The effect of different micro-osteoperforation depths on the rate of orthodontic tooth movement: A single-center, single-blind, randomized clinical trial

  • Objective: This study aimed to identify the clinical effectiveness of two different penetration depths of micro-osteoperforations (MOPs) on the rate of orthodontic tooth movement. Methods: Twenty-four patients requiring the removal of the upper first premolar teeth were selected and randomly divided into two groups. The control group participants did not undergo MOPs. Participants in the experimental group underwent three MOPs each at 4-mm (MOP-4) and 7-mm (MOP-7) depths, which were randomly and equally performed to either the left or right side distal to the canine. The retraction amount was measured on three-dimensional digital models on the 28th day of retraction. MOP-related pain was measured using a visual analog scale (VAS). Between-group statistical differences in the VAS scores were determined using an independent t-test and those in canine retraction were determined using analysis of variance and post-hoc Tukey test. Results: No significant difference was found between the MOP-4 (1.22 ± 0.29 mm/month) and MOP-7 (1.29 ± 0.31 mm/month) groups in terms of the canine retraction rate. Moreover, both the groups demonstrated a significantly higher canine movement than the control group (0.88 ± 0.19 mm/month). MOPs did not significantly affect the mesialization of the posterior teeth (p > 0.05). Moreover, the pain scores in the MOP-4 and MOP-7 groups were similar and showed no statistically significant difference. Conclusions: Three MOPs with a depth of 4 mm can be performed as an effective method to increase the rate of tooth movement. However, three MOPs with depths of 4-7 mm does not additionally enhance tooth movement.

Ipriflavone 투여가 백서의 실험적 치아이동 후 치주조직의 재형성에 미치는 영향 (The effects of ipriflavone on the periodontal reorganization following experimental tooth movement in the rat)

  • 본 연구는 치아이동 후 ipriflavone 투여가 치주조직의 재형성, 나아가 치아회귀 억제에 미치는 영향을 알아보기 위하여 시행되었다 웅성 백서 72마리를 대상으로 상악 좌우측 제1, 2구치 사이에 교정용 고무밴드를 끼워 제1, 2구치를 3주간 이동시킨 다음, 교정용 고무밴드 제거 1일 전부터 매일 체중 kg당 50 mg 또는 400 mg의 ipriflavone을 투여한 실험군과 ipriflavone을 투여하지 않은 대조군으로 구분하였다. 교정용 고무밴드를 제거함으로써 회귀가 나타나기 시작한 날로부터 5일 간격으로 15일까지 각각 실험동물을 희생시켜가며 치아회귀 거리를 계측하는 한편 치주조직 재형성 소견을 관찰하였다. Ipriflavone을 투여하고 치아회귀를 관찰한 결과 용량에 관계없이 대조군에 비하여 적은 양의 회귀를 나타내었으며 시간 간격에 따른 치아회귀를 관찰한 결과 대부분의 회귀가 5일째 나타난 대조군과 달리, 늦게 그리고 점진적으로 나타나는 양상을 보였다. Iprinavone을 투여하고 조직소견을 관찰한 결과 대조군에 비하여 치조골연이 매끄러운 양상을 보였으며 시간 간격에 따른 조직소견을 관찰한 결과 대조군에 비하여 치주인대 재배열이 빠르게 나타나는 양상을 보였다. 이상의 결과는 ipriflavone의 투여가 치주조직의 재형성을 통하여 회귀를 억제함으로 교정적 치아이동 후 보정에 도움이 될 수 있음을 시사하였다.

비타민 C 결핍이 guinea pig의 실험적 치아이동속도와 치조골 개조에 미치는 영향 (Effect of vitamin C deficiency on the rate of orthodontic tooth movement and alveolar bone remodeling)

  • 이 연구는 비타민 C 결핍이 guinea pig에서의 실험적 치아이동속도와 치조골 개조에 미치는 영향을 알아보고자 시행되었다 이를 위하여 웅성 guinea pig 30마리를 대상으로 정상량의 비타민 C (5 mg/day) 를 투여한 군(정상군)과 결핍량(0.2mg/day)을 투여한 군(결핍군)으로 나누고 치아이동 실험을 시행하였다. 초기 교정력 75gm으로 상악의 좌우 중절치를 이개시키는 치아이동을 시행하고, 순차적으로 실험 경과기간이 지나면 치아이동양을 계측하고 희생시켜 조직 소견을 관찰하였다. 실험 결과. 결핍군에서 치조골과 치주인대의 교원섬유 함량은 현저한 감소와 불규칙한 배열을 보였으며 인대세포의 수적 감소 및 출혈이 관찰되었고 치조골에서 골형성의 감소와 파골세포의 출현 및 골소강의 크기증가와 골소강내의 다수의 파골세포가 관찰되었다. 정상군의 인장측 치조골은 치아이동 시간경과에 따라 조골세포와 골형성이 지속적인 증가를 보였으나, 결핍군에서는 조골세포가 감소하고 골형성은 미약한 양상을 보였다. 치아 이동양은 실험 초기 1일, 3일, 5일, 7일 결핍군의 치아이동양이 정상군보다 많게 나타났다 (p<0.05) 이상의 결과에서 비타민 C 결핍은 치주조직의 교원질의 합성을 억제하여 치조골의 형성을 감소시키고 흡수를 증가시키는 골개조를 초래하며, 치아이동중 인장측 치조골 형성을 억제하고 압박측 치조골 흡수를 촉진하여 치아이동 초기에는 빠른 치아이동을 가져오는 것을 시사한다.

