Effects of a supportive workplace environment on the success rate for smoking cessation camp

  • Background: This study was conducted to identify the success rate for smoking cessation over time after participation in a therapeutic smoking cessation camp, and to identify how participant characteristics, including a supportive workplace environment for smoking cessation (SWESC), affect the success rate for smoking cessation. Methods: In all, 296 participants at smoking cessation camps in Ulsan between 2015 and 2020 were investigated. The success rates of smoking cessation after weeks 4, 6, 12, and 24 at camp were investigated. The participants were grouped as workers with an SWESC, and workers without an SWESC, and variables (age, education, household income, marital status, drinking, exercise, body mass index, morbidity, job, number of counseling sessions, cigarettes smoked per day and smoking initiation age) were investigated. Multiple logistic regression analysis was conducted at each time point. In addition, Cox regression analysis was performed to evaluate the variables affecting the success rate for smoking cessation over time. Results: The smoking cessation success rate of workers with an SWESC at week 24 (90.7%) was higher than that for workers without an SWESC (60.5%). Multiple logistic regression was performed to determine the relationship between each variable and the success rates for smoking cessation at week 6, 12, and 24. SWESC was confirmed as significant (p < 0.05) variables for increased success rate for smoking cessation at all 3 time points. After adjusting for all variables, the Cox proportional hazards survival analysis showed a hazard ratio of 6.17 for SWESC (p < 0.001,; 95% confidence interval: 3.08-12.38). Conclusions: At a professional treatment smoking cessation camp, participants with an SWESC showed a significantly higher success rate for smoking cessation. Supportive workplace environment for workers' health is expected to be an important factor for smoking cessation projects as well as other health promotion projects at workplace.

반사파가 박동형 체외막산화기 제어에 사용되는 심층신경망의 심장 박동 감지에 미치는 영향 분석 (Analysis of the Impact of Reflected Waves on Deep Neural Network-Based Heartbeat Detection for Pulsatile Extracorporeal Membrane Oxygenator Control)

  • It is necessary to develop a pulsatile Extracorporeal Membrane Oxygenator (p-ECMO) with counter-pulsation control(CPC), which ejects blood during the diastolic phase of the heart rather than the systolic phase, due to the known issues with conventional ECMO causing fatal complications such as ventricular dilation and pulmonary edema. A promising method to simultaneously detect the pulsations of the heart and p-ECMO is to analyze blood pressure waveforms using deep neural network technology(DNN). However, the accurate detection of cardiac rhythms by DNNs is challenging due to various noises such as pulsations from p-ECMO, reflected waves in the vessels, and other dynamic noises. This study aims to evaluate the accuracy of DNNs developed for CPC in p-ECMO, using human-like blood pressure waveforms reproduced in an in-vitro experiment. Especially, an experimental setup that reproduces reflected waves commonly observed in actual patients was developed, and the impact of these waves on DNN judgments was assessed using a multiple DNN (m-DNN) that provides accurate determinations along with a separate index for heartbeat recognition ability. In the experimental setup inducing reflected waves, it was observed that the shape of the blood pressure waveform became increasingly complex, which coincided with an increase in harmonic components, as evident from the Fast Fourier Transform results of the blood pressure wave. It was observed that the recognition score (RS) of DNNs decreased in blood pressure waveforms with significant harmonic components, separate from the frequency components caused by the heart and p-ECMO. This study demonstrated that each DNN trained on blood pressure waveforms without reflected waves showed low RS when faced with waveforms containing reflected waves. However, the accuracy of the final results from the m-DNN remained high even in the presence of reflected waves.

640 nm LED 조사와 아로마테라피가 목 피부 상태 개선에 미치는 영향 (Effect of 640 nm LED Irradiation and Aromatherapy on Ameliorating Neck Skin Condition)

