The Isokinetic exercise and effects of combined exercises on the change of postural control, functional status in patients with osteoarthritis of the knee (등속성 운동과 복합 운동이 퇴행성 슬관절염 여성의 근기능, 균형능력, 기능적 능력 및 관절기능상태에 미치는 영향)

  • Objective: The purpose of this study was to investigate the effects of combined exercise on the change of postural control, functional status in patients with osteoarthritis of the knee. Methods : The subjects were consisted of 24 women patients fifties and sixties with knee osteoarthritis. Each group had a exercise for 30 minutes per day and three times a week during 8 weeks period. Was used to measure muscular function(60, $180^{\circ}/sec$), postural control. functional capacity(15 M walking test, Rising form a chair test, stair climbing and descending test) and the functional state of a joint(WOMAC). Results: This study results in following 1. In case of muscular function have significantly increased in both groups. 2. There was no significant difference in the postural control of the bilateral between two groups. However, the postural control of a isokinetic exercise program group showed a significant difference in the bilateral side(overall, $M{\cdot}L$ stability index). Especially, the change of postural control in the isokinetic plus postural control exercise program group decreased more significantly in the bilateral(overall, $A{\cdot}P$ stability index). 3 It was significantly decreased in climbing and descending the stairs, and rising from the chair, although functional capacity decreased in lam walking in both groups. 4. The functional state of a joint(WOMAC) showed a significant decrease in both groups. Conclusion : In order to improve the functional status and postural control of patients with osteoarthritis of the knee, performing exercise programming of isokinetic plus balance will be more effective than the exercise program composed of only the isokinetic exercise program.

The Effects of the Stretching and Recreation Exercise Including Health Education on Physical Fitness and Emotion of Women in Elderly (건강교육을 포함한 신장운동 및 레크레이션 운동이 여성노인의 체력 및 정서에 미치는 효과)

  • The purpose of this study was to examine the effects of the stretching and recreation exercise including health education on physical fitness and emotion of women in elderly. This study has been done between February and April, 2004 and the subjects of the study were 28 women in elderly. Each of stretching and recreation exercise was carried out for 30 minutes a day, 5 times a week for 8 weeks and health education was carried out for 30 minutes a day, 2 times a week for 8 weeks. Vital capacity, flexibility, balance and grip strength were measured using the Health Management System developed by the Korea Physical Science Institution. The scores of depression, anxiety and hostility were measured by Korean Manual of Symptoms-Checklist -90 revision. The collected data were processed using the SPSS PC program and analyzed using Wilcoxon Signed Ranks Test The result of this study are as follows : 1. Flexibility was increased after 8 weeks more than before stretching and recreation exercise including health education were done. 2. Vital capacity, balance and grip strength were not increased after 8 weeks more than before stretching and recreation exercise including health education were done. 3. The score of anxiety, depression and hostility was decreased after 8 weeks less than before stretching and recreation exercise including health education was done. In conclusion, stretching and recreation exercise including health education promoted the flexibility and decreased anxiety, depression and hostility of women in elderly.

Gynostemma pentaphyllum extract and its active component gypenoside L improve the exercise performance of treadmill-trained mice

