Pancreatoduodenectomy following neoadjuvant chemotherapy in duodenal adenocarcinoma

  • A 51-year-old male patient had four times of massive hematochezia episode three days before arrival. Carbohydrate antigen (CA) 19-9 level was extremely elevated. Computed tomography, magnetic resonance imaging, and positron emission tomography-computed tomography identified 5.7 cm sized periampullary duodenal cancer with regional metastatic lymph nodes and vascular invasion to aberrant right hepatic artery, main portal vein, and superior mesenteric vein. Diagnosed as duodenal adenocarcinoma through endoscopic biopsy, 16 times of FOLFIRI (5-fluorouracil, leucovorin, irinotecan) was conducted. The regimen changed to XELOX (capecitabine, oxaliplatine), four times of administration was done, and the CA19-9 level dramatically decreased. The tumor decreased to 2.1 cm. After R0 laparoscopic pylorus preserving pancreatoduodenectomy, no adjuvant therapy was given. No sign of recurrence or metastasis was reported, and the patient reached complete remission after five years. We reported a case where neoadjuvant chemotherapy for locally advanced duodenal adenocarcinoma was shown to be effective.

A Case of Neonatal Tetanus Cured with Neuromuscular Blocking Agent and Ventilatory Support (근이완제 및 인공 환기요법으로 치료한 신생아 파상풍 1례)

  • A neonate born at 38 gestational weeks was admitted due to generalized tonic-clonic seizure and cyanosis. The neonate was born six days previously at home through normal delivery and the umbilical cord was cut using scissors sterilized in boiling water. The neonate weighed 3,180 g at admission. Physical examination revealed cyanosis, opisthotonus, trismus and reactive muscle spasms. Laboratory exam, brain sonogram and EEG showed no significant abnormal findings. Based on her history and physical examination, the neonate was diagnosed with tetanus and put in an incubator isolated in a quiet, dark room. Treatment with tetanus human immunoglobulin along with antibiotics (penicillin G) were started immediately, and mechanical ventilation, administration of neuromuscular blocking agent and muscle relaxant were also started off. The frequency of seizure episode decreased gradually, and on the 32nd hospital day, mechanical ventilatory support was stopped along with extubation two days later. Thereafter, the neonate was in continuous generalized hypertonic state and showed feeding difficulty, but there was gradual improvement. She was dismissed on the 49th hospital day and is currently under OPD follow-up, doing well with no special problems. Respiratory management is critical to neonatal tetanus. We report here a case of tetanus treated with inhibition of self-respiration, neuromuscular blocker and application of ventilator, and present this method as a useful direction for future treatment of neonatal tetanus.

Ozone Exposure Assessment by Population Characteristics: A Case Study for High Ozone Days in Busan (인구특성을 고려한 노출평가: 부산지역 고농도 오존일 사례연구)

  • Objectives: Photochemical ozone pollution is associated with increased mortality risk. This study aims to assess the population exposure to ozone according to population characteristics for high ozone days in the Busan metropolitan region (BMR). Methods: The ozone exposure assessment in this study was performed using the WRF-CMAQ simulated ozone concentrations and the population data in the BMR. The settled and daytime population and their activity were considered to conduct the static and dynamic ozone exposure assessment. Results: Applying a static exposure assessment, in case that high ozone occurred throughout Busan area, the highest exposure levels were evaluated in urban neighborhoods. In case of ozone pollution in outer Busan, because sensitive groups have been relatively higher exposure, this case was also evaluated as part of that should not be overlooked. The dynamic exposure was higher than static exposure because the number of population exposed to ozone of high concentration is increased. This approach is important in a regard consider that daytime population distribution when high ozone occur. Conclusion: This study shows the different population exposure according to various ozone distributions for each episode day. Considering demographic characteristic such as population density and activity should be important to understanding the population exposure assessment when ozone pollution occurs.

Laparoscopic-assisted Colopexy by Two-Portal Access in a Dog with Recurrent Rectal Prolapse

  • A case of rectal prolapses in a one-year-old intact male mixed-breed dog, weighing 6.8 kg was presented with one-week history of protruded tubular pink mass through the anus along with mucosal necrosis. The prolapse was non-reducible, so rectal amputation and anal purse-string suture was performed. Prolapse recurred 9 days after the surgery, purse-string suture and conservative treatment were attempted with no-good results. Laparoscopic-assisted incisional colopexy technique was applied to treat the recurrent rectal prolapse, but failed 6 days after surgery. This was followed by non-incisional colopexy technique, which prevented recurrence during the 3 months of follow-up period. Despite the episode of recurrence, the laparoscopic-assisted colopexy technique treated rectal prolapse successfully. This is the first report in Korea, which describes laparoscopic-assisted colopexy in the dog.