Relationship Between the Dose of Clodronate and Serum Level of Alkaline Phosphatase, Calcium, and Phosphate During Orthodontic Tooth Movement

  • Objective : To evaluate the relationship between the dose of Clodronate and serum level of alkaline phosphatase (ALP), calcium (Ca), and phosphate (PO4) during orthodontic tooth movement MaterialS and MethodS: A total of 18 sex-matched Wistar rats (weight=180~230g, mean age=8 weeks) were allocated into the 2.5mM Clodronate (2.5C) group, 10mM Clodronate (10C) group, or control group (n=6 for each group). After the application of a nickel-titanium closed coil spring (force of 60g) between the upper central incisors and first molars (UFM), 2.5C, 10C, or saline was injected every third day into the subperiosteum of the alveolar bone adjacent to UFM for the experimental and control groups. The animals were sacrificed 17 days later. Trunk blood was quickly collected into a heparinized tube and centrifuged at 2,000 rpm for 20 min. The plasma was used for the biochemical assays of the serum level of ALP, Ca, and PO4. Kruskall-Wallis test and Mann-Whitney test with Bonferroni correction were performed for the statistical analyses. Results : Dose-dependent increase in the level of ALP (P<0.01) and decrease in the level of Ca (P<0.001) were observed among the control, 2.5C, and 10C groups. Although there was no significant difference in PO4 between the 2.5C and 10C groups, the 10C group showed a significantly higher level of PO4 than the control group (P<0.01). Conclusion : Since Clodronate induced significant dose-dependent change in the serum level of ALP, Ca, and PO4 during orthodontic tooth movement, orthodontists should consider these biochemical markers not only as a diagnostic tool for bone turnover rate but also as a monitoring tool for orthodontic tooth movement.

실험적 치아 이동시 성견 치주조직의 변화에 대한 면역조직화학적 연구 (IMMUNOHISTOCHEMICAL STUDY ON THE PERIODONTAL TISSUE REACTION DURING EXPERIMENTAL TOOTH MOVEMENT IN THE ADULT DOG)

  • The purpose of this study was to evaluate the effect of orthodontic force on periodontal cellular activity by immunoperoxidase stain of epidermal growth factor, one of the tissue hormone. And supplementarily, to investigate of the changes of periodontal structures, periodontium was stained by H-E, Masson's Trichrome, P. A. S. stain after orthodontic force application. The experimental animals were four young adult dogs of average 8 month old. The fixed orthodontic appliance was cemented on mandibular right 4th premolar and 1st molar of each animal as experimental site. Mandibular left 4th premolar area of the same animal was used as control. The appliance consist of two silver crown soldered with 0.030' tube, $0.018\times0.022'$ S.S. sectional arch wire, and 0.009' open coil spring for manifestating of orthodontic force for bodily tooth movement of mandibular 4th premolar toward mesial direction. Experimental group was sacrificed at 1, 2, 3, 5 weeks from beginning of the experiment, and was investigated immunohistochemically and bistochemically by several staining methods. Findings were as follows: 1. The degree of EGF staining in control group was highest in epithelium of periodontium, and osteoclasts, osteoblasts and fibroblasts around the capillary were stained at higher level in periodontium. Generally, control group shows positive distribution of EGF all around the periodontal area. 2. The degree of EGF staining in control and 5 week group were similar, and did not show the significant different level between tension and pressure side. 3. All of 1, 2, 3 week group showed the same staining degree and distribution of EGF, and the tension side was more positive reaction of EGF stain than the pressure side. 4. The features of collagen fiber and periodontal fiber arrangement observed by H-E, Masson's Trichrome and P. A. S. stain revealed that oblique periodontal fibers were strectched in tension side, compressed in pressure side of all experimental group. Some fiber group in pressure side of 5 week group recovered the regular arrangement along the capillaries.