  • 본 연구의 목적은 640 nm LED 조사와 아로마테라피가 목 피부 상태에 미치는 영향을 체계적으로 평가하고 비교하는 것이다. 20명의 여성 참여자를 대상으로 A그룹(640 nm LED 조사 + 아로마테라피)와 B그룹(640 nm LED 조사만)으로 나뉘어, 주 2회씩, 총 8회의 실험을 진행하였다. 실험 결과, 수분 함량은 A그룹(p<0.01)과 B그룹(p<0.001) 모두 유의하게 나타났으며, 주름 지수는 A그룹(p<0.01)과 B그룹(p<0.05) 모두 유의하게 나타났다. 또한, 치밀도 변화는 A그룹(p<0.001)과 B그룹(p<0.05) 모두 유의하게 나타났으며, 탄력 변화는 A그룹(p<0.001)과 B그룹(p<0.001) 모두 유의하게 나타났다. 피부색 변화는 A그룹에서는 (p>0.05) 유의하지 않았고, B그룹에서는 (p<0.05) 유의하게 나타났다. 기미 및 색소 침착 변화는 A그룹에서 (p<0.05) 유의하게 나타났으며, B그룹에서는 (p>0.05) 유의하지 않았다. 이러한 결과는 아로마테라피와 640 nm LED 조사를 병행하는 것이 목 피부 상태를 향상시키는 데 높은 효과를 나타낼 수 있음을 시사한다.

살충제 저항성 목화진딧물에 대한 afidopyropen과 chlorfenapyr, cyantraniliprole의 혼합효과 평가 (Combined Effect of Afidopyropen, Chlorfenapyr and Cyantraniliprole to Insecticide-resistant Cotton Aphid, Aphis gossypii (Hemiptera: Aphididae))

  • 살충제 저항성 목화진딧물 집단(완주, 춘천, 고성)에 대한 3종(afidopyropen, chlorfenapyr, cyantraniliprole) 약제별 감수성과 혼용에 따른 방제효과를 평가하였다. 3개 야외집단에 대해서 3약제 모두100배 이상의 저항성비를 보였다. 혼합제의 협력작용을 평가하기 위해 반수치사농도(LC50)을 이용한 조합지수(CI), %M(synergism), 연합독성계수(CTC), 와들리 비율(WR), 상승비율(SR)와 사충률을 이용한 아보트 비율(AR)로 비교하였다. Afidopyropen + chlorfenapyr은 모든 야외집단에 대해서 살충활성의 증가를 보였고, chlorfenapyr + cyantraniliprole, afidopyropen + cayntraniliprole은 완주, 춘천 집단에 대해서 살충활성의 증가를 보였다. 그러나 chlorfenapyr + cyantraniliprole (CI, 1.63; %M(synergism), 30; CTC, 64.0; WR, 0.6, AR, 0.54)과 afidopyropen + cyantraniliprole (CI, 6.7; %M(synergism), 1; CTC, 19.8; WR, 0.2, AR ≤ 0.55)은 고성 집단에서 길항작용을 나타내었다. 포장조건에서도 afidopyropen + chlorfenapyr, chlorfenapyr + cyantraniliprole, afidopyropen + cyantraniliprole은 99% 이상의 높은 방제가를 보여 향후 살충제 저항성 목화진딧물 방제용 혼합제 개발에 유용하게 활용될 것이다.

Use of custom glenoid components for reverse total shoulder arthroplasty

  • Background: Our purpose was to evaluate a custom reverse total shoulder arthroplasty glenoid baseplate for severe glenoid deficiency, emphasizing the challenges with this approach, including short-term clinical and radiographic outcomes and complications. Methods: This was a single-institution, retrospective series of 29 patients between January 2017 and December 2022 for whom a custom glenoid component was created for extensive glenoid bone loss. Patients were evaluated preoperatively and at intervals for up to 5 years. All received preoperative physical examinations, plain radiographs, and computed tomography (CT). Intra- and postoperative complications are reported. Results: Of 29 patients, delays resulted in only undergoing surgery, and in three of those, the implant did not match the glenoid. For those three, the time from CT scan to implantation averaged 7.6 months (range, 6.1-10.7 months), compared with 5.5 months (range, 2-8.6 months) for those whose implants fit. In patients with at least 2-year follow-up (n=9), no failures occurred. Significant improvements were observed in all patient-reported outcome measures in those nine patients (American Shoulder and Elbow Score, P<0.01; Simple Shoulder Test, P=0.02; Single Assessment Numeric Evaluation, P<0.01; Western Ontario Osteoarthritis of the Shoulder Index, P<0.01). Range of motion improved for forward flexion and abduction (P=0.03 for both) and internal rotation up the back (P=0.02). Pain and satisfaction also improved (P<0.01 for both). Conclusions: Prolonged time (>6 months) from CT scan to device implantation resulted in bone loss that rendered the implants unusable. Satisfactory short-term radiographic and clinical follow-up can be achieved with a well-fitting device. Level of evidence: III.