A Research on Comparing the Women's Portrayal between Chinese Family Dramas and Korean Family Dramas (한국 드라마와 중국 드라마의 며느리 이미지에 나타난 여성상 비교)

  • The article compares the women's portrayal between Korean family drama and Chinese family drama. The two dramas are Korean family drama 'the Golden Days of Daughter-in-law ' and Chinese family drama 'the Beautiful Days of Daughter-in-law'. There are many ways to see dramas but dramas are the basement which express the society and the culture in that country, and here the women's portrayal is the main theme. The article follows steps: first, through the dramatic episode of two dramas, show the characters of main images. And through the main characters analyze the reared culture and show the same points and differences between the two chosen episodes. As a result of the women's portrayal analysis through the sex-role model, there were some differences between two dramas : new generation of Korean daughter-in-law tend to be very independent and they make effort to develop themselves even though they got married ; for Chinese daughter-in-law, very dependent, family is prior to everything without opportunity of self-development.

The Features Associated with the Yellow Sand Phenomenon Observed in Korea in Wintertime (겨울철 황상 현상의 특징)

  • Spring time is a favorable season to be easily observed the Yellow Sand phenomenon in East Asia. In particular most of the phenomenon tend to occur in April. However, Yellow Sand phenomenon was observed from almost the whole country of Korea in winter of 1966, 1977 and 1999. The features of the synoptic weather pattern in the source regions, air stream flow between the source region and Korea, the measurement of TSP concentration, aerosol size distribution, and chemical composition of snow samples associated with Yellow Sand phenomenon were investigated. The result showed the characteristic evolutionary feature of the synoptic system associated with Yellow Sand phenomena, that is, a strong low level wind mobilized the dust within 2 or 3 days before Yellow Sand phenomenon being observed in Seoul. The wind was remarkably intensified in the source region on January 24, 1999 under the strong pressure gradient, A trajectory analysis showed that the Yellow Sand particle could be reached to Korea within 2 days from the source region, Gobi desert, through Loess plateau and Loess deposition region. The TSP concentration at the top of Kwanak mountain during the Yellow Sand phenomenon is abruptly increasing than the monthly mean concentration. The size resolved number concentration of aerosols ranging from 0.3 to 25${\mu}{\textrm}{m}$ was analyzed during Yellow Sand episode. It was evident that aerosols were distinguished by particles in the range of 2-3 ${\mu}{\textrm}{m}$ to result in the abrupt increase in January 1999, After Yellow Sand phenomenon, there was heavy snow in Seoul. By the analysis of snow collected during that time, it was observed that both the Ca(sup)2+ concentration and pH were increased abnormally compared to those in the other winter season.

Sports injuries: a 5-year review of admissions at a major trauma center in the United Kingdom

  • Purpose: Sports offer several health benefits but are not free of injury risk. Activity dynamics vary across sports, impacting the injury profile and thereby influencing healthcare resource utilization and health outcomes. The purpose of this study was to investigate sports-related major trauma cases and compare differences across sports and activity groups. Methods: A retrospective case notes review of sports-related major traumas over a 5-year period was conducted. Demographic, hospital episode-related, and health outcome-related data were analyzed, and differences were compared across sports and activity groups. The Glasgow Outcome Scale (GOS) at discharge was used as the primary outcome measure and the length of hospital stay as the secondary outcome measure. Results: In total, 76% of cases had good recovery at discharge (GOS, 5), 19% had moderate disability (GOS, 4), and 5% had severe disability (GOS, 3). The mean length of hospital stay was 11.2 days (range, 1-121 days). The most severely injured body region was the limbs (29.1%) and vertebral/spinal injuries were most common (33%) in terms of location. A significant difference (P<0.05) existed in GOS across sports groups, with motor sports having the lowest GOS. However, no significant differences (P>0.05) were found in other health-outcome variables or injury patterns across sports or activity groups, although more competitive sports cases (67%) required admission than recreational sports cases (33%). Conclusions: Spinal injuries are the most frequent sports injuries, bear the worst health outcomes, and warrant better preventive measures. Head injuries previously dominated the worst outcomes; this change is likely due to better preventive and management modalities. Competitive sports had a higher injury frequency than recreational sports, but no difference in health outcomes or injury patterns.

