이하선 기능 보존을 위한 심층엽의 선택적 절제술: 증례보고 (SELECTIVE DEEP LOBE PAROTIDECTOMY FOR PRESERVATION OF PAROTID FUNCTION: A CASE REPORT)

  • Tumors of the deep lobe of parotid gland are rare. These benign tumors have usually been treated by total parotidectomy, which has functional and esthetic side effects. Recently, there has been a trend for operations of parotid gland benign tumors to be less radical and selective deep lobe parotidectomy has been introduced. This technique preserves the superficial lobe and facial nerve when tumor is located in the deep lobe. Selective deep lobe parotidectomy preserves parotid salivary function, minimizes the incidence of facial nerve damage and gustatory sweating (Frey's syndrome) and improves cosmetic outcome. We report a case of pleomorphic adenoma of the deep lobe that was successfully treated by selective deep lobe parotidectomy with satisfactory result.

Bimaxillary orthognathic surgery and condylectomy for mandibular condyle osteochondroma: a case report

  • Osteochondroma is rarely reported in the maxillofacial region; however, it is prevalent in the mandibular condyle. This slowly growing tumor may lead to malocclusion and facial asymmetry. A 39-year-old woman complained of gradual development of anterior and posterior unilateral crossbite, which resulted in facial asymmetry. A radiological study disclosed a large tumor mass on the top of the left mandibular condyle. This bony tumor was surgically removed through condylectomy and the remaining condyle head was secured. Subsequently, bimaxillary orthognathic surgery was performed to correct facial asymmetry and malocclusion. Pathological diagnosis was osteochondroma; immunohistochemistry showed that the tumor exhibited a conspicuous expression of BMP-4 and BMP-2 but rarely expression of PCNA. There was no recurrence at least for 1 year after the operation. Patient's functional and esthetic rehabilitation was uneventful.

Treatment of anterior open bite by posterior maxillary segmental osteotomy and miniplates: a case report

  • Background: Anterior open bite is a challenging malocclusion to correct orthodontic treatment. Anterior open bite associated with over-erupted posterior teeth and long lower facial height should be treated by reduction of posterior dimension for esthetic results. Although the possibility of orthodontic treatment of an anterior open bite has increased with the introduction of skeletal anchorage, there are still cases requiring surgery for various reasons. Case presentation: This case report covers an anterior open bite of a 25-year-old man successfully treated with the posterior maxillary segmental osteotomy (PMSO) and miniplates. After the pre-surgical orthodontic treatment, the PMSO between canines and first premolars was performed under local anesthesia and miniplates were placed on the zygomatic buttress. As a result of 28 months of treatment, an impaction amount of 3.5 mm was obtained in the maxillary posterior teeth, and the facial esthetics improved at rest and smile. Conclusion: The impaction of the posterior dentoalveolar segment using the PMSO can be a good treatment option in patients with anterior open bite showing long lower facial height.

Facial 'Phi' Mask를 이용한 3급 부정교합 환자의 악교정수술 후 정면부 연조직의 변화 (Soft Tissue Change in Frontal View after Orthognathic Surgery for Class III Malocclusion: Analysis Using Facial 'Phi' Mask)

