Development of a Tele-Rehabilitation System for Outcome Evaluation of Physical Therapy

  • This paper presents a portable tele-assessment system designed for remote evaluation of the hypertonic elbow joint of neurologically impaired patients. A patient's upper limb was securely strapped to a portable limb-stretching device which is connected through Internet to a portable haptic device by which a clinician remotely moved the patient's elbow joint and felt the resistance from the patient. Elbow flexion angle and joint torques were measured from both master and slave devices and bilaterally fed back to their counterparts. In order to overcome problems associated with the network latency, two different tele-operation schemes were proposed depending on relative speed of tasks compared to the amount of time delay. For slow movement tasks, the bilateral tele-operation was achieved in real-time by designing control architectures after causality analysis. For fast movement tasks, we used a semi-real-time tele-operation scheme which provided the clinicians with stable and transparent feeling. The tele-assessment system was verified experimentally on patients with stroke. The devices were made portable and low cost, which makes it potentially more accessible to patients in remote areas.

Correlation between BMI and Physical Fitness of College Women in Seoul

  • This study investigated the correlation of the physical measurements and the basic physical fitness of 158 female students in the city of Seoul. The average age of the subjects was 22.0 ${\pm}$ 0.13 years old, the average height was 160.9 ${\pm}$ 0.7cm and the average weight was 53.4 ${\pm}$ 0.6kg. The mean BMI (Body Mass Index) was 20.7 ${\pm}$ 0.2kg/㎡. The average muscle mass was 36.5 ${\pm}$ 0.3 and the average body fat percentage was 28.0 ${\pm}$ 0.4%. The basic fitness levels of the subjects were measured based on the evaluation chart of the Korea Health and Science Research Institution (1994). The subjects were divided into 3 groups based on BMI(Group I : BMI < 20, Group II : 20 $\leq$ BMI < 25, Group III BMI $\geq$ 25) and the correlation between BMI and physical fitness was assessed. 1) The muscle strength of the subjects was measured by their grip strength, among other tests. Groups 1 and 2 were evaluated as “su” (level 1) and group 3 was “woo” (level 2). 2) Endurance 1 was tested by push-ups, and while groups 1 and 2 were evaluated as “su” (level 1), group 3 was “ga” (level 5). This result did not suggest any significant relevance among the subjects. 3) Endurance 2 was tested by sit-ups : groups 1 and 2 were evaluated as “mi” (level 3) and group 3 as “yang” (level 4). Group 2 and group 3 showed a significant difference. 4) Instant power was tested by standing high jumps, and although there was a significant difference between group 1 and group 3, all of the subjects were evaluated as “ga” (level 5). 5) Flexibility was measured by how far the subjects could bend forward. There was no significant relevance between the groups and they were all respectively evaluated as “woo” (level 2). 6) Agility was tested with side-steps and all the subjects showed poor agility as “yang” (level 4). 7) Heart and lung endurance was tested by the step test, calculating the maximum oxygen intake with the Physical Energy Index (PEI) and using the numbers according to the evaluation chart. Group 1 was evaluated as “mi” (level 3) and groups 2 and 3 were “woo” (level 2). From these results, we could see the group III (obese group) had a tendency of lower levels in all the variables related to body fitness They showed significantly lower endurance assessed with time for sit-ups and instant power by the standing high jumps. 복근 지구력 평가 도구로서 등척성 수축 후 버티기 방법의 타당성 비교 검증 (Testing for validity of isometric muscle endurance test as an abdominal muscular endurance evaluation methods)

  • 본 연구는 복근의 정적 지구력 평가 도구로서의 등척성 버티기 방법의 타당성을 검증하고 최적의 적용 각도를 결정해내기 위해 수행되었다. 92명(남 68명, 여 24명)의 피험자가 자발적으로 실험에 참여하여 다음과 같은 결과를 얻었다. 유산소 역량과 근지구력 역량 사이의 상관성은 남성에서보다 여성에게서 더 높게 나타났으며 등척성 버티기의 수행 시간과 윗몸일으키기의 반복 횟수 사이에 정적인 상관관계를 나타냈다. 또한 40도의 경사도를 적용 시 30도 및 50도와 비교해 근지구력과의 가장 높은 상관성을 나타냈다. 이와 같은 결과를 통해 복근지구력의 평가 도구로서의 등척성 버티기 방법이 요부의 잠재적 상해 위험을 지닌 것으로 보고되고 있는 윗몸일으키기 방법을 대체할 수 있는 대안으로 사용될 수 있으며 적용 시 40도의 각도를 적용하는 것이 적절하다는 결론을 내릴 수 있었다. 서로 다른 연령 및 체력 상태에 따른 최적의 적용 각도에 대해서는 후속 연구를 통해 밝혀져야 할 것이다.

