스웨덴의 아동보육제도 연구 (A Study on the child care system in Sweden)

  • This study is to describl the child care system in Sweden and to understand how the progressive child care system has been developed. The sex categories of the child care issues were studied based on a review of the literature related to the Swedish child care, family welfare system and the socio-economic information on Sweden. Six categories were : (1) historical and socio-economic background of child care system, (2) supply of and demand for the child care service and the types of public and private child care, (3) child care program activities, (4) child care staff training system, (5) administration and financial support system for the child care, and (6) the family policy and the family support programs in Sweden. For the Korean child care system, this study on the Swedish child care implied that : (1) To provide a progressive system of the child care system in Korea will require serious discussion about concentration of the child care administration system which is currently distributed to Ministry of Education and Ministry of Health and Welfare. (2) The extensive family support programs such as the parental leave and flexible work conditions for women are needed for the infant care by parents at home. (3) to expand the child care services in Korea, public support should be primarily focused on the supply of the public day care centers even though the demands for the various child care services are to be met by public financial support. (4) Most of all, societal recognition that all children are the resposiblity of our society is needed to develop a progressive child care system in Korea.

가족관련변인과 영유아발달의 관계 : 종일제 보육과 어머니 단독양육의 비교 (The Relations between Familial Predictors and Infant-Toddler Development: Comparison between Full-Time Child Care vs. Exclusive Mother Care)

  • 본 연구는 영유아기 종일제 어린이집 이용 여부에 따라 가족관련 변인이 영유아발달에 가지는 관계성의 강도가 다를 수 있다는 가설을 검증하는 데에 목적이 있다. 한국아동패널연구의 3차년도 및 4차년도 조사자료를 분석하여 종일제 어린이집을 이용(주당 30시간 이상의 어린이집 이용)집단과 어린이집이나 대리양육자를 이용하지 않는 어머니 단독양육 집단을 추출하여, 일련의 가족관련 변인과 영유아의 발달적 결과 간의 관계성의 양상과 그 강도를 비교하였다. 분석방법으로 이변량 상관관계분석을 실시하였으며 두 집단의 상관계수 간의 유의한 차이를 검증하였다. 연구결과, 만2세의 경우 어머니의 사회적 양육유형이 영아의 의사소통을 정적으로 예측하는데 있어 종일제 어린이집을 사용하는 가족에서 그 크기가 유의하게 큰 것으로 나타났다. 만 3세 때에는 가정환경검사 중 다수의 하위변인에서 어린이집을 이용하지 않는 가족의 경우, 종일제 어린이집을 이용하는 경우에 비해 유의하게 강한 상관관계로 표현어휘 및 수용어휘 발달을 예측하였다. 즉, 어린이집에서 시간을 보내는 경우, 가정환경 외에 다른 요인이 발달을 예측할 수 있음을 시사한다. 하지만 이들 변인 외에 분석에 사용된 다양한 변인과 영유아 발달 간의 상관관계에 집단차는 발견되지 않아 발달에 기여하는 가족과정이 종일제 어린이집과 어머니 단독양육 집단 간에 유사 또는 차별적이라고 단순히 결론 내릴 수 없고, 발달연령, 가족과정 내의 예측요인, 그리고 발달적 결과에 따라 다소 복잡한 유형으로 차이를 보임을 알 수 있다.

보육경험과 아동특성 및 어머니의 양육행동이 아동의 공격성에 미치는 영향 (The Effect of Day-Care, Child′s Characteristics and Maternal Behavior on Child Aggression)

  • The purpose of this study is to evaluate child - care effects on young child's aggression in ecological context. For this, main effects, interaction effects and cumulative effects between child care experiences(quantity, quality, age of entry and stability) and maternal behavior as well as characteristics of child(sex, age and temperament) were examined. A total of 62 young children(34 boys and 28 girls) enrolled in child - care center were observed, and mothers and caregivers completed questionnaires. Data was analysed by three - way ANOVA, logistic regression and crosstabs. As result, first, main effects of quantity, rejective and regulative mothering on children's aggression were found. Second, there were significant interaction effects between both quality and quantity of day-care and rejective mothering. In particular, the interaction effects provided evidence that high - quality child - care served a compensatory function for extensive care and rejective mothering. Third, logistic regression analysis revealed risk factors for child's aggression and the effects of these risk factors were cumulative. Interaction effects of day-care quality and cumulative effects of day-care, characteristics of child and maternal behavior on child aggression were discussed.

