물을 함유한 화장품의 복합효능 평가)

  • This study conducted an experiment to develop new raw ingredients for natural cosmetics by evaluating the efficacy of cosmetic products containing the extracts of Pleurotus eringii mushrooms grown without agricultural chemicals. To this end, purchased were Pleurotus eringii mushrooms grown without agricultural chemicals in South Gyeongsang province, South Korea in 2016; extraction was conductedwith 80% EeOH at room temperature after dry ing them. The study identified the excellent efficacy of the extracts after evaluating the antioxidant activity using the DPPH assay and ROS assay. According to the evaluation of he categories of moisture, oil, pH and pigment after the application of a cream-type facial mask pack containing 5% of the extracts, there were significant differences in the categories of moisture and oil on the T-zone area, and no statistically significant differences in the categories of oil change on the U-zone, pH and pigment. However as the extracts of Pleurotus eringii mushrooms had excellent efficacy in all categories compared to the control group, the efficacy of the extracts as a cosmetic raw ingredient could be identified. It is expected that this study may further contribute to developing more advanced raw ingredients, given the extract and cultivation conditions by concentration.

Orthodontic upright treatment for mesioangular impacted lower second molar (근심경사 매복된 하악 제2대구치의 직립 치료)

  • The lower $2^{nd}$ molar eruption is beginning to mesiolingually, then rotate to distobuccally so it has a tendency to be tilted and impacted mesially. Signs and symptoms of impacted $2^{nd}$ molar are similar to impacted $3^{rd}$ molar's. However, treatment plan for impacted $2^{nd}$ molar is different from that of impacted $3^{rd}$'s. The former is the preservation and uprighting of $2^{nd}$ molar so that it could act to recovery of mastication, symmetrical facial growth, maintaining the symmetry of dental arch, stable occlusion, while the latter is the extraction of tooth. If the uprighting treatment is planned, most proper protocol of treatment and the additional treatment opition should be applied with consideration for it's crown exposure, present of $3^{rd}$ molar which interrupt the uprighting process, extrusion of opposite tooth. Although it could not improve the esthetic result, it could prevent many dental problems. Therefore, uprighting for impacted lower $2^{nd}$ molar is meaningful treatment.

A CEPHALOMETRIC EVALAUATION OF ANTERIOR OPENBITE MALOCCLUSIONS TREATED BY MULTILOOP EDGEWISE ARCHWIRE TECHNIQUE (Multiloop edgewise Archwire 기법으로 치료된 전치 개교 증례의 두부방사선사진 계측학적 평가)

  • The purpose of this study was to evaluate the change of before and after treatment of anterior openbite malocclusions treated by Multiloop Edgewise Archwire technique. The openbite sample consisted of 4 male and 12 female adults, treated with nonextraction or third molar extraction. The normal sample consisted of 58 subjects, which have pleasing facial profile and normal occlusion and no experience of orthodontic or prosthodontic treatment. The 58 subjects of normal sample were subdivided by cephalemetric vertical relationship of face. The 40 subjects, cephalometric vertical relationship of face was in normal range, classified as Normal Sample group 1. The 18 subjects, increased cephalometric vertical relationship of face, classified as Normal Sample group 2. The computerized cephalometric analysis was accomplished with 50 reference points for 22 skeletal measurements, 46 dentoalveolar measurements, 8 soft tissue measurements. Statistical analysis of the data was carried out with paired t-test, Student's t-test, and DUNCAN test using SAS(PC version), The results were as follows : 1. There were no statistically significant differences in skeletal measurement between before and after treatment. The major changes were in dentoalveolar region. 2. After treatment, the long axis of maxillary and mandibular posterior teeth were distally tipped-back, and uprighted to bisected occlusal plane. The interincisal angle was increased. 3. There were no statistically significant increase in the upper posterior dental height and statistically significant decrease in the lower posterior dental height. The upper anterior dental height was increased, but there was no statistically significant increase in the absolute upper anterior dental hight. The lower anterior dental height was increased. 4. After treatment, the maxillary occlusal plane to palatal plane angle and the mandibular occlusal plane to mandibular plane angle were statistically significant increased. Then, there were no statistically significant difference between after treatment group and normal sample group 2. 5. After treatment, the percentage of upper lip length to upper anterior dental height was decreased. Then, There were no statistically significant difference between after treatment group and normal sample group 2.

Face Recognition Based on Polar Coordinate Transform (극좌표계 변환에 기반한 얼굴 인식 방법)

  • In this paper, we propose a novel method for face recognition which uses polar coordinate instead of conventional cartesian coordinate. Among the central area of a face, we select a point as a pole and make a polar image of a face by evenly sampling pixels in each direction of 360 degrees around the pole. By applying conventional feature extraction methods to the polar image, the recognition rates are improved. The polar coordinate delineates near-pole area more vividly than the area far from the pole. In a face, important regions such as eyes, nose and mouth are concentrated on the central part of a face. Therefore, the polar coordinate of a face image can achieve more vivid representation of important facial regions compared to the conventional cartesian coordinate. The proposed polar coordinate transform was applied to Yale and FRGC databases and LDA and NLDA were used to extract features afterwards. The experimental results show that the proposed method performs better than the conventional cartesian images.

Cutaneous Basal Cell Carcinoma Arising in Odontogenic Cutaneous Fistula

  • An odontogenic cutaneous fistula is a pathological communication between the outer skin surface of the face and the oral cavity. Facial cutaneous fistula is a complication of odontogenic infection that is often misdiagnosed with skin infection. We report a rare case, which was diagnosed as basal cell carcinoma based on the biopsy of skin lesions in the patient who had been diagnosed with odontogenic cutaneous fistula. A 64-year-old male patient presented with a cutaneous odontogenic fistula. The patient had undergone surgical extraction of fistula tract and loose tooth before dermatology or plastic surgery consultation. With the biopsy and computed tomography, it was confirmed that fistula and basal cell carcinoma. However, the connection between the fistula and skin cancer was not clear. Positron emission tomography-computed tomography scan was performed and was not detected as other local or distant metastasis. After that, wide excision of the skin lesion was performed. Although skin cancer is not commonly observed, it is necessary to rule out this disease entity by performing biopsy of skin lesions.
