지주막하출혈 후 안검하수와 안구운동장애를 호소하는 동안신경마비 환자의 한방치료 증례보고 2례 (Case Reports of Korean Medicine Treatment of Two Patients with Oculomotor Nerve Palsy Complaining of Ptosis and Limitation of Eye Movement After Subarachnoid Hemorrhage)

  • Background: A number of studies have identified the effect of Korean medicine treatment for oculomotor nerve palsy, but few have reported treatment of subarachnoid hemorrhage. This paper reports two cases of oculomotor nerve palsy with ptosis and limitation of eye movement after subarachnoid hemorrhage whose conditions were improved by Korean medicine treatment. Methods: Two patients with ptosis and limitation of eye movement after subarachnoid hemorrhage were treated with Korean medicine, including acupuncture, electroacupuncture, herbal medicine, cupping, moxibustion, and pharmacoacupuncture. Clinical symptoms were measured as the distance of the interpalpebral fissure and eyeball movement (adduction, abduction). Results: After treatment, their clinical symptoms showed improvement. Conclusion: Korean medicine treatment, including pharmacoacupuncture and electroacupuncture, could be adopted as a treatment method for oculomotor nerve palsy after subarachnoid hemorrhage.

IgG κMonoclonal Gammopathy of Undetermined Significance와 연관된 운동신경 다발성 신경병증과 다발선 뇌신경병증 (Sensorimotor Polyneuropathy and Multiple Cranial Neuropathies Associated with IgG κMonoclonal Gammopathy of Undetermined Significance)

  • The occurrence of polyneuropathy in association with monoclonal gammopathy of undetermined significance (MGUS) is quite common. However, reports of MGUS associated cranial neuropathies are rare. A 63 year-old women was presented with diplopia and swallowing difficulty. Neurological examination showed limitation of abduction of right eye, right peripheral facial palsy, decreased hearing and gag reflex, left side deviation of uvula, and decreased DTR. Sensorimotor polyneuropathy were observed with elctrophysiological studies. Protein and immunoelectrophoresis revealed IgG ${\kappa}$monoclonal gammopathy. She was treated with intravenous immunoglobulin and steroid, and her symptoms and signs were improved. This case suggested that she had sensorimotor polyneuropathy and multiple cranial neuropathies associated with IgG ${\kappa}$MGUS.

  • Objective : In 1983 Autosomal dominent spinocerebellar ataxia(ADCA) has been classified to four types by a useful clinical features according to Harding. Since 1993 ADCA referred to as SCA by molecular genetic characteristics. We focused on the improvement of clinical symptoms in SCA patient through oriental medical treatment. Methods & Results : The assessment included neurophysiological examination and clinical symptoms. For example slow saccade, gaze limitation, upward and lateral, nystagmus, truncal and gait ataxia. The patient(M/30) was clinically charaterized by ocular abnormalities, trunkal and gait ataxia and the MRI showed atrophy of cerebellar vermis. The patient was taken both common acupuncture and Dong-Si venepuncture on Hwasan with herbal medicine. Before the treatment Rt. ocular movement were slowed and a significant eye fixation was observed always in abduction and intermittently presented trunkal and gait ataxia. During the treatment trunkal and gait ataxia has not been presented and ocular obnomalities changed to normal state. This case suggests the possibility of oriental medical treatment on SCA but further observation is needed on this patient.

마비성사시의 한방치험 2례 (Two Cases of Paralytic Strabismus Treated with Acupuncture and Herbal Medicine)