Effects of Acylase Treatment Episodes on Multispecies Biofilm Development (Acylase의 처리 시기 및 기간이 다종 생물막 형성에 미치는 영향)

  • Acylases can have a significant effect on biofilm formation owing to their quorum quenching activity. In this study, we investigated the effects of acylase treatment episodes on multispecies biofilm development. A consortium composed of 9 species belonging to different genera was allowed to form biofilms for 5 days under various treatment episodes (different treatment periods, 1, 2, 3, or 4 days; and two application timings, beginning or later) at 1, 5, 10, 20 and 50 mg·l-1 acylase concentrations. The acylase treatment for 5 days showed that acylase concentration was negative with biofilm development (linear regression, Y = -0.05·x + 2.37, p < 0.05, R2 = 0.88). Acylase was more effective in reducing biofilm formation when it was applied in the beginning (vs. in later development stage) at all acylase concentrations (p < 0.05). ANOVA indicated that treatment period was significant on biofilm formation in both application timings at ≥ 10 mg·l-1 (p < 0.05). Linearity test results showed that all slope values between period and biofilm were negative in both timings at ≥ 10 mg·l-1 (p < 0.05, except for the later application at 20 mg·l-1). When temporal biofilm dynamics were monitored at 20 mg·l-1, biofilms gradually increased with time at all treatment episodes (p < 0.05), and slope values in linear regression between biofilm and time were lower when acylase was applied in the beginning (p < 0.05). Our findings suggest the importance of the acylase treatment period and application timing on biofilm control.

Clinical Analysis of Disease Recurrence for the Patients with Secondary Spontaneous Pneumothorax (이차성 자연기흉 환자의 재발양상에 관한 분석)

  • Background: Secondary spontaneous pneumothorax is caused by various underlying lung diseases, and this is despite that primary spontaneous pneumotherax is caused by rupture of subpleural blebs. The treatment algorithm for secondary pneumothorax is different from that for primary pneumothorax. We studied the recurrence rate, the characteristics of recurrence and the treatment outcomes of the patients with secondary spontaneous pneumothorax. Material and Method: Between March 2005 to March 2007, 85 patients were treated for their first episodes of secondary spontaneous pneumothorax. We analyzed the characteristics and factors for recurrence of secondary spontaneous pneumothorax by conducting a retrospective review of the medical records. Result: The most common underlying lung disease was pulmonary tuberculosis (49.4%), and the second was chronic obstructive lung disease (27.6%), The recurrence rate was 47.1% (40/85). The second and third recurrence rates were 10.9% and 3.5%, respectively. The mean follow up period was $21.1{\pm}6.7$ months (range: $0{\sim}36$ month). For the recurrence cases, 70.5% of them occurred within a year after the first episode. The success rates according to the treatment modalities were thoracostomy 47.6%, chemical pleurodesis 74.4%, blob resection 71% and Heimlich valve application 50%. Chemical pleurodesis through the chest tube was the most effective method of treatment. The factor that was most predictive of recurrence was 'an air-leak of 7 days or more' at the first episode. (p=0.002) Conclusion: The patients who have a prolonged air-leak at the first episode of pneumothorax tend to have a higher incidence of recurrence. Further studies with more patients are necessary to determine the standard treatment protocol for secondary spontaneous pneumothorax.

Effect of Octreotide on Patients with Malignant Bowel Obstruction (악성장폐색 환자에서 Octreotide의 치료 효과)

  • Department of Internal Medicine, The Catholic University of Korea College of Medicine, Seoul, Korea Purpose: Malignant bowel obstruction causes gastrointestinal symptoms and leads to diminished quality of life in patients with advanced cancer. Several studies have shown the efficacy of octreotide for the relief of malignant bowel obstruction-related symptoms. The aim of this study is to assess the efficacy and safety of octreotide in patients with malignant bowel obstruction. Methods: We retrospectively reviewed medical records of twenty nine patients who had suffered from malignant bowel obstruction without clinical improvement of conservative care and subsequently, received octreotide treatment. Initial dosage of octreotide was 0.1 mg/day, and dose was escalated depending on the clinical effect. For each patient, we assessed visual analogue scale (VAS) of pain, number of vomiting episode, and amount of nasogastric tube drainage. Results: Median dosage of octreotide was 0.2 mg/day (range 0.1~0.6), and median duration from initial medication to death was 20 days (range 2~103). VAS before and after octreotide treatment were 5.6$\pm$1.24, and 2.7$\pm$0.96, respectively. The numbers of vomiting episode before and after octreotide treatment were 3.6/day$\pm$2.5, and 0.4/day$\pm$0.8, respectively. The mean amounts of nasogastric tube drainage before and after octreotide treatment were 975$\pm$1,083 cc/day and 115$\pm$196 cc/day, respectively. Statistically significant reduction in VAS, the number of vomiting episode and the amount of nasogastric tube drainage were observed after octreotide treatment (P<0.05). Conclusion: Administration of octreotide in patients with malignant bowel obstruction, which is uncontrolled by other medication, was effective and safe. In such clinical situations, physicians should consider to add of octreotide for symptomatic control.
