Gastric Cancer Presenting as a Krukenberg Tumor at 22 Weeks' Gestation

  • Gastric cancer is rare during pregnancy, and often advanced upon presentation. A Krukenberg tumor presents a diagnostic and therapeutic challenge in the pregnant patient. We present a case of a 38-year-old woman at 22 weeks' gestation who presented with worsening epigastric pain, and was found to have a left pelvic mass on ultrasound, which was confirmed by magnetic resonance imaging. She went into active labor and delivered a viable infant via vaginal delivery. An exploratory laparotomy revealed a large mass originating from her left ovary and diffuse thickening of the lesser curvature of the stomach. Frozen section investigation revealed the presence of signet cell adenocarcinoma. Subsequent upper endoscopy showed linitis plastica, while biopsy confirmed the presence of adenocarcinoma. In conclusion, the occurrence of gastric cancer in pregnancy is rare despite extremely common symptoms. The management poses a challenge because of the need for early treatment, and the continuation of the pregnancy.

  • Helicobacter pylori(H. pylori) is the causative agent of chronic gastritis and the single most important factor in peptic ulcer disease, however, the pathogenetic mechanisms underlying H, pylori infection are not well understood. Futhermore, there is a strong association between H. pylori infection and gastric cancer. Various diagnostic methods for detecting H. pylori infection are available. These can be divided into invasive methods, requiring endoscopy, and non-invasive tests, mainly 13C-urea breath tests and serologic detection of antibodies. Rapid urease test is the most recommendable endoscopic test for the diagnosis of H. pylori infection, presently. CLO test kit is the represent of rapid urease test kits. The principles of CLO test kit is that hydrolysis of urea by urease Is detected by a dye indicators showing a color change. Our device is used same principle but we improved the reaction time is more faster and positive color change is more distinctive from the color of the negative specimen. So, this kit is more reliable because it response faster and accuracy.

  • Object : To report the case of the improved patient after the extraction with hemostst forcept. Methods : We treated one patient who visited Gabsan Korean medical clinic.First two times, We used acupuncture, fumigation, exsanguination, external medincine. He had some agonistic response. But It was not satisfactory. So We operated extraction with hemostat forceps. Result and Conclusion : 1. We extracted nasal polyp, the length of polyp was 5.5 cm2. After extraction, the patient had not headache, nasal congestion and so on.3. On intranasal endoscopy, left nose did not have polyp any more.4. Hemostat forceps extraction is effective to nasal polyp.

  • Vocal cord polyps are generally removed using a direct laryngoscope and surgical microscope. In some case of laryngeal microsurgery, laryngeal exposure with surgical microscope is difficult because of severalfactors. We experienced a case of vocal polyp with difficult laryngeal exposure in a 56 year old male patient. The vocal cord polyp was removed successfully through endoscopic approach.

  • In many medical image devices, dc noise often prevents normal diagnosis. In wireless capsule endoscopy systems, multipath fading through indoor wireless links induces inter-symbol interference (ISI) and indoor electric devices generate impulsive noise in the received signal. Moreover, dc noise, ISI, and impulsive noise are also found in optical fiber communication that can be used in remote medical diagnosis. In this paper, a blind signal processing method based on the biased probability density functions of constant modulus error that is robust to those problems that can cause error propagation in decision feedback (DF) methods is presented. Based on this property of robustness to error propagation, a DF version of the method is proposed. In the simulation for the impulse response of optical fiber channels having slowly varying dc noise and impulsive noise, the proposed DF method yields a performance enhancement of approximately 10 dB in mean squared error over its linear counterpart.

  • A 23-year-old female residing in a village of Cao Bang Province, North Vietnam, visited the Hospital of Hanoi Medical University in July 2013. She felt dim eyes and a bulge-sticking pain in her left eye for some days before visiting the hospital. In the hospital, a clinical examination, an eye endoscopy, and an operation were carried out. A nematode specimen was collected from the eye of this patient. The body of this worm was thin and long and measured $22.0{\times}0.3mm$. It was morphologically suggested as an immature female worm of Angiostrongylus cantonensis. By a molecular method using 18S rRNA gene, this nematode was confirmed as A. cantonensis. This is the first molecular study for identification of A. cantonensis in Vietnam.

  • Most solitary gastrointestinal (GI) polyps in children are either inflammatory or hamartomatous. Solitary hyperplastic polyp, sentinel polyp and solitary adenomatous polyp have been occasionally diagnosed in adults, but very rarely reported in Korean children. We recently came across a case with adenomatous polyp in the colon, a case with hyperplastic polyp beneath the gastroesophageal junction, a case with hyperplastic polyp in the prepyloric area, and a case with sentinel polyp in the distal esophagus, which are unusual pathologic types in children. These mucosal lesions were diagnosed incidentally during elective endoscopic examinations for GI symptoms. Most polyps do not cause significant symptoms, so the diagnosis might be delayed, especially in children, in whom GI endoscopy is not commonly performed for screening purpose as in the adults.

  • Prolonged air leakage is a major cause of morbidity in pneumothorax. When conservative management is not effective, surgery should be performed. However, surgery is not appropriate in patients with low pulmonary function. In these patients, occlusion of the airway with endobronchial blockers may be attempted under bronchoscopy. We treated two patients with prolonged air leakage using endobronchial Watanabe spigots under fibrobronchoscopy.

  • Due to improve form accuracy and surface roughness of a aspheric lens core that is made of Ni, the study is carried out on localization about a Subminiature Optics of Proximity and Wide Field of View. The required form accuracy P-V $0.2{\mu}m$ and surface roughness is Ra 10 nm. The design of experiment(DOE) is adopted to find a optimal cutting conditions which are spindle speed, depth of cut, feedrate. Finally, the effects of this study are replacing importation and strengthening competitiveness through the localization of the Subminiature Optics of Proximity and Wide Field of View.

Jejunogastric intussusception prone to misdiagnosis as gastric cancer

  • The authors report a case of a 78-year-old female with a history of gastric surgery 35 years ago. She was initially misdiagnosed as gastric cancer bleeding and underwent an emergency laparotomy under the diagnosis of jejunogastric intussusception (JGI), 23 hours after the onset of symptoms. We also reviewed 116 JGI case reports and analyzed clinical features and outcomes. Compared to the past, diagnosis of JGI is easier with diagnostic examinations such as an endoscopy, computed tomography, and the upper gastrointestinal series. And a good prognosis can be expected with proper fluid resuscitation and surgical reduction, even if the symptoms persist more than 48 hours.
