A Clinical Study of Patent Ductus Arteriosus (동맥관개존증의 임상적 고찰)

  • A clinical study on 139 cases of operated PDA was performed during period from Aug. 1982 to Apr. 1991 at the Dept. of Thoracic and Cardiovascular Surgery of Chonbuk National University Hospital. The following results are obtained. 1. The 35 males and 104 females ranged in age from 6 months to 40 years. [mean 10.2 yrs. ] 2. Chief complaints of the patients were frequent URI in 50%, dyspnea on exertion in 31.2%, palpitation in 11.1%, and no subjective symptoms in 28.78% 3. On auscultation, continuous machinery murmur heard in 79.86% and systolic murmur in 20.14%. 4. Radiologic findings of chest P-A showed increased density of pulmonary vascularity in 80.58%, cardiomegaly in 61.87%, and within normal limit in 19.42% of the patients. 5. The signs of LVH[44.4%], RVH[17.4%], BVH[7.6%] were noted on the EKC. 6. Cardiac catheterizations were performed in 114 patients. The mean Qp/Qs was 2.65 and the mean Pp /Ps was 0.41 and the mean systolic pulmonary artery pressure was 46.6 mmHg. 7. Operative methods were as followed: The 130 cases[93.52%] of ligation and 3 cases[2.16%] of division & suture for PDA were performed through the left posterolateral thoracotomy. And the remained cases were managed under the cardiopulmonary bypass. 8. Operative complications were hoarseness in 8 cases, atelectasis in 6 cases, intraoperative ductal rupture under the left thoracotomy approach 2 cases, recannalization 1 case and others in 3 cases. 9. One patient died due to ductal rupture intraoperatively and the overall mortality was 0.7%.

Psychophysical and Physiological Study on Various Lifting Tasks (여러 가지 들기 작업에서의 인체심리학적 · 생리학적 연구)

  • The muscular-skeletal disorders(MSDs) that have become a major issue recently in Korean industrial safety area are mainly caused by manual material handling task. The objective of this study is to provide scientific data for the establishment of work safety standard for Korean workers through the experiments of lifting task under various conditions, in order to prevent the muscular-skeletal disorders in the industrial work site. Eight male college students were recruited as participants. Three different lifting frequencies(1, 3, 5 lifts/min) and three twisting angles(including the sagittal plane and two asymmetric angles; i.e., 0°, 45°, 90°) for symmetric and asymmetric tasks, respectively, with three lifting range from floor to knuckle height, knuckle to shoulder, floor to shoulder height for one hour's work shift using free style lifting technique were studied. The maximum acceptable weight of load(MAWL) was determined under the different task conditions, and the oxygen consumption, heart rate, and RPE were measured or recorded while subjects were lifting their MAWLs. The results showed that: (1) The MAWLs were significantly decreased as the task frequency and task angle increased.; (2) The heart rate, oxygen consumption, RPE significantly increased with an increase in lifting frequency although maximum acceptable weight of lift decreased.; (3) The highest heart rate and oxygen consumption was recorded at the lifting range of floor to shoulder, followed by floor to knuckle and knuckle to shoulder.; (4) The RPE value showed that subjects perceived more exertion at the high frequency rate of lifting task and lifting range of floor to shoulder height. (5) The modeling for MAWL using isometric strength, task angle and lifting frequency were developed. It is expected that use of the results provided in this study may prove helpful in reducing MMH hazards, especially from lifting tasks for Korean, and can be used as a basis for pre-employment screening.

Analysis of Maximum Acceptable Frequencies for Upper Extremity Motions with Forces in a Seated Position (앉은 자세에서 힘 수준에 따른 상지관절 동작별 최대 수용 반복 빈도수 분석)

  • Evaluation of repetitiveness for upper extremity intensive tasks is essential to determine the level of risk for upper extremity musculoskeletal disorders at the workplace. However, experimental data available to establish the acceptable levels of repetitiveness for various postures and forces is lacking. The present study examined the maximum acceptable frequencies(MAFs; motions/min.) of shoulder, elbow, wrist, and index finger motions at different forces(1kgf and 4kgf for shoulder, elbow, and wrist; 0.25kgf and 1 kgf for index finger) in sitting. Seventeen right-handed males in 20s without having any history musculoskeletal disorders participated in the MAF experiment. The participants determined their MAFs for the upper extremity motions by using the self-adjustment method and their work pulse(increase in heart rate; beats/min.) and rating of perceived exertion(RPE) were measured when working at MAF. The MAFs of elbow, wrist, and index finger motions for each force level were about 2, 3, and 6 times the corresponding MAF(9 at the high force and 24 at the low force) of shoulder motion and the MAFs at the low force increased about 2 times those at the high force. The work pulses of elbow, wrist, and index finger motions for each force level were 70%, 50%, and 30% of the corresponding work pulse(17 at the high force and 12 at the low force) of shoulder motion and the work pulses at the low force were about 70 % of those at the high force. Lastly, the RPEs of the upper extremity regions were about level 3(moderate) or below.