동화와 관련된 확장활동이 유아의 언어 창의성에 미치는 영향 (The effect of the extensive program using the storytelling on young children's linguistic creativity)

  • The purpose of the study was to explore the effect of the extensive program using the story telling on young children's linguistic creativity. 20 five-year-olds and 18 four-year-olds from a day care center were selected for this study. After performing pre-test, they were divided into control and experimental group. Children's linguistic creativity was measured using 'Imagining Red' of 'Korean Comprehensive Creativity Test for Young Children' standardized by Chun(2000). 6 books were selected for the extensive program for the promotion of children's linguistic creativity was carried out to experimental group for three weeks. After three weeks, post-test was performed on experimental and control group. The results indicate that the extensive program using the storytelling had an effect on promoting young children's linguistic creativity including the sub-elements such as fluency, flexibility, and originality. As the results of experiencing the extensive program related to storytelling, the linguistic creativity of 5 year olds improved more than that of 4 year aids and that of girls improved more than that of boys. For the developing children's linguistic creativity, various extensive activities need to be programmed.

한국 미숙아 관리의 현황과 전망 (Current Status of Korean Premature Infant Care and its Prospective)

  • Advances in neonatal care system and research have resulted in an increased survival rate among low birth weight infants in the industrialized countries. Recent Korean neonatal mortality and morbidity statistics, and current status of Korean neonatal intensive care facilities were reviewed here for the sake of future improvement and research. Morbidity statistics revealed that perinatal diseases accounted for 80% of the death of premature infants implying the possibility of its reduction by the vigorous prenatal care service in future.On the basis of extensive studies of nutritional support and growth rate of premature infants, commercial formulas for premature infants have been developed and various aspects of feeding techniques are standardized. However, problem of growth deficit of premature infants remains unsolved as medical problem. NICU specialists are challenged with the tasks of prenatal education or provision of care that minimizes the neurodevelopmental problems seen in preterm infants, various short-term outcome researches have been reported and those are reviewed here to promote research interest in the field of neonatal nursing. Systemic long-term outcome studies are also awaited in Korea for the formulation of welfare policy in future. Nursing science has to embrace all these interdisciplinary studies as their own research field in collaboration with neonatologist, nutritionist, rehabilitation therapist, social workers and teachers.

Waardenburg Syndrome 환아의 치과치료를 위한 정주 깊은 진정법 -증례보고- (IV Deep Sedation Management of Waardenburg Syndrome Child during Dental Treatment - A case report -)

  • IV Deep sedation is useful for Waardenburg syndrome Child undergoing extensive dental treatment. We experienced a case of dental treatment under W deep sedation in a 3 years old boy with Waardenburg syndrome. He was treated on an outpateint basis. He was diagnosed hypothyroidism and received thyroid hormone. Premedication was done using ketamine and atropine IM. Induction using $N_2O$-enflurane mask ventilation was uneventful and intravenous cathetering was carried out. IV deep sedation was maintained with midazolam-$N_2O$ combination. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful IV Deep sedation management of Waardenburg syndrome child during dental treatment in as out patient.

가계 내 경제적 자원이 자녀의 건강에 미치는 영향에 대한 실증분석 (Economic Resources and Child Health: An Assessment of Certain Mechanisms)

  • 본 연구는 가계 내 경제적 자원이 자녀의 건강에 미치는 영향을 분석한다. 경제적 자원과 건강 간의 관계는 두 변수 상호 간의 영향으로 인해 분석이 용이하지 않다. 그러나 성인의 건강이 노동공급 등을 통해 경제적 자원에 영향을 미치는 것과는 달리, 자녀의 건강은 경제적 자원에 미치는 영향이 적으므로 가계의 경제적 자원이 자녀의 건강에 미치는 영향은 분석이 상대적으로 용이하다. 인도네시아 자료(Indonesian Family Life Survey)를 이용하여 분석한 결과, 가계의 경제적 자원이 자녀의 건강에 통계적으로 유의한 수준에서 긍정적인 영향을 끼치는 것으로 나타났다. 또한 경제적 자원이 자녀의 건강에 영향을 끼치는 경로는 영양상태, 의료서비스의 이용, 가계의 위생상태 등인 것으로 나타났다. 따라서 저소득 가구 자녀의 건강증진을 위해서는 의료서비스의 이용 증대뿐만 아니라 가구의 위생상태 등을 개선하는 정책 또한 중요한 것으로 판단된다.

