Clincal Efficacy of Korean Red Ginseng for Erectile Dysfunction -Multi-national Approach

  • Ginseng has been used in maintaining physical vitality all over the East Asian countries and recently its metabolism and actions on neurologic, cardiovascular and endocrinologic systems are being elucidated. Korean red ginseng (KRG) has been used in various aliments, and to prove its efficacy icy erectile dysfunction an international study on Asians other than Korean was performed. Patients with borderline organic and psychogenic erectile dysfunction were included. KRG were given daily, and placebos were given as controls. Treatment lasted a total of 3 months. Surveys including libido, erectile potency, the sexual satisfactions were given. Serum testosterone and erectile function study were taken. Among the 23 patients with KRG, 18 patients were followed. Four had diabetes, 2 hypertension, 3 hyperchole, iterolemia, 1 low testosterone,4 psychogenic, and 4 idiopathic. In 10 patients with placebo, 7 were followed for more than three months. The clinical efficacy of KRG was 66.7% on objective Questionnaire and 72.2% on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the controls. Serum testosterone Bevels were normalized in 2 patients with KRG, whose serum testosterone levels were reduced from prestudy. When the erectile functions after audiovisual stimuli evaluated using Rigiscan in 6 patients with KRG,4 showed rigidity move than 70oyc. One patient reported constipation, and 2 gastric up-sets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with minimal side effects. Thus KRG has been proven effective in Koreans, and a result on other Asians is pending. The exact action mechanism and the active ingredients in KRC need to be itudied.

Applications of proportional odds ordinal logistic regression models and continuation ratio models in examining the association of physical inactivity with erectile dysfunction among type 2 diabetic patients

  • [Purpose] Many studies have observed a high prevalence of erectile dysfunction among individuals performing physical activity in less leisure-time. However, this relationship in patients with type 2 diabetic patients is not well studied. In exposure outcome studies with ordinal outcome variables, investigators often try to make the outcome variable dichotomous and lose information by collapsing categories. Several statistical models have been developed to make full use of all information in ordinal response data, but they have not been widely used in public health research. In this paper, we discuss the application of two statistical models to determine the association of physical inactivity with erectile dysfunction among patients with type 2 diabetes. [Methods] A total of 204 married men aged 20-60 years with a diagnosis of type 2 diabetes at the outpatient unit of the Department of Endocrinology at PSG hospitals during the months of May and June 2019 were studied. We examined the association between physical inactivity and erectile dysfunction using proportional odds ordinal logistic regression models and continuation ratio models. [Results] The proportional odds model revealed that patients with diabetes who perform leisure time physical activity for over 40 minutes per day have reduced odds of erectile dysfunction (odds ratio=0.38) across the severity categories of erectile dysfunction after adjusting for age and duration of diabetes. [Conclusion] The present study suggests that physical inactivity has a negative impact on erectile function. We observed that the simple logistic regression model had only 75% efficiency compared to the proportional odds model used here; hence, more valid estimates were obtained here.

In vitro and in vivo evaluation of tissue-cultured mountain ginseng on penile erection

  • Background: Progressed tissue culture techniques have allowed us to easily obtain mass products of tissue-cultured mountain ginseng over 100 yr old (TCMG-100). We investigated the effects of TCMG-100 extract on erectile function using in vitro and in vivo studies. Methods: To examine the relaxation effects and mechanisms of action of TCMG-100 on rabbit cavernosal strips evaluated in an organ bath. To investigate the long-term treatment effect of TCMG-100, 8-wk administration was performed. After administration of TCMG-100, intracavernosal pressure, cyclic guanosine monophosphate and nitric oxide (NO) levels of cavernosal tissue, serum testosterone level, histological observation of collagen fiber, endothelium, smooth muscle cell, and transforming growth factor-${\beta}1$ were investigated. Results: TCMG-100 extract displayed dose-dependent relaxation effects on precontracted rabbit corporal smooth muscle. The TCMG-100-induced relaxation was significantly reduced by removing the endothelium, and treatment with an NO synthase inhibitor or NO scavenger. Eight weeks of TCMG-100 administration increased intracavernosal pressure in a rat model. The levels of cyclic guanosine monophosphate and NO in the corpus callosum and serum testosterone level were also increased by TCMG-100 treatment. Furthermore, histological evaluation of collagen, smooth muscle, and endothelium showed increases in endothelium and smooth muscle, and a decrease in transforming growth factor-${\beta}1$ expression. Conclusion: These relaxation effects on corporal smooth muscle and increased erectile function suggest that TCMG-100 might be used as an alternative herbal medicine to improve erectile function.