운동성 조음장애에서 폐쇄자음 발성의 음향학적 특성 (Acoustic Characteristics of Stop Consonant Production in the Motor Speech Disorders)

  • Background and Objectives : Dysarthria refers to speech disorder that causes difficulties in speech communication due to paralysis, muscle weakening, and incoordination of speech muscle mechanism caused by damaged central or peripheral nerve system. Pitch, strength and speed are influenced by dysarthria during detonation due to difficulties in muscle control. As evaluation items, alternate motion rate and diadochokinesis have been commonly used, and articulation is also an important evaluation items. The purpose of this study is to find acoustic characteristics on sound production of dysarthria patients. Materials and Methods : Research subjects have been selected as 20 dysarthria patients and 20 subjects for control group, and voice sample was composed of bilabial, alveolar sound, and velar sound in diadochokinetic rate, while consonant articulation test was composed of bilabial plosive, alveolar plosive, velar plosive. Analysis items were composed of 1) speaking rate, energy, articulation time of diadochokinesis, 2) voice onset time (VOT), total duration (TD), vowel duration (VD), hold of plosives. Results and Conclusions : The number of diadochokinetic rate of dysarthria was smaller than control group. Both control group and dysarthria group was highly presented in the order of /t/>/p/>/k/. Minimum energy range per cycle during diadochokinetic rate of dysarthria group was smaller than control group, and presented statistical significance in /p/, /k/, /ptk/. Maximum energy range was larger than control group, and presented statistical significance in /t/, /ptk/. Articulation time, gap, total articulation time during diadochokinetic rate of dysarthria group was longer than control group and presented statistical significance. The articulation time was presented in both control group and dysarthria group in the order of /k/>/t/>/p/, while Gap was presented in the order of /p/>/t/>/k/ for control group and /p/>/k/>/t/ for dysarthria group. VOT, TD, VD regarding plosives of dysarthria group were longer than control group. Hold showed large deviation compared to control group that had appeared due to declined larynx and articulation organ motility.

  • The objective of this study is to report the clinical effects of applying an integrated rehabilitation protocol to four patients with traumatic cervical spinal cord injuries. The treatments applied included acupuncture, cupping and moxibustion, chuna manual therapy, functional electrical stimulation, and other physical therapies. The evaluation methods included American Spinal Injury Association Impairment Scale, functional independence measure (FIM), modified Barthel index (MBI), numeric rating scale (NRS) and other scales. All patients presented marked improvements in FIM, MBI, NRS and other scales, along with increased muscle strength in the manual muscle test. The application of our protocol resulted in clear clinical benefits and enhanced the recovery and quality of life for the patients in this study.

카프레이즈(Calf raise) 처치가 하지 순발력에 미치는 영향 (The Effect of Calfraise Treatment on the Quickness of the Legs)

  • 본 연구의 목적은 기초체력요인 중 일회적인 카프라이즈 동작이 하지의 순발력에 미치는 영향을 분석하는 것입니다. 본 연구의 대상은 D광역시 소재 Y대학교 체육학과 16명이며, 모든 대상자들은 측정항목과 방범에 대한 내용을 숙지하였으며, 설문에 응답하고 동의한 학생들을 대상으로 실험을 진행하였습니다. 요약하면, 단 한 번의 카프레이즈만이 하지의 순발력 강화에 효과가 있는 것으로 보이며, 특히 종아리 근육의 둘레가 두꺼울수록 하지의 순발력에 영향을 미치는 것으로 판단됩니다. 또한, 본 연구의 결과가 모든 연령대에 효과가 있다고 볼 수는 없지만, 단 한 번의 운동만으로 기록을 향상시키는 효과를 보여주고 있어 체력 평가의 기초 자료로 활용하기에 좋을 것으로 사료됩니다. 향후 연구에서는 연령별 대상자를 모집하여 연령 차이를 통해 운동의 필요성을 높일 수 있는 연구를 진행하면 좋을 것으로 판단됩니다.

Isolated Ruptures of the Infraspinatus: Clinical Characteristics and Outcomes

  • Background: Isolated infraspinatus tear is very rare and clinical features are not as well known, therefore the purpose of this study was to evaluate clinical characteristics and outcomes of isolated infraspinatus tear that authors experienced. Methods: Authors reviewed 288 cases of full-thickness rotator cuff tear involving infraspinatus between 2010 and 2015, and retrospectively analyzed six cases of isolated infraspinatus tear. Perioperative clinical characteristics, postoperative functional outcomes of 6 months were investigated. Functional evaluation included visual analogue scale (VAS), range of motions, American Shoulder and Elbow Surgeons (ASES) score, and Constant score. Results: Calcific tendinitis was accompanied in 4 cases (66.7%). Three of them received steroid injection or aspiration or extracorporeal shockwave therapy. Mean preoperative pain VAS was 7.1 (range, 5-9), and mean postoperative pain VAS at 6 months later was 1.6 (range, 0-5). Preoperative muscle strength by isokinetic muscle performance test showed 52% deficit of abduction and 37.6% deficit of external rotation. All 6 patients had arthroscopic repair of the infraspinatus tendon. All the patients at the 6 months follow-up exhibited clinical improvement in the Constant score (67.8 [range, 45-77] to 89.3 [range, 81-100], p=0.029), and ASES score (52.3 [range, 30-77] to 90.0 [range, 80-100], p=0.002). There was no healing failure on imaging. Conclusions: Isolated infraspinatus tendon tear was frequently accompanied by calcific tendinitis, but pathophysiologic relationship should need more study. To rule out neurogenic etiology, magnetic resonance imaging and electromyography would be helpful. Arthroscopic infraspinatus tendon repair and supraspinatus debridement showed relatively good result in painful shoulder.

