Generation of Weather Data for Future Climate Change for South Korea using PRECIS (PRECIS를 이용한 우리나라 기후변화 기상자료의 생성)

  • According to the Fourth Assessment Report of the Inter governmental Panel on Climate Change(IPCC), climate change is already in progress around the world, and it is necessary to start mitigation and adaptation strategies for buildings in order to minimize adverse impacts. It is likely that the South Korea will experience milder winters and hotter and more extreme summers. Those changes will impact on building performance, particularly with regard to cooling and ventilation, with implications for the quality of the indoor environment, energy consumption and carbon emissions. This study generate weather data for future climate change for use in impacts studies using PRECIS (Providing REgional Climate for Impacts Studies). These scenarios and RCM (Regional Climate Model) are provided high-resolution climate-change predictions for a region generally consistent with the continental-scale climate changes predicted in the GCM (Global Climate Model).

Resection of Cervical Trachea and Ene-to-end Anastomosis : A Review of 11 Cases (경부기관절제 및 단단문합술 11례에 대한 분석)

  • Despite improvement in respiratory care, including widespread use of low pressure and high volume cuffed tubes, tracheal stenosis remains a feared complication of prolonged intubation and tracheostomy. In such patients, other coexisting problems such as vocal cord paralysis, tracheoesophageal fistula, noncontiguous stenotic segments and laryngeal stenosis may occasionly be encountered. Therefore tracheal stenosis still presents a significant management problem, despite recent endoscopic advances and surgical techniques. Between 1991 and 1994, authors preformed tracheal resection with end-to-end anastomosis on 11 patients with tracheal stenosis. The total success rate (asymptomatic patients with patent airway) was 72.7% and there were no serious complication. This report reviews our experience about this procedure and surgical results. And it investigates associated factors for successful results.

Comparison of Meaning in Life and Death Attitude between Participants and Non-participants in Well-dying Education (죽음준비교육 참여군과 비 참여군의 삶의 의미 및 죽음에 대한 태도 비교)

  • Purpose: The purposes of this study was to compare the meaning in life and death attitude between the participants and nonparticipants of the well-dying education program. Methods: This study adopted the descriptive comparative design. Data were collected by interviewing 85 participants and 94 non-participants of well-dying education. The instruments used for this study were a self-report questionnaire. Results: There were significant differences in age, gender, marital status, health status, and volunteer experience. The program participants showed higher scores in the death attitude than non-participants. There were significant correlations between meaning in life and death attitude in participant group. Death attitude was significantly associated with meaning in life in participant group with 6.0% variance. Conclusion: Based on the results, well-dying education program was effective to prepare good death with more comprehensive vision. Therefore, this program should be served for patient with life-threatening illness by nurse and this is the expended role of oncology and hospice palliative nurses.

  • For predictable posterior implants, appropriate soft tissue thickness, called the biologic width, is required around the implant for crestal bone stability. In order to do so, it seems that there are many cases where the implant should be positioned deeper than the depth that we previously thought was appropriate or inevitable limit. I would like to share my clinical experience about the vertical position of the posterior implant with the case reports and the related surgical technique.

  • It is a relatively rare clinical experience to see the impacted first molar. The first permanent molar is the key in normal occlusion. Abnormal eruption of the first permanent molars would inflict normal development of dental arches and give rise to abnormal root resorption of the adjacent teeth. Therefore, the abnormally erupted first permanent molars should be detected and treated early in order to obtain proper occlusal guidance in mixed dentition period. This report presents two cases on distally tilted and impacted mandibular first molars which were treated by surgical exposure and orthodontic traction using facial mask.

Aesthetic Facial Correction of Cleidocranial Dysplasia

  • We report two cases of cleidocranial dysplasia, which was managed without significant craniofacial osteotomy. A mother and daughter, both of normal intelligence, presented with central forehead depression, mid-face hypoplasia, and blepharoptosis. The fact that they have an identically deformed face implied a genetic basis. In both patients, radiologic evaluation revealed the underdeveloped maxilla, persistent fontanelle opening, and cleidal aplasia. Clinical findings and radiologic studies were consistent with the diagnosis of cleidocranial dysplasia. Both patients underwent forehead plasty via bicoronal approach, augmentation rhinoplasty using tip plasty, and epicanthoplasty. In addition, the mother underwent malar augmentation using Medpor implantation and reduction genioplasty. The patients did not experience any postoperative complication and remained satisfied with the operation at 6-year follow-up.

  • Intraparotid facial nerve schwannoma is a rare benign neoplasm. Due to its rarity, it is not usually a prioritized diagnosis before surgery and may therefore lead to an unintentional treatment error. In this article, we report a single case of intraparotid facial nerve schwannoma. We were able to make a diagnosis with frozen biopsy. A complete resection of the mass while preserving the facial nerve was performed. Herein we present our clinical experience with regards to the treatment process of intraparotid facial nerve schwannoma.

  • Myasthenia gravis is usually defined as a state of abnormal fatigability. The cause of myasthenia gravis is not known. Several disorders tend to occur more frequently in patients with myasthenia gravis, such as hyperthyroidism or other thyroidal disorder. Anesthetic experience with thymectomies for two patients with myasthenia gravis has been reported. Both of them tolerated the surgical procedures under endoteracheal Nitrous oxide-Oxygen-Halothane anesthesia well. Diagnosis and clinical features including choice of preanesthetic medication, anesthetic agents, techniques and of neuromuscular blocking agents for myasthenic patients have been discussed. Paramount importance of rigid attention to ventilation, the maintenance of a patent airway and the removal of secretions whenever necessary for the patient safety throughout operative and postoperative period has been stressed.

  • For the safety of railway, it should be evaluated for the running safety by measuring the derailment coefficient. Although railway has run the fixed and maintained rail, some of railway is derailed. This report shows the results that performed the static load test, wheelset manufacturing for test, main line running test on the basis of the derailment theory and experience. It is executed main line test into more than 80km/h for estimating the curving performance and running safety of Gwangju EMU. As the test results, could confirm the curving performance and running safety of Gwangju EMU from the results of the wheel unloading, lateral force, derailment coefficient etc. Derailment coefficient was less than 0.8, and lateral force allowance limit and wheel load reduction ratio were enough safe.