Ethanol 에 의해 발기부전을 유도한 흰쥐의 성기능 개선에 미치는 합환피(合歡皮)의 영향 (Effects of Albizzia Julibrissin on Chronic Ethanol-treated Erectile Dysfunction in Rats)

  • Objectives : Albizzia Julibrissin was formulated to contain various natural products known to cure erectile dysfunction. This study was aimed to investigate the effects of Albizzia Julibrissin on the nitric oxide synthase (NOS) activity, nitrite level, antioxidation and erectile responses induced by ethanol in corpus cavernosum penis of rats. Methods : The crushed Albizzia Julibrissin was extracted 3 times, each time with 3 volumes of methyl alcohol at $60^{\circ}C$ for 24 h. The extract was filtered and evaporated under a reduced pressure using a rotary evaporator to yield 45.3 g. Albizzia Julibrissin extract was oral-administered 100 mg per 1 kg of body weight for 20 days, while the normal group was administered only with a saline. The efficacy of Albizzia Julibrissin against erectile function was examined as described in the text. Results : The level of urethral NOS activity and nitrite were increased by Albizzia Julibrissin. The level of lipid peroxide was decreased by Albizzia Julibrissin. The level of urethral lipid peroxide in the ethanol-Albizzia Julibrissin double administered rats was decreased as low as in the norma! group, while the one in the ethanol-treated group was increased. The level of urethral nitrite, NOS activity, glutathione and serum testosterone in the ethanol-Albizzia Julibrissin double administered rats were as high as in the normal group, while the one in the ethanol-treated group was decreased. The erectile response to cavernous nerve stimulation in the ethanol-Albizzia Julibrissin double administered rats increased as high as in the normal group while the one in the ethanol-treated group decreased. Conclusions : Albizzia Julibrissin was shown to be effective for the treatment of erectile dysfunction induced by ethanol in rats.

고지혈증 흰쥐에서 발기부전에 대한 생약제제 KH-305의 효과 (The Effect of Herbal Formula KH-305 on Erectile Dysfunction in Hyperlipidemic Rat)

  • 저자들은 일반 흰쥐를 1.5% cholesterol과 0.25% cholic acid, 우지 25% 첨가한 식이로 고지혈증을 유발시킨 후 KH-305를 8주간 경구투여하여 혈중 cholesterol과 HDL-C, LDL-C 등을 측정, 지질대사에 미치는 영향을 알아보았다. 또한, 최대해면체 내압과 내피 관련 NOS 발현정도를 측정하여 음경발기 촉진 및 지속에 미치는 영향을 알아보았다. 그 결과 KH-305를 투여한 모든 그룹에서 고지방식이 그룹보다 총 cholesterol 수치와 LDL-C를 낮추었으며 HDL-C의 수치를 높였다. 최대해면체 내압에서는 KH-305 투여 그룹이 고지방식이 그룹보다 최대해면체 내압에 이르는 시간이 짧았고 최대해면체 내압의 수치가 높았으며 이 중 300mg/kg일 경우 가장 효과가 좋았다. 또한, KH-305 모든 투여 그룹은 고지방식이 그룹과 비교해 볼 때 모든 그룹에서 eNOS와 nNOS의 발현이 뚜렷이 증가되었다. 따라서, 혈중 cholesterol을 낮추면서 NOS의 발현을 증가, 최대해면체 내압이 큰 KH-305는 고지혈증으로 유발된 발기부전에 효능이 있는 약물이 될 수 있으리라 사료된다.