소뇌 손상에 의한 사지 마비 환자의 보행 및 운동 기능 회복 양상 연구 (Characteristics of Gait and Motor Function Recovery in Quadriplegia Patients with Cerebellar Injury)

  • Purpose: Cerebellar injury can be caused by a variety of factors, including trauma, stroke, and tumor. Cerebellar injury can manifest in different clinical symptoms and signs depending on the size and location of the injury. The purpose of this study was to examine and compare the recovery patterns of each motor function by tracking the motor levels of patients with cerebellar injury. Methods: This study recruited 11 patients with quadriplegia resulting from cerebellar injury. The motricity index (MI), modified Brunnstrom classification (MBC), and functional ambulation category (FAC) methods were used to evaluate motor levels. The motor function evaluation was performed immediately after the onset of the condition and at intervals of one month, two months, and six months after onset. Results: The MI values of the upper and lower extremities and hand function (MBC) indicated severe paralysis in the early stages of onset. Compared to the onset time, significant motor function recovery was observed after 1, 2, and 6 months (p < 0.05). In contrast, there was no significant pattern of recovery between 1, 2, and 6 months after onset (p > 0.05). FAC indicated showed significant recovery at one month compared to onset (p<0.05), and there was also a significant difference between 1 and 2 months (p < 0.05). On the other hand, there was no significant difference in FAC between 2 and 6 months (p > 0.05). Conclusion: Patients with cerebellar injury showed significant recovery in functions related to muscle strength and voluntary muscle control one month after onset and gradually recovered further over the next six months. On the other hand, gait function, which is closely related to balance, showed a relatively slow recovery pattern from the beginning of the disease to the six month follow-up.

상지 보조 소프트로봇의 의복화를 위한 디자인 개발 프로세스 (Design Development Process for Clothing of Upper Limb Assistive Wearable Soft Robot)

  • This study proposes a design process for an upper limb assistive wearable soft robot that will enable the development of a clothing product for an upper limb assistive soft robot. A soft robot made of a flexible and soft material that compensates for the shortcomings of existing upper limb muscle strength assistive devices is being developed. Consequently, a clothing process of the upper limb assistive soft robot is required to increase the possibility of wearing such a device. The design process of the upper limb auxiliary soft robot is presented as follows. User analysis and required performance deduction-Soft robot design-upper limb assistive wearable soft robot prototype design and production-evaluation. After designing the clothing according to the design process, the design was revised and supplemented repeatedly according to the results of the clothing evaluation. In the post-production evaluation stage, the first and second prototypes were attached to actual subjects, and the second prototype showed better results. The developed soft robot evaluated if the functionality as a clothing function and the functionality as the utility of the device were harmonized. The convergence study utilized a process of reducing friction conducted through an understanding and cooperation between research fields. The results of this study can be used as basic data to establish the direction of prototype development in fusion research.

한국 여성 노인을 대상으로 한 피트니스 압박웨어 선호도 및 착용 특성 평가 (Evaluation of Preference and Wearing Characteristics of Fitness Compression wear for Korean Elderly Women)

  • This study evaluated the preference and wearing characteristics of existing fitness compression wear for elderly women and identified the problems and improvements of existing fitness apparel. Preference and wearing characteristics for 6 types (A-F) of upper and lower fitness compression wear were evaluated. Photos and drawings as well as the preferred designs and details were investigated after presenting the evaluation clothing. Evaluations were made on a 5 point Likert scale for the fitness, allowance, pressure, ease of movement, fabric material, and overall satisfaction after wearing. The design preference indicated that B type (26.6%) and C type (23.4%) were preferred in the top with C, E and F type preferred to the same ratio of 19.4% in the slacks. The fitness and allowance amount of the top B type were the most appropriate (${\geq}4$), the C type was in close contact (2.86), and the F type was inadequate in the neck circumference (1.77). The feeling of pressure was high in the waist, abdomen, thighs, and knees. The area where the elderly people want to improve their strength was the legs; in addition, a pattern design was needed to strengthen leg muscles when designing fitness compression wear. However, a design with excessive adhesion due to a muscle support band was shown to be not preferred. Therefore, depending on the activity, it is necessary to develop a fitness compression wear that applies an optimum stretching ratio of fabric based on body surface area changes.