영유아 시기의 어머니 취업이 모-자녀 관계에 미치는 영향에 관한 종단적 연구 (A Longitudinal Approach to the Effects of Early Maternal Employment on Mother-Child Relationship)

  • 본 연구는 미국 NICHD Study of Early Child Care에 참여하는 1364 가족을 대상으로 하여 어머니의 초기취업이 이후 어머니-자녀 관계에 미치는 영향을 종단적으로 밝혀보고자 하였다. 자녀 출생 이후 6개월부터의 취업에 관한 데이터를 이용하여 자녀의 영유아기 시기에 언제나 일주일에 30시간 이상 취업해 왔던 어머니, 파트타임으로 취업해왔거나 일부 기간 동안만 일했던 어머니, 그리고 늘 전업주부였던 어머니 집단으로 나누어 자녀가 36개월일 때와 초등학교 1학년일 때 세 집단 간 어머니의 지지행동, 자녀 자율성 존중, 적대감의 정도를 비교하였다. 자녀 연령 36개월까지 언제나 취업했던 어머니는 전혀 취업하지 않은 어머니에 비해 낮은 수준의 지지행동을 36개월에 보였다. 자녀 생후 1년 동안 부분적으로만 일했던 어머니는 다른 집단 어머니에 비해 유의하게 높은 수준의 지지행동을 보였다. 이외의 36개월에 관찰된 변수들에서는 유의한 집단 차이가 존재하지 않았다. 초등학교 일학년 때 모-자녀 관계에서는 어머니의 36개월까지의 취업집단 및 1학년 때 취업집단 사이 모두 유의한 집단차를 찾을 수 없었다. 전반적인 결과는 어머니의 취업, 특히 영유아기의 초기 취업이 2-3년 이후 어머니가 자녀와 가지는 상호작용의 질에 일관되게 부정적인 영향을 주지는 않는다는 점을 시사하였다.

Effects of A Picture Book Reading Intervention Program on Young Children's Language Development and Print Concept

  • The purpose of this study was to investigate the effects of picture book reading intervention activities on language development of low-income children. The subjects were 60 children in low-income families, aged 5, selected from 24 child care centers located in three cities nearby Seoul, South Korea. The experimental group had received intervention program for 8 weeks, two days a week, and three teachers conducted the intervention program for 30 to 40 minutes for each session. The intervention program was administered to the children with picture book reading activities in the first session, followed by providing more extensive activities in the second session. Afterwards, the study allowed the children to take one picture book to read at home. To evaluate the effect of the picture book reading intervention program, this study utilized instruments called the Preschool Receptive-Expressive Language Scale and the Concepts about Print. Significant differences found between the two groups. The experimental group showed higher scores compared with the control group in the post-test of expressive language development. Also, children in the experimental group showed a significant increase in the concepts about print after the intervention program was administered. In conclusion, findings indicate several changes in positive outcomes after implementation of the picture book reading intervention program.

정신지체 환아의 치과치료를 위한 외래마취관리 -증례보고- (Anesthetic Management of a Mentally Retarded Child during Dental Treatment -A case report -)

  • General anesthesia is often required for mentally retarded children undergoing extensive dental treatment. We experienced a case of dental treatment under general anesthesia in a 14-year-old boy with mental retardation. He was treated on an outpatient basis. He was diagnosed of Noonan syndrome and received heart surgery when he was six years old. Induction using thiopental and vecuronium was uneventful and nasotracheal intubation were carried out. General anesthesia was maintained with sevoflurane for 2.5 hours. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful anesthetic management of a mentally retarded child during dental treatment in as an out-patient.
