  • Zygomatic fractures are the second most common fractures of the facial skeleton, after nasal bone fractures. Due to its uniqueness, the malar bone plays a very important role in maintaining appropriate facial contours. Zygomatic fractures can cause ocular and mandibular functional impairment, along with cosmetic defects. With the help of advanced imaging techniques and various treatment options, the management of zygomatic fractures has become more sophisticated and less invasive. This article discusses zygomatic fractures in detail: their clinical and radiographic features, and the various treatment options available.

Pox viral infection in a rufous turtle dove

  • A dead dove was found on the road and submitted for diagnosis. The bird was severely emaciated, with deformation in its facial area. Grossly, white coalescing nodules were seen on the cut surface of the nasal cavity. Histopathologically, epithelial cells of the upper respiratory tract were markedly proliferated, with ballooning degeneration, down growth of the rete ridge, and large eosinophilic intracytoplasmic inclusion bodies. Parakeratotic hyperkeratosis and focal necrotic focus was present in the proliferative area. The facial bones showed partial bone resorption. Transmission electron microscopy revealed numerous viral particles in epithelial cells with dumbbell-shaped bodies, consistent with poxvirus.

Skin Condition Analysis of Facial Image using Smart Device: Based on Acne, Pigmentation, Flush and Blemish

  • In this paper, we propose a method for skin condition analysis using a camera module embedded in a smartphone without a separate skin diagnosis device. The type of skin disease detected in facial image taken by smartphone is acne, pigmentation, blemish and flush. Face features and regions were detected using Haar features, and skin regions were detected using YCbCr and HSV color models. Acne and flush were extracted by setting the range of a component image hue, and pigmentation was calculated by calculating the factor between the minimum and maximum value of the corresponding skin pixel in the component image R. Blemish was detected on the basis of adaptive thresholds in gray scale level images. As a result of the experiment, the proposed skin condition analysis showed that skin diseases of acne, pigmentation, blemish and flush were effectively detected.

Ramsay Hunt syndrome

  • Ramsay Hunt syndrome is a type of acute herpes zoster, which occurs by reactivation of the varicella-zoster virus at the geniculate ganglion. Clinical presentation of Ramsay Hunt syndrome includes a vesicular rash on the ear (herpes zoster oticus) or in the oral mucosa accompanied by acute peripheral facial nerve paralysis. Other cranial nerves such as V, IX, XI, and XII are often involved. Additional variability of the clinical picture of Ramsay Hunt syndrome is produced by varying patterns of skin involvement explained by individual anastomoses between cranial and cervical nerves. Combination treatment containing anti-viral agents and steroids is recommended for the treatment of Ramsay Hunt syndrome. Additionally, early diagnosis of Ramsay Hunt syndrome is a crucial factor to improve damaged nerves in Ramsay Hunt syndrome, which initiates treatment as soon as possible.

Maxillofacial Trauma Trends at a Tertiary Care Hospital: A Retrospective Study

  • Purpose: Maxillofacial fractures are rapidly increasing from car accidents, industrial accidents, teenaged criminal activity, and sports injuries. Accurate assessment, appropriate diagnosis, and preparing individual treatment plans are necessary to reduce surgical complications. We investigated recent trends of facial bone fracture by period, cause, and type, with the objective of reducing surgical complications. Methods: To investigate time trends of maxillofacial fractures, we reviewed medical records from 2,196 patients with maxillofacial fractures in 1981~1987 (Group A), 1995~1999 (Group B), and 2008~2012 (Group C). We analyzed each group, comparing the number of patients, sex ratio, age, fracture site, and etiology. Results: The number of patients in each period was 418, 516, and 1,262 in Groups A to C. Of note is the increase in the number of patients from Group A to C. The sex ratios were 5.6:1, 3.5:1, and 3.8:1 in Groups A, B, and C. The most affected age group for fracture is 20~29 in all three groups. Traffic accidents are the most common cause in Groups A and B, while there were somewhat different causes of fracture in Group C. Sports-induced facial trauma was twice as high in Group C compared with Group A and B. Mandible fracture accounts for a large portion of facial bone fractures overall. Conclusion: We observed an increase in facial bone fracture patients at Kyungpook National University Dental Hospital over the years. Although facial injury caused by traffic accidents was still a major cause of facial bone fracture in all periods, the percentage decreased. In recent years, isolated mandible fracture increased but mandible and mid-facial complex fracture decreased, possibly because of a reduction in traffic accidents.