Effects of the Nuegra from Male Silkworm Extract on Enhancement of the Masculine Function and Activation of Overall Physical Function

  • The purpose of this investigation is to evaluate the effects of the Nuegra on enhancement of the masculine and physical activities in general through measuring changes of the testosterone, FSH and subjective symptoms like fatigue, insomnia, urinary stream, muscular weakness, libido and erectile dysfunction. Total 168 male subjects were enrolled from 12 urology, internal medicine clinics and general practitioner, During the 6-week investigational period, 2 capsules of Nuegra were given to the subjects right after meal for 4 weeks, and 1 capsule of Nuegra was added each time in subjects with no or minimal effect. Testoster-one and FSH levels were measured at first visit and last visit, for evaluating masculine activities. To avoid bias and standardize the test results, only one clinic was assigned as a central lab, and all blood samples were transferred. General information and subjective symptoms were evaluated at first visit and at 2 weeks interval, week 2, 4 and 6 using VAS (Visual Analogue Scale). The mean age of the subjects were 51.8${\pm}$8.2 years old (range: 36.1-82.1). Based on the subjects who were tested on testosterone and FSH levels at day l and week 6, the means were 4.4${\pm}$1.4 nmol/L (range: 2.6-7.7), 8.6${\pm}$9.6 mIU/mL (range: 0.3-40.4), respectively at day 1. At week 6, the results were 4.9 ${\pm}$1.6 (2.6-8.9 range), 9.4${\pm}$13.1 (1.0-53.9 range), respectively. Marginally significant difference between pre-dose and post-dose was present. Statistically significant differences were revealed in general assessment for subjective symptoms, fatigue, insomnia, erectile dysfunction, etc. In fatigue, response rates were 39.6, 65.4 and 76.4% at week 2, 4 and 6, respectively (P < 0.0001). Response vates for erectile dysfunction were 13.4, 41.2 and 72.7% at week 2, 4, and 6 (P < 0.0001), respectively, Response rates for libido were 13.6, 51.6 and 73.5% at week 2, 4, and 6 (P < 0.0001), respectively. For urinary stream response rates were 26.9, 44.7 and 66.8% at week 2, 4, and 6 (P < 0.0001), respectively. VAS for muscular weakness did not show significant results that response rates were 40, 60 and 80% at week 2, 4, and 6 from 8.2 (P = 0.24), respectively. Response rates for insomnia were 50, 60, 100% at week 2, 4, and 6 (P < 0.0001), respectively. The results shows that Nuegra tends to enhance masculine activities including libido, erectile dysfunction and urinary stream and also effective for improving general conditions especially insomnia, muscular weakness and fatigue. In conclusion, this investigation has demonstrated that Nuegra does not only have tendency to increase masculine activities through increased secretion of the testosterone and FSH but also improve general conditions such as erectile dysfunction, libido, fatigue and muscular power.

Effects of Geiji-Bokryung-Hwan on eNOS, nNOS, Caveolin-1 and bFGF Protein Expressions and the Endothelial Cells of the Corpus Cavernosum in Hypercholesterolemic Rat

  • We examine the effect of Geiji-Bokryung-Hwan(GBH) on erectile function in a rat model of hypercholesterolemic erectile dysfunction. GBH, a drug preparation consisting of five herbs of Cinnamomi Ramulus (Geiji), Poria Cocos (Bokryun), Mountan Cortex Radicis (Mokdanpi), Paeoniae Radix (Jakyak), and Persicae Semen (Doin) is a traditional Korean herbal medicine that is widely used in the treatment of atherosclerosis-related disorders. In this study, 3-month-old Sprague-Dawley rats were used. The 6 rats control animals were fed a normal diet and the other 18 rats were fed 1% cholesterol diet for 3 months. After 1 months, GBH was added to the drinking water of the treatment group of 12 rats but not the cholesterol only group of 6 rats. Of the 12 rats 6 received 30 mg/kg per day (group 1) and 6 received 60 mg/kg per day (group 2) of GBH. At 3 months erectile function was evaluated with cavernous nerve electrostimulation in all animals. Penile tissues were collected for electron microscopy, and to perform Western blot for endothelial nitric oxide synthase (eNOS), neuronal nitric oxide synthase (nNOS), basic fibroblast growth factor (bFGF) and caveolin-1. Systemic arterial pressure was not significantly different between the animals that were fed the 1% cholesterol diet and the controls. Conversely erectile function was not impaired in the herbal medicine treated rats. Electron microscopy showed many caveolae with fingerlike processes in the cavernous smooth muscle and endothelial cell membranes in control and treated rats but not in the cholesterol only group of rats. Western blot showed differences among groups in protein expression for eNOS, nNOS, caveolin-1 and bFGF protein expression in penile tissue. Increased eNOS and nNOS protein expressions dy high cholesterol diet were significantly decreased in group 1 and group 2. Interestingly, caveolin-1 and bFGF protein expression was significantly higher in groups 1 and 2 than in the cholesterol only and control groups.

