Idiopathic type-4 hiatal hernia in a middle-aged cat

  • A 6-year-old female domestic short hair cat presented with acute onset of vomiting, anorexia, lethargy, and tachypnea. The cat was apparently healthy prior to presentation without history of trauma. On diagnostic imagings, the entire stomach and spleen were detected in the thoracic cavity. An emergent celiotomy was performed, and a type-4 hiatal hernia was confirmed; the stomach, spleen, pancreas, and duodenum were herniated through the esophageal hiatus. It was corrected using phrenicoplasty, esophagopexy, and left-sided gastropexy and there was no recurrence 16 months after surgery. This is the first case report of an idiopathic type-4 hiatal hernia in a cat.

영아에서 발생한 식도열공탈장 수술치험 -1례 보고- (Esophageal Hiatal Hernia in Infant -A Report of Case-)

  • The esophageal hiatal hernia is a rare disease in Korea especially in children and infant. We experienced a case of type III esophageal hiatal hernia in 9 months female. She had no specific past history and familial history except recurrent URI and postprandial habitual vomiting. The chest X-ray and Barium swallowing showed herniated stomach in Rt. thoracic cavity and posterior mediastinum. We performed modified Belsey Mark IV procedure with using the 3-0 and 4-0 Pledgeted Ticrons. In operative field, the stomach cardia portion was herniated into the Rt.thoracic cavity and posterior mediastinum with elevation of the esophagogastric junction above the diaphragm. But there was no evidence of short esophagus and combined anomaly. The postoperative courses were uneventful and good without specific complication for about 5 months to this point.

  • Fourteen cases of diaphragmatic hernias surgically treated at the Dept. of Thoracic & Cardiovascular Surgery in Kosin Medical Center from Jan. 1979 to Feb. 1991, were reviewed in this study. This report includ 9 cases of traumatic diaphragmatic hernia, 5 cases of congenital diaphragmatic hernia. Among the traumatic hernias, 5 cases were blunt trauma and 4 cases were penetrating trauma. Five cases of congenital diaphragmatic hernias were presented, including 2 cases of esophageal hiatal hernia, 2 cases of Bochdalek's hernia and a case of Morgagni's hernia. Operation was performed in all patients. Results of all treated cases were excellent.

신생아에서 발생한 식도열공탈장 수술치험;1례 보고 (Paraesophageal Hiatal Hernia in Newborn - A Case Report -)

  • Esophageal hiatal hernia is common disorder in western sociey, and mainly affects mid-aged women, There are two types of hiatal gernia; common and more benign type is sliding [type I], and more severe type is paraesophageal [type II], and Skinner subdivided type II as true II, IIA, III, and paraesophageal group, As Skinner`s nomination, this case could be belong to IIA, which imply the state that natural hiatus is occupied with gastric antrum or duodenum instead of normal esophago-gastric junction, Main problem of paraesophageal hernia comes from its complication; intestinal obstruction, volvulus, strangulation, and incarceration, as well as pulmonary aspiration. So, as soon as confirm diagnosis, it should be corrected surgically to prevent above complications, and sometimes it could result in serious condition. We have experienced paraesophageal hiatal hernia in 3-day newborn infant and have repaired it successfully. We used transabdominal approach to repair and to prevent reflux Nissen`s fundoplication was performed. We would report that with reference study.

횡경막 허니아 (Diaphragmatic Hernia: Report of 20 Cases)

  • Congenital diaphragmatic hernia is an important cause of respiratory distress in the newborn. Eventration, with which these conditions are easily confused, may also Produce severe respiratory distress in infancy or be an asymptomatic radiographic finding. Harrington`s classification of diaphragmatic hernias into two categories, traumatic and nontraumatic, is most widely accepted. Nontraumatic hernias are [1] the congenital types, composed of the posterolateral [Bochdalek], those through the esophageal hiatus, the parasternal [/Morgagni], and those through a defect left by partial absense posteriorly, and [2] the acquired types, composed of those through the esophagea/hiatus [sliding and paraesophageal] and those the sites mentioned above under the congenital hernias. During the period from 1970 up to October 1978, 21 cases of diaphragmatic hernia were treated in department of cardiovascular and thoracic surgery. 11 cases of Bochdalek hernias, 1 case of Morgagni hernia, 5 cases of diaphragmatic eventration and 3 cases of hiatal hernia [2 cases of paraesophageal and 1 cases of sliding type], were experienced. 3 cases of 20 died of respiratory insufficiency, 2 cases of mortality were combined with left lung hypoplasia with Bochdalek hernia.

휘발유 폭발에 의한 식도파열 1례 (Esophageal Rupture due to Explosion of Gasoline: A Case Report)

  • Esopkageal rupture is one of the rarest disease. Mackler described that esophageal rupture was differentiated from esophageal perforation, the perforation is produced by esophagoscopy, and continuous erosion, such as esophagitis,gastric reflux, hiatal hernia and malignant neoplasm of the esophagus, the rupture is occurred by severe vomiting, cough and strong positive pressure into the esophageal lumen. Since,at first Boerhaave reported the esophageal rupture due to severe vomiting in 1742, several case reports of esophageal rupture have been in the literatures. Authors reported a case of the esophageal rupture due to explosion of gasoline in 50 year old female. The rupture occurred a longitudinal rent on the left posterolateral aspect of lower one third of esophagus and accompanied wlth second degree burn on the entire face and neck. The treatment consists of immediate thoracotomy in order to drainage of pyothorax and gastrostomy for nutritional problem, but patient expired because of septicemia probably due to uncontrollable empyema of thorax on 45th admitted day.