  • BACKGROUND/OBJECTIVES: The effectiveness of natural compounds in improving athletic ability has attracted attention in both sports and research. Gynostemma pentaphyllum (Thunb.) leaves are used to make traditional herbal medicines in Asia. The active components of G. pentaphyllum, dammarane saponins, or gypenosides, possess a range of biological activities. On the other hand, the anti-fatigue effects from G. pentaphyllum extract (GPE) and its effective compound, gypenoside L (GL), remain to be determined. MATERIALS/METHODS: This study examined the effects of GPE on fatigue and exercise performance in ICR mice. GPE was administered orally to mice for 6 weeks, with or without treadmill training. The biochemical analysis in serum, glycogen content, mRNA, and protein expressions of the liver and muscle were analyzed. RESULTS: The ExGPE (exercise with 300 mg/kg body weight/day of GPE) mice decreased the fat mass percentage significantly compared to the ExC mice, while the ExGPE showed the greatest lean mass percentage compared to the ExC group. The administration of GPE improved the exercise endurance and capacity in treadmill-trained mice, increased glucose and triglycerides, and decreased the serum creatine kinase and lactate levels after intensive exercise. The muscle glycogen levels were higher in the ExGPE group than the ExC group. GPE increased the level of mitochondrial biogenesis by enhancing the phosphorylation of peroxisome proliferator-activated receptor-γ coactivator-1α (PGC-1α) protein and the mRNA expression of nuclear respiratory factor 1, mitochondrial DNA, peroxisome proliferator-activated receptor-δ, superoxide dismutase 2, and by decreasing the lactate dehydrogenase B level in the soleus muscle (SOL). GPE also improved PGC-1α activation in the SOL significantly through AMPK/p38 phosphorylation. CONCLUSIONS: These results showed that GPE supplementation enhances exercise performance and has anti-fatigue activity. In addition, the underlying molecular mechanism was elucidated. Therefore, GPE is a promising candidate for developing functional foods and enhancing the exercise capacity and anti-fatigue activity.

Effects of a High Protein Diet and Aerobic Exercise on Body Weight Changes and Blood Lipids in Slightly Overweight Women

  • To examine the combined effects of a high-protein diet and aerobic exercise on body weight and composition and blood lipid profiles in overweight women, 30 young women were recruited and placed into three groups: The high-protein diet and exercise group (HPE), the exercise-only group (EXO) and the control group (CON) (30$\pm$3%, 27$\pm$2%, and 29$\pm$3% body fat, respectively) for an 8-week experimental period. Daily diet included 25% isolated soybean protein (>90% protein, approximately 400 kcal) combined with each subject s usual diet for the HPE group. The exercise program consisted of aerobic-type exercises undertaken >3 times/wk and for>30 min/session at 50-60% of maximal capacity. Physical fitness, body composition, serum total cholesterol (TC), high-density lipoprotein-cholesterol (HDL-C), triglycerides (TG) and glucose were measured before and after the experiment. Maximal aerobic capacity increased by the end of experiment in both the HPE (from 27.2$\pm$3.5 to 35.l$\pm$5.9 ml/kg/min, p<0.01) and EXO (from 30.3$\pm$5.4 to 33.8$\pm$3.8 mㅣ/kg/min, p<0.05) groups. Percent body fat decreased by 3.3% (p<0.01) in the HPE group and by 1.5% (p<0.05) in the EXO group by the end of the experiment, but not in the CON group. Lower back strength and agility increased only in the HPE group. In the HPE group, TC decreased from 168$\pm$20 to 155$\pm$18 mg/dL and HDL-C increased from 57$\pm$l0 to 61$\pm$9 mg/dL in HPE (p<0.01). But TC and HDL-C did not change in the EXO and CON groups. TG and glucose did not vary among the groups. Although the EXO group showed a similar outcome to that of the HPE group, a favorable change in body composition and blood lipids as well as an improvement in aerobic capacity was more marginal in the latter group.

The Relationship between Exercise, Bone Mineral Density and Antioxidant Enzyme Activity of Postmenopausal Women

