Fabricating a crown to retrofit an existing abutment tooth for a partial removable dental prosthesis (PRDP) is one of the most time-consuming and labor-intensive clinical procedures. In particular, when the patient is concerned with esthetic aspects of restoration, the task of fabricating becomes more daunting. Many techniques for the fabrication of all-metallic or metal-ceramic crowns have been discussed in the literature. This article was aimed to describe a simple fabrication method in which a retrofitting crown was fabricated for a precise fit using a ceramic-pressed-to-metal system.

국소의치 장착자에서 2차 우식 등의 문제가 발생하여 지대치 치료 후 크라운 재수복이 필요한 경우가 있다. 기존 국소의치가 잘 기능하고 있고 환자가 여러 가지 원인으로 국소의치를 새로 제작할 수 없을 경우에는 기존 국소의치에 맞도록 지대치 보철물을 제작해야 한다. 그러나 이 과정은 기술적으로 난이도가 높고, 기공과정이 복잡하다. 국소의치를 지지하는 지대치에 합병증이 발생하기 전의 원본 모형이 있고 참고점이 될 수 있는 다른 치아들을 포함하고 있다면, CAD/CAM을 이용하여 retrofitting 보철물을 매우 간단하고 효율적으로 제작할 수 있다. 본 증례는 지대치의 포스트, 코어 및 보철물이 복합적으로 탈락하여 내원한 환자에서, computer-aided design and computer-aided manufacturing (CAD/CAM)을 이용해 기존 국소의치에 맞는 보철물을 제작하였고 높은 적합도와 조정 과정의 최소화 등 만족스러운 결과를 얻어 이를 보고하는 바이다.

Fabricating a surveyed prosthesis beneath an existing partial removable dental prosthesis (PRDP) is a challenging and time-consuming procedure. The computer-aided design/computer-assisted manufacturing (CAD/CAM) technology was applied to fabricate a retrofitted, surveyed zirconia prosthesis to an existing PRDP. CAD/CAM technology enabled precise and easy replication of the contour of the planned surveyed crown on the existing abutment tooth. This technology ensured excellent adaptation and fit of newly fabricated crown to the existing PRDP with minimal adjustments. In this case report, a seventy-year-old male patient presented with fractured existing surveyed crown. Because the existing PRDP was serviceable, new crown was fabricated to the existing PRDP.

In this study 240 geriatric patients, aged 60 year and over were evaluated using a questionnaire and oral examination for their oral status, demand and satisfaction of prosthesis treatment. 1. The single main complaint was regarding from the existing removable prosthesis. When the complaints were catagorized into larger groups, complaints regarding existing prosthesis were 32% and followed by 30% in caries and endodontic problems. 2. DMFT index woman appeared the man the age when will increase more highly recording where highly and considered statistically. 3. According to the result from questionnaire, 48% of patients wearing removable prosthesis and 43% with fixed prosthesis were not satisfied with existing prosthesis. Thirty-seven percentages of patients were not satisfied with aesthetics and unable to chew food properly. 4. Thirty-nine percentage of patients were wearing removable prosthesis. More complete dentures were found on maxilla and partial dentures on mandible. 5. Seventy-seven percentage of patients requested for new prosthesis and the majority of these patients had complaints of difficulties in chewing and discomfort.

본 증례는 기존의 국소의치에서 파절된 metal-ceramic surveyed crown을 complete contour zirconia surveyed crown으로 전환하는 간단하고 편리한 CAD/CAM 기술을 보고하고 있다. 항암치료를 받고 있는 환자의 기존 metal-ceramic surveyed crown 외형, 형태학적 특성, 레스트 시트를 complete contour zirconia surveyed crown으로 제작하는 복제 기법에 대해 보고한다.

치조제의 조직 변화, 의치 또는 지대치의 파절 등으로 인해 국소의치의 수리가 필요한 경우가 종종 발생한다. 기존 국소의치의 금속 구조물에 맞는 보철물을 제작하면 몇 가지 장점이 존재한다. 환자의 경제적 부담이 줄어들고, 새로운 의치를 제작하기 위해 여러 번 내원할 필요가 없다. 자연치아가 파절되어 의치를 수리할 때 사용되는 인공치는 자연치아와 유사한 형태를 갖기 어려우며, 심미적인 인공치 수리를 위해서는 환자 개개인의 치아 형태와 유사한 맞춤형 인공치 제작이 필요하다. 최근에는 CAD/CAM 기술을 이용한 기존 국소의치에 맞는 보철물을 제작하여 높은 유지력과 적합도를 확보할 수 있으며, 맞춤형 인공치아를 디자인하여 더욱 심미적이고 조화로운 의치 수리가 가능하다. 디지털 방식을 이용하여 기존 국소의치에 맞춤형 보철물과 인접치열을 미러링한 맞춤형 인공치를 제작하여 의치를 수리한 증례로 시간과 비용을 절약하고 과정을 단순화하면서 심미적, 기능적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

A well-planned, precise occlusal adjustment of natural teeth has some distinct advantages over other forms of occlusal therapy. It should be emphasized, however, that an occlusal adjustment is an irreversible procedure and has definite contraindications in some mouths. Generally, the treatment methods for the patients that has open-bite will be following as below. : (1) Use of removable orthopedic repositioning appliance, (2) Orthodontics, (3) Full or partial reconstruction of the dentition, (4) Orthognathic surgical procedure, (5) Occlusal adjustment of the existing natural teeth, (6) Any combination of the above. Above all, the advantages of occlusal adjustment of natural teeth are : (1) the patient is more able to adapt to the changes in jaw position and posture; (2) the phonetic or speaking ability of the patient is not significantly changed and usually is improved; (3) the esthetics of the natural teeth is not altered and often is better; (4) the hygiene of the individual teeth is easily maintained; and (5) the functional usage of the teeth as cutting and chewing devices is markedly improved. The objective of an occlusal adjustment, as with any form of occlusal therapy, is to correct or remove the occlusal interferences, or premature contacts, on the occluding parts of the teeth which prevent a centric relation closure of the mandible. A systematic, disciplined approach can be followed in treatment, the objectives should be listed. They are : (1) Centric relation occlusion of the posterior teeth. (2) Proper "coupling" of the anterior teeth. (3) An acceptable disclusive angle of the anterior teeth in harmony with the condylar movement patterns. (4) Stability of the corrected occlusion. (5) Resolution of the related symptoms. For the patient with open-bite on anterior and posterior teeth, this case report shows the treatment methods in combination the fixed prosthesis with the selective cutting of the natural teeth. Occlusal adjustment is no longer an elective procedure but a mandatory one for patients requiring restorations and those in treatment for TMD dysfunctions or those whose dentitions show signs of occlusal trauma. Occlusal adjustment is essential for all who do not display the above lists.