백서구치의 실험적 치아이동시 치근막 혈관변화에 관한 혈관주형법을 이용한 연구 (AN EXPERIMENTAL STUDY ON THE VASCULAR CHANGES OF RAT MOLAR PERIODONTAL LIGAMENT FOLLOWING ORTHODONTIC TOOTH MOVEMENT USING VASCULAR CORROSION CASTING METHOD)

  • This study was undertaken to investigate the three dimensional vascular changes of periodontal ligament following orthodontic tooth movement. Experimental tooth movement was carried out in 96 Sprague-Dawley rats with the weight of 250g. They were divided into four experimental groups (each 24 rats). The left maxillary first molar was moved mesially with 25g force in group I, and with 75g force in group II. Each three animals were sacrificed after 1, 6, 12, 24 hours, and 3, 7, 14, 21 days. In group III, 25g mesial force was applied for 3 days, and in group IV, 75g mesial force was applied for 3 days. Then the appliances were removed, and each three animals were sacrificed after 1, 6, 12, 24 hours, and 3, 7, 14, 21 days from removal of appliance. The contralateral molars were used for control group. Casting media was injected via left ventricle and polymerized in warm water. After corrosion of surrounding soft tissue, three dimensional vascular changes were examined using scanning electron microscopy. The findings of this study were as follows: 1. Pressure side of group I and II showed degenerative vascular changes such as vascular compression, reduction of vasculature, leakage of casting media. But, regenerative changes were dominant after 7 days of tooth movement. Although the degenerative vascular changes were more severe in group II, which was exposed to heavy force, the timing of these changes was not different between two groups. 2. Periodontal vasculature was reestablished by the growth of new capillaries and their differentiation and union from the remaining periodontal vessels and vessels of alveolar bone marrow. Although vascular regeneration was more rapid in group I, which was exposed to light force, the vasculature was not fully normalized in both groups even after 21 days. 3. There was no remarkable changes in tension side of group I and II, but looping of capillary, new capillary growth, dilation of vessels, redirection of vessels in the direction of tensile force were occurred. 4. In pressure side of group III and IV, in which appliance was removed after 3 days of orthodontic force, bone resorption was continued even after removal of appliance. Regeneration of vasculature was initiated after 1-6 hours, and it was more rapid in group III than group IV. In both groups, the vasculature was not fully normalized even after 21 days. 5. After removal of appliance, tension side of group III and IV showed vascular compression and loss of vasculature.

Impact of piezocision on orthodontic tooth movement

  • Objective: This study investigated the impact of a single piezocision in the maxillary alveolar process on the speed of tooth movement. The null hypothesis was that the speed of tooth movement will be equal with and without piezocision. Methods: All maxillary molars on one side were moved against the combined incisors in 10 ten-week-old male Wistar rats. Under general anesthesia, a force of 25 cN was applied on either side using a Sentalloy closed coil spring. After placing the orthodontic appliance, vertical corticision was performed using a piezotome under local anesthesia, 2 mm mesial from the mesial root of the first molar on a randomly selected side; the other side served as the control. At the beginning of the treatment, and 2 and 4 weeks later, skull micro-computed tomography was performed. After image reconstruction, the distance between the mesial root of the first molar and the incisive canal, and the length of the mesial root of the first maxillary molar were measured. Moreover, the root resorption score was determined as described by Lu et al. Results: Significantly higher speed of tooth movement was observed on the corticision side; thus, the null hypothesis was rejected. The loss of root length and root resorption score were significantly more pronounced after piezocision than before. A strong correlation was observed between the speed of tooth movement and root resorption on the surgical side, but the control side only showed a weak correlation. Conclusions: Piezocision accelerates orthodontic tooth movement and causes increased root resorption.

치과에서 사용되는 수종의 소염진통제가 치아이동에 미치는 영향에 관한 실험적 연구 (EXPERIMENTAL STUDY IF THE EFFECTS OF NON-STEROIDAL ANTI-INFLAMMATORY AGENTS USED IN DENIAL CLINIC ON ORTHODONTIC TOOTH MOVEMENT IN RATS)

  • Non-steroidal anti-inflammatory agents are used to relieve pain and to reduce swelling in dental clinics. This experiment was performed to study the effect of non-steroidal anti-inflammatory agents in Sprague-Dawley rats on orthodontic tooth movement. Thirty rats were used and divided six groups of five rats each. The first group, administered saline and no orthodontic force, served as a normal group. The second group, administered saline and applied experimental force, was control group. The other four groups were administered Aspirin, Pontal, Tyrenol and Indomethacin each, and applied experimental orthodontic force by 1/4 inch elastic, inserted into the interproximal space between maxillary first and second molar in rats. All experimental rats were sacrificed after three days, and the specimens were sectioned horizontally five times serially, and counted the number of osteoclasts appeared at the compressed surface of interradicular bone on first buccal root of first molar on light microscope. The obtained results were as follows: 1. The number of osteoclast on the compressed surface of the interradicular bone on first buccal root of the first molar in the four non-steroidal anti-inflammatory agents groups decreased in contrast to control group. 2. In non-steroidal anti-inflammatory agents group, the number of osteoclast in Indomethacin group was least among the all non-steroidal anti-inflammatory agents groups. From the above results, it was believed that the non-steroidal anti-inflammatory agents may have the inhibitory effect of tooth movement during orthodontic treatment.
