Purpose: This study was conducted to investigate the effects of skater exercise on cardiopulmonary fitness and balance function in patients with mild chronic stroke. Methods: A total of ten chronic stroke patients with mild neurologic deficits were recruited. The participants were divided into two groups, a skater exercise (n=5) and a control group (n=5). Skater exercise was performed by physical therapists in the experimental group, whereas patients in the control group conducted self-exercise. One session of the intervention was carried out for 30 minutes, three times per week for eight weeks. Cardiopulmonary function, falling index, and Euro-Quality of Life-5 Dimension (EQ-5D) were measured before and after the intervention. Results: No side effects were reported during and after skater exercise intervention. The peak aerobic capacity, falling index, and EQ-5D improved significantly in the experimental group compared with the control group after eight weeks of skater exercise (p=0.002, p=0.010, and p=0.006). Significant relationships were identified between the change in EQ-5D and peak aerobic capacity (R2=0.75, p=0.002)/falling index (R2=0.65, p=0.002). Conclusion: Skater exercise was an effective physical therapy to improve cardiopulmonary fitness and balance function in patients with mild chronic stroke.
Purpose: This study aims to analyze the outcome of exercise programs for Rheumatoid Arthritis patients and its tendency with network analysis. Method: 30 articles from Medline search of foreign nursing journals(1966-2002) and 5 from three Korean nursing journals(1970-2002) were surveyed. The contents analyses were focused on outcome measures and network analysis of exercise programs. Results: Exercise programs result in improving muscle strength(91.7%), aerobic capacity(72.9%), and 50 feet walk time(57.3%). Among several exercises, water exercise, and Tai-chi are in the middle of weight-bearing exercises. In the experimental design, non-randomized control trials were 40%, and the functional status of subjects were not classified in 40% of the articles surveyed. Conclusion: Considering the results, muscle strength, aerobic capacity, and 50 feet walk time were tested as valid outcome measures. Tai-chi exercise could be recommended as proper exercise for Rheumatoid arthritis patients because of its low intensity of weight-bearing exercise. In terms of research methods, the randomized trials and functional classifications of rheumatoid arthritis should be done more strictly.
본 연구는 유산소운동능력의 차이를 보이는 두 집단이 각각 로윙에르고미터 최대하 운동을 실시한 후 혈중 활성산소의 변화에 어떠한 차이를 나타내는지 구명하는데 목적이 있으며 연구대상으로는 조정 선수집단 6명과 비선수집단 6명을 대상으로 최대유산소운동능력을 검사한 후 각각의 목표심박수 85~90%를 적용하여 로윙에르고미터 운동을 실시하였고 운동 전, 직후, 10분, 20분, 차30분에 각각 채혈하여 활성산소의 변화를 분석하였다. 그 결과는 유산소운동능력의 차이에 따른 활성산소의 변화에서 집단과 시기 간 상호작용 효과는 유의한(p<.05) 차이를 나타내어 시기에 따른 변화의 양상이 집단 간 차이가 있었고 사후검증 결과 회복기 10분에서 20분 그리고 20분에서 30분 구간에서 유의한 차이가 나타났다. 결론적으로 인체에 부정적인 영향을 미치는 활성산소로부터 유산소운동능력이 우수한 조정선수집단이 비선수집단에 비해 긍정적인 회복 패턴을 보였다.
This study was directed to further clarify the effect of resting time or dietary condition for the improvement of exercise capacity and physical fitness, and the changes of some body consitiuents and physiological functions which are related to the exercise metabolism. Sixteen male students(8 athletes and 8 nonathletes)were participated during 3 weeks(Aug. 20-Sep.9, 1989). Each subject performed two treadmill running trials at an absolute intensity (1 mintute in 3.4mph/15% slope and 2 minutes in 5.5mph/20% slope). In the resting time trials, general diet was fed before 1, 2, 4 and 12 hours of the treadmill exercise loaded. in the dietary condition trials, high carbohydrate(HC), high fat(HF) and high protein(HP) diet were fed before 2 hours of the treadmill exercise loaded. Control trial was that of resting time before treadmill exercise loaded after 12 hours of general diet feeding. Measurement were made to study the change of blood glucose, palmitate, lactate, blood pressure and heart rate. The results obtained are summarized as follows: Mean daily metabolizable period. In resting time trial, the blood glucose concentration of athlete and nonathlete from 2 hours group was similar to control group. The blood palmitate concentration of athlete was increased in 1 and 2 hours group but those of nonathlete was not only increased in 1 and2 hours group but was more increased in 12 hours group, compared with both control group. The blood lactate concentration was increased in all experimental group, compared with both control group and those of nonathlete was much higher than athlets. The elevation rate of blood pressure in pre-and after-exercise of athlete was lower than those of nonathlete. In dietary composition trial, the blood glucose concentration of athlete and nonathlete in HC group was higher than other diet groups. The blood lactate concentration of athlete and nonathlete in HC group was lower than other diet groups. There was no remarkable change of the blood palmitate concentration and heart rate in each dietary composition trial, but those of nonathlete was low in HC and high in HP group. In above results, it was suggested that the effective condition of resting time and dietary composition for the improvement of exercise capacity of nonathlete may be 2 hours and HC diet, respectively. But it was showed that the exercise capacity of athlete may not be affected by experimental condition of resting time, except 1 hour after feeding or of dietary composition because of well adaptation in new exercise condition.