Workload Evaluation of Various Shoulder Posture by using Muscle Force, Fatigue and Psychophysical Workload

  • Objective: The purpose of this study is to evaluate the potential risk of shoulder muscle at particular working postures in sitting. Background: The cause of shoulder pain needs to be specifically studied in relation with particular shoulder postures to prevent shoulder MSDs in workplace. Method: In this study MVC, fatigue and subjective workload were investigated depending on the change of shoulder posture. An experiment was designed to evaluate the six shoulder muscles at nine shoulder postures including the combination of 30(adduction), 0, 30(abduction) degrees and 60, 90, and 120 degrees of shoulder flexion. Surface electrodes were attached to the middle trapezius, inferior middle trapezius, anterior deltoid, posterior deltoid, serratus anterior and teres major. Thirteen subjects participated in the experiment. Dependent variables were RPE (rating of perceived exertion), MVC(maximum voluntary contraction) and MPF(mean power frequency) shift by EMG (electromyography). Results: The middle trapezius and inferior middle trapezius were not significantly fatigued at all postures. The decline of MPF slope was less than 10% at all postures. The anterior deltoid was significantly fatigued all postures. The decline of MPF slope was more than 10% at all postures. The posterior deltoid was significantly fatigued 30 degrees of adduction and 90 degrees of flexion. And, neutral and 30 degrees of abduction postures were fatigued more than 90 degrees of flexion. The serratus anterior was significantly fatigued except for 30 degrees of adduction and 60 degrees of flexion posture. The teres major was significantly fatigued except for neutral and 60 degrees of flexion, 30 degrees of abduction and 60 degrees of flexion posture. Conclusion: It was found that a certain muscle was fatigued fast at particular posture compared to other muscles, which would mean that a certain shoulder muscle at particular posture could be easily exposed to the risk of musculoskeletal disorders than other muscles. Application: It is expected that the result can be applied to design workplace using shoulder muscles.

Current Status and Prospects on Biofuel Conversion Technologies and Facilities, Using Lignocellulosic Biomass (목질계 바이오연료 생산을 위한 산업화 기술 및 전망)

  • This study investigated to understand the trend of international commercializing technologies and industrial status of the transportation biofuel based on lignocellulosic biomass. Two major commercializing technologies for the lignocellulosic biofuel are biochemical conversion technology and thermochemical conversion technology. It was reported that a total of 93 industrial companies were using lignocellulosic biomass of all facilities related to advanced biofuel. On the basis of commercial type, the biochemical conversion technology was identified to be the major technology in the lignocellulosic biofuel industries, showing 84% of all. Also the main products of commercial type industrial companies are bioethanol (1,155,000 tons/yr) and bio-oil (120,000 tons/yr), which are in a remarkably inadequate amount to substitute for the transportation biofuel worldwide. It was suggested that the transportation biofuel market was currently in need of further development in both technology and scale, and was in high demands of technological development and commercializing exertion.

The Effect of Sport Drink Ingestion on the Physical Fatigue Level and Heart Rate during Prolonged Exposure in Hot Ambient Temperature in Professional Golfers (장시간 고온 환경 노출시 수분의 섭취가 프로골프선수들의 신체피로도 및 심박수에 미치는 영향)

  • We investigated the effects of prolonged exposure in hot environmental condition and ingestion of fluid on various physiological variables including plasma glucose, lactate, the rating of perceived exertion (RPE), and heart rate as well as golf putting performance. Six male professional golfers were voluntarily participated in three different putting trials which were separated by seven days of time interval period. Three different putting trials were conducted at either 20℃ or 32℃, or 32℃ + Fluid ingestion. Performing 32℃ + Fluid ingestion trial, all subject ingested sport drink as much as their body mass was decreased. For each experiment, all subjects were undertaken total 48 putting, which separated by four x 12 putting in four different time points (i.e., Rest, 1 hr, 2 hr, and 3 hr). Plasma glucose concentration was significantly decreased with hot ambient condition but it was almost fully recovered by fluid ingestion. Plasma lactate concentration was significantly higher when subjects were exposed in hot environmental condition, and it did not change with fluid ingestion. There was a no different in putting performance and psychological fatigue level (performed by GRID test) at any environmental conditions. The RPE, commonly used for evaluating of physical fatigue level, was significantly dropped by fluid ingestion which indicates lower physical fatigue level. In addition to this, heart rate (HR) was also significantly decreased after fluid ingestion. Based on these results, it was concluded that the ingestion of fluid during prolonged exposure in hot ambient condition decrease the degree of physical fatigue levels and heart rate, which will possibly improve the golf performance when exposed in extreme weather condition in summer. Study on the Feasibility of High-Temperature Immersion Ultrasonic Sensor with a Strip Waveguide (도파띠를 이용한 고온 액침 초음파센서의 가능성 연구)