  • Purpose: Dr. Marquardt made the facial 'phi' mask using golden ratio. Most class III patients have bulky faces and want a smaller face. Using a facial golden mask, this study estimated and compared frontal photographs before and after operation for soft tissue measurement. The golden mask can be considered as a reference tool for facial esthetic analyses especially in lower face. Methods: Forty patients who had undergone orthognathic surgery at Samsung Medical Center from January 2006 to December 2009 were included in this study. These patients had Class III malocclusion. Lateral cephalometric radiographs, frontal clinical photos of pre-op and 8~12 month later post-op, and the facial 'phi' mask using golden ratio, were used for analysis. Reduction of the lower face area, occlusal plane changes, amounts of mandible setback and amounts of maxilla posterior impaction were estimated. Results: Lower facial reduction ratio and mandibular setback amounts were significantly different between 1-jaw and 2-jaw groups. Average postoperative changes in the area of lower face between bilateral sagittal split ramus osteotomy (BSSRO) and BSSRO combined maxilla posterior impaction were compared by using an independent simple t-test and $P$ value was 0.016. Therefore, the lower facial reduction ratio and mandibular setback amount were significantly different in maxilla posterior impaction. Conclusion: The two-jaw surgery group showed more reduction of the lower facial area than the 1-jaw surgery group. The amount of lower facial reduction was more related with the amount of mandibular setback. There was no significant relation in lower facial reduction with amount of maxilla posterior impaction, pre-op occlusal plane, post-op occlusal plane and the mandibular angle. A relationship between the change in the lower facial area and the amount of maxilla posterior impaction or the change of mandibular angle occlusal plane at pre-op could not be found because of the difference in the amount of setback between two groups.

II급 1류 부정교합 환자에서 Bionator의 적응증에 관한 연구 (AN EVALUATION ON THE INDICATIONS OF BIONATOR IN CLASS II DIVISION 1 MALOCCLUSION)

  • 본 연구의 목적은 bionator를 사용한 II급 1류 부정교합 환자의 치료후 결과를 토대로 치료전에 치료후 결과를 예측할 수 있는 항목들을 알아봄으로써 성장하는 II급 1류 부정교합을 가진 환자들의 치료에 도움을 얻는데 있다. bionator를 사용한 앵글 II급 1류 부정교합 환자 48명의 치료후 두부방사선계측사진을 통해 치료결과가 양호한 군(1군)과 치료결과가 불량한 군(2군)으로 나눈 후 양군의 치료전 측모두부방사선계측사진의 비교분석을 통해 두군 사이에 차이를 보이는 계측항목들을 알아보았고, 판별분석을 통해 다음의 결과를 얻을 수 있었다. 1. 치료전 골격계측 항목으로는 ANB, facial convexity angle, AB to facial plane angle 등이, 치성계측 항목으로는 L1 to A-Pog, U1 to facial plane, L1 to facial plane 등이, 연조직 계측항목으로는 Ricketts esthetic line 상에서 상, 하순의 돌출도가 양군 사이에 유의한 차를 보였다(SAS t-test, p<0.05). 2. 판별분석을 통해 유의성있게 나타나는 항목의 순위를 본 결과 L1 to facial plane, 하순의 돌출도, ANB과 FMIA 등이 양군의 치료결과의 예측에 도움을 주는 것으로 나타났다. 3. 증감판별분석을 통해 서로 독립적이며 상관계수가 높은 3개의 변수 - L1 to facial plane, articular angle, ANB- 를 선택하였으며, 이를 토대로 판별식을 도출하였다.

Demographic review of aesthetic surgery for patients with facial palsy

  • Background: This study analyzed the demographic characteristics of patients with facial palsy who were treated using either dynamic or static procedures. This study aimed to compare the frequency of procedure implementation and age distribution between the two groups. Methods: This study retrospectively analyzed the medical records of patients treated for facial palsy at a single institution from 2014 to 2022. Among cases included in our study, dynamic procedures involved cross-facial nerve graft and latissimus dorsi or gracilis muscle flap transfer. Static procedures included gold weight insertion, canthopexy, browlift, and thread lift/static slings. Results: Among the 31 patients included in our study, eight (25.8%) incorporated dynamic techniques, and the average age of patients was 44.75 years (range, 24-68 years) with a male to female ratio of 1:4. The remaining 23 patients (74.2%) underwent a static procedure, of which the average age was 59.17 years (range, 23-81 years) which was statistically significantly higher than the average age of 44.75 of dynamic patients (p= 0.013). Regarding the timing of treatment after diagnosis, no patient underwent dynamic procedures more than 20 years after initial diagnosis. A greater diversity in the timing of treatment was observed in the static group. All patients who underwent dynamic procedures were treated using static procedures during the study period. Conclusion: Because aesthetics-based static techniques are typically quick outpatient procedures that can be performed under local anesthesia, our study shows that these are often preferred treatments for all age groups, especially for debilitated or older patients. Further research is required to investigate the long-term functional outcomes of these surgical techniques in a wider population of patients.