  • Strabismus refers to a extra-ocular muscle imbalance that results in improper alignment of the visual axes of two eyes. It may be divided into paralytic and non-paralytic strabismus. Paralytic strabismus is primarily a neurological problem: non-paralytic strabismus is more strictly an ophthalmologic problem. This case report focuses on paralytic strabismus resulting from palsies of the third and the sixth cranial nerves, respectively. Oculomotor nerve palsies result in binocular diplopia with characteristic patterns of strabismus. Oculomotor nerve provides motor and parasympathetic innervation to the eyes. Acquired oculomotor nerve palsies are not uncommon. Injury to the third nerve may result in complete or partial dysfunction. Complete third nerve palsy is manifested by ptosis, dilated pupil, an eye that is deviated down and out in primary position, and limited adduction, elevation, and depression. Patients with unilateral sixth nerve palsy complain of binocular, horizontal diplopia esotropia in the primary position due to unopposed action of the medial rectus and limitation of abduction due to weakness of the lateral rectus. Diplopia is worse in the direction of the paretic lateral rectus muscle. Paralytic strabismus are treated, based on the theory of Oriental medicine. with berbal medicines having gun-bi(健脾), bae-to(培土), gue-pung(祛風) effect of acupuncture around the eyes and etc. We describe a 63-year-old woman with complete the 3rd cranial nerve palsy and a 32-year-old woman with the unilateral 6th cranial nerve palsy who treated with acupuncture and herbal medicines and showed complete recovery.

  • Objectives: Atopic dermatitis (AD) is an easily recurrent inflammatory skin disease. Since AD has complex pathology, people have been investigating it on different aspects with various experimental models. One of them is in vivo model which has spontaneously developed AD-like skin lesions by various inducers. Methods: In this study, two kinds of mouse species were applied in the experiment; BALB/c and C57BL/6 mice. We compared features among the animal species making AD mouse model with protein allergen, ovalbumin. AD-like skin lesions were induced by ovalbumin on two kinds of scheme and were evaluated with the histological results and size of spleen which is a critical immunological organ. Also, we measured the level of immunoglobulin E in serum. In addition, we investigated the results of ovalbumin induced-AD-like skin lesions along with obesity. Results and Conclusion: We evaluated weight of organs and thickness of epidermis. These results suggest the possibility of the appropriate in vivo experimental model for AD or the comorbidity with obesity.

Changes in lower extremity alignment in standing position using a foot plate

  • Objective: Eversion of the foot is created with internal rotation of the shank, and inversion of the foot is created with external rotation of the shank. The purpose of the study was to investigate the effect of continuous changes in the angle of the subtalar joint on lower extremity alignments. Design: Cross-sectional study. Methods: Seventeen healthy young adult subjects recruited. The subjects were asked to stand up in a natural standing position on a footplate with eye open and equal weight on each foot for 10s in two different conditions: The right subtalar joint was everted continuously $0^{\circ}-20^{\circ}$ and in separate segments of $0^{\circ}$, $5^{\circ}$, $10^{\circ}$, $15^{\circ}$, $20^{\circ}$. The averages of three trials were used. The observation of the changes in the lower extremity was performed with the use of 3-dimensional motion analysis. For data analysis, the SPSS 18.0 software using paired t-test and repeated measures analysis of variance (ANOVA) was applied. Results: The angle was significantly increased at the horizontal rotation angle of the shank, thigh, and ankle without anterior rotation of the pelvis (p<0.05). The maximum horizontal rotation angle at the thigh on $20^{\circ}$ was $-4.52^{\circ}$ in static, and $-3.10^{\circ}$ in the dynamic conditions compared to $0^{\circ}$. Conclusions: Increased unilateral foot pronation, thigh, shank, ankle horizontal rotation variance was significantly effective. The observation of the changes in foot abduction with the use of a 3-dimensional motion analysis augmented in predicting the angle values of each segment of the lower extremity. In further studies, a comparison of the right and left subtalar joints need to be investigated.