Pharmacoacupuncture for the Treatment of Frozen Shoulder: protocol for a systematic review and meta-analysis

  • Objectives: Frozen shoulder (FS) is one of the most challenging shoulder disorders for patients and clinicians. Its symptoms mainly include any combination of stiffness, nocturnal pain, and limitation of active and passive glenohumeral joint movement. Conventional treatment options for FS are physical therapy, nonsteroidal anti-inflammatory drugs, injection therapy, and arthroscopic capsular release, but adverse and limited effects continue to present problems. As a result, pharmacoacupuncture (PA) is getting attention as an alternative therapy for patients with FS. PA is a new form of acupuncture treatment in traditional Korean medicine (TKM) that is mainly used for musculoskeletal diseases. It has similarity and specificity compared to corticosteroid injection and hydrodilatation, making it a potential alternative injection therapy for FS. However, no systematic reviews investigating the utilization of PA for FS have been published. Therefore, this review aims to standardize the clinical use of PA for FS and validate its therapeutic effect. Methods: The protocol was registered in Prospero (CRD42023445708) on 18 July 2023. Until Aug. 31, 2023, seven electronic databases will be searched for randomized controlled trials of PA for FS. Authors will be contacted, and manual searches will also be performed. Two reviewers will independently screen and collect data from retrieved articles according to predefined criteria. The primary outcome will be pain intensity, and secondary outcomes will be effective rate, Constant-Murley Score, Shoulder Pain and Disability Index, range of motion, quality of life, and adverse events. Bias and quality of the included trials will be assessed using the Cochrane handbook's risk-of-bias tool for randomized trials. Meta analyses will be conducted using Review Manager V.5.3 software. GRADE will be used to evaluate the level of evidence for each outcome. Results: This systematic review and meta-analysis will be conducted following PRISMA statement. The results will be published in a peer-reviewed journal. Conclusion: This review will provide scientific evidence to support health insurance policy as well as the standardization of PA in clinical practice.

Case Study on an Oral Health Care Program for Older Adults Based on a Public-Private-Academic Partnership

  • Background: The population of Gangneung City in South Korea has shown a gradual increase in the proportion of individuals aged 65 years and older, and the most frequently reported diseases for several years have consistently been periodontal diseases, including gingivitis and periodontitis. The regional imbalance in the distribution of dental personnel and resources has emerged as a problem of inequality in the use of dental care. It has been advocated to identify areas with disadvantages in dental care and develop public dental policies based on that. This study aimed to develop a customized oral healthcare program for local seniors based on a Public-Private-Academic Partnership, and to evaluate the oral health status of older adults in Gangneung City. Methods: The participants were residents aged 60 years and above in Gangneung City. A questionnaire including general information, systemic health status, and oral health status was administered to the participants. In addition, oral healthcare and education tailored to each individual's health status were provided once or twice based on their oral health status. The collected data were analyzed using IBM SPSS Statistics 25 for descriptive statistical analysis. Results: Among the older adults in Gangneung City, 75% had at least one prosthesis and exhibited symptoms of gingivitis or periodontitis. Additionally, the modified sulcus bleeding index decreased among participants who underwent the program twice. Over 90% of the participants expressed satisfaction with the program. Conclusion: The program appeared to contribute positively to the oral health promotion among local seniors. Further oral healthcare programs should focus on seniors in rural and old urban areas to reduce disparities in oral health across regions.

Pancreaticoduodenectomy with superior mesenteric artery first-approach combined total meso-pancreas excision for periampullary malignancies: A high-volume single-center experience with short-term outcomes