하안면부에서 입술의 돌출 정도와 안면 비대칭의 인지도에 관한 연구 (Level of perception of changed lip protrusion and asymmetry of the lower facial height)

  • 교정 치료를 하고자 하는 가장 큰 동기 중의 하나는 좋은 안면을 얻고자 하는 것이므로 교정 치료 후 안면부에 나타나는 변화에 대한 환자들의 인지도를 이해하는 것은 교정 치료의 진단과 치료 계획 수립에 매우 중요한 일이다. 이에 본 연구는 정면 및 측모에서 하 안면부위의 입술의 위치와 안면 비대칭의 변화에 관하여 인지할 수 있는 최소한의 변화량을 알아보고자 근형 잡힌 비율을 지닌 가상의 정모와 측모 사진을 컴퓨터 영상으로 제작한 후 입술의 위치는 Ricketts의 E-line을 기준으로, 안면의 비대칭은 턱 끝의 중앙 지점을 기준으로 각각 1, 2, 3, 4 mm 변화시킨 디지털 영상을 이용하여 40명의 미술학도를 관찰자로 인지도 조사를 하였다. 연구 결과 환자가 입술의 위치와 비대칭의 변화를 인지하기 위해서는 측모에서 최소한 2 mm 이상, 정모에서 3 mm 이상의 변화가 필요하였으며 하 안면부 변화에 대한 인지도는 정모에서보다 측모 변화에 대한 인지도가 높았다. 또한 변화에 대한 사전 정보는 변화에 대한 인지율을 현저하게 높여 주었다.

한국인의 연조직측모 선호경향에 대한 연구 (A Study of Esthetic Facial Profile Preference In Korean)

  • 연조직 측모는 교정치료와 진단에 있어서 중요한 부분이며, 안모 심미성에 대한 개념은 긴 역사를 가지고 변천하여 왔으며 지역이나 문화, 성별, 인종, 관습 등에 따라서 그 심미관이 상이하다. 본 연구에서는 대중들의 연조직측모 선호도를 조사하기 위하여 교정환자들의 주 연령층인 사춘기성장가속기 전의 초등학생, 사춘기 연령의 학생, 20대의 대학생들과 보호자의 주연령층인 40-50대 층을 대상으로 설문조사를 시행하였다. 설문지의 제작을 위해 기존의 심미적인 안면의 경, 연조직 형태에 관한 연구에서 얻은 연조직 평균계측치에 가까운 연조직 측모를 가진 20대 여성 한 명을 선정한 후 측모천연색 사진을 촬영하여 표준측모로 사용하였다 전체 설문지는 7 set로 이루어져 있으며, 각 set는 측모에 영향을 줄 수 있는 입술의 돌출도, 코, 이부의 높이와 Sn point를 표준측모에서 변화시킨 천연색 측모형들을 포함하고 있다. 가장 좋다고 생각되는 안모는 1점, 가장 좋지 않다고 생각되는 안모를 5점의 순서로 통계처리를 하여 각 응답자군들의 연조직측모 선호도를 알아보았으며, 성별, 연령에 따른 선호도의 차이를 조사하였다. 또한 대중들이 선호하는 안모와 선호하지 않는 안모와 하안면 측모의 심미적 평가를 위해 기존에 사용되는 기준선들간의 신뢰성을 평가하여 다음의 결과를 얻을 수 있었다. 1.응답자의 신뢰도를 평가시 유년기에서 다른 group들에 비해 낮은 값을 나타내었다. 이는 유년기에서는 아직은 측모선호도에 대한 관심이 없는 것으로 평가하였다. 2. 측모 선호도의 연령, 성별 차이는 뚜렷하지 않은 것으로 나타났다. 3. 가장 선호하는 측모보다 가장 선호하지 않는 측모에 대한 선택의 일치도가 더 높았다. 4.응답자의 group별로 순위의 일치성을 나타내는 Kendall의 일치계수(W)는 20대에서 가장 높게 나타났다 이는 다른 연령에 비해 측모에 대한 선호성에 대한 기준이 명확한 것을 나타낸다. 5. 응답자들이 가장 선호하는 측모의 구순돌출도는 심미적인 안모에 대한 이전 연구에서의 Ricketts의 nose tip-Pog line(E-line), Burstone의 Sn-Pog line(B-line)에 대한 평균치에 가까운 값을 나타내었다 이는 이들 기준선들이 임상적으로 유용하게 사용할 수 있음을 의미한다. 6. 응답자들이 가장 선호하지 않는 측모는 E-line과 B-line에 대해 구순이 너무 돌출되거나 후퇴된 경우였다. 7. 응답자들의 측모 선호도는 straight, concave, convex profile순으로 나타났다.

