Application of epoxy putty external skeletal fixator for stabilization of tarsocrural arthrodesis in small dogs and cats

  • Two small-breed dogs and two cats, with an average body weight of 4.88 kg (range: 4.3-5.5 kg), suffered hindlimb lameness due to luxation with or without fractures of the tarsocrural joint. These patients underwent tarsocrural arthrodesis with epoxy putty external skeletal fixator. The animals' skins were incised minimally, and the articular cartilage of the tarsocrural joint was removed, followed by autogenous cancellous bone grafting. Epoxy putty and positive Centerface®, pins with diameters 1.2 mm and 2.0 mm, were used for connecting bar and as a full pin fixation, respectively. All the patients regained the ability to bear weight on the affected limb within 3-7 days and resumed a normal gait within 9-15 weeks. The external skeletal fixator frame was removed within 13-17 weeks without major complications. Tarsocrural arthrodesis using epoxy putty external skeletal fixator resulted in excellent outcomes without severe postoperative complications in this study. Epoxy putty external skeletal fixator can be a valuable surgical option for tarsocrural arthrodesis in patients weighing less than 5.5 kg.

Computer Simulation of Double Osteotomy in Tibia Using Double-Hexapod External Fixator (이중 헥사포드 외고정 기구를 이용한 선천성 왜소증 환자의 경골 교정 시뮬레이션)

  • In this paper, we proposed a new method of pre-operative planning for tibial deformity correction using double hexapod external fixator in pseudoacondroplasia. The 3-D computer graphic model of deformed tibia was reconstructed from 3 mm sliced CT data, and CAD model of double hexapod external fixator was developed. The fixator was composed of 170 mm diameter of three rings and 90 mm of twelve struts. The bone deformities and the osteotomy lines for double osteotomy were measured using X-rays, and the necessary joint values to correct the given deformities were obtained by inverse kinematics analysis. The computer graphic simulation was performed to visualize the deformity correction process and evaluate the analysis result. By examining the pre-op and post-op X-rays, the simulation result was in good agreement with the clinical outcomes.

Effects of Pre-tension and Additional Half-pin on Fracture Stability in Hybrid External Fixator System (강선의 인장력과 추가 Half pin이 혼성외고정장치 시스템의 안정성에 미치는 영향)

  • It is clinically well known that pre-tension of wires increases the fracture stability in ring or hybrid external fixation. In some cases, additional half pin should be necessary to increase the stability when soft tissue impalement occurs during fixation. In this paper, the fracture stability of a hybrid external fixator system with different pre-tension effects and additional half-pins was analysed using FEM to investigate the effects of these pre-tension and half pin on the system stability quantitatively. 3-D finite element models of five different fixator frames were developed using by beam elements. In axial compression analysis, the fracture stiffness was increased maximally 62% as the pre-tension increased. In torsion analysis, in the other hand, there is little variations in the fracture stiffness. Additional half pin increased the system stiffness about 200 %. From the results, proper pre-tension and additional half pin would provide good methods to increase the fracture stability of the hybrid external fixator and provide more surgical options to minimize soft tissue damage at the fracture site.

  • Purpose: To evaluate the result of percutaneous fixation with cannulated screws and Ilizarov external fixator in triplane fracture of the distal tibial epiphysis in children. Materials and Methods: Between May 2004 and December 2007, 14 cases with triplane fractures were treated by percutaneous fixation with cannulated screws and Ilizarov external fixator after underwent CT imaging to assess the fracture pattern, articular disruption and to plan further management. Mean age and follow-up period were 14.1 years old and 15 months respectively. Results: There were satisfactory results in all 14 cases that had excellent reduction and stable fixation. All cases regained full range of movement within 6 weeks. Conclusion: We obtained satisfactory result after percutaneous fixation with cannulated screws and Ilizarov external fixator in triplane fractures of the distal tibial epiphysis in children.

  • A five-month old female dog was presented to Teaching Anilmal Hospital, Gyeongasng National University for femur fracture. Previously the dog had been presented to a local hospital. Orthopedic and blood examinations were conducted. There was absent response of conscious propri- oception. The dog stood knuckled onto the digits and had hock dropped, but showed no117a1 values in WBC.RBC, Hb, PCV, BUN, Creatine, Glucose, Cholesterol, AST and ALT. Plain radioaraphs were taken and there were abundant periosteal reaction and malalignment of fragments. The dog was anesthetized and prepared for aseptic surgery. Joshi external fixator and half pins were applied(uni lateral). AT 8 weeks after surgery, the pins were removed from the bone. The fracture was well healed wish no evidence of original fracture line. but mild muscle atropy and shortening of femur were found.

  • A 5-year-old, male mixed breed dog was presented to Gyeongsang National University Animal Hospital for bilateral mandibular open fracture by bite. The dog had loss of the teeth and injury of the oral mucosa. Radio-graphic and blood examinations were conducted. Fracture lines were observed between first premolar and second premolar, but temporomandibular luxation was not observed on radiographs. In serum analysis, elevated levels of CPK and LDH were found. The dog was anesthetized and prepared for aseptic surgery. Acrylic external skeletal fixator with Kirschiner wire was applied. Pin loosening and malalignment of the right fracture line was observed at 7 weeks after surgery. We applied Kirschner-Ehmer fixator and half pins during the second surgical procedure. At 4 weeks after a second surgical procedure, the fixator and pins were removed from mandible. The fractures were healed with no complications.

  • This paper presents a model and analysis techniques for a unilateral external fixator to achieve fracture reduction and deformity correction in long bones precisely. through fixator joint adjustment. Combining the kinematic analysis with a graphic model of the tibia and the fixator allowed 3D simulation and visualization of the adjustments required to reduce fracture or correct bone deformity after osteotomy. The model and analysis technique can be used for fixator evaluation and clinical application planning.

  • Two day-old male and 1 month-old female Hanwoo calves presented to Chonbuk Animal Medical Center in Chonbuk National University. Two calves were stepped by mother cow and had right (2 day-old male) and left (1 month-old female) forelimb lameness. The male calf was diagnosed as right radius-ulnar fracture through physical and radiographic examination. The other female calf was diagnosed as left third and fourth metacarpal bone fracture by the same examinations as male calf. Acrylic frame external fixator was used to reduce the fractures. Arcrylic frame external fixator in two calves lasted for 5~6 weeks. After removing the acrylic frame external fixator, radiographic evaluation and physical examination were performed. Fractured radius-ulnar and metacarpal bones were unionized and the calves were able to stand and bear weight. In these two calves, application of acrylic frame external fixator in calf fracture was effective. Acrylic frame external fixator should be considered for the repair of radius-ulnar and metacarpal bone fracture in calf.

Analysis of bone-remodeling by the influence of external fixator with FEM (FEM을 이용한 외부고정구 영향에 의한 골-재형성에 대한 해석)

  • A computational method has been developed to analyze the bone-remodeling induced by external fixator. The method was based on the Finite Element Method (FEM) in combination with numerical formulation of adaptive bone-remodeling theories. As a feed-back control variable, compressive strain and effective stress were used to determine the surface remodeling and internal (density) remodeling respectively. Surface remodeling and internal remodeling were combined at each time step to predict the rel situation. A noticeable shape and density change were detected at the region between two pins and density change was decreased with time increment. At final time step, the shape and density distribution were converged closely to its original intact bone model. Similar change was detected in stress distribution. The altered stress distribution due to the pin and external fixator converged to the intact stress distribution with time.