  • Exertion has been made to develop high-temperature (about $250{\sim}650^{\circ}C$) immersion ultrasonic sensor for the visualization of objects, temperature measurement, dimensional check, or nondestructive testing of welds under liquid sodium. In this study, the feasibility of the ultrasonic sensor taking advantage of a strip waveguide was confirmed by water-experiment. The lowest order of antisymmetric Lamb wave was used in the frequency range with negligible dispersion. This plate wave was excited in the stainless steel strip waveguide of 1.0mm thickness and 13mm width by the comb-structure transducer of 2.3MHz frequency. Its attenuation coefficient was 1.2dB/m in air and 380dB/m in water. The signal to noise ratio of 25dB was obtained from a planar reflector 270mm away from the beam aperture of $13mm{\times}39mm$ size.

The Result of Autologous Osteochondral Grafting for the Osteochondral Lesion of the Talus (거골의 골연골 병변에 대한 자가 골연골 이식술의 결과)

  • Purpose: The purpose of this study is to assess the results of the autologous osteochondral grafting for the osteochondral lesion of the talus. Materials and Methods: This study included 21 patient who had been treated with the open autologous osteochondral grafting between December, 1999 and December 2003. We treated the patients with autologous osteochondral grafting method who had the lesion of stage II or medial lesion of stage III without improvement for at least 6 months, the medial lesion of stage IV, the lateral lesion of stage III, IV by Berndt and Harty's classification or who had not improved with previous operation. The average duration of follow up was 26 (12-56) months. 6 patients had been treated with the previous operation that 3 patients with drilling, 3 patients excision, curettage and drilling. Results: The average AOFAS Ankle/Hindfoot score was 92.7 points and no patient had a limitation of the daily activities due to pain or abnormal function. Postoperative radiography showed good joint congruency and postoperative MRI good incorporation of the graft. The average Lysholm knee score was 96 points and only two patients had mild knee pain during severe exertion. Conclusion: The autologous osteochondral grafting is the useful operative method when the lesion is advanced stage or when previous operative treatment has failed.

A Case of Isolated Right Pulmonary Artery Agenesis (고립성 우측 폐동맥 형성부전증 1례)

  • Unilateral pulmonary artery agenesis is a rare congenital anomaly usually associated with other cardiovascular anomalies such as tetralogy of Fallot or septal defect. Unilateral pulmonary artery agenesis without other coexisting cardiovascular abnormality (isolated unilateral pulmonary artery) is extremely rare and often asymptomatic until adulthood. In these patients, diagnostic clue is found in a plain chest roentgenogram, showing a hyperlucent contracted hemithorax. We have recently experienced a case of isolated right pulmonary artery agenesis, which was diagnosed by chest dynamic CT, perfusion scan, echocardiogram and 3-dimensional reconstruction cardiac CT angiography in a 50-year old female who had suffered from mild dyspnea on exertion and improved with conservative treatment. We report this case with a brief review of the relevant literature.

Short-term Glycemic Control and the Related Factors in Association with Compliance in Diabetic Patients (당뇨병 환자의 치료순응도에 따른 단기간 혈당조절정도와 관련 요인)

  • Objectives : Generally, it seemed that the therapeutic result in diabetic patients was changed by compliance. This study was conducted on the basis of assumption that the therapeutic result id diabetic patients could control according to compliance. This study was conducted to analyze the related factors in association with compliance to drug, diet and exercise therapy. Methods : 224 diabetic patients in Kyungpook National University Hospital were selected through the interviews and HbA1c values from 1 Jan. to 28 Feb.1997. The drug compliance was tested by regularity of drug administration, the diet compliance was tested by restriction of food, exactly allocation, balance of nutrient, measuring food and the exercise compliance was tested by regularity of exercise per day. We assessed compliance by percentage, $x^2-test$ and generalized logit regression model(method:enter). Results : The significant variable was the satisfaction to medical personnels in drug, the knowledge to disease in diet, the participation of the diabetic education in exercise therapy and the satisfaction to medical personnels in HbA1c. Using the generalized logit model(method : enter) in compliance change, the significant variables were the satisfaction to medical personnels and the complication in drug; the significant variables were the age at the first diagnosis, the family history, the concern of health, the knowledge of disease, the self-exertion for therapy and the complication in diet: the only significant variable was the gender in exercise therapy. Conclusions : The degree of glycemic control in diabetic patients was influenced by compliance. In order to improve patient's compliance, we must foster the knowledge on the diseases, lead participation for diabetic education. Because the satisfaction to medical personnels was the important variables, we must build up good relationship between doctors and patients.