Purpose : The Purpose of this study was on determine whether thoracic expension exercise might increase the pulmonary function of the patients with stroke. Methods : Fourty paients with stroke were randomly assigned to experimental(n=20) and control group(n=20). During four weeks, each group participated thirty minutes for five times per week. Subjects were assessed using pre-value and post-value measurement pulmonary function(Forced vital capacity, Forced expiratory volume at one second, FEV1/FVC, Peak expiratory flow, Tidal volume, vital capacity, Inspiratory capacity, Expiratory reserve volume, Inspiratory reserve volume). Results : These finding suggest that experimental group was significant increase in FVC, FEV1, PEF, TV, IC, IRV, ERV($p$<.05). In comparison of two group, experimental group was high pulmonary function than control group. Conclusion : This study showed experimental group can be used to improve pulmonary function than control group. Thus it indicates that the thoracic expension exercise will be more improved through the continued respiratory exercise program.

Background and Objectives: Pathophysiological changes of right ventricle (RV) after repair of tetralogy of Fallot (TOF) are coupled with a highly compliant low-pressure pulmonary artery (PA) system. This study aimed to determine whether pulmonary vascular function was associated with RV parameters and exercise capacity, and its impact on RV remodeling after pulmonary valve replacement. Methods: In a total of 48 patients over 18 years of age with repaired TOF, pulmonary arterial elastance (Ea), RV volume data, and RV-PA coupling ratio were calculated and analyzed in relation to exercise capacity. Results: Patients with a low Ea showed a more severe pulmonary regurgitation volume index, greater RV end-diastolic volume index, and greater effective RV stroke volume (p=0.039, p=0.013, and p=0.011, respectively). Patients with a high Ea had lower exercise capacity than those with a low Ea (peak oxygen consumption [peak VO2] rate: 25.8±7.7 vs. 34.3±5.5 mL/kg/min, respectively, p=0.003), while peak VO2 was inversely correlated with Ea and mean PA pressure (p=0.004 and p=0.004, respectively). In the univariate analysis, a higher preoperative RV end-diastolic volume index and RV end-systolic volume index, left ventricular end-systolic volume index, and higher RV-PA coupling ratio were risk factors for suboptimal outcomes. Preoperative RV volume and RV-PA coupling ratio reflecting the adaptive PA system response are important factors in optimal postoperative results. Conclusions: We found that PA vascular dysfunction, presenting as elevated Ea in TOF, may contribute to exercise intolerance. However, Ea was inversely correlated with pulmonary regurgitation (PR) severity, which may prevent PR, RV dilatation, and left ventricular dilatation in the absence of significant pulmonary stenosis.
The purpose of this study was to identify the differences in pulmonary functioning after respiratory exercise with IPPB (Intermittent Positive Pressure Breather) in patients with progressive muscular dystrophy (PMD). The subjects were 46 patients with PMD who were admitted to the Rehabilitation Medicine Department of Youngdong Severance Hospital. The subjects were assigned into one of 2 groups. The control group received comprehensive treatments such as ROM exercise, deep breathing exercise, moist hot packs, and ultrasound twice a day while admitted at the Rehabilitation Medicine Department. Unlike the control group, the subjects at the experimental group received respiratory exercise treatment with IPPB. The subjects were admitted for 10~19 days, and the average length of hospital stay was 12.2 days. Pulmonary functioning was evaluated at admission and discharge by SENSOR MEDICS. The data were analyzed by a paired t-test and a independent t-test. The results were as follows: 1) The change of each parameter of pulmonary function tests were significantly improved in all groups after respiratory exercise treatment during admission (p<0.05). 2) By comparing the change of each parameter of pulmonary function tests between the experimental group and control group, the parameters of vital capacity (VC), forced vital capacity, forced vital capacity predicted (FVCP) and forced expiratory volume in 1 second (FEV1) were significantly improved in the experimental group which had received the pulmonary exercise treatment with IPPB (p<0.05). In conclusion, this study suggests that the pulmonary exercise treatments with IPPB facilitated improvement in the pulmonary functioning for the PMD patients during their hospital stay.