  • This study was carried out to elucidate the relationship among exercise, bone mineral density and antioxidant enzyme activity of postmenopausal women. 60 women residing in the Iksan, Korea area were recruited. The questionnaires were designed to find out exercise habits. Bone mineral density (BMD) was measured by dual energy X-ray absorptiometry. Parameters of antioxidative capacity, including the activities of superoxide dismutase (SOD), glutathione peroxidase (GPx), catalase (CAT) and total antioxidant capacity (TA) were analyzed in fasting blood. The mean age, height, weight, and BMI of subjects were 65.0 years, 151.1 cm, 59.5 kg $26.0\;m/kg^2$, respectively. The mean BMDs of subjects were $0.85\;g/cm^2$ (lumbar spine), $0.6\;g/cm^2$ (Femoral neck), $0.49\;g/cm^2$ (trochanter), and $0.40\;g/cm^2$ (Ward's triangle). There was a significant difference in BMD among different age groups (50's, 60's and 70's) showing lower value with increasing age (p<0.05). The mean SOD, GPx, and CAT activities were 138.5 U/mL, 1,273.8 U/mL and 314.3 kU/L respectively, and TA was 1.16 mmol/L. TA of the group which exercised 3$\sim$4 times a week was significantly higher than those of the other exercise groups (p<0.05). The subjects with higher SOD activity also have a higher the T values in the lumbar spine, femoral neck, trochanter, and Ward's triangle. In conclusion, this study revealed that the levels of antioxidant enzyme activity were closely associated with the exercise status and bone mineral density in postmenopausal women.

The Biofeedback Scapular Stabilization Exercise in Stroke Patients Effect of Muscle Activity and Function of the Upper Extremity

  • Purpose: This study attempts to understand the effect of stabilization exercise of biofeedback scapular on muscle activity and functional evaluation of the upper extremity in stroke patients. Methods: Patients were divided into two groups; a biofeedback scapular stabilization exercise group comprised of 8 patients and a task-oriented training group including another 8 patients, and 30-minute exercise was performed 5 times a week for 8 weeks. Electromyogram was used to measure muscular activity of lower trapezius, deltoid middle, and serratus anterior. Fugl-Meyer Assessment and Manual Function Test were used to evaluate functions of the muscles mentioned. Results: Significant difference was observed in the comparison group before and after exercise in muscular activity of lower trapezius, deltoid middle, and serratus anterior, Fugl-Meyer Assessment, and Manual Function Test. Conclusion: Therefore, we could see that biofeedback scapular stabilization exercise is more effective than task-oriented training in facilitating muscle activation and functional capacity of upper limb.

  • Patients with chronic obstructive pulmonary disease receive a range of treatments including but not limited to inhaled bronchodilators, inhaled and systemic corticosteroids, supplemental oxygen, and pulmonary rehabilitation. Pulmonary rehabilitation is a multidisciplinary intervention that seeks to combine patient education, exercise, and lifestyle changes into a comprehensive program. Programs 6 to 8 weeks in length have been shown to improve health, reduce dyspnea, increase exercise capacity, improve psychological well-being, and reduce healthcare utilization and hospitalization. Although the use of pulmonary rehabilitation is widely supported by the literature, controversy still exists regarding what should be included in the programs. The goal of this review was to summarize the evidence for pulmonary rehabilitation and identify the areas that hold promise in improving its utilization and effectiveness.

A Behavior-Modification Approaches to Improved Exercise Performance for Athletes Through the Multipe Nutritional Counseling (영양상담을 통한 식사행동의 변화가 운동선수의 운동능력 향상에 미치는 효과)

  • Management of nutrient intakes through behavior-mordification can be important for improving exercise performance in athletes. The purpose of this study was to determine the effectiveness of nutritional counseling for improving exercise performance capability in athletes. The subjects were seven golfers and fourteen ju-do athletes from Hoseo university, and they have nor received professional hel[ from dieticians. Prior to nutritional counseling, their dietary intakes, lifestyles and dietary habits were evaluated. Nutritional counseling sessions were conducted by a trained dietician every 2-3 weeks for 3-4 month. After 5 counseling sessions, nutrient intakes, lifestyle and dietary habits were re-evaluated. Exercise performance capability was measured by maximal exercise stress test using treadmills. Individual data were assessed as quality index represents the better nutritional status. The common dietary and lifestyle problems were overeating binging, overeating, snacking and drinking alcohol prior to the nutritional counseling. After counseling sessions, the quality index of dietary habits seemed to decrease, especially in ju-do athletes, representing nutritional problems were resolved. Body fat and body mass index decreased in ju-do athletes, but not in golfers. Both athletes have shown to increase oxygen uptake at anaerobic threshold and maximal oxygen uptake status, which represents that the athletic performance capacity was improved after counseling sessions. Their respiratory quotients were decreased. In conclusion, nutritional counseling sessions over five times are an effective and efficient approach to change dietary habits to improve exercise performance capacity. Furthermore, clients can have good dietary habits and learn how to manago muscle strength by behavior modification through multiple nutritional counseling sessions. The Effect of the Core Stabilization Exercise and Thoracic Manipulation on the Respiratory Function of Chronic Low Back Pain Patients (척추 안정화 운동과 등뼈 가동성 운동이 만성 허리 통증 환자의 호흡 기능에 미치는 영향)