  • 음식물괴에 의한 갑작스런 식도폐쇄는 아주 드문 현상은 아니다. 주로 고령자나 치아의 결손이 있어 불충분한 저작후 큰 덩어리를 무리하게 삼켜 잘 일어난다. 대개는 해부학적 이상이 있는 사람에게서 일어나며 정상적인 식도를 가진 사람에서도 드물게 일어난다. 그러나 가장 흔한 원인중의 하나는 식도부식 등의 기왕력이 있은 후 이차적으로 생긴 식도협착증 이라고 할 수 있으므로 음식물괴에 의한 식도이물의 경우 기왕력을 확인하는 것이 무엇보다도 중요하다고 하겠다. 저자들은 최근 식도협착이 있는 58세 된 남자 환자에게서 10일 동안에 연거퍼 2회의 음식물괴에 의한 식도이물례를 경험하였기에 보고하는 바이다.

실패한 Nissen 술을 Belsey Mark IV 위바닥주름술로 교정한 1예 (A Case of Belsey Mark IV Fundoplication after Failed Nissen Antireflux Surgery)

  • Belsey Mark IV 위바닥주름술은 이전의 역류 방지 수술이 실패하여 재수술을 해야 하는 경우 혹은 식도 열공 탈장이 오래 되어 유착이 있는 경우에 적응증이 된다. 본 증례는 식도 열공 탈장과 GERD로 진단되어 Nissen 위바닥 주름술을 시행하였으나 실패하였고 재수술을 시도하였지만 수술 부위의 유착이 심하여 수술적 접근이 불가능하였다. 저자들은 흉강을 통하여 Belsey Mark IV 위바닥주름술을 실시하였고, 수술 후 증상과 영양상태의 호전을 보인 1예를 문헌 고찰과 함께 보고하는 바이다.

정상 한국성인의 식도내압 측정 (Esophageal Pressure Monitoring in Normal Korean Adults)

  • Here, I and wer report the results of our studying about; 1. The length of esophagus and sphincters; 2. Resting pressure of upper sphincter, upper esophagus, mid-esophagus, lower esophagus and lower sphincter; 3. Pressure changes in swallowing at these points of esophagus; 4. Resting and swallowing pressure curves in these points in 50 normal Korean adults. In addition to these we wbserved pressure inversion point, slow and fast components of phasic pressure which are originating from respiration and heart beat. And we studied transportation time and speed of peristalsis. The speed of peristalsis is faster in the lower esophagus than in the upper. I can probalby be proud in the results of these study because these will become a standard criteria in the further evaluation of esophageal functional disturbances in such lesions as; Achalasia, Hiatal hernia, Esophageal canceer, Scleroderma, diverticula.

양성식도질환(良性食道疾患)에 대(對)한 임상적(臨床的) 고찰(考察) (Surgical Management of the Benign Esophageal Diseases)

  • A clinical analysis was performed on 118 cases of the benign esophageal diseases experienced at Department of Thoracic Surgery, Seoul National University Hospital during 20 year period from 1957 to 1976. Of 118 cases of the benign esophageal diseases, there were 84 patients of esophagenal stenosis, 14 of esophageal perforation, 8 of esophageal atresia, 7 of achalasia, 2 of hiatal hernia, 2 of esophageal foreign body and one of esophageal diverticulum. Fifty-one patients were male and sixty-seven were female, and ages ranged from one day to sixty-four years with peak incidence in the age group of 20 to 29 years. All but one of the esophageal stenosis were caused by corrosive esophagitis and ages ranged from three to sixty-four years with peak incidence in third decade. Main symptoms of the esophageal stenosis were dysphagia, weight loss and chest pain in order and mostly began between one month and one year after ingestion of corrosive agents. Corrosive esophageal stenosis developed most frequently in middle one-third of the esophagus and about one-forth of them were diffuse. Operations were performed on 72 patients of esophageal stenosis of whom 26 patients had esophagocologastrostomy, 21 gastrostomy, 20 esophagogastrostomy, 4 esophagojejunogastrostomy and 2 pharyngogastrostomy. There were 5 deaths in the postoperative period, an operative mortality of 6.9 percent, and 20 patients had one or two complications; eight were anastomotic leaks, 6 gangrenes of replaced loop, 4 wound abscesses and others. The causes of the esophageal perforation were traumatic in 7 cases, caustics in 4 and spontaneous in 3, and the most frequent site of the perforation was lower one-third of the esophagus. Frequent symptoms of the esophageal perforation were pain, fever, dysphagia and dyspnea, and preoperatively there were mediastinitis in 8 cases, empyema in 7, lung abscess in 3 and others. All 14 patients of the esophageal perforation underwent operation: primary closure in 7 cases, drainage in 4, esophagogastrostomy in 2 and 'esophageal diversion in one. There were 4 postoperative deaths and 11 postoperative complications occurred in 7 patients. The duration of symptoms in achalasia was between 3 months and 25 years, with an average duration of 6. 2 years. Frequent symptoms of the achlasia esophagi were dysphagia, regurgitation, pain and weight loss in order. All 7 patients of achlasia underwent modified Heller's operation where 2 patients had complications, restenosis in one and esophageal perforation in another. All 8 patients of congenital esophageal atresia had distal tracheoesophageal fistula and were admitted within 5 days of life, but there were pneumonic consolidation on chest X-ray in patients. Five patients underwent one staged operation with the result of 2 deaths and one anastomotic leak.