Prevalence of considering revision rhinoplasty in Saudi patients and its associated factors

  • Background: Primary rhinoplasty outcomes may not meet individual expectations. Consequently, reoperation may be advocated to improve results. This study examines the prevalence of individuals considering revision rhinoplasty, while identifying the main cosmetic and functional complaints and factors associated. Methodology: This is a cross-sectional study conducted in Saudi Arabia using a self-reported online questionnaire distributed through social media channels. The sample included 1370 participants who were all Saudi nationals over the age of 16 who had undergone primary rhinoplasty at least 1 year prior. Results: The prevalence of individuals considering revision rhinoplasty was 44.7%. The primary reason for considering it was the desire for further esthetic improvement in an already acceptable result (50.16%). The most common cosmetic complaints subjectively reported were poorly defined nasal tip (32.35%). The most prevalent nasal function symptom was nasal obstruction (56.9%). Significant factors associated with considering revision rhinoplasty included the physician not understanding the patient's complaints, short consultation time, low monthly income, inadequate information about the expected results, not using computer imaging to predict outcomes, lack of rapport with the surgeon, and inadequate information about the risks and complications. Conclusions: A thorough understanding of patient concerns and expectations, as well as thoughtful consideration of risk factors, may help surgeons achieve more successful outcomes and potentially reduce the incidence of revision rhinoplasties. Level of evidence: III.

한국인 유치열기 아동의 두부방사선 계측학적 연구 (ROENTGENOCEPHALOMETRIC STUDY FOR THE CHILDREN IN THE PRIMARY DENTITION OF KOREAN)

  • In order to know cephalometric norms for the preschool children, this roentgeno cephalometric study was undertaken in each 50 Korean male and female children of primary dentition age from 4 to 5 year. The following results were obtained. 1. In the skeletal analysis, there was no significant difference between male and female in angular measurement and the linear measument of the male was generally greater than that of the female. 2. Saddle angle was $122.3^{\circ}$, articular angle was $147.6^{\circ}$, gonial angle was $119.4^{\circ}$ and the sum of each angle was $396.1^{\circ}$ in male and $396.6^{\circ}$ in female. 3. The ratio of mandibular body to anterior cranial base was about 1 : 0.91. 4. In the primary dentition, suggested that the nasion and point A move forward relative to sella turcica in a fashion, pogonion and point B are equal in angular position relative to plan S-N, bony chin and chin button was yet underdeveloped, and the forward growth of mandible was seen rapid than maxilla after 4 years. 5. Suggested that the percentage of anterior facial height to the posterior facial height were 64.4% in male and 64.1% in female. 6. Maxillary primary incisors was more upright than the permanent incisors, mandibular primary incisors was inclined lingually relative to the permanent incisor, and primary incisors was more upright than the permanent incisors. 7. Maxillary primary incisors in female was inclined labially than male. 8. In the the relationship of the upper lip and lower lip to the esthetic line, the upper lip was 2.11mm and the lower lip was 2.33mm front of the esthetic line.

아시아 성인 연조직 측모의 비교분석 (SOFT TISSUE PROFILES OF YOUNG ORIENTAL ADULTS)