Among the 64 patients, 37 patients were followed with KRG. Five had diabetes, 5 hypertension, 5 hypercholesterolemia, 6 low testosterone, 6 psychogenic, and 11 idiopathic. The improvement after KRG administration was $70.2\%$ on objective questionnaire and $75.7\%$ on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the placebo. The effects of KRG in Chinese and Singapores were similar to the Koreans. Serum testosterone levels were nonnalized in 6 patients with KRG, who's serum testosterone levels were reduced from pre-study. Two patient reported constipation, and 2 gastric upsets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with little side effects. KRG is effective in Koreans and also Asians. The exact action mechanism and the active ingredients in KRG need to be studied.

Antioxidative Properties of Brown Algae Polyphenolics and Their Perspectives as Chemopreventive Agents Against Vascular Risk Factors

  • Several polyphenolic compounds and complex mixtures were isolated from brown algae species. The 1,1-diphenyl-2-picryhydarzyl (DPPH) radical scavenging activity and ferric reducing antioxidant power (FRAP) of these compounds were evaluated to determine their physiological usefulness as antioxidants for vascular protection. The antioxidative protection of low-density lipoprotein (LDL) was also evaluated and compared with that of catechin, because the generation of oxidized LDL is one of the most active and specific risk factors contributing to atherogenesis. Oral administration to rats of a commercially available sample ($VNP^{TM}$) containing 30% of these polyphenolic compounds and 70% dietary fiber revealed that the serum reducing capacity measured in terms of FRAP value was significantly elevated 30 min after the treatment, but declined rather quickly thereafter, indicating the good oral absorption of the compounds and their fast binding to the lumenal surface of the blood vessels. An eight-week, human, clinical trial (n=31) of $VNP^{TM}$ showed significant improvement in erectile function measured by IIEF (international index of erectile function) score. These results collectively demonstrated the usefulness of these polyphenolic compounds as fundamental chemopreventive agents against vascular risk factors originating from oxidative stress.

Impact of irregular marital cohabitation on quality of life and sexual dysfunction in infertile men from upper Egypt

  • Objective: Irregular patterns of marital cohabitation are a common problem in upper Egypt due to employment conditions. The objective of this study was to investigate the effect of irregular marital cohabitation on the quality of life and sexual function of infertile men. Methods: In total, 208 infertile men were included and divided into two groups. The first group included 134 infertile men with an irregular pattern of marital cohabitation and the second group included 74 infertile men with a regular pattern of marital cohabitation. All subjects were assessed through a clinical evaluation, conventional semen analysis, the fertility quality of life (FertiQoL) questionnaire, the International Index of Erectile Function (IIEF-5) score, and the premature ejaculation diagnostic tool (PMEDT). Results: The two groups were compared in terms of conventional semen parameters, FertiQoL, IIEF-5 score, and PMEDT. Infertile men with an irregular pattern of marital cohabitation had significantly lower subscale and total FertiQoL and IIEF-5 scores. Additionally, they had significantly higher PMEDT scores. Erectile dysfunction and premature ejaculation were more common in them than in infertile men with a regular pattern of marital cohabitation. Conclusion: Irregular patterns of marital cohabitation had an adverse effect on quality of life and sexual function in infertile men.