Background: This study was to determine whether the diaphragmatic breathing exercise using a DiP Belt(Diaphragmatic Pressure Belt) is effective in increasing the diaphragmatic motion and forced vital capacity. Design: Pretest-Posttest design. Methods: A total of 44 subjects(15 male, 29 female) participated in this study. All subjects were measured the diaphragmatic motion with a sonography and the Forced Vital Capacity(FVC) was measured with a digital spirometer. After 4 weeks, the subjects were intervened the diaphragmatic breathing exercise using a DiP belt and were remeasured for diaphragm motion and FVC. Results: After exercise intervention, quiet breathing significantly increased with the change in diaphragmatic motion and showed a moderate effect size (p<.01, Cohen's d = -0.53). In addition, it was significantly increased in deep breathing and showed a high effect size (p<.001, Cohen's d = -1.32). The mean diaphragmatic contraction pressure increased, but there was no significant difference and the peak diaphragmatic contraction pressure increased significantly (p<.05). Both diaphragmatic contraction pressure showed small effect sizes (respectively Cohen's d = -0.28, -0.33). In spirometry, FVC, Forced Expiratory Volume in 1 second (FEV1), and FEV1/FVC% all increased, but there was no significant difference. Only peak expiratory flow increased significantly and showed a small effect size (p<.05, Cohen's d = -0.41). Conclusion: The DiP belt diaphragmatic breathing exercise that the principle of visual feedback can correct diaphragm breathing in a short time, so it is a useful breathing exercise device that can help the diaphragm breathing exercise in the right way in clinical practice.
The purpose of this thesis is to investigate whether taurine supplementation in combination with fructose improves both energy metabolism and exercise capacity. Eight collegiate female subjects were recruited for the study. Each subject went through threecross-over designs: control(fluid), fructose, and taurine plus fructose supplementation trials. Subjects received taurine supplementation 100 mg/kg a day for two weeks. After the supplementation, all subjects take 10% fructose at 15 min prior to exercise, immediately before exercise, and every 15 min during exercise. Subjects received 150 ml fluid as placebo during the same procedure. The subjects performed submaximal exercise at the exercise intensity of 60% for 45 min and then 80% of maximal oxygen uptake (VO2max) until exhaustion time. A 10ml blood sample was taken for measuring the level of glucose, ammonia, lactate, free fatty acids, and insulin every 15 min during exercise at 60% of VO2max. The blood glucose levels was significantly higher at 45 min and 50 min exercise after supplementation of fructose, and immediately before exercise and 50 min exercise after taurine plus fructose compared to the placebo trial. However, the values tended to be lower in taurine plus fructose supplementation compared to the fructose trial. The levels of both lactate and ammonia were significantly lower compared to the placebo, while the exhaustion time was significantly increased. The level of free-fatty acids was significantly lower at 30, 45, and 50 min after fructoseand fructose plus taurine supplementation compared to the placebo trial. The level of glucagon was significantly lower at 15, 30, 45, and 50 min after fructose and fructose plus taurine supplementation compared to the placebo trial. There was no differences in insulin concentration among three treatments. This thesis concludes that combined taurine and fructose supplementation prior to exercise may improve exercise tolerance time and energy metabolism, lowering the muscle fatigue factors such as lactate and ammonia.
Purpose : The purpose of this study is to investigate the effect of squat, bracing and aerobic exercise on lung function, which is known to be effective for strength training, on lung function. Methods : The study was conducted with 33 students from Busan K university. Eleven students were assigned to squats, bracing, and aerobic exercise, six weeks three times a week. In order to measure lung activity, pony Fx manufactured the change amount of FVC (forced vital capacity), FEV1 (Forced expiratory volume at one second), and FEV1/FVC % (forced vital capacity/forced expiratory volume at one second) was analyzed after inputting the information of experimental group A and B controls. As a method of measurement, the difference between the three groups was analyzed using repeated ANOVA. Results : As a result of analyzing the effects of squat, bracing, and aerobic exercise for 6 weeks, all values of FVC, FEV1, FEV1/FVC % were increased from 0 weeks to 6 weeks except FEV1/FVC %. There was no significant difference in FVC from week 3 to week 6. In the squat, bracing, and aerobic exercise, the changes in spirometry showed that the FVC, FEV1, and FEV1/FVC % values in bracing exercise were significantly increased with time than before exercise. As a result of analyzing the changes in the spirometry of squat, bracing, and aerobic exercise, the FVC, FEV1, FEV1/FVC % values in the squat exercise showed statistically significant difference according to the period, but the lowest increase among the three groups. Conclusion : In conclusion, aerobic, bracing and squat exercises all had a significant impact on improving lung function. Therefore, even without aerobic exercise, squat or bracing exercise alone can be expected to improve lung function.