  • Backgrounds/Aims: Pancreaticoduodenectomy (PD) is the only radical treatment for periampullary malignancies. Superior mesenteric artery (SMA) first approach combined with total meso-pancreas (MP) excision was conducted to improve the oncological results. There has not been any previous research of a technique that combines the SMA first approach and total MP excision with a detailed description of the MP macroscopical shape. Methods: We prospectively assessed 77 patients with periampullary malignancies between October 2020 and March 2022 (18 months). All patients had undergone PD with SMA first approach combined total MP excision. The perioperative indications, clinical data, intra-operative index, R0 resection rate of postoperative pathological specimens (especially mesopancreatic margin), postoperative complications, and follow-up results were evaluated. Results: The median operative time was 289.6 min (178-540 min), the median intraoperative blood loss was 209 mL (30-1,600 mL). Microscopically, there were 19 (24.7%) cases with metastatic MP, and five cases (6.5%) with R1-resection of the MP. The number of lymph nodes (LNs) harvested and metastatic LNs were 27.2 (maximum was 74) and 1.8 (maximum was 16), respectively. Some (46.8%) patients had pancreatic fistula, but mostly in grade A, with 7 patients (9.1%) who required re-operations. Some 18.2% of cases developed postoperative refractory diarrhea. The rate of in-hospital mortality was 1.3%. Conclusions: The PD with SMA first approach combined TMpE for periampullary malignancies was effective in achieving superior oncological statistics (rate of MP R0-resection and number of total resected LNs) with non-inferior short-term outcomes. It is necessary to evaluate survival outcomes with long-term follow-up.

Programmed Follow-up and Quality Control of Treatment Techniques Enhance Chronic Thromboembolic Pulmonary Hypertension Management: Lessons From a Multidisciplinary Team

  • Background and Objectives: The recent developments in chronic thromboembolic pulmonary hypertension (CTEPH) are emphasizing the multidisciplinary team. We report on the changes in clinical practice following the development of a multidisciplinary team, based on our 7 years of experience. Methods: Multidisciplinary team was established in 2015 offering both balloon pulmonary angioplasty (BPA) and pulmonary endarterectomy (PEA) with technical upgrades by internal and external expertise. For operable cases, PEA was recommended as the primary treatment modality, followed by pulmonary angiography and right heart catheterization after 6 months to evaluate treatment effect and identify patients requiring further BPA. For patients with inoperable anatomy or high surgical risk, BPA was recommended as the initial treatment modality. Patient data and clinical outcomes were closely monitored. Results: The number of CTEPH treatments rapidly increased and postoperative survival improved after team development. Before the team, 38 patients were treated by PEA for 18 years; however, 125 patients were treated by PEA or BPA after the team for 7 years. The number of PEA performed was 64 and that of BPA 342 sessions. World Health Organization functional class I or II was achieved in 93% of patients. The patients treated with PEA was younger, male dominant, higher pulmonary artery pressure, and smaller cardiac index, than BPA-only patients. In-hospital death after PEA was only 1 case and none after BPA. Conclusions: The balanced development of BPA and PEA through a multidisciplinary team approach proved synergistic in increasing the number of actively treated CTEPH patients and improving clinical outcomes.

Beneficial effects of oral antioxidant supplementation on semen quality parameters, reproductive hormones, and sperm DNA integrity in men with idiopathic oligoasthenoteratozoospermia

  • Objective: Recently, oral antioxidants in combined forms have been used to treat men with idiopathic infertility. This study aimed to evaluate the effects of treatment with vitamin C, vitamin E, selenium, zinc, arginine, L-carnitine, and coenzyme Q10 on sperm quality parameters, DNA integrity, reproductive hormones, and pregnancy rates in men with infertility and idiopathic oligoasthenoteratozoospermia (OAT). Methods: A prospective study was conducted on 420 men with infertility and idiopathic OAT who took an oral supplement of antioxidant SP-Power tablets twice daily for 6 months. Semen quality, reproductive hormones, and the DNA fragmentation index (DFI) were evaluated at baseline and at 3 and 6 months after supplementation, using the World Health Organization 2021 guidelines. Results: No significant difference was observed in volume or the percentage of typical morphology during treatment. A significant improvement in sperm concentration was observed after supplementation (8.67±1.41, 12.17±1.91, and 19.01±0.86 at baseline, 3, and 6 months respectively, p<0.01). The total motility, progressive motility, and total motile sperm count also increased significantly (p<0.01), whereas the DFI decreased after 6 months. There was an increase in normal FSH levels and testosterone levels after 6 months of supplementation of antioxidant SP-Power but these differences were not statistically significant (p=not significant and p=0.06, respectively). Conclusion: Supplementation with SP-Power tablets improved sperm quality parameters, sperm DFI, some reproductive hormones, and pregnancy rates in men with infertility and idiopathic OAT, which could be attributed to the supplement's synergistic antioxidant action. Further studies are needed to determine the effects of supplementation on oxidative stress markers.