급성 원발성 교뇌 출혈 환자의 하나반증후군, 현기증, 이상감각에 대한 한양방 협진치료 : 증례보고 (A Case Report of Acute Primary Pontine Hemorrhage with One-and-a-Half Syndrome, Vertigo, and Paresthesia Treated with Eastern-Western Integrative Medicine)

  • Background: The purpose of this study was to report the improvement of a patient with one-and-a-half syndrome, vertigo, and paresthesia caused by acute primary pontine hemorrhage (PPH) after a combination treatment of traditional Korean and Western medicine. Case report: A 51-year-old female with one-and-a-half syndrome, vertigo, and paresthesia after PPH was treated with Korean medicine, including herbal medication, acupuncture, and moxibustion, and Western medicine, including medication and rehabilitation therapy during hospitalization. Her progress was evaluated by checking for changes in symptoms with the extraocular muscle (EOM) function test, numeral rating scale (NRS), and follow-up brain computed tomography scans and magnetic resonance imaging (MRI). After 41 days of treatment, the EOM movement was improved, leaving limited abduction of the left eye. The NRS scores for vertigo and paresthesia decreased from 10 to 5 and from 10 to 3, respectively. Improvement was noted in hematoma in MRI, but a new ischemic lesion was also discovered. Conclusion: This case reports the clinical course of one-and-a-half syndrome and suggests that a combined therapy of traditional Korean and Western medicine can be useful for PPH patients with one-and-a-half syndrome, vertigo, and paresthesia. However, studies of larger populations are required.

양측성 외전신경 마비 환자 치험 1례 (A Case Report of Bilateral Abducens Nerve Palsy Using Korean Medicine)

  • ■ Objectives The purpose of this case report is to show an effect of korean medicine for patient with bilateral abducens nerve palsy. ■ Methods This patient was 58 year-old man who was diagnosed with bilateral abducens nerve palsy. The patient had Diabetes Mellitus, hypertension, benign prostatic hyperplasia and dyslipidemia as underlying diseases. The patient had symptoms of limited eye movement to lateral, diplopia, dizziness and mild headache. He was treated by korean medicine, containing electroacupuncture, pharmacoacupuncture, moxibustion and herb medicines during 13 days of hospitalization and following outpatient department. His eye movement was evaluated by the method of Scott and kraft and discomfort from his symptoms was scored by the Numerous Rating Scale. And other general conditions were measured. ■ Results After that period, patient's abduction deficit was improved and subjective discomfort from his symptoms was getting better. His other conditions were also better, compared with his admission. ■ Conclusion This study suggests that Korean medicine could have a therapeutic effect for bilateral abducens nerve palsy. So, It could help to relieve patient's symptoms and make general conditions better.

Kepler의 망막 상 이론 형성 과정에서의 과학적 문제 발견과 귀추적 사고 (Kepler's Scientific Problem Finding and the Abductive Reasoning in his Discovery of the Retinal Image Formation)

  • 이 연구의 목적은 시각에서의 망막 상 이론을 세우는 데 있어 Kepler가 어떻게 과학적 문제를 발견하고 그의 이론 형성 과정에서 어떻게 귀추적 사고를 하였는가를 조사하고, 그 분석 결과를 통해 과학에서의 창의성 교육을 위한 시사점을 얻는 것이었다. 관련 문헌들의 심층적인 분석을 통해, Kepler의 망막 상 이론에서의 과학적 문제 발견은 당시 시각 이론에 대한, 그의 정확하고 적절한 광학적 지식을 바탕으로 한 비판적 분석으로부터 이루어졌으며, 그는 새로운 이론을 세우는 과정에서 이미 알고 있는 구면 렌즈에서의 광학적 현상을 눈의 메카니즘 설명에 도입함으로써 귀추적 사고의 과정을 거쳤음이 밝혀졌다. 이러한 결과들로부터 과학 창의성 교육을 위한 시사점이 다음과 같이 도출되었다. 첫째는, 과학 창의성교수 자료의 개발에 있어서 프로그램 속에 Kepler의 과학적 문제 발견과 같은 상황이 제시될 필요가 있다는 것이며, 둘째로는, 적절하고 정확한 과학적 지식이 과학적 문제를 발견하고 가설을 형성하는 데 있어 필수적이라는 것이 지도되어야 하고, 마지막으로는, 학생 스스로 성공적인 문제 해결 경험을 가질 수 있도록 하고, 그것이 그들로 하여금 새로운 과학적 문제를 발견하는 데까지 이르도록 프로그램을 구성해야 한다는 것이다.