  • 이 연구는 정상 교합과 양호한 안모를 가진 169명의 남자와 174명의 여자 아시아 각국 인을 대상으로 연조직 측모의 특징을 비교 분석하기 위하여 시행되었다. 100명의 한국인, 100명의 중국인, 72명의 월남인 및 71명의 일본인으로부터 촬영된 측모두부방사선사진 상에서 9개의 계측점을 설정하고 전산 입력한 뒤 6 개의 연조직 계측 항목에 대한 측정을 시행한 후 통계 처리하였다. 분산 분적에서 중국인의 총 측모(GI'-Pr-Pg')는 한국인이나 월남인에서 보다 적은 돌출도를 보였다(P<0.01). 중국인의 안면평면측모(GI'-Sn'-Pg') 역시 월남인 보다 적은 돌출각을 나타내었다(P<0.01). 홀더웨이의 하순각(Pg'-LS:N-B)은 월남인에서 가장 컸고 다음으로 중국인, 한국인, 일본인의 순으로 크게 나타났다. 월남인의 상순은 중국인에서보다 리???츠의 심미적 기준선에 더 많이 근접하였고 한국인과 일본인에서의 상순은 기준선보다 후방에 위치하였다. 한국인과 일본인의 하순은 심미적 기준선에 근접한 반면 중국인과 월남인의 하순은 기준선 보다 2 mm 전방에 위치하였다. 한국인과 월남인의 비순각은 중국인과 일본인에서 보다 큰 것으로 나타났다. 남녀 간의 성차는 기본적으로 코의 높이에서 상이하였고 이로 인하여 여자에서의 총 측모각과 비순각이 남자보다 크게 나타났다(P<0.001). 이러한 결과는 아시아 각국인 들의 연조직 측모에 대하여 단일 표준치에 의한 분석 평가가 적절하지 않으며 각 인종별 표준치 설정의 필요성을 시사한다.

심미적 측모 판단에 미치는 성별과 문화의 영향력 (The Influence of gender or culture on determining esthetic facial profile)

  • 교정치료를 받고자 하는 많은 사람들이 관심을 갖는 중요한 동기중의 하나가 미적 안모의 추구이기 때문에, 미적 안모를 평가하는 기준이 성별이나 환자가 속해있는 문화권에 따라서 달라진다면, 이에 대한 정확한 이해가 치료목표의 설정에 있어서 중요한 역할을 한다고 할 수 있다. 본 연구에서는 성별이나 문화가 매력적인 측모를 판단하는데 영향력을 발휘하는지를 알아보기 위해 4개의 평가 군으로 하여금 133명의 일반인 사진 중에서 좋은 측모를 가진 사람의 사진을 채점방식에 의해 선택하게 하였고, 좋은 측모 군을 평가하는 경향이나 평가점수의 차이를 이용하여, 4평가 군 사이에 좋은 측모를 선정하는데 있어서 차이가 있는지를 Bonferroni's multiple comparison과 randomized block designed ANOVA를 통하여 통계적으로 검증하였다. 4개의 평가 군은 한국에 거주하는 10명의 20대 남자 군과 10명의 20대 여자 군, 10명의 20대 재미한국인 남자 군, 그리고 10명의 20대 재미한국인 여자 군으로 구성되었으며, 선택된 좋은 안모 군의 연조직 형태는 두부방사선 사진의 투사도를 사용하여 선계측, 각계측, 그리고 비율계측을 함으로써 비교 분석하였고, 이를 통해서 다음의 결과를 얻었다. 1. 심미적인 측모를 판별하는데 있어서의 개인적인 차이 여부를 통계적으로 입증한 결과, 아름다움에 대한 개념이 지극히 주관적이라는 통념과 일치하는 것으로 사료 된다. 2. 동일한 문화권에 살고있는 한국인에서, 성별의 차이는 심미적 측모의 평가에 영향을 주지 않았다. 3. 다른 문화권에서 생활하는 같은 민족적 동질성을 가진 두 집단의 한국인간에 심미적 측모를 선택하는데 서로 다른 양상을 보였으며, 이는 미의 기준을 설정하는 데에 있어서 문화가 더 큰 변수임을 입증하고 있다. 4. 좋은 안모 군과 좋지 않은 안모 군에서의 연조직 분석에서 하안면부 수직 고경의 비율, 안면 돌출도 및 상하순의 전후방적 위치 관계에서 특징적인 차이를 나타내었다.