약관절 장애의 보존적 치료효과(I I) (Effect of Conservative Treatment of Temporomandibular Disorders(II))

  • In this study, 94 patients with temporomandibular disorders were interviewed to evaluate the effect of the treatment they received at the department of Oral diagnosis and oral Medicine in Seoul national university Hospital. The treatment administered to the patient were patient education, relaxation procedures, physical therapy, occlusal splint therapy and selective grinding of teeth. The following results were disclosed : 1. 97.9% of the patients reported no pain or considerably less pain than they had reported at their first visits to the department. 2. 89.3% of the patients thought that the treatment provided was either complete or considerably successful. 3. TMJ and facial pain was resolved within average 3.0 months 4. Of the 70 patients who previously reported having jaw joint sound, 34 patients (48.6%) no longer reported int. 5. Of the 48 patients who previously reported having headache regularly, 3 patients (68.8%) no longer reported headache regularly. 6. Of the 66 patients who previously reported having masticatory muscle pain, 41 patients (62.1%) no longer reported masticatory muscle pain. 7. Analysis of the data did not disclose a subgroup or factor, such as age, the stage of internal derangement, capsulitis, bruxism, MPD, duration of symptoms, that could be correlated with the reduction of pain or the patient's perception of success of treatment.

Early-diagnosed silent sinus syndrome and cone-beam computed tomography in a pediatric patient: a case report

  • Chronic maxillary atelectasis (CMA) is a progressive alteration in the volume of the maxillary sinuses that may result in facial asymmetry. CMA in asymptomatic patients is known as silent sinus syndrome (SSS) and is a rare entity, especially in pediatric patients. This study reports a case of SSS in a pediatric patient who received an early diagnosis through cone-beam computed tomography (CBCT). An asymptomatic 12-year-old female patient in orthodontic treatment presented with opacification of the left maxillary sinus on a panoramic radiograph. Clinically, the patient had discrete hypoglobus and enophthalmos. CBCT and nasal video-endoscopy revealed ostiomeatal obstruction with bone deformity, leading to diagnosis of SSS. Endonasal endoscopic maxillary sinusotomy was performed. Two years later, the patient remained asymptomatic, and a second CBCT exam confirmed a stable condition. This case highlights the role of optimal radiographic interpretation for early diagnosis of maxillofacial alterations in pediatric patients.

Elongated styloid syndrome mimicking temporomandibular joint disorders: a case report and short literature review

  • Elongated styloid syndrome (ESS) can present with myriad symptoms that mimic common features of orofacial pain, such as temporomandibular joint disorders (TMJDs), often causing a challenge and delay in diagnosis. We report the case of a 52-year-old male with a three-year history of non-painful clicking during jaw movement initially diagnosed as TMJD-related internal derangement. The patient presented with a history of annoying jaw sounds for three years, described as a popping sound without bilateral clicking or crepitation. Tinnitus and progressive hearing loss were observed in the right ear, and a hearing aid was recommended by an otolaryngologist. The patient was initially diagnosed with TMJD and managed accordingly; nevertheless, his symptoms persisted. Imaging revealed prominent bilateral styloid process elongation that exceeded the recognized cut-off level of >30 mm for elongation. The patient was informed of his diagnosis and its treatment but opted only for further swallowing and auditory assessments of his ear and nose symptoms. Clinicians should consider including ESS as a differential diagnosis in patients presenting with non-specific chronic orofacial symptoms for timely diagnosis and favorable clinical outcomes.