  • Purpose : The aim of this study was to investigate the change of the respiratory function of chronic low back pain patients after the thoracic manipulation and the trunk stability exercise on chronic low back pain patients. Methods : For this study, the group of the experiment was consisted of 44 patients suffering from the low back pain chronically. The participants were divided into two groups: the control group, which was assigned for core stabilization exercise (CSE), and the experimental group, which was randomly assigned for core stabilization exercise after thoracic manipulation (CSE+TM). Both groups carried out each assigned treatment on three times a week for 8 weeks. To study the change related to measurement variable from each test groups before and after intervention, paired t-test was performed. Further, the statistics for an intergroup comparison was analyzed by covariance analysis, ANCOVA. The measurement was conducted by the respiratory function, the respiratory function was measured by forced vital capacity (FVC) and forced expiratory volume in one second (FEV1). Results : As a result, forced vital capacity (FVC) and forced expiratory volume in one second (FEV1) showed significant differences not only by each groups but also between those groups before and after the test. Conclusion : In conclusion, both experiments, the core stabilization exercise and the core stabilization exercise conducted after thoracic manipulation, on chronic low back pain patients resulted in the significant level of difference in the respiratory function. This result indicates that the thoracic manipulation is an efficient treatment for improving the respiratory function for chronic low back pain patients.

The Influence of Maximal Aerobic Capacity on the Two Years Cardiac Related Re-Hospitalization in Patients with Heart Failure with Reduced Ejection Fraction in Korean Society

  • Purpose: This study examined the influence of the maximal aerobic capacity on the two-year cardiac-related re-hospitalization in patients with heart failure with a reduced ejection fraction (HFrEF) in Korean society. Methods: The maximal aerobic capacity of the study population (n=95, male 63%) was evaluated using a cardiopulmonary exercise (CPX) testing system. Each patient was followed up for two years to divide the HFrEF patients into two groups according to cardiac-related re-hospitalization: re-hospitalization (RH) group (n=29, 30%) and no re-hospitalization (NRH) group (n=66, 70%). Results: The relative peak $VO_2$ (mL/kg/min, p<0.001), exercise duration (p<0.001), respiratory exchange ratio ($VCO_2/VO_2$, p=0.001), systolic blood pressure (SBP) reserve (p=0.004), heart rate (HR) reserve (p=0.007), SBP max (p=0.02), and HR max (p=0.039) were significantly lower in the RH group than the NRH group during the CPX test. On the other hand, the ventilatory efficiency (VE/VCO2 slope, p=0.02) and age (p=0.022) were significantly higher in the RH group than in the NRH group. In binary logistic regression analysis, the relative peak $VO_2$ (p=0.001, Wald Chi-square 10.137) was the strongest predictive factor on cardiac-related re-hospitalization, which was followed by $VCO_2/VO_2$ (p=0.019, Wald Chi-square 5.54). On the other hand, age (p=0.063, Wald Chi-square 3.445) did not have a significant influence on cardiac related re-hospitalization. Conclusion: The maximal aerobic capacity, especially the relative peak $VO_2$, is the strongest factor on cardiac-related re-hospitalization within two years in patients with HFrEF in Korean society.
