A Rupture of Proximal Ureter Caused by Renal Calculi in a Dog (개의 신장결석에 의한 근위요관파열의 1예)

  • A 3-year old female Poodle dog was presented with a history of anorexia, vomiting, abdominal pain and depression for 2 days. After physical and blood examination, excretory urogram(EU) and ultrasonography of the abjomen were conducted. On radiographs and ultrasonographs, dilation of diverticulum, sinus, proximal ureter of right kidney and mild dilation of left renal medullar were found. And there was some leakage of contrast agent in the proximal ureter area of right kidney. So this was diagnosed as a rupture of right proximal ureter. This dog was undertaken a surgery of nephrectomy of right kidney. On surgery, dilation of right kidney and hemorrhage, adhesion were found at the proximal ureter and some calculi were also found in the pelvis of right kidney. There were no complications after surgery.

Radiographic Estimation of Renal Dimension in Korean Domestic Cats (한국 재래 고양이 신장크기에 대한 방사선적 평가)

  • Renal length, width, and ratio to the length of the second lumbar vertebrae body were assessed on survey radiographs and excretory urograms in 50 Korean domestic cats. Renal dimensions on plain radiographs were: right kidney length $4.31{\pm}0.50cm\;(mean{\pm}SD)$, right kidney width $3.10{\pm}0.36cm$, left kidney length $4.17{\pm}0.48cm$, left kidney width $3.03{\pm}0.34cm$, right kidney length ratio $2.37{\pm}0.21$, right kidney width ratio $1.71{\pm}0.15$, left kidney length ratio $2.30{\pm}0.20$, and left kidney width ratio $1.38{\pm}0.08$, respectively. Renal dimensions on excretory urograms were: right kidney length $4.56{\pm}0.53cm$, right kidney width $3.29{\pm}0.37cm$, left kidney length $4.43{\pm}0.47cm$, left kidney width $3.22{\pm}0.33cm$, right kidney length ratio $2.51{\pm}0.22$, right kidney width ratio $1.81{\pm}0.17$, left kidney length ratio $2.45{\pm}0.20$, and left kidney width ratio $1.78{\pm}0.16$, respectively. There were significant difference in renal size and renal dimension ratio measured from contrast studies compared with survey radiographs (P<0.05). There were also significant difference in renal size and renal length ratio between right and left kidneys (p<0.05).

  • Polyps of the male urethra are relatively uncommon lesions. They usually arise from the prostatic urethra mostly verumontanum or Just lateral to the verumontanum. Posterior urethral polyp often presents with hematuria, hemospermia and sometimes lower urinary tract obstruction. We recently experienced a pedunculated prostatic urethral polyp in a 63-year-old man who complained of gross total hematuria and difficult micturition for several years. On rectal examination the prostate was slightly enlarged with normal consistency. Excretory urogram showed a round filling defect in the right side of the bladder neck measuring about $2{\times}2cm.$ in size. Cystourethroscopy revealed trabeculation of the vesical wall, mild bilateral prostatic hypertrophy and a round cystic mass with a long stalk arising from the prostatic floor 1 cm. Proximal to the verumontanum which caused ball-valve obstruction in the bladder neck. This pedunculated polypoid mass was then resected at its base with resectoscope and was removed transurethrally using Lowsley's grasping forceps. The specimen was proved as fibrous polyp histologically.

Retroperitoneal Abscess Caused by Staphylococcus intermedius in a Dog (개에서 Staphylococcus intermedius에 의한 복막후농양 발생례)

  • Two year-old female miniature schnauzer was presented with depression, abdominal pain and inappetence. Physical examination revealed tachypnea, fever, and abdominal pain. Leukocytosis with neutrophilia was shown in complete blood count. Radiograph revealed the increase of opacity in parallel to vertebra and dislocation of descending colon. Calculi and sludge in bladder, hydronephrosis and dilation of proximal ureter in right kidney were observed in ultrasonographic examination; also, irregular shape of structure in retroperitoneal cavity was observed. Urinalysis showed protein urine and hemtauria. Many epithelial cells, spindle cells, calcium oxalate dihydrate crystals, struvite crystals, amorphous crystals and granular casts, WBC, cocci were examined in urine sediment test. ERD-kit test result was highly positive. Based on those test and excretory urogram pyelonephritis and complete unilateral obstruction of ureter were confirmed. A balloon type structure in retroperitoneal cavity was found through the exploratory laparotomy. In cytologic examination of aspirated fluid from dilated area many degenerated neutrophils and phagocytized cocci were observed. Staphylococcus intermedius was cultured from aspirated fluid. Based on these clinical results retroperitoneal abscesses caused by Staphylococcus intermedius was diagnosed with ureter obstruction and concurrent cystic calculi. Nephrectomy of right kidney with ureter was performed.

Intrapelvic Urethral Anastomosis in a Dog with Complete Obstruction of Proximal Membranous Urethra (막성요도 근위부 완전폐쇄를 지닌 개에서 골반내 요도문합술)

  • A 4.65 kg 13-month-old male Pekingese dog was referred to veterinary teaching hospital of Konkuk University for evaluation of dysuria. On physical examination, severe distention of urinary bladder was found in abdominal palpation. Urinary catheter could not be guided into urinary bladder. On serum biochemistry, blood urea nitrogen (35.6 mg/dl) and creatinine (1.9 mg/dl) were increased. Obstruction part of proximal membranous urethra was founded on urethrogram. The length (13 mm) of obstruction part was callipered by cystourethrogram and urethrogram on operation. Surgical resection of obstruction part of urethra was performed without pubic osteotomy, and anastomosis was performed with 5-0 polyglycolic acid. Omentum was placed around the urethral anastomosis. On first day after surgery, appetite was good. On day 5, complete blood count and serum biochemistry showed normal range but mild urinary incontinence was showed after removing catheter. On day 7, urinalysis showed normal condition. On day 14, no leakage of surgical site was observed in excretory urogram. On day 21, no more urinary incontinence and good micturition were found. On 1 year later, the patient showed healthy condition without recurrence.
